BACKGROUND: Myxomatous mitral valve disease is the most common cardiac disease among dogs. The mitral valve consists of valve leaflets, chordae tendineae and papillary muscles. Much attention has been given to lesions of the valve leaflets, but there is still a lack of information in the literature about the chordae. During myxomatous mitral valve disease, an episode of chordae rupture can occur. Predisposing factors for such an episode are known in human cardiology.
MATERIALS AND METHODS: The 54 mitral valves were prepared from the hearts of older dogs, from which the chordae tendineae were

BACKGROUND: Prosthetic valve dysfunction due to entrapment of the subvalvular apparatus is a rare condition, particularly when occurring as a delayed postoperative complication. Spontaneous papillary muscle rupture in the absence of an ischemic event is also uncommon. The simultaneous presence of both conditions renders this case of intermittent symptomatic mitral regurgitation exceptional.
CASE PRESENTATION: We present the case of a 72-year-old woman with a history of rheumatic heart disease and a mechanical mitral valve prosthesis. Although the initial postoperative course was uneventful, s

BACKGROUND: Tricuspid regurgitation is classified broadly by its etiology being either structural or functional. New or worsening tricuspid regurgitation upon weaning from cardiopulmonary bypass is rare and often functional. However, iatrogenic structural causes are possible and should be investigated with a high index of suspicion when issues separating from cardiopulmonary bypass arise. Currently, isolated coronary artery bypass grafting is not a class I indication for intraoperative transesophageal echocardiography, leading to significant practice variation. However, intraoperative transeso

BACKGROUND: Although radiofrequency ablation is effective for treating left ventricular papillary muscle (PM) arrhythmias, procedural complications like catheter entrapment and risk of PM injury are incompletely understood. Furthermore, cardiac catheters lack tensile force monitoring at the catheter tip, creating procedural blind spots that prevent the detection and potential mitigation of such complications. Therefore, this study aimed to evaluate the force required to rupture PMs before and after ablation, providing insights into procedural safety and potential risks associated with PM ablat

Mitral valve closure is a complex process and requires coordinated actions of all its interrelated anatomical components: the left atrium, the mitral annulus, the valve leaflets, the tendinous chordae, and the papillary muscles with its surrounding left ventricular wall for an effective mitral valve closure. Research of last three-decades has shown that the mitral annulus starts contracting during atrial systole which significantly prevent early mitral regurgitation (MR). MR can be acute or chronic, and primary, or secondary or mixed; their etiologies, mechanisms and natural progression are ve

BACKGROUND: The management of severe pectus excavatum (PEx) in patients with multiple prior sternal reconstructions presents unique challenges for safe median sternotomy for cardiac surgery and concomitant sternal reconstruction. Promising outcomes using various approaches have been reported; however, limited literature is available for urgent scenarios.
CASE PRESENTATION: We report our surgical approach in a young female patient with Marfan syndrome and PEx status after multiple sternal reconstruction procedures who presented with acute congestive heart failure due to mitral valve insufficie

Acute ischemic mitral regurgitation is a rare but potentially catastrophic complication following acute myocardial infarction (AMI), characterized by severe clinical presentation and high mortality. Meanwhile, advancements in primary percutaneous coronary intervention (PCI) have reduced the incidence of acute mitral regurgitation (AMR). The surgical approach remains the standard treatment but is associated with high rates of complications and in-hospital mortality, particularly in patients with cardiogenic shock or mechanical complications, such as papillary muscle rupture. Mitral transcathete

Mechanical complications following acute myocardial infarction (MI) represent some of the most challenging conditions in contemporary cardiology, often leading to rapid clinical deterioration and high mortality despite advances in reperfusion therapy. These complications span a spectrum of presentations, from early-phase structural disruptions such as ventricular septal rupture, papillary muscle rupture with acute mitral regurgitation (MR), and left ventricular free wall rupture (LVFWR), to later-stage manifestations, including true ventricular aneurysms and pseudoaneurysms. While surgical int

Acute mitral regurgitation typically presents with dyspnea, chest pain, and hemodynamic instability. It is an uncommon cause of hemoptysis. We present a case of a patient presenting with dyspnea and hemoptysis without hemodynamic instability along with right-sided infiltrate on chest radiography a few days after an acute inferolateral STEMI who was found to have posterior papillary muscle rupture resulting in acute mitral regurgitation. Our case illustrates that the aforementioned symptoms and signs should raise concern for acute mitral regurgitation and prompt cardiac evaluation in the approp

