The use of veno-veno-arterial ECMO as a successful strategy in acute mitral regurgitation secondary to papillary muscle rupture causing cardiogenic shock and profound hypoxemia: a case report.

Acute mitral regurgitation (MR) secondary to papillary muscle rupture is a rare mechanical complication of acute myocardial infarction occurring in 0.05-0.26% of all cases of myocardial infarction. The only treatment is emergency mitral valve surgery with high operative mortality reaching up to 39%. The use of extracorporeal membrane oxygenator (ECMO) as a stabilization strategy and a bridge to recovery may potentially improve the outcome of such cases. Here, we report a case of acute MR presenting with cardiogenic shock and severe hypoxia that required insertion of veno-veno-arterial ECMO ini

Spontaneous Papillary Muscle Rupture: An Uncommon Presentation in the Absence of Myocardial Infarction.

BACKGROUND Papillary muscle rupture (PMR) is a rare complication of myocardial infarction (MI); experiencing PMR without MI makes it even more uncommon, thereby complicating its diagnosis. Therefore, we report a case of spontaneous PMR to raise awareness of this entity. CASE REPORT A 48-year-old man with type 2 diabetes presented to the Emergency Department (ED) after experiencing sudden shortness of breath that began the day before. He had no history of chest trauma, fever, chills, or ischemic chest pain. His vital signs showed stable blood pressure and mild tachycardia. The patient had hypox

Case Report: Non-ischemic Papillary Muscle Rupture due to MRSA Myocarditis with Concurrent Thromboembolic Myocardial Infarction Secondary to Infective Endocarditis.

Non-ischemic papillary muscle rupture (PMR) is rare. PMR caused by myocarditis in the presence of concurrent infective endocarditis (IE) and myocardial infarction (MI) has not been described. We report a 46-year-old male with recurrent MRSA bacteremia who presented in septic shock and suffered cardiac arrest. Echocardiography revealed acute mitral valve regurgitation resulting from posteromedial PMR. An intra-aortic balloon pump was implanted. Angiography revealed thrombotic occlusion of a small distal left circumflex artery. Emergent mitral valve replacement surgery was performed. MRSA myocar

[Percutaneous treatment of post-acute myocardial infarction mechanical complications: state of the art].

Left ventricular free wall rupture, ventricular pseudoaneurysm, papillary muscle rupture and ventricular septal rupture are life-threatening mechanical complications of acute myocardial infarction. Despite significant improvements over the last decades in overall mortality for patients with myocardial infarction, the outcome of subjects who develop post-infarction mechanical complications remains poor. Surgical treatment is considered the standard of care. However, percutaneous approaches (such as pericardial fibrin-glue injection for left ventricular free wall rupture, transcatheter edge-to-e

OMI/NOMI: Time for a New Classification of Acute Myocardial Infarction.

Forty percent of patients with acute coronary occlusion myocardial infarction (OMI) do not present with STEMI criteria, which delays their treatment and increases morbidity and mortality. The need to identify these patients promptly is crucial, and this sets the stage for the proposed reclassification. Many of these patients can be identified by other ECG and clinical features. Background/Objectives: We sought to evaluate cases of STEMI and NSTEMI that result in OMI. Additionally, we focused on the consequences of delayed revascularization in NSTEMI patients with acute coronary occlusion (NSTE

Acute torrential mitral regurgitation secondary to iatrogenic papillary muscle rupture.

Papillary Muscle rupture is a rare and potentially life-threatening complication that is typically observed in the days following acute myocardial infarction. Less commonly, rupture of a papillary muscle may arise as an iatrogenic complication of cardiac procedures. We present a case of cardiogenic shock in the setting of torrential mitral regurgitation secondary to iatrogenic papillary muscle rupture during a left ventricular angiogram. The patient was placed on percutaneous mechanical circulatory support with Impella CP and transferred for emergent mitral valve replacement. This case highlig

Trends in In-Hospital Mortality in Patients Admitted With Cardiovascular Diseases in the United States With Demographics and Risk Factors of All Cardiovascular In-Hospital Mortality: Analysis of the 2021 National Inpatient Sample Database.

