A Case Report of I-Cell Disease (Mucolipidosis Type II) With a Novel Placental-Focused Diagnostic Approach.

Inclusion cell disease (I-cell disease/Mucolipiodsis Type II) is an autosomal recessive lysosomal storage disorder that results in accumulation of substrates (cholesterol, phospholipids, and glycosaminoglycans) in various tissues. The clinical presentation of I-cell disease includes hepatomegaly, splenomegaly, cardiomegaly, upper respiratory infections, skeletal deformities, developmental delay, abnormal facies, and failure to thrive. We describe a case of I-cell disease in a neonate admitted to the Neonatal Intensive Care Unit for respiratory distress with initial suspicion of infection. Plac

Loss of p300/CBP-associated factor aggravates cardiac remodeling via regulation of CAMKK2 acetylation.

Here we aim to elucidate the role of the p300/CBP-associated factor (PCAF) in pathological cardiac remodeling. Specifically, we explore how PCAF-mediated acetylation of calcium/calmodulin-dependent protein kinase kinase 2 (CAMKK2) influences AMPK signaling, thereby regulating cardiac hypertrophy and dysfunction under pathological stress. A genetically engineered PCAF-knockout (KO) mouse model was generated using the CRISPR-Cas9 system to evaluate the effect of PCAF deficiency on cardiac remodeling induced by isoproterenol infusion and transverse aortic constriction (TAC). PCAF deficiency signi

Cardiomyocytes FGFBP1 induction by STAT3 drives pathological cardiac remodeling.

Cardiac remodeling underlies the progression to heart failure, yet the molecular drivers remain incompletely defined. Here, we identify fibroblast growth factor binding protein 1 (FGFBP1) as a previously unrecognized regulator of pathological cardiac remodeling. FGFBP1 expression was markedly increased in hypertrophic human myocardium and in murine hearts exposed to angiotensin II (Ang II) or transverse aortic constriction (TAC). Using FGFBP1 knockout mice, we found that loss of FGFBP1 significantly attenuated Ang II- and TAC-induced cardiac dysfunction, hypertrophy, and interstitial fibrosis.

A comprehensive bedside chest radiography dataset with structured, itemized and graded radiologic reports.

Automated analysis of bedside chest radiographs remains challenging due to limited large-scale datasets with expert annotations and standardized severity grading. We provide TAIX-Ray, a comprehensive dataset of 215,381 bedside chest radiographs collected from 47,724 intensive care unit patients (30,306 male, 17,418 female, median age 68 years) collected over 14 years (01/2010-12/2023) at the University Hospital Aachen, Germany. During routine clinical reporting, 134 trained radiologists provided structured, itemized reports using a standardized template. They systematically assessed eight path

Predictive factors for new-onset atrial fibrillation in patients with embolic stroke of undetermined source and left atrial enlargement - development of a risk score.

BACKGROUND: In patients with embolic stroke of undetermined source (ESUS), detecting new-onset atrial fibrillation (AF) is crucial because it leads to treatment change and reduces stroke recurrence and mortality. However, the high cost and relatively low AF detection rate of implantable cardiac monitors (ICM) necessitate the identification of predictive factors to better select candidates. This study aimed to identify AF predictors in ESUS patients with left atrial enlargement (LAE) to improve ICM selection. METHODS: We conducted a retrospective observational study with consecutive patients w

Routine chest X-ray in atraumatic chest pain with normal vital signs: A propensity-matched TriNetX analysis.

BACKGROUND: Patients presenting to the emergency department (ED) with chest pain frequently undergo a range of diagnostic evaluations aimed at rapidly identifying life-threatening conditions while stratifying overall risk. However, the utility of chest x-rays (CXR) in patients with chest pain requires further investigation, as the diagnostic yield is uncertain. This study examines the prevalence of clinically relevant diagnoses and procedures identified on CXR in a cohort of patients with chest pain who had CXR, versus those with chest pain without CXR. METHODS: A retrospective analysis was c

Percutaneous Correction of Aortic Coarctation and Patent Ductus Arteriosus in a Young, Failing Heart.

BACKGROUND: Congenital anomalies of the great vessels can coexist in a significant proportion of pediatric patients, but a delayed diagnosis can present with fulminant heart failure later in life. CASE SUMMARY: A 12-year-old boy with failure to thrive and recurrent chest infections was referred to the cardiac outpatient department for cardiomegaly noted on chest x-ray. Echocardiography revealed a patent ductus arteriosus (PDA) and coarctation of the aorta (CoA) with impaired left ventricular function. Both lesions were successfully corrected through percutaneous intervention. DISCUSSION: Thi

Microtubule dynamics control the direction of cardiomyocyte growth.

