Are general practitioners less likely to recommend treatment when given absolute data on its risks and benefits? A web-based randomized survey.

BACKGROUND: Doctors need to balance benefits and harms of treatments to advise patients and enable shared decision-making, yet studies indicate general practitioners (GPs) may not be familiar with treatment effect sizes. In atrial fibrillation, national guidelines on anticoagulation recommend risk scoring tools, which do not provide details of the absolute effects. OBJECTIVES: To determine if presenting absolute data on benefits and risks of anticoagulation in atrial fibrillation influences GPs' treatment recommendations. METHODS: One hundred and fifty-nine GPs and 93 GP trainees in South Ea

Cardiomyocyte-derived OTUD7B promotes cardiac hypertrophy by deubiquitinating SERCA2a.

Rationale: Pathological cardiac hypertrophy, triggered by persistent neurohumoral or hemodynamic stress, is a key precursor of ventricular dysfunction and heart failure. Deubiquitinating enzymes (DUBs) have emerged as critical regulators of cardiovascular biology. This study examined the function of a DUB, ovarian tumor domain-containing 7B (OTUD7B), in cardiac hypertrophy. Methods: Cardiomyocyte-specific OTUD7B knockout and overexpression mouse models were generated to evaluate myocardial hypertrophy and cardiac dysfunction in response to angiotensin II (Ang II) infusion or transverse aortic

Intraosseous venous malformation of the clivus.

This case report presents a female patient in her teens who experienced a 1-month history of intermittent right-sided tongue numbness and twitching.Investigations included laboratory tests and an electroencephalogram, both of which were unremarkable. A brain MRI revealed an expansile mass occupying the entirety of the clivus and the condylar segment of the right occipital bone. A CT scan further demonstrated an osteolytic and expansile mass within the clivus, with multiple areas of thickened bone trabeculae resembling a 'honeycomb pattern'. A trans-sphenoidal surgical biopsy disclosed an intra

Clinical phenotypes of ANCA-associated vasculitis using ensemble clustering: prognostic and therapeutic insights from two Japanese cohorts.

OBJECTIVES: Clinical heterogeneity in ANCA-associated vasculitis (AAV) is not fully captured by conventional disease subtypes or ANCA serotypes. We aimed to identify data-driven, clinically meaningful disease phenotypes based on patterns of organ involvement in microscopic polyangiitis (MPA) and granulomatosis with polyangiitis (GPA), and to evaluate their prognostic and therapeutic implications. METHODS: We conducted a multicentre retrospective cohort study using two nationwide Japanese registries (J-CANVAS as derivation; JPVAS as validation). Ensemble clustering was applied to high-dimensio

Timing to restart direct oral anticoagulants after traumatic intracranial haemorrhage (RESTART tICrH): study protocol for a randomised controlled multicentre trial.

BACKGROUND: Head injury in older adults is increasingly common, often caused by low-impact falls. A significant proportion of these patients are taking oral anticoagulants (OACs) to prevent stroke or thromboembolism. When traumatic bleeding within the head occurs, OACs are usually stopped to reduce the risk of worsening haemorrhage; however, prolonged interruption increases the risk of stroke, systemic thromboembolism and death. There is currently no consensus on when it is safest to restart OACs. Direct oral anticoagulants (DOACs) are now widely prescribed and may offer advantages over warfar

Sodium-myo-inositol cotransporter-1, SMIT1, promotes cardiac hypertrophy and fibrosis induced by pressure overload in mice.

AIMS: Recent clinical studies have reported that myo-inositol is consistently elevated in plasma of patients with heart failure (HF), yet its role in cardiac dysfunction remains poorly understood. Myo-inositol is specifically transported into cells by the sodium-myo-inositol co-transporter-1 (SMIT1), a member of the sodium-glucose co-transporter (SGLT) family expressed in the heart. While myo-inositol is essential for phosphoinositide signalling, osmoregulation, and metabolic homeostasis, dysregulation of SMIT1-mediated myo-inositol transport may contribute to key pathological mechanisms in HF

T-cadherin, a major adiponectin binding partner, suppresses ERK signaling in metabolic tissues.

T-cadherin, which is a major adiponectin binding partner, exerts various organ-protective effects. However, the specific changes in intracellular signaling that are induced by T-cadherin in metabolic tissues/cells remain unclear. We demonstrated that T-cadherin suppresses ERK signaling in both cultured cells and murine tissues. T-cadherin knockdown increased ERK phosphorylation in C2C12 myocytes and F2 endothelial cells, whereas T-cadherin overexpression suppressed ERK phosphorylation. Proteomic analysis revealed that many proteins that are downstream targets of ERK signaling were upregulated

Early identification of vascular cognitive impairment from a multimodal perspective: a combined diagnosis from targeted cognitive assessments, imaging biomarkers, and molecular fluid biomarkers to ecological behavioral characteristics.

Vascular cognitive impairment (VCI), the second leading cause of dementia, is characterized by heterogeneous pathophysiology and a potentially reversible early phase, underscoring the need for timely identification. This review synthesizes advances across four complementary domains - targeted cognitive assessments, imaging biomarkers, molecular fluid biomarkers, and ecological behavioral characteristics - conceptualized as the TIME framework. Emerging markers, including the peak width of skeletonized mean diffusivity (PSMD), oxygen extraction fraction, brain-derived extracellular vesicles, and

Structural Diversity of 2-O-(β-d-Glucopyranosyl)benzyl Quinates from Casearia tardieuae and Their Anti-Myocardial Hypertrophy Activity in Rat Cardiomyocytes.

Thirteen undescribed 2-O-(β-d-glucopyranosyl)benzyl quinates (1-13) and two reported derivatives (14, 15) were obtained from the twigs and leaves of Casearia tardieuae. The structures of the new compounds were elucidated by comprehensive spectroscopic analysis (1D/2D NMR, HRESIMS, IR, UV), coupled with acid hydrolysis and derivatization studies of the sugar moieties. Biological evaluation showed that compound 1 exhibited antihypertrophic potency comparable to the positive control prazosin, and its cytotoxicity was significantly lower than that of prazosin. Preliminary mechanism investigations

Paired vagus nerve stimulation for post-stroke shoulder motor recovery: corticospinal-C5 motor network plasticity and rehabilitation applications.

Shoulder motor impairment after stroke restricts reaching, self-care, balance reactions, and the use of mobility aids. Paired vagus nerve stimulation (pVNS), in which brief cervical vagus nerve stimulation is temporally coupled to task-specific rehabilitation, improves overall upper-extremity impairment and activity in selected people with chronic ischemic stroke. Its shoulder-specific effects, however, remain uncertain. This Mini Review evaluates pVNS through a corticospinal-C5 motor-network frameworkthat links movement-related cortical ensembles, descending corticospinal and reticulospinal p
