Association of lipoprotein(a) with ischemia-reperfusion injury in patients with ST-elevation myocardial infarction.

BACKGROUND: Hypercholesterolemia adversely affects ischemia-reperfusion injury in acute ST-elevation myocardial infarction (STEMI). Lipoprotein(a) (Lp(a)) is an independent cardiovascular risk factor beyond traditional lipid markers; however, its impact on ischemic myocardial injury remains unclear. We therefore investigated the association of Lp(a) with myocardial and microvascular damage assessed by multiparametric cardiovascular magnetic resonance (CMR), as well as clinical outcomes in STEMI patients. METHODS AND RESULTS: In this prospective MARINA-STEMI (NCT04113356) cohort study, 518 STE

MR-409 promotes neurogenesis and improves neurological function after cerebral ischemia through attenuation of microglial activation.

Neurological recovery during the chronic phase of ischemic stroke (IS) remains a major clinical challenge. Our previous studies showed that the growth hormone-releasing hormone analogue (GHRH-A) MR-409 promotes neurogenesis and improves neurological outcomes. However, the underlying mechanisms remain unclear. Here, a transient middle cerebral artery occlusion (tMCAO) mouse model was used to investigate whether early modulation of microglial activation mediates these effects. MR-409 (2.5, 5, or 10 μg, s.c.) was administered immediately after tMCAO and daily for 3, 7, 14, or 28 days depending on

The Citrus Flavonoid Naringin Confers Neuroprotection Against Cerebral Ischemia by Enhancing Endogenous Antioxidant Capacity and Suppressing Neuroinflammation.

Naringin is a naturally occurring flavanone glycoside found in citrus fruits that has been widely studied for its antioxidant and anti-inflammatory activities. The precise mechanisms by which naringin exerts protective effects against ischemic stroke are not yet fully understood. To address this gap, the current study examined the efficacy of naringin administration (100 mg/kg BW) in an experimental rat model involving permanent right middle cerebral artery occlusion (Rt.MCAO). Administration of naringin significantly improved neurological performance while reducing cerebral infarct volume and

Electroacupuncture Ameliorating Cerebral I/R-Induced Inflammatory Response through the TLR/TRAF6/NF-κB Pathway.

BACKGROUND: The inflammatory response plays a crucial role in cerebral ischemia/reperfusion (I/R) injury and may offer a new therapeutic approach for stroke. Electroacupuncture has shown potential in treating cerebral ischemia/reperfusion injury. METHODOLOGY: Male Sprague-Dawley rats were randomly assigned to four groups: the sham group, middle cerebral artery occlusion/reperfusion (MCAO/R) group, sham electroacupuncture (SEA) group, and electroacupuncture (EA) group. The SEA and EA groups received 30 min of treatment after 60 min of ischemia. Body weight was measured before and 24 h after th

Gastrodin inhibits neuronal ferroptosis by regulating mir-455-5p/ACSL4 pathway after stroke via exosome.

Recent studies have demonstrated that ferroptosis plays a crucial role in neuronal cell death following cerebral infarction. Our previous research showed that gastrodin (GAS) significantly improved neurological function in rats subjected to middle cerebral artery occlusion (MCAO); however, the underlying mechanisms remain unclear. In the present study, we employed Western blotting, quantitative PCR (qPCR), and immunofluorescence assays to investigate the changes in the expression levels of ferroptosis-related regulatory proteins (ACSL4, TFR1, GPX4, and MBL-2) in both in vivo and in vitro model

Middle Cerebral Artery Occlusion-induced Deficits in Fine Skilled Motor Movement and Its Recovery Following Combined Treatment with Sodium Valproate and CIMT.

BACKGROUND: Preclinical studies have shown that sodium valproic acid (VPA) has neuroprotective properties following ischaemic injury. However, its compatibility as an adjuvant therapy with physical rehabilitation in ischaemic stroke recovery, with underlying neurophysiological mechanisms, has not been explored yet. PURPOSE: The present study has investigated the advantage of combination therapy over monotherapy in ameliorating ischaemic stroke-induced deficits in skilled fine motor movements with associated changes in local and interhemispheric neural connectivity. METHODS: Cerebral ischaemi

Intranasal Delivery of an Antioxidant Nanoassembly for Cerebral Ischemia-Reperfusion Injury via Drd2/β-arrestin2/ERK Signaling.

Cerebral ischemia-reperfusion injury (CIRI) is caused by reduced cerebral blood flow, leading to excessive oxidative stress and neurological deficits. Curcumin (Cur) is a promising therapeutic agent, but its antioxidant activity is limited by low bioavailability and poor brain penetration. Herein, an intranasal delivery strategy using a tetrahedral framework nucleic acid (tFNAs)-Cur nanoassembly (tFNAs-Cur) was employed for CIRI treatment. tFNAs-Cur was synthesized through self-assembly, achieving a high Cur loading efficiency of 87% and exhibiting potent scavenging activity against reactive o

Quantitative Chemical Exchange Saturation Transfer Imaging With Golden-Angle Radial k-Space Sampling and Locally Low-Rank Reconstruction.

