UNLABELLED: Pseudoaneurysm of the mitral-aortic intervalvular fibrosa is a rare and typically acquired cardiac lesion, with congenital forms being exceptionally uncommon, particularly when identified prenatally. We report the case of a 23-year-old gravida 2 woman with one previous history of miscarriage who was referred at 25 weeks' gestation for a fetal intracardiac mass. Fetal echocardiography revealed a cystic echogenic mass at the crux of the heart, consistent with a pseudoaneurysm arising from the mitral-pulmonary intervalvular fibrosa (MPIVF) associated with transposition of the great ar

BACKGROUND: Transthoracic echocardiography is the first-line test for congenital heart disease (CHD), but accurate targeted triage and lesion subtyping require expertise and synthesis across multiple heterogeneous cine clips. We developed a video-based deep learning approach for examination-level targeted triage and subtype classification of common left-to-right shunt lesions.
METHODS: We retrospectively assembled 2,373 echocardiography examinations (800 normal; 493 VSD, 751 ASD, and 329 PDA) from The First Affiliated Hospital of Xinjiang Medical University. Each examination comprised multipl

OBJECTIVES: Post-infarction ventricular septal defect is a rare but severe mechanical complication of acute myocardial infarction that can lead to cardiogenic shock and carries a high mortality rate. Early recognition and multidisciplinary management are essential for improving survival.
CASE PRESENTATION: A 58-year-old previously healthy woman presented to the Emergency Department with syncope and cardiogenic shock due to an anterior myocardial infarction. Echocardiography revealed an apical ventricular septal defect with severe biventricular dysfunction and right ventricular failure, confir

BACKGROUND: Residual shunts after tetralogy of Fallot (TOF) repair are rare and can stem from residual defects, unrecognized VSDs, or, rarely, acquired coronary cameral fistulas (CCFs). Correctly distinguishing these is vital since treatments differ.
CASE SUMMARY: Two paediatric cases following TOF repair are described: one shows features of an acquired CCF with continuous flow from the coronary artery into the right ventricle. Because of the low shunt fraction, asymptomatic presentation, and lack of guidelines for definitive treatment, the patient is managed conservatively with regular monit

Recent advances in congenital heart disease management reflect a paradigm shift towards less invasive, physiology-preserving, and durable interventions. Transcatheter pulmonary valve implantation has become an established therapy for right ventricular outflow tract dysfunction, supported by multiple valve systems tailored to increasingly complex anatomies. These devices have demonstrated high procedural success and sustained hemodynamic performance, although infective endocarditis and arrhythmic complications remain clinical concerns. In parallel, transcatheter edge-to-edge repair for atrioven

BACKGROUND: Atrial septal defect (ASD) is one of the most common congenital heart defects and leads to chronic right ventricular (RV) volume loading, which may progress to heart failure if untreated. Surgical or transcatheter closure unloads the RV and improves hemodynamics, but the impact on intraventricular hemodynamic forces (HDF) remains unclear. Here, we evaluated the effects of ASD closure on biventricular HDF.
METHODS: Cardiac magnetic resonance imaging was performed at 1.5T on 21 patients and 11 sex- and age-matched healthy controls. Whole-heart 4D flow and cine images in standard vie

INTRODUCTION: CHD affect approximately 1% of live births globally and are the most common congenital anomaly. Contemporary data on CHD risk in offspring of affected mothers remains limited. This study evaluated CHD incidence in offspring of mothers with CHD at a southeastern U.S. academic medical centre.
METHODS: We conducted a retrospective review of patients with CHD who received prenatal care between 2015 and 2023 at a multidisciplinary Cardio-Obstetrics clinic. Patients with acquired heart disease, syndromic or inherited CHD, and deliveries at outside hospitals with missing delivery data

BACKGROUND: Infants with ventricular septal defect (VSD) and concurrent respiratory compromise exhibit significant heterogeneity in their recovery after surgical repair. Objective tools for preoperative risk stratification are lacking.
AIMS: The primary aim of this study was to determine if preoperative hemodynamic data, acquired noninvasively using Electrical Cardiometry (EC), could predict prolonged mechanical ventilation (PMV) in infants undergoing VSD repair.
METHODS: We conducted a retrospective study of 51 infants. EC monitoring (ICON) was performed from admission to the day before sur

BACKGROUND: Coexisting coronary artery disease (CAD) and congenital heart disease (CHD) is rare in adults and poses complex management challenges requiring multidisciplinary coordination.
CASE SUMMARY: A 79-year-old man with a history of CAD and stenting was referred for evaluation given worsening dyspnea. Coronary angiography revealed severe ostial left circumflex stenosis and moderate left main disease. In addition, computed tomography demonstrated a previously undiagnosed sinus venosus atrial septal defect with partial anomalous pulmonary venous return. Multidisciplinary evaluation at a pa

