Myxedema-related cardiac tamponade diagnosed by focused cardiac ultrasound (FoCUS): a case report.

INTRODUCTION: Pericardial effusion can progress to cardiac tamponade, a life-threatening condition in which clinical findings are often nonspecific and a complete Beck's triad is frequently absent. Focused cardiac ultrasound (FoCUS) provides rapid bedside recognition of tamponade physiology and can expedite definitive management. This case highlights its pragmatic value for promptly identifying tamponade in a rare endocrine etiology-severe hypothyroidism (myxedema)-particularly in resource-limited settings. CASE PRESENTATION: A 62-year-old man with a 6-month history of progressive lower-limb

PGK1 Drives Cardiac Hypertrophy by Regulating the Vimentin/PI3K/Akt Pathway.

BACKGROUND: Pathological cardiac hypertrophy is a major risk factor for heart failure. PGK1 (phosphoglycerate kinase 1) plays an important role in cellular energy metabolism. However, the functions of PGK1 in cardiac hypertrophy remain largely unexplored. METHODS: The expression and activity of PGK1, as well as its metabolite 3-phosphoglycerate, were examined in cardiac hypertrophy patients and mice subjected to transverse aortic constriction or Ang II (angiotensin II). Liquid chromatography-tandem mass spectrometry and co-immunoprecipitation analyses were used to identify the interacting pro

TMC6 Is a Novel Therapeutic Target for Pathogenic Cardiac Hypertrophy.

BACKGROUND: Pathogenic cardiac hypertrophy, often driven by mechanical stress, is a leading cause of heart failure. However, effective therapeutic targets remain limited. TMC6 (transmembrane channel-like protein 6) is abundant in healthy myocardium but downregulated in hypertrophic hearts; its role in cardiac hypertrophy remains undefined. METHODS: We combined cardiac-specific Tmc6 knockout mice subjected to transverse aortic constriction surgery, neonatal rat ventricular myocytes, and CRISPR/Cas9-edited human pluripotent stem cell-derived cardiomyocytes to assess hypertrophy and signaling re

Point-of-Care Ultrasound as a Key Diagnostic Tool in the Emergency Detection of Severe Pericardial Effusion.

Point-of-care ultrasound (POCUS) has become an essential extension of the physical examination in the emergency department, enabling rapid bedside assessment and timely clinical decision-making. We report a case of a 58-year-old female presenting with progressive dyspnea initially suggestive of a pulmonary etiology. Chest radiography revealed a right pleural effusion and apparent cardiomegaly. Bedside POCUS promptly identified a large circumferential pericardial effusion with echocardiographic signs of early cardiac tamponade, leading to immediate cardiology referral, intensive care admission,

Cor Triatriatum Dexter: The Largest Comprehensive Review in the Field on 124 Worldwide Cases (1968-Now).

BACKGROUND: Cor triatriatum dexter (CTD) is a rare congenital heart defect where a membrane divides the right atrium into two chambers, resulting from the incomplete regression of the right valve of the sinus venosus. Due to its rarity, only individual case reports and a limited number of case series have been published to date. This study constitutes the most extensive comprehensive review conducted in this area. Eight factors were evaluated: age at diagnosis, sex, clinical presentation, electrocardiographic findings, imaging (ultrasound, CT, or MRI), associated cardiac anomalies, and patient

Hederagenin Attenuates Cardiac Remodeling by Targeting Phosphodiesterase 9A.

BACKGROUND AND AIM: New therapeutic strategies for heart failure are urgently needed. The protective effects of cyclic guanosine monophosphate (cGMP)-protein kinase G (PKG) pathway on heart have been widely reported. Despite phosphodiesterase 9A (PDE9A) inhibitors combating cardiac remodeling, clinically available drugs are lacking. Hederagenin (HED) is a natural bioactive compound that possesses a wide range of pharmacological activities. However, the role of HED in cardiac remodeling and its underlying mechanisms remains elusive. This study aimed to investigate the effects of HED on cardiac

UBA1 promotes cardiac hypertrophy by suppressing autophagy via targeting ATG5 for ubiquitination.

BACKGROUND: Pathological cardiac hypertrophy frequently leads to heart failure (HF). UBA1, the key E1 ubiquitin-activating enzyme, initiates ubiquitin-proteasome signaling and contributes to various diseases, yet its mechanism in cardiac hypertrophy remains unclear. METHODS: Cardiac hypertrophy model was induced by either Ang II stimulation or TAC in vitro and in vivo. Mice received rAAV9-UBA1-siRNA or rAAV9-UBA1 for UBA1 knockdown or overexpression, respectively. RESULTS: We found UBA1 upregulated in murine and human hypertrophic hearts. Cardiomyocyte-specific UBA1 knockdown protected again

Transient elevation in cellular glucose uptake exacerbates pressure overload-induced cardiac hypertrophy and dysfunction.