Chemical exchange saturation transfer magnetic resonance fingerprinting (CEST-MRF) is a promising quantitative molecular imaging technique. To reduce scan time, current CEST-MRF implementations typically use echo-planar imaging (EPI) readouts, which are prone to geometric distortion and signal dropout. In this study, we developed a motion-robust, geometrically accurate CEST-MRF method using radial k-space sampling, locally low-rank reconstruction, and neural network-based quantification. The acquisition schedule was optimized using deep learning, and its accuracy was validated in numerical sim

Laminarin attenuates cerebral ischemia-reperfusion injury by inhibiting Notch1 signaling and reprogramming microglial inflammatory responses.

BACKGROUND: Neuroinflammation contributes to cerebral ischemia-reperfusion injury, and microglial inflammatory responses are regulated in part by Notch signaling. This study investigated whether laminarin, a brown algae-derived β-glucan polysaccharide, protects against cerebral ischemia-reperfusion injury by modulating Notch1-dependent microglial responses. METHODS: A rat middle cerebral artery occlusion/reperfusion model was established, and laminarin was administered intraperitoneally at 10 mg/kg for seven consecutive days. Neurological deficits and infarct volume were assessed by modified

Essential oil from Angelica sinensis Radix attenuates ischemic stroke via the cGMP/PKG/MAPK pathway: insights from network pharmacology and molecular mechanism evaluation.

ETHNOPHARMACOLOGICAL RELEVANCE: As a traditional herbal remedy, the root of Angelica sinensis has long been used to treat headaches, strokes, and dementia. As the primary active ingredient in A. sinensis, A. sinensis essential oil (ASEO) also plays a significant role in the therapy of stroke. Although ASEO has shown clear therapeutic effects, its specific mechanism of preventing ischemic stroke still needs to be further clarified. AIM OF THE STUDY: Through network pharmacological analysis combined with in vitro and in vivo experimental verification, this study investigated the protective effe

Zinc activates autophagy by regulating HDAC6 expression following cerebral ischemia/reperfusion injury.

Intracellular zinc accumulation and excessive autophagy have been reported to contribute to neuronal death following cerebral ischemia/reperfusion (I/R). Histone deacetylase 6 (HDAC6), which contains a zinc-dependent active site, is closely associated with autophagy. However, the relationship between zinc, HDAC6 and autophagy following cerebral I/R remains unclear. In this study, we aim to investigate whether ischemia-induced zinc accumulation activates autophagy through HDAC6. Rats were treated with zinc chelator, autophagy activator/inhibitor or HDAC6 inhibitor during a 90-minute middle cere

When not to explore: a combat-specific decision framework for selective neck exploration at Role 2.

Current civilian algorithms for penetrating neck trauma assume access to definitive vascular reconstruction, endovascular intervention, advanced imaging, and specialist multidisciplinary care. These assumptions may not apply to Role 2 military facilities, where neck exploration can commit the surgeon to procedures that cannot be completed under optimal conditions. This position paper was developed through a focused narrative review of contemporary recommendations and literature on penetrating neck trauma, vascular and aerodigestive injury, temporary hemorrhage control, and combat casualty care

Abdominal ultrasound mitigates neurodegenerative pathology and alters specific gut microbial taxa in senescence-accelerated mice.

Age-related changes in gut microbiota composition have been implicated in brain aging and cognitive decline, acting through the gut-brain axis on amyloid accumulation, tau pathology, synapse damage, and glial activation. Live microbial approaches such as probiotics, prebiotics, and fecal microbiota transplantation address this mechanism, but they still suffer from low efficacy and unknown safety issues. We tested whether abdominal ultrasound, an approach that does not involve microbes, can alleviate age-related neurodegeneration in the senescence-accelerated mouse strain SAMP8. We treated 6-mo

Atrial fibrillation in stroke patients: facts, numbers and current challenges.

Atrial fibrillation (AF) is a leading, modifiable risk factor for ischaemic stroke, with 20-30% of stroke patients harbouring underlying AF. Management of AF, however, differs substantially depending on clinical context. In ischaemic stroke patients with pre-known AF, direct oral anticoagulants (DOACs) are the cornerstone of secondary prevention, with initiation timing guided by infarct severity following a risk-adapted approach. In patients with no history of AF, prolonged cardiac rhythm monitoring after stroke or transient ischaemic attack improves AF detection rates. However, it remains unc