The adult heart grows by the addition of sarcomeres along the length or width of individual cardiomyocytes, yet how directional growth is spatially coordinated remains unclear. We found that microtubule dynamics could act as a toggle to direct cardiomyocyte growth. Increasing microtubule stability drove cellular widening, concomitant with redirecting messenger RNA (mRNA) export and translation along the width of the cell and reinforcement of the intercalated disc. Conversely, decreasing microtubule stability promoted cellular lengthening, disrupting the intercalated disc and biasing translatio

AI-Based Denoising for Simulated Dose Reduction in Pediatric Chest Radiography: A Prospective Multicenter Evaluation of Image Quality and Diagnostic Performance.

RATIONALE AND OBJECTIVES: Radiation dose reduction in pediatric chest radiography is a clinical priority due to increased radiosensitivity and cumulative lifetime risk. However, substantial dose reduction may degrade image quality and compromise diagnostic performance. Artificial intelligence (AI)-based denoising has emerged as a potential solution, although robust prospective multicenter validation remains limited. To prospectively evaluate whether a vendor-neutral deep learning-based denoising system improves image quality while assessing the magnitude of change in diagnostic performance in

Paradoxical Sinus Bradycardia in the Setting of Acute Cardiac Tamponade: A Case Report.

Cardiac tamponade is a life-threatening condition classically associated with compensatory sinus tachycardia due to impaired diastolic filling and reduced cardiac output; however, paradoxical sinus bradycardia is rare and may delay recognition. We report a case of a 43-year-old woman with obesity status post gastric sleeve, asthma, and hyperlipidemia who presented with two weeks of progressive dyspnea, pleuritic chest pain, and chest tightness following a suspected viral syndrome, with symptoms worsening supine and improving when leaning forward. Initial evaluation demonstrated muffled heart s

Evaluation of Urinary Tubular Biomarkers in Dogs with Myxomatous Mitral Valve Disease Across ACVIM Stages.

Myxomatous mitral valve disease (MMVD) is the most common acquired heart valve disease in dogs and it may contribute to cardiovascular-renal axis disorders (CvRD) in dogs. Sensitive and early biomarkers of renal involvement are needed. In this prospective and observational study, 84 dogs were enrolled (20 healthy dogs and 64 dogs with MMVD), categorised using the American College of Veterinary Internal Medicine guidelines. Serum and urinary parameters were analysed, including tubular biomarkers expressed as creatinine-ratios: urinary alkaline phosphatase (uALPc), gamma-glutamyl transferase (uG

Cytochrome P450-derived arachidonic acid metabolites as emerging therapeutic targets in menopause-associated cardiac hypertrophy.

Menopause is associated with a marked increase in cardiac hypertrophy and progression to heart failure with preserved ejection fraction (HFpEF), a condition that disproportionately affects women and lacks effective targeted therapies. Although estrogen deficiency has long been implicated in postmenopausal cardiac remodeling, emerging evidence highlights a critical role for cytochrome P450 (CYP)-derived arachidonic acid metabolites as central regulators of myocardial structure and function. In a healthy premenopausal heart, a balance between cardioprotective epoxyeicosatrienoic acids (EETs), pr

ALPK1 promotes cardiomyocyte hypertrophy by activating NF-κB/NLRP3 inflammasome-mediated pyroptosis.

BACKGROUND: Cardiac hypertrophy is a leading risk factor for cardiovascular morbidity and mortality. Alpha-protein kinase 1 (ALPK1) is a novel essential player in innate immunity and inflammation. Therapeutic agents targeting ALPK1 have successively entered clinical trials and become research hotspots. However, the role of ALPK1 on cardiac hypertrophy remains unknown. METHODS: In the present study, transverse aortic constriction (TAC) was used to establish in vivo models of cardiac hypertrophy. Neonatal mouse cardiomyocytes (NMCMs) and AC16 human cardiomyocytes treated with angiotensin Ⅱ (Ang

Extracellular vesicles from hypertrophic cardiomyocytes promote breast cancer progression.

Several cardiovascular diseases are associated with tumor development. But the mechanisms underlying how the hypertrophic heart promotes tumor progression remain unknown. Here, we show that extracellular vesicles from hypertrophic cardiomyocytes facilitate cancer progression. Following transverse aortic constriction to induce cardiac hypertrophy, 4T1-Luc cells are orthotopically implanted into mice. Concurrently, AC16 cells are treated with angiotensin II, and extracellular vesicles isolated from these cells are administered to BALB/c nude mice previously implanted with MDA-MB-231 cells in the
