Temporary Mechanical Circulatory Support in Acute Myocardial Infarction-Related Cardiogenic Shock: Lessons from Contemporary Randomized Trials.

Background: Acute myocardial infarction-related cardiogenic shock (AMI-CS) remains associated with high mortality despite advances in revascularization, critical care, and temporary mechanical circulatory support (MCS). Recent randomized trials have challenged the concept of routine device-based therapy and highlighted the importance of matching MCS strategy to shock phenotype, timing, and patient risk profile. Methods: We reviewed contemporary randomized clinical trials and prespecified or post hoc analyses evaluating temporary MCS strategies in adult patients with AMI-CS. PubMed was searched

Study of the energy efficiency of various drive systems of an innovative implantable left ventricular assist device.

This paper presents the results of studies of the energy efficiency of an innovative implantable left ventricular assist device. One of the advantages of the proposed LVAD is in cases where end-stage left ventricular failure is accompanied by mitral valve regurgitation and pulmonary hypertension. Another advantage is minimally invasive implantation with minimal thoracic trauma, without disrupting the breathing process, which shortens the length of hospital stay. The best drive system for converting the rotary motion of the servo motor into reciprocating motion of the piston of the suction-disc

Weaning in early neurological-neurosurgical rehabilitation in Germany - results from a nationwide online survey.

BACKGROUND: In Germany, approximately 1,100 beds are allocated to early neurological-neurosurgical rehabilitation (ENNR) for patients with severe neurological illness requiring weaning from mechanical ventilation. Specialized ENNR institutions play a major role in maintaining ICU capacity in acute care hospitals and in reducing the number of patients dependent on home intensive care. However, nationwide structural data on ENNR weaning centers-together with detailed patient characteristics-remain unavailable. This lack of information poses significant challenges for healthcare planning, resourc

Alternative Polyadenylation Signatures Distinguish Maladaptive Right Ventricular Remodeling in Pulmonary Hypertension: Implications for RNA-Based Diagnostics and Therapeutics.

Increased pulmonary vascular pressures due to vascular remodeling, elevated vascular resistance, and vasoconstriction characterize Pulmonary Arterial Hypertension (PAH). The narrowing of the pulmonary arteries and obstruction of blood flow increase the Right Ventricular (RV) afterload, forcing the RV to undergo structural and functional changes. While adaptive remodeling leads to RV compensation by maintaining function, maladaptive remodeling leads to RV decompensation, characterized by worsening function and eventual failure. At present, there is no effective treatment for these patients as t

Clinical Profile and Outcome of Hypertensive Emergencies: A Prospective Observational Study.

INTRODUCTION: Hypertensive emergencies are among the most common life-threatening acute medical conditions encountered in clinical practice. They are characterized by a sudden, severe elevation of blood pressure (systolic ≥180 mmHg and/or diastolic ≥120 mmHg) in association with acute target organ damage, leading to significant short-term mortality and long-term morbidity. In low- and middle-income regions, delayed detection, poor treatment adherence, and limited health infrastructure amplify this burden. This prospective observational study aimed to evaluate the clinical profile, organ involv

Advancements and Applications of Focused Assessed Transthoracic Echocardiography: A Narrative Review.

BACKGROUND AND AIMS: Focused Assessed Transthoracic Echocardiography (FATE), developed by Dr. Erik Sloth in 1989, has become a key ultrasound technique for rapid bedside assessment of cardiac function. Its non-invasive nature, real-time imaging, and portability make it ideal for facilitating swift clinical decisions across various specialties. This review aims to explore the evolution of FATE, evaluate its current clinical applications, and discuss prospects, with a focus on training protocols, image interpretation, and the incorporation of emerging technologies. METHODS: A narrative review o

Cardiorenal Hemodynamic Coupling: Mechanical Circulatory Support Augments Renal Blood Flow via Renal Vasodilation.

Robust cardiorenal hemodynamic coupling influences outcomes in cardiogenic shock (CS), with concomitant renal failure a persistent challenge despite the use of mechanical circulatory support (MCS). The authors investigated cardiorenal hemodynamic coupling during progressive left ventricular failure and percutaneous ventricular assist device (pVAD) unloading in a porcine model. Renal blood flow (RBF) and aortic, left ventricular, and central venous pressures were continuously measured across progressive CS and varying pVAD support. During CS progression, increased left ventricular end-diastolic

Rate of decline in residual kidney function and cognitive impairment in incident haemodialysis patients: A prospective, longitudinal analysis of the BISTRO trial cohort.

BACKGROUND: Previous studies suggest loss of residual kidney function (RKF) on haemodialysis is associated with cognitive impairment (CI). This study investigated the association of rate of change in RKF with risk of new CI and change in cognition after haemodialysis initiation and the association between dialysis treatment parameters and changes in cognition. METHODS: This prospective cohort study used data from the Bioimpedance Spectroscopy to Maintain Renal Output trial (BISTRO). Cognition was assessed using the Montreal Cognitive Assessment (MoCA). Learning from a pitfall: Atrial septostomy in LV failure under VA-ECMO for pulmonary hypertension.

BACKGROUND: Atrial septostomy has been successfully used in patients with pulmonary hypertension and in those supported with veno-arterial extracorporeal membrane oxygenation (VA-ECMO) complicated by pulmonary congestion. CASE DESCRIPTION: We report a patient on VA-ECMO for decompensated pulmonary hypertension who subsequently developed stress-induced cardiomyopathy and pulmonary congestion. An atrial septostomy was performed, which led to worsening respiratory status due to the creation of a right-to-left shunt, and hemodynamic deterioration resulting from increased left ventricular preload.

