Mechanisms and Management of Acquired Aseptic Intracardiac Communications After Transcatheter Aortic Valve Implantation: Single-Center Case Series Insights.

BACKGROUND: Acquired aseptic intracardiac communications (AICCs) are rare but clinically relevant structural complications following transcatheter aortic valve implantation (TAVI), typically resulting from mechanical injury to the aortic annulus, interventricular septum, or adjacent structures. Evidence regarding their incidence, mechanisms, and optimal management remains limited. AIM: To review the mechanisms and management of post-TAVI AICCs and to present a single-center experience focusing on multimodality imaging, procedural characteristics, and outcomes of transcatheter closure. METHOD

Excision of calcified left atrial myxomas: a surgical challenge.

Atrial myxomas are the most frequent primary cardiac tumors and are typically cured by surgical resection. Prognosis is excellent in most cases, but complex tumors that involve adjacent valvular or septal structures may lead to challenging repairs and adverse outcomes. We present two patients with atrial myxomas who underwent surgery with strikingly different results. The first, a 72-year-old woman, had a broad-based, calcified left atrial myxoma encroaching on the mitral annulus. Following resection, annular stability was preserved with annuloplasty and patch reinforcement, resulting in succe

The pivotal role of echocardiography in structural heart interventions: from planning to success.

Structural heart disease (SHD) encompasses a wide range of congenital and acquired cardiac abnormalities, increasingly treated with transcatheter interventions. Echocardiography plays a crucial role in optimising these procedures, providing high-resolution imaging for pre-procedural planning, real-time guidance and post-intervention assessment. The shift from two-dimensional to advanced three/four-dimensional (3D/4D) echocardiography has significantly enhanced the visualisation of complex cardiac structures, improving procedural success and patient safety. The purpose of this review article is

Transcatheter Edge-to-Edge Repair in Adult Congenital Heart Disease: Anatomic Challenges, Device Adaptations, and Emerging Evidence.

Atrioventricular valvular regurgitation (AVVR) is a principal determinant of morbidity and mortality in adults with congenital heart disease (ACHD), and surgical management carries substantial operative risk owing to prior sternotomies, distorted intracardiac anatomy, impaired ventricular function, and lesion-specific hemodynamic fragility. Transcatheter edge-to-edge repair (TEER)-using MitraClip or TriClip (Abbott) or PASCAL/PASCAL ACE (Edwards Lifesciences)-has emerged as a compelling alternative in high-surgical-risk acquired valvular disease and is rapidly expanding into ACHD. We review th

Berlin Heart EXCOR sVAD implantation technique for neonates and infants with functionally univentricular ductal-dependent systemic circulation.

The Berlin Heart EXCOR is a pulsatile paracorporeal ventricular assist device (VAD) for neonates, infants, children and adults with congenital or acquired severe ventricular dysfunction. Berlin Heart EXCOR VADs are routinely used as either a bridge to a cardiac transplantation, or occasionally as a bridge to ventricular recovery. Our programmatic philosophy is to bridge neonates and infants with functionally univentricular ductal-dependent systemic circulation or functionally univentricular ductal-dependent pulmonary circulation who are at high risk for staged palliation because of important c

Acquired ventricular septal defect secondary to aortic valve endocarditis.

Infective endocarditis remains a deadly disease with a significant mortality rate. While ventricular septal defects (VSDs) have been linked to an increased risk of infective endocarditis, cases of acquired VSDs resulting from infective endocarditis are not well-documented in the literature. Our report highlights a rare case of acquired VSD that resulted directly from aortic valve endocarditis, treated with successful repair and placement of permanent pacemaker.

Missed or Delayed Diagnosis of Heart Disease by the General Pediatrician.

Missed or delayed heart disease diagnoses pose a major challenge in pediatric primary care. Many cardiac conditions present with subtle or nonspecific symptoms that resemble benign childhood illnesses, making their prompt recognition difficult. This review describes congenital and acquired heart diseases prone to diagnostic delays, including critical congenital heart disease, coarctation of the aorta, atrial and ventricular septal defects, myocarditis, Kawasaki disease, heart failure, and pulmonary arterial hypertension. The atypical presentations of these disorders and the associated diagnost

A new association between Kleefstra syndrome and Panayiotopoulos epilepsy.

BACKGROUND: Kleefstra syndrome is a rare genetic disorder attributed to loss of function of EHMT1, either due to a point mutation or a microdeletion in the chromosome region 9q34.3. This gene encodes an enzyme that modifies histone function and is essential for normal development. Individuals with Kleefstra syndrome typically present intellectual disability (from moderate to severe), language delay, autism spectrum disorders, generalized hypotonia, and distinctive facial dysmorphic features. Additional manifestations in children may include cardiac defects, renal and urological malformations,

Diagnosis and treatment options for sinus of Valsalva aneurysms: A narrative review.

Sinus of Valsalva aneurysm (SoVA) is a rare cardiac defect that may be congenital or acquired. It is characterized by abnormal dilatation of the aortic root due to a weakened elastic lamina at the junction of the annulus fibrosus and the aortic media. SoVAs are present in approximately 0.09% of the general population and comprise up to 3.5% of all congenital cardiac defects. It is usually found incidentally on cardiac imaging, with a higher incidence observed in the Western populations and a male-to-female ratio of 4:1. A transthoracic two-dimensional echocardiogram is the initial diagnostic t

Blunt Chest Trauma-Related Cardiovascular Intervention in Two Pediatric Cases. A Silent Threat.

Blunt thoracic traumas due to traffic accidents or falls from a height may conceal the degree of the underlying injury, leading to an underestimation of the actual clinical picture. Two pediatric poly-traumatized patients were admitted to the emergency room following traffic accidents. The first patient was overrun by a motor vehicle and seemed to be doing well in ICU when, all of a sudden, he developed heart failure with hemodynamic instability on day 14. Transesophageal and color flow Doppler echocardiography demonstrated an acquired 10 mm ventricular septal defect with left-to-right shuntin

Acquired von Willebrand syndrome in young children with congenital heart defects: focus on patent ductus arteriosus and ventricular septal defect.

BACKGROUND: Acquired von Willebrand syndrome (AVWS) is a rare bleeding disorder due to a deficiency of von Willebrand factor (VWF). High shear stress causes stretching and rupture of VWF multimers, leading to functional loss and increased proteolysis. This occurs in cardiovascular diseases, reducing high-molecular-weight multimers (HMWMs). Patent ductus arteriosus (PDA) and ventricular septal defect (VSD) cause blood shunting between systemic and pulmonary circulation, increasing shear stress, and may contribute to AVWS. OBJECTIVES: To investigate whether children with PDA and VSD experience
