Diagnosis of congenital and acquired Gerbode defects by cardiac MRI: two cases.

The Gerbode defect is a rare left ventricle-to-right atrium shunt, often misdiagnosed as tricuspid regurgitation, and may be either congenital or acquired. We report two cases: one acquired Gerbode defect following redo mechanical mitral valve replacement surgery, presenting with dyspnea and progressive heart failure; and one incidental congenital Gerbode defect in an asymptomatic patient. In both cases, Cardiac Magnetic Resonance (CMR) clarified shunt anatomy and informed management decisions. The first patient underwent successful percutaneous closure under Digital Subtraction Angiography (D

Post-acute Myocardial Infarction Complication: A Ventricular Septal Defect.

A ventricular septal defect (VSD) is usually a congenital cardiac condition. In the elderly, although it may be present from birth, VSD can be acquired and manifest as a complication of acute myocardial infarction (AMI), as endocarditis, or as a result of cardiac procedures. It can lead to heart failure and other severe consequences demanding medical intervention. This case study describes an 80-year-old obese woman who was diagnosed with inferolateral myocardial infarction with right ventricular (RV) involvement after being admitted with symptoms of acute coronary syndrome (ACS). VSD, a rare

Effect of Chronic Pediatric Heart Conditions on Family Dynamics in Saudi Arabia: A Cross-Sectional Study.

INTRODUCTION: Chronic pediatric heart diseases significantly affect family dynamics, therefore affecting caregivers' emotional, financial, and social spheres of life. Understanding how healthcare systems and family structures differ in Saudi Arabia helps us improve support systems since they directly affect families. This study investigated how chronic pediatric cardiac diseases affected family dynamics and what elements supported caregivers' stress and coping strategies. METHODS: Between February and March 2025, a cross-sectional survey was conducted at King Fahad Medical City (KFMC), Riyadh

Application of an interactive, stereoscopic three-dimensional viewer to evaluate atrial septal defects in five dogs.

Definitive closure of atrial septal defects (ASDs) has been reported in dogs utilizing a variety of methods, including device occlusion. We report five dogs with ASD in which multidimensional imaging with computed tomography angiography (CTA) and three-dimensional reconstruction was utilized to assess suitability for device occlusion. Three dogs were initially evaluated for an asymptomatic murmur noted during routine preventative care visits, and two had presenting complaints of mild exercise intolerance. Concurrent congenital heart disease was diagnosed in four of the dogs including pulmonary

Acquired Gerbode Defect Diagnosed Seven Years After Surgical Mitral Valve Repair for Severe Primary Mitral Valve Regurgitation: A Case Report.

Surgical mitral valve repair is the main treatment for patients with severe primary mitral regurgitation who are symptomatic; this technique is usually done in patients with myxomatous degeneration of the mitral leaflets, such as mitral valve prolapse. In these patients, surgical mitral valve repair is recommended over surgical mitral valve replacement; also, surgical mitral valve repair is recommended over percutaneous repair in patients with acceptable surgical risk. We report herein the case of a 54-year-old man who underwent mitral valve repair seven years previously for myxomatous anterio

Is the Use of Intraoperative Neuromonitoring Justified During Lumbar Anterior Approach Surgery?

BACKGROUND: Intraoperative neuromonitoring (IONM) became widely used in spine surgery to reduce the risk of iatrogenic nerve injury. However, the proliferation of IONM has fallen into question based on effectiveness and costs, with a lack of evidence supporting its benefit for specific spine surgery procedures. The purpose of this study was to evaluate the use of IONM and the rate of neurological injury associated with anterior lumbar spinal surgery. METHODS: This was a retrospective study on a consecutive series of 359 patients undergoing lumbar anterior approach surgery for anterior lumbar

Population-based retrospective cohort study on community-acquired pneumonia hospitalization in children with a ventricular septal defect.

The cohort consisted of 9400 exposed children diagnosed with ventricular septal defect (VSD). The risk of community-acquired pneumonia (CAP) or asthma with VSD was assessed using the Cox proportional hazard model with an inverse probability of treatment weighting. During a mean follow-up of 6.67 years (starting from 12 months after birth), there were 2100 CAP admission cases among exposed patients (incidence rate: 33.2 per 1000 person-years) and 20,109 CAP admission cases among unexposed children (incidence rate: 29.6 per 1000 person-years), with hazard ration of 1.09 (95% CI 1.04-1.14).

Diagnosis and surgical approach of congenital Gerbode defect: A case report.

INTRODUCTION: The Gerbode defect is an infrequent cardiac anomaly, with an incidence below 1 % in current worldwide literature. It consists of a communication between the left ventricle and right atrium in the membranous septum with consequential hemodynamical and structural heart changes and can present as either congenital or acquired. The concomitant affectation of the tricuspid valve poses its difficulty as a diagnostic and therapeutical challenge. PRESENTATION OF CASE: This case report presents a 27-year-old patient with an incidental finding of congenital Gerbode defect with hemodynamic

Intervention needs assessment in children with heart diseases presenting to Tibebe-Ghion Specialized Teaching Hospital, Bahir Dar, North West Ethiopia - Tip of the iceberg of the intervention gap in Low-and-middle-income Countries (LMICs).

BACKGROUND: Pediatric heart disease is becoming a major contributor to childhood mortality. Almost half of congenital heart defects require intervention, either surgical or trans-catheter. Rheumatic heart disease also remains a global health problem in Low-and-middle-income-countries (LMICs). Intervention timing depends on the natural course and hemodynamic significance of the lesion with an emphasis on earlier intervention to prevent the damage of volume or pressure load and hypoxia. OBJECTIVE: describe the current unmet intervention need of children with heart diseases presenting to Bahir D

Using a customized telescoped stent complex in the percutaneous treatment of a residual superior sinus venosus defect.

The management of superior sinus venosus defects (SVD) via transcatheter covered stent (CS) placement is becoming an acceptable alternative to open heart surgery. Though the medium-term success of this procedure has been described, residual shunting from damage to the covering of the implanted stents, use of stents which are too short and unanticipated shortening of stents may result in immediate or short-term procedural failure. In such cases, placement of a second CS may be required to address a residual defect. Preprocedural prediction of the length of stent required for residual leak treat

Phonocardiogram (PCG) Murmur Detection Based on the Mean Teacher Method.

Cardiovascular diseases (CVDs) are among the primary causes of mortality globally, highlighting the critical need for early detection to mitigate their impact. Phonocardiograms (PCGs), which record heart sounds, are essential for the non-invasive assessment of cardiac function, enabling the early identification of abnormalities such as murmurs. Particularly in underprivileged regions with high birth rates, the absence of early diagnosis poses a significant public health challenge. In pediatric populations, the analysis of PCG signals is invaluable for detecting abnormal sound waves indicative

Special considerations for the stabilization and resuscitation of patients with cardiac disease in the Neonatal Intensive Care Unit.

Effective resuscitation of neonates with congenital heart disease (CHD) depends on comprehensive planning, thorough understanding of physiology, vigilant monitoring, and interdisciplinary collaboration to achieve the best outcomes. Neonatal heart disease can affect cardiac structure, rhythm, or ventricular function, and may be either congenital or acquired. Critical congenital heart disease (CCHD) can result in inadequate pulmonary blood flow, impaired intracardiac mixing, airway obstruction, or insufficient cardiac output. Tailored resuscitation strategies are important as early as the delive