Isolated prenatal foramen ovale closure or restriction presenting after birth: a distinct, under-recognized clinical condition.

BACKGROUND: The foramen ovale (FO) plays a central role in fetal circulation. Prenatal FO closure or restriction is well-characterized in fetuses and in neonates with congenital heart defects, but is rarely described as an isolated finding after birth. This study aimed to characterize the clinical presentation, echocardiographic features, management, and outcomes of neonates with isolated prenatal FO closure or restriction. METHODS: We retrospectively analyzed 10 consecutive neonates admitted to our tertiary neonatal intensive care unit between September 2018 and August 2025 with FO closure o

Medical Management and Mechanical Circulatory Support Escalation in Right Ventricular Failure.

Right ventricular (RV) failure is a clinically distinct entity with significant prognostic implications across multiple cardiovascular conditions. Its diagnosis is challenging due to overlapping features with left ventricular failure, often leading to delayed recognition. This article reviews the unique pathophysiology, clinical presentation, and diagnostic strategies for RV failure, emphasizing hemodynamic assessment. A structured approach to medical management is presented, focusing on optimization of preload, afterload, contractility, and rhythm control. Disease-specific considerations and

The Ca2+-Sensitivity of Contraction is Increased in the Left Atrium and Left Ventricle of Patients with Ischemic Heart Failure.

BACKGROUND: Ischemic heart failure (IHF) has been shown to impair contractility and disrupt sarcomere function in the left ventricle. Left ventricular failure can cause left atrial dysfunction, which is associated with a greater risk of patient mortality. Despite this, the biochemical and biomechanical characteristics of the left atrium in IHF remain obscure. METHODS: Myocardial mechanical properties were measured using permeabilized muscle isolated from the left ventricle (LV) and left atrium (LA) of donors and patients with IHF. Tissue homogenates from these samples were used to measure tit

Transient pseudo-P pulmonale unmasking cardiopulmonary interactions in cardiogenic shock.

The "pseudo-P pulmonale" mimics right atrial enlargement, often mimicking acute pulmonary embolism (PE). We report a 70-year-old male with cardiogenic shock and elevated D-dimer post-thrombolysis, initially suggesting massive PE. However, dynamic ECG monitoring revealed extreme pseudo-P pulmonale (0.55 mV) temporally uncoupled from the D-dimer peak. Postural experiments and extubation confirmed this alteration was the electrophysiological projection of severe backward pressure from acute left ventricular failure, exacerbated by high positive end-expiratory pressure (PEEP 14 cmH2O). This case h

Left Ventricular Failure Despite Patent Coronary Arteries in Acute Type A Aortic Dissection.

Severe myocardial failure after acute type A aortic dissection is usually attributed to structural coronary malperfusion. We report catastrophic global left ventricular failure despite preserved epicardial coronary anatomy, successfully bridged with durable mechanical circulatory support. A 57-year-old man presented with acute type A dissection and cardiogenic shock with extensive precordial ST-segment elevation. Emergency total arch replacement with a frozen elephant trunk was performed. Intraoperative inspection and postoperative coronary angiography confirmed patent coronary ostia and arter

The β1 adrenoceptor (AR) blocker metoprolol normalises βAR cascade and caveolar protein expression and restores β1AR responsiveness in failing right ventricular myocytes.

Right ventricular (RV) failure is the leading cause of death in pulmonary artery hypertension (PAH) and treatments that preserve RV contractility are urgently required. β-adrenoceptor blockers (BB) are used clinically in left ventricular failure to improve β-adrenoceptor (AR) responsiveness but it is not known whether they confer the same benefits in RV failure. Here we tested this using the rat monocrotaline (MCT) model of PAH. When PAH was established, treatment commenced with β1AR-selective metoprolol (10 mg/kg/day; MCT + BB group) or vehicle (MCT group). In isolated RV myocytes from MCT, i

Extracorporeal life support for severe heart failure in a diffuse panbronchiolitis patient awaiting lung transplant: a case report.

A 55-year-old woman with chronic respiratory failure due to diffuse panbronchiolitis, on a lung transplant waitlist, developed acute left ventricular failure with an ejection fraction of 18%. Although cardiac reversibility was uncertain, it was considered potentially reversible and transplant eligibility was judged to be maintainable; therefore, veno-arterial extracorporeal membrane oxygenation with Impella was initiated as extracorporeal life support (ECLS), with combined support selected to prevent left ventricular distension and provide circulatory stabilization. Cardiac function improved,

Right ventricular dysfunction in cardiogenic shock on temporary mechanical circulatory support.

AIMS: Right ventricular dysfunction (RVD) is common in cardiogenic shock (CS) due to left ventricular failure and is associated with increased mortality, but its impact in patients requiring temporary mechanical circulatory support (tMCS) remains unclear. We explored RVD and its association with clinical outcomes in CS patients under tMCS. METHODS AND RESULTS: Individuals with CS from the SWISS Circulatory Support Registry (NCT04117230) were included. RVD was defined by ≥1 of the following parameters: tricuspid annular plane systolic excursion (TAPSE) < 14 mm, TAPSE/systolic pulmonary artery