Clinically prior hyperglycemia may lead to long-lasting adverse cardiovascular effects, a process referred to as 'glycemic memory.' Epigenetic modifications, specifically DNA methylation changes, may play a key role in this phenomenon. This study investigated if prior high glucose delivery to cardiomyocytes, led to worsened cardiovascular effects upon pressure overload and to ascertain the gene expression and corresponding DNA methylation signatures linked to glycemic memory. Using inducible and cardiomyocyte-specific glucose transporter 4 (GLUT4) overexpressing mice. We induced glucose delive

The 129S1/SvlmJ mouse strain recapitulates severe hypertensive target organ damage under moderate angiotensin II-induced hypertension.

Hypertension remains the leading cause of cerebral, cardiac, renal, and retinal vascular damage. However, genetic determinants underlying organ-specific vulnerability are poorly understood, and commonly used mouse models, notably C57BL/6J, often fail to recapitulate severe hypertensive complications seen in humans. This study compares the widely used C57BL/6J mouse strain with the genetically distinct 129S1/SvlmJ strain under hypertensive stress, aiming to identify a model that better reproduces hypertensive target organ damage. Moderate hypertension was induced in 129S1/SvlmJ and C57BL/6J mic

AJUBA Attenuates Pathological Cardiac Hypertrophy by Enhancing NEDD4-Mediated Ubiquitination and Degradation of DVL2.

Cardiac hypertrophy, a response to pathological stimuli, eventually progresses to heart failure (HF). AJUBA is a multifunctional protein expressed in various human tissues, serving as a critical modulator of cellular processes and signalling pathways involved in cardiac hypertrophy and remodelling. It plays significant roles in cellular signalling, gene transcription, and cytoskeletal organisation, modulating cardiac growth, inflammation, and cell survival. However, the precise function of AJUBA in cardiac hypertrophy remains unclear. In the present study, we examined its function as a novel r

Association of artificial intelligence derived cardiothoracic ratio assessment on non-cardiac chest CT with heart failure and all-cause mortality: A retrospective single center study.

BACKGROUND: The cardiothoracic ratio (CTR) is estimated by dividing cardiac width by thoracic width. The Area Deprivation Index (ADI) is a metric to quantify socioeconomic conditions. This study investigates the use of artificial intelligence (AI) to identify elevated CTR as a predictor for heart failure (HF) and mortality and assessed ADI's influence on these risks. METHODS: This retrospective cohort study included 9,693 consecutive patients with non-cardiac chest CTs. An AI algorithm automated measurement of cardiac and thoracic diameters to calculate the cardiothoracic ratio (CTR). Patient

Infantile hypertrophic cardiomyopathy masquerading as cardiac tamponade: A case report.

Hypertrophic cardiomyopathy in infancy often follows a rapidly progressive clinical course, with many cases becoming fulminant and fatal. We reported a case of a 4-month-old infant with worsening breathlessness. While admitted, she was in severe respiratory distress and appeared drowsy. We also noted indiscernible heart sounds from physical examination and cardiomegaly with a globular cardiac silhouette from chest X-ray. Considering her distressing clinical presentation, we suspected cardiac tamponade and performed an emergent transthoracic echocardiography to confirm our suspicion. Echocardio

Exercise blood pressure response does not differentiate endurance athletes with paroxysmal atrial fibrillation.

AIMS: Atrial fibrillation (AF) is more prevalent among endurance athletes than in the general population. An exaggerated blood pressure (BP) response to exercise may contribute to atrial pressure excess and AF risk. We hypothesized that higher exercise BP would be associated with left atrial enlargement and AF prevalence in endurance athletes. METHODS AND RESULTS: One hundred seventy-eight endurance-trained athletes, 89 with paroxysmal AF [median age 55 (IQR: 46-62 years), 92% male] and 89 age- and sex-matched athletes without AF, underwent resting echocardiography and cardiopulmonary exercis

Sirtuin 2 inhibits global protein synthesis via Rheb-GTPase degradation.

Increased global protein synthesis is associated with the development and progression of several aging-related diseases and disorders. Strategies like calorie restriction and pharmacological inhibition of protein synthesis have exhibited health-promoting effects. However, the complex molecular events that regulate global protein synthesis are not completely understood. Here, we report that SIRT2, a histone deacetylase, negatively regulates global protein synthesis by inhibiting the mTORC1 pathway via deacetylating Rheb and promoting its degradation. Our in vitro results suggest that SIRT2 defi