Introduction and background Cardiovascular diseases (CVDs) encompass a range of disorders involving coronary artery diseases, valvular heart diseases, myocardial diseases, pericardial diseases, hypertensive heart diseases, heart failure (HF), and pulmonary artery diseases. Given the high prevalence of CVDs, understanding both overall and in-hospital mortality rates from these diseases is crucial. Unsurprisingly, most research, procedures, and new pharmacological interventions aim to reduce these rates. No recent studies have comprehensively detailed in-hospital mortality rates, demographics, a

Papillary Muscle Rupture in the Setting of Streptococcus lutetiensis Endocarditis.

Papillary muscle rupture is commonly linked to ischemic events, occurring typically postmyocardial infarction. However, iatrogenic and nonischemic etiologies can play a role in papillary muscle rupture, including infective endocarditis. The case presented herein is an 85-year-old female with a history of heart failure with preserved ejection fraction (HFpEF), who presented with progressive dyspnea despite furosemide therapy, and was discovered to have a flail posterior mitral valve leaflet secondary to papillary muscle rupture associated with Streptococcus lutetiensis endocarditis. Transthorac

Left ventricular papillary muscle rupture due to acute myocardial infarction after transcatheter aortic valve replacement.

INTRODUCTION: Mitral regurgitation is a potential complication of transcatheter aortic valve replacement. Here, we report a case of severe acute mitral regurgitation caused by papillary muscle rupture occurring 16 days after transcatheter aortic valve replacement. PRESENTATION OF CASE: An 82-year-old woman with severe AS was referred to our hospital. Transfemoral transcatheter aortic valve replacement was scheduled. Preoperative computed tomography revealed that the height of the left coronary artery was 8.7 cm, and a self-expandable valve was selected. The procedure was performed successfull

Late mitral leaflet tear after transcatheter edge-to-edge repair for acute ischaemic mitral regurgitation: a case report.

BACKGROUND: Acute mitral regurgitation due to papillary muscle rupture is a severe complication of acute myocardial infarction. Transcatheter edge-to-edge repair is emerging as an effective alternative to surgical treatment, with encouraging outcomes. Leaflet adverse events are rare and are associated with relapse of significant mitral regurgitation. CASE SUMMARY: A 54-year-old man arrived at our hospital with a late presentation of ST-elevation myocardial infarction. During primary percutaneous coronary intervention of the circumflex coronary artery, a partial papillary muscle rupture occurr

Severe tricuspid regurgitation after a horse kick: a case report of a rare cause of acquired valvulopathy.

BACKGROUND: Many rare complications are associated with blunt chest trauma and right ventricular contusion. Among these, post-traumatic severe tricuspid regurgitation is a relatively rare clinical entity. Furthermore, only a few cases reported in the literature are associated with trauma due to kicking by a horse. CASE SUMMARY: We present the case of a 56-year-old woman who was diagnosed with early massive tricuspid regurgitation caused by traumatic rupture of the anterior papillary muscle, which was successfully treated by surgical tricuspid repair. The patient had no symptoms suggestive of

Acute Mitral Papillary Muscle Rupture Secondary to Cardiac Amyloidosis.

Acute mitral valve regurgitation due to papillary muscle rupture secondary to cardiac amyloidosis is rare. A 75-year-old woman without prior heart disease or chest trauma presented with acute mitral regurgitation and shock. Coronary angiography demonstrated patent coronary arteries. Emergency surgery revealed a prolapse of the anterior leaflet with posteromedial papillary muscle rupture. Mitral valvuloplasty was performed using artificial chordae and an annuloplasty ring. Histopathologic examination of the ruptured papillary muscle confirmed amyloid deposition between the cardiomyocytes. This
