[Ischemic mitral regurgitation: from the emergency room to the cardiac surgery recovery unit].

We present the case of a 69-year-old male with no cardiovascular history, who was admitted to the emergency department due to acute coronary syndrome with ST elevation and cardiogenic shock. Immediately after the percutaneous coronary intervention, he developed a greater requirement of vasopressors, the addition of inotropes, and an increase in oxygen supply by mechanical respiratory assistance. An echocardiogram was performed, which showed acute ischemic mitral regurgitation due to complete rupture of the posteromedial papillary muscle. Under ventricular assistance with a counterpulsation bal

Prediction of mechanical complications post-acute myocardial infarction in individuals with type 2 diabetes mellitus.

BACKGROUND: Acute myocardial infarction (AMI) patients with type 2 diabetes mellitus (T2DM) represent a unique population characterized by poorer prognoses, which may be further exacerbated by mechanical complications. This study aims to develop a predictive model to identify high-risk individuals within this populations. METHODS: This study enrolled AMI patients with T2DM and categorized them into complication and control groups. The mechanical complications were defined as papillary muscle rupture (with or without acute mitral regurgitation), ventricular septal defect, left ventricular pseu

Repair Versus Replacement in Mitral Valve Papillary Muscle Rupture: A Multicenter Study.

OBJECTIVES: Papillary muscle rupture (PMR) is a rare but potentially fatal mechanical complication after acute myocardial infarction (AMI). Although surgery is considered the gold-standard treatment for post-AMI PMR, the optimal surgical strategy remains unclear. METHODS: Data from post-AMI PMR patients submitted to mitral valve replacement (MVR) or mitral valve repair (MVr) surgery in the period between 2001 and 2019, from 20 international centres, were collected in the CAUTION study database. In-hospital and long-term post-discharge mortality were the endpoints. A multivariable logistic reg

Impella 5.5 for Preoperative Optimization in the Setting of Ruptured Papillary Muscle.

Cardiogenic shock in the setting of a ruptured papillary muscle can be extremely challenging to manage. We report two cases of successful and safe hemodynamic optimization with a modern axial-flow temporary left ventricular assist device, the Impella 5.5, prior to mitral valve replacement for this indication.

Papillary Muscle Rupture During Percutaneous Transvenous Mitral Commissurotomy: An Uncommon Scenario Exempt From Emergency Surgical Conversion.

Papillary muscle rupture is a rare but serious complication during percutaneous transvenous mitral commissurotomy (PTMC). In many cases, it leads to acute significant mitral regurgitation (MR), requiring urgent surgical repair. We performed PTMC for a 49-year-old woman with symptomatic moderate rheumatic mitral stenosis. Initial balloon inflation resulted in papillary muscle rupture; however, it did not induce leaflet prolapse and worsening of MR. Finally, we succeeded in achieving a mean pressure gradient of less than 5 mmHg without worsening of MR through several balloon inflations. We exper

Risk prediction of mechanical complications in acute myocardial infarction patients with prior malignancy.

BACKGROUND: Acute myocardial infarction (AMI) patients with prior malignancy have been largely understudied, despite potentially facing higher risks of adverse outcomes. This case-control study aimed to identify independent risk factors for in-hospital mechanical complications among AMI patients with prior malignancies. METHODS: This study enrolled AMI patients with prior malignancy who were hospitalized for treatment. Patients were divided into complication and control groups based on the occurrence of in-hospital mechanical complications. The mechanical complications in this study were defi

An Incomplete Anterolateral Papillary Muscle Rupture in Patient with Suspected Septic Shock.

Papillary muscle rupture (PMR) is a cardiac surgical emergency, and early treatment is associated with better short- and long-term survival outcomes. The sensitivity of the transthoracic echocardiogram for PMR is rather low (65%), and in acute, the diagnosis could be difficult given the clinical, laboratory, and radiological aspects not specific for the disease. We report a case of incomplete anterolateral PMR in a patient admitted to intensive care with bilateral pneumonia and septic shock. Transesophageal echocardiogram was fundamental for a better visualization of mitral subvalvular apparat

Simultaneous Percutaneous Ventricular Septal Closure and Mitral Valve Repair in Postinfarction Mechanical Complications: Case Report.

Ventricular septal rupture (VSR) and papillary muscle rupture are lethal complications of acute myocardial infarction. We present the case of a 76-year-old woman presenting with chest pain with electrocardiogram showing ST-segment elevation in the inferior leads. Angiography showed chronic right coronary artery occlusion. A VSR and partially torn papillary muscle was seen on echocardiogram. Percutaneous VSR closure with an Amplatzer device (Abbott) and mitral valve repair with a MitraClip (Abbott) were successfully performed simultaneously, and she was discharged to a skilled nursing facility.

Anterolateral Papillary Muscle Rupture: A Rare Complication of Myocardial Infarction.