BACKGROUND: Mechanical complications (MC) are rare but significant sequelae of acute myocardial infarction (AMI). Current data on sex differences in AMI with MC is limited.
METHODS: We queried the National Inpatient Sample database to identify adult patients with the primary diagnosis of AMI and MC. The main outcome of interest was sex difference in-hospital mortality. Secondary outcomes were sex differences in the incidence of acute kidney injury (AKI), major bleeding, use of inotropes, permanent pacemaker implantation (PPMI), performance of percutaneous coronary intervention (PCI), coronary

AIMS: Mechanical complications (MCs) are rare but potentially fatal sequelae of acute myocardial infarction (AMI). Surgery, though challenging, is considered the treatment of choice. The authors sought to study the early and long-term results of patients undergoing surgical treatment for post-AMI MCs.
METHODS AND RESULTS: Patients who underwent surgical treatment for post-infarction MCs between 2001 through 2019 in 27 centres worldwide were retrieved from the database of the CAUTION study. In-hospital and long-term mortality were the primary outcomes. Cox proportional hazards regression model

UNLABELLED: Papillary muscle rupture is usually caused by myocardial infarction although rare cases of non-ischemic etiology have also been described. Among these, infective endocarditis represents an important cause. Herein, we report a case due to Streptococcus agalactiae involving the posteromedial papillary muscle.
LEARNING OBJECTIVE: Non-ischemic papillary muscle rupture should be suspected when there is no evidence of atherosclerotic coronary artery disease. In the febrile patient, infective endocarditis should be considered in the differential diagnosis.

BACKGROUND: Percutaneous transvenous mitral commissurotomy (PTMC) is the first-line therapy of clinically significant rheumatic mitral stenosis. While the procedure is generally safe, new onset or aggravation of mitral regurgitation (MR) may occur, mainly due to commissural splitting and, less frequently, to leaflet tear and chordal rupture. Papillary muscle rupture (PMR) is exceedingly rare in this setting.
CASE SUMMARY: A 74-year-old woman with a history of aortic valve replacement and prior rheumatic mitral commissurotomy presented for worsening exercise intolerance and exertional dyspnoea

PURPOSE OF REVIEW: Although rare, the development of mechanical complications following an acute myocardial infarction is associated with a high morbidity and mortality. Here, we review the clinical features, diagnostic strategy, and treatment options for each of the mechanical complications, with a focus on the role of echocardiography.
RECENT FINDINGS: The growth of percutaneous structural interventions worldwide has given rise to new non-surgical options for management of mechanical complications. As such, select patients may benefit from a novel use of these established treatment methods.

Penetrating cardiac trauma is a fatal condition and can result in the injury of various parts of the heart. Ventricular Septal Defect (VSD) following these traumas occurs only in 1-5% of cases. The patients' conditions depend on location, size, and concomitant injuries. One of the uncommon coincidences with the VSD is Mitral Regurgitation (MR) due to injury to sub-valvular structures. In this study, we report a case of concomitant traumatic-induced VSD and MR in a 14-year-old boy following a stab wound to his chest. The patient was a teenage boy coming to the Rajaei Cardiology Hospital emergen

The most common mechanical complications of acute myocardial infarction include free-wall rupture, ventricular septal rupture (VSR), papillary muscle rupture and pseudoaneurysm. It is rare for a patient to experience more than one mechanical complication simultaneously. Here, we present a case of ST-segment elevation myocardial infarction (STEMI) complicated with three mechanical complications, including ventricular apical wall rupture, ventricular aneurysm formation and ventricular septal dissection (VSD) with VSR. Cardiac auscultation revealed rhythmic S1 and S2 with a grade 3 holosystolic m

A 59-year-old man was admitted to our hospital with left hemiplegia. A computed tomography( CT) scan and echocardiography revealed a cerebral infarction in the right middle cerebral artery's territory, as well as a large pseudoaneurysm (4×3 cm) of the lateral left ventricular wall. The patient agreed to undergo cardiac surgery because of the high risk of rupture and recurrent cerebral infarctions. Owing to the high probability of damaging the posterior papillary muscle and coronary arteries, an extracardiac approach was used, and the pseudoaneurysm cavity was closed using double-patch repair.