BACKGROUND: Acquired Gerbode defect is a complication of cardiac surgery. Diagnostic challenges arise because of frequent mischaracterization as severe tricuspid regurgitation on imaging, often delaying definitive management.
CASE SUMMARY: After undergoing surgical tricuspid valve repair, a patient developed an acquired Gerbode defect located in the membranous septum beneath the noncoronary cusp of the aortic valve. The defect remained undetected for over 3 years owing to misinterpretation of echocardiographic findings as severe tricuspid regurgitation. Comprehensive transesophageal echocardi

Vertical right axillary thoracotomy (VRAT) has become a useful minimally invasive technique for treating certain congenital and acquired heart conditions. By making a vertical cut along the right axillary line, VRAT provides excellent access to the heart while reducing chest wall trauma and improving cosmetic results. It has been effectively used for repairing atrial septal defects, partial anomalous pulmonary venous return, tricuspid valve issues, and some aortic and left-sided lesions in suitable patients. Although these benefits are clear, broader adoption is constrained by concerns about l

BACKGROUND: Cardiovascular diseases (CVDs) are major causes of premature mortality and disability, especially in settings where timely cardiac surgery or percutaneous interventions are limited. Globally, the prevalence of CVDs is rising, with significant regional differences. Africa bears a high burden of congenital heart disease (CHD) and rheumatic heart disease (RHD) but has limited access to surgical and interventional resources. This study describes the spectrum and management of cardiac diseases at the Cardiac Center of Ethiopia (CCE), highlighting treatment gaps. METHODS: A hospital-base

BACKGROUND: Patients with adult congenital heart disease (ACHD) frequently develop late complications despite early surgical repair, including ventricular dysfunction and arrhythmias. In conditions with predominant right-sided pathology such as atrial septal defect (ASD), pulmonary stenosis (PS) and Tetralogy of Fallot (ToF), leading to right ventricular dilatation or systolic/diastolic dysfunction, ventricular interdependence may result in impaired left ventricular filling and left atrial (LA) dysfunction, despite the absence of primary left-sided disease. LA strain (LAS) has demonstrated pro

IMPORTANCE: Maternal adult congenital heart disease (ACHD) has been associated with increased offspring congenital heart disease (CHD), but evidence from resource-limited regions remains scarce. The association between maternal acquired heart disease (AHD) and offspring CHD is unknown.
OBJECTIVE: To quantify the overall and subtype-specific CHD risk in offspring associated with maternal ACHD and AHD, examine the association of maternal ACHD and AHD with outcomes in offspring with CHD, and identify maternal factors that may modify the associations between maternal cardiac diseases and offsprin

BACKGROUND: Cardiac computed tomography angiography (CTA) has been shown to be an excellent modality not only to assess the coronary arteries but also the interatrial septum. We aimed to develop a classification system to grade interatrial septal patency with a focus on clinical applicability.
METHODS: Patients who underwent cardiac CTA from January 1st, 2018 to December 31st, 2022 at a specialized imaging center were included into a registry study. Scans were acquired on a third-generation dual-source CT scanner and retrospectively analyzed using a dedicated post-processing software.

BACKGROUND: Absence of the pericardium (CAP) is a rare condition that can be either congenital or acquired. The acquired form typically occurs following pericardiectomy to treat constrictive or recurrent pericarditis. The congenital variant is extremely rare, with an incidence of less than 1 in 10,000. CAP can be further classified based on the location of the defect and whether the absence is partial or complete. Complete left-sided defects are the most common, accounting for approximately 70% of all pericardial defects.
CASE PRESENTATION: The patient is a 16-year-old Black African male who

BACKGROUND: Acquired ventricular septal defect (VSD), typically following myocardial infarction (MI) or infective endocarditis (IE), remains a rare but life-threatening condition. Surgical repair represents the standard of care, yet data on long-term outcomes and health-related quality of life (HRQoL) are limited.
METHODS: We retrospectively analyzed 44 consecutive patients (median age 65 years, interquartile range [IQR] 57-72.5; range 20-83) who underwent surgical VSD closure at our institution between 2008 and 2023. Clinical characteristics, perioperative variables, and outcomes were assess

BACKGROUND: In resource-limited settings lacking routine newborn screening, common lower respiratory tract infections like community-acquired pneumonia (CAP) can obscure or delay the diagnosis of underlying cyanotic congenital heart disease (CHD). Tetralogy of Fallot (TOF) is the most common cyanotic CHD, yet its co-occurrence with CAP creates significant diagnostic masking, as acute pulmonary signs often conceal cardiac abnormalities.
CASE PRESENTATION: A 5-month-old male presented in Borama, Somalia, with severe respiratory distress, fever, cough, and bilateral crackles, leading to an initi