BACKGROUND: Myocardial infarctions can result in mechanical complications such as rupture of papillary muscles. Rupture of the anterolateral papillary muscle is rarer because of its dual blood supply. CASE SUMMARY: A 44-year-old man presented with subacute chest pain and was diagnosed with a non-ST-segment elevation myocardial infarction. Coronary angiography revealed multivessel coronary artery disease, for which the patient underwent urgent coronary artery bypass grafting to revascularize. Postoperatively, he developed acute respiratory failure due to pulmonary edema in the setting of decom

Severe mitral valve papillary muscle rupture of isolated Whipple's endocarditis: a case report and review of the literature.

BACKGROUND: Tropheryma whipplei endocarditis (TWE) is rarely reported. Diagnosis is particularly challenging when it occurs as isolated TWE without classical manifestations of Whipple's disease. CASE PRESENTATION: A 35-year-old Asian female with systemic lupus erythematosus presented with acute heart failure secondary to mitral valve papillary muscle rupture as her sole symptom, requiring emergent veno-arterial extracorporeal membrane oxygenation support and urgent valve replacement. Intraoperative absence of vegetations and negative conventional microbiological examination preliminarily rule

Chronic papillary muscle rupture, a case report.

BACKGROUND: Papillary muscle rupture is a rare and potentially fatal complication of acute myocardial ischemia, which typically results in acute severe mitral regurgitation and cardiogenic shock. Due to the urgent need for medical and surgical intervention and high mortality, chronic presentation of mitral regurgitation from papillary muscle rupture is unusual and, to our knowledge, with no reported incidence. CASE PRESENTATION: A 70-year-old female presented with several months of increased fatigability and dyspnea on exertion. Examination and diagnostic echocardiographic studies confirmed s

Mobile Transcatheter Edge-to-Edge Repair Team for Acute Post-Myocardial Infarction Mitral Regurgitation.

BACKGROUND: Cardiogenic shock (CS) due to papillary muscle rupture causing acute mitral regurgitation (MR) after acute coronary syndrome carries poor prognosis and frequently necessitates urgent MR reduction. CASE SUMMARY: A 70-year-old man presented in CS due to papillary muscle rupture at a hospital without cardiac surgery or expertise in structural heart interventions. He was deemed at high risk for transfer by the heart team, and a percutaneous treatment approach was chosen. A mobile mitral transcatheter edge-to-edge repair (m-TEER) team was dispatched from a tertiary center, and within h

Risk factors for mechanical complications in very elderly patients with acute myocardial infarction.

BACKGROUND: The mortality rate for mechanical complications in very elderly acute myocardial infarction (AMI) patients is extremely high, but predictive tools specifically for this population are lacking. METHODS: Mechanical complications (free-wall rupture, ventricular septal rupture, papillary muscle rupture/dysfunction, ventricular aneurysm) were independently validated. Differences in baseline characteristics, laboratory markers, and outcomes were compared. Missing data underwent sensitivity analysis, with >20% exclusion and <15% imputation. Regression modeling entered variables with univ

Anterolateral Papillary Muscle Rupture After Acute Myocardial Infarction Leading to Severe Mitral Regurgitation: A Case Report.

Due to dual blood supply, ischemic mitral anterolateral papillary muscle rupture is less common than that of posteromedial papillary muscle and is rare in acute myocardial infarction without left main lesion. "Pseudo-normalization" phenomenon of left ventricular ejection fraction may occur in the early stage, which is inconsistent with clinical symptoms.

The use of venoarterial ECMO as a successful strategy in acute severe mitral regurgitation secondary to papillary muscle rupture due to acute myocardial infarction: A case report and narrative review.

RATIONALE: Acute mitral regurgitation (MR) secondary to papillary muscle rupture is a rare but often life-threatening mechanical complication post-acute myocardial infarction (MI). The use of peripheral venoarterial extracorporeal membrane oxygenation (VA-ECMO) as a bridge to mitral valve replacement surgery may improve outcome of such patients. PATIENT CONCERNS: We reported the case of a 70-year old woman with past history who presented to the emergency department at People's Hospital of Rizhao with "a 5-day history of chest distress." She developed refractory cardiogenic shock, severe pulmo

Edge-to-edge repair with ring annuloplasty for post-transplant biopsy-related tricuspid regurgitation.

We experienced an early biopsy-related tricuspid regurgitation post-transplant. Transthoracic and transoesophageal echocardiography showed a frail anterior leaflet with a possible ruptured chorda. The tricuspid valve was inspected, and a ruptured chorda was noted in the anterior leaflet with a single torn chorda on the septal leaflet. The corresponding papillary muscle was difficult to identify. The patient underwent successful tricuspid valve repair with edge-to-edge repair and ring annuloplasty. Compared to a complex repair procedure such as the replacement of multiple artificial chordae, th
