Acute Myocardial Infarction Complicated by Papillary Muscle Rupture and Cardiogenic Shock Requiring ECMO Support in a Patient with Bipolar Disorder and Chronic Cannabis Use.

Cardiogenic shock secondary to acute myocardial infarction complicated by mechanical failure remains associated with high mortality despite advances in cardiac surgery and mechanical circulatory support. We report the case of a 42-year-old patient with posterior papillary muscle rupture leading to severe mitral regurgitation, managed with emergency surgical intervention and extracorporeal membrane oxygenation. The patient, with a history of Type I Bipolar Disorder under long-term lithium therapy and chronic Cannabis use, presented in critical condition with cardiogenic shock (Killip IV), acute

Can FoCUS Speed Up the Management of Acute Coronary Syndrome in the Emergency Department?

Focused Cardiac Ultrasound (FoCUS) is a targeted bedside imaging modality with an established role in the management of critically ill patients. Acute Coronary Syndrome (ACS) is a common cause of presentation to the Emergency Department (ED), and although electrocardiography (ECG) and cardiac biomarkers are the cornerstones for its diagnosis, FoCUS may facilitate diagnostic evaluation and disposition of patients in different levels of care. Initially, FoCUS plays a crucial diagnostic role through the identification of Regional Wall Motion Abnormalities (RWMAs), enabling direct visualization of

From Papillary Muscle Rupture to Left Atrial Pseudoaneurysm: A Rare Sequential Complication After Acute Myocardial Infarction and Mitral Valve Replacement.

Papillary muscle rupture is a rare but life-threatening mechanical complication of acute myocardial infarction, resulting in acute severe mitral regurgitation and cardiogenic shock. Emergency surgical intervention is essential for survival. Left atrial intramural hematoma and subsequent pseudoaneurysm formation are exceptionally rare complications of mitral valve surgery, with no established management strategy. We report the case of a 77-year-old man presenting with acute severe mitral regurgitation secondary to complete rupture of the posteromedial papillary muscle following an inferior myoc

Acute mitral regurgitation following giant coronary artery aneurysm resection: a case report.

BACKGROUND: Giant coronary artery aneurysms carry a high risk of rupture, thrombosis, and distal embolization. When percutaneous coronary intervention is not feasible in symptomatic cases, surgical resection combined with coronary artery bypass grafting is recommended. However, surgical ligation can interrupt flow to downstream branch arteries and precipitate regional myocardial ischemia. CASE PRESENTATION: In a 70-year-old woman with coronary artery disease, coronary angiography showed a 20-mm aneurysm with 90% stenosis of the proximal left anterior descending (LAD) artery, 90% stenosis of t

Fulminant recurrent myocarditis complicated by papillary muscle rupture and cardiogenic shock managed with ExtraCorporeal membrane oxygenation and impella support.

We present a case of acute-on-recurrent viral myocarditis complicated by papillary muscle rupture of the mitral valve. This rare clinical scenario, combining refractory cardiogenic shock with acute severe mitral valve regurgitation, illustrates the therapeutic challenges associated with temporary mechanical circulatory support (tMCS). Although mitral valve replacement would theoretically correct the mechanical complication, cardiac surgery in the setting of profound cardiogenic and septic shock carries a high risk of failure. In this context, the combination of veno-arterial extracorporeal mem

Massive tricuspid regurgitation caused by iatrogenic papillary muscle rupture during leadless pacemaker retrieval: a case report.

BACKGROUND: Leadless pacemakers (LPMs) reduce many complications associated with transvenous systems; however, tricuspid regurgitation (TR) caused by mechanical interference after LPM implantation has been reported. We describe a rare case of massive TR caused by papillary muscle rupture during LPM deployment and retrieval. CASE SUMMARY: An 86-year-old man presented with syncope due to complete atrioventricular block. Because of his advanced age and limited activities of daily living, a leadless pacemaker was selected. During deployment, the anterior papillary muscle became entrapped between

Aberrant, Elevated Papillary Muscle and LV Wall Forces Associated With Mitral Valve Prolapse Are Normalized by Surgical Neochordal Repair but Not by Transcatheter Edge-to-Edge Repair.

BACKGROUND: Recent studies have linked mitral valve prolapse to increased papillary muscle (PM) traction in mitral valve prolapse and MitraClip patients. We aimed to biomechanically compare edge-to-edge repair via MitraClip implantation and neochordal repair for mitral valve prolapse on PM forces ex vivo. METHODS: We developed a force transduction system to measure the forces exerted on the PMs throughout the cardiac cycle in a left heart simulator. Using a randomized, paired procedure with porcine mitral valves (n=8), we measured the hemodynamic parameters and PM forces for control, prolapse

Surgical relevance of a pedicled omental flap for the treatment of a recurrent giant mycotic left ventricular pseudoaneurysm: a case report and review of the literature.

BACKGROUND: Myocardial necrosis following acute myocardial infarction may result in three major mechanical complications: papillary muscle rupture, free-wall rupture, and ventricular septal rupture (VSR), all of which are associated with devastating hemodynamic consequences and may compromise short-term survival. Although the incidence of these complications has markedly declined with the widespread adoption of early percutaneous coronary interventions, the surgical management of VSR remains highly demanding and requires substantial expertise. Moreover, the optimal timing of surgery remains co

Prophylactic temporary mechanical circulatory support: The new paradigm in cardiac surgery.

BackgroundPost-cardiotomy cardiogenic shock (PCCS) remains one of the most lethal complications of cardiac surgery, with high in-hospital mortality despite major improvements in operative techniques, and intensive care management. Temporary mechanical circulatory support (tMCS) is widely applied as a rescue therapy for refractory PCCS. However, outcomes remain suboptimal, also due to delayed initiation after the onset of severe hemodynamic collapse and irreversible compromised end-organ states. This review summarizes current concepts of PCCS management and evaluates the emerging role of prophy

Silent Myocardial Infarction Presenting With Concurrent Left Ventricular Aneurysm, Intracavitary Thrombus, and Ventricular Septal Defect: A Case Report.

Mechanical complications of acute myocardial infarction (AMI) - including left ventricular aneurysm (LVA), ventricular septal defect (VSD), free wall rupture, and papillary muscle rupture - are rare in the current reperfusion era but carry high mortality. Silent myocardial infarction (MI), particularly in diabetic patients, often goes unrecognized until these complications manifest. The concurrent occurrence of multiple mechanical complications following a silent AMI is exceedingly rare and poses significant diagnostic and therapeutic challenges. We report a 70-year-old woman with diabetes mel

Severe Tricuspid Regurgitation: Traumatic Papillary Muscle Rupture Mimicking Infective Endocarditis-A Case Report.

BACKGROUND Differentiating traumatic tricuspid regurgitation (TR) from infective endocarditis (IE) is clinically crucial in patients presenting with abnormal tricuspid valve findings after blunt chest trauma. Particularly, papillary muscle rupture caused by trauma can produce mobile valvular structures that closely resemble vegetations, making accurate diagnosis challenging in the acute setting. CASE REPORT A man in his 60s with pulmonary sarcoidosis was admitted after a 5-m fall, sustaining multiple fractures. Preoperative transthoracic echocardiography revealed a 7-mm mobile structure on the
