BACKGROUND AND OBJECTIVES: The training of cerebrovascular neurosurgeons faces significant challenges, particularly due to the decreasing volume of aneurysm clipping procedures. Traditional training methods rely heavily on clinical case availability, which limits skill development. This study aimed to implement and validate a Microsurgical Aneurysm Training Simulator (MATS) that offers a comprehensive, realistic, and cost-effective solution for neurosurgical training.
METHODS: MATS was designed using semiautomated algorithms and additive manufacturing to replicate a bifurcation aneurysm of th

Dural arteriovenous fistulas (dAVFs) are a type of vascular malformation that form within the dura mater. Though historically considered rare, their detection is expected to increase with the broader use of advanced cerebral angiography techniques. We describe two 80-year-old men presented for neurovascular procedures. The first, following an emergency craniotomy for subdural hematoma (SDH) evacuation, underwent middle meningeal artery embolization (MMAE). During the procedure, an incidental, asymptomatic middle meningeal artery and superficial temporal artery to superior sagittal sinus (MMA/S

In their recently published INR study, Qureshi et al. present their results on intra-arterial lidocaine administration in the middle meningeal artery for the short-term treatment of subarachnoid hemorrhage (SAH)-related headaches. The authors demonstrate that their proposed intra-arterial treatment consistently alleviates headaches in patients with SAH. The purpose of this commentary is to commend the authors on their paper and the notable results they have achieved. It is always pleasant to encounter studies that not only make it to the "Latest Online" section of neurointerventional journals

BACKGROUND: Aneurysmal subarachnoid hemorrhage caused by cerebral aneurysm rupture has a poor prognosis, with mortality exceeding 30% despite treatment advancements. Surgical neck clipping remains the standard for preventing rupture, but intraoperative rupture rates vary significantly (3-50%) and are influenced by vascular complexity and technical challenges. Thinning of the vascular wall near the aneurysm neck, particularly with microaneurysm formation, has emerged as a significant risk factor, yet these changes often go undetected in preoperative imaging.
OBJECTIVE: This study aimed to eval

The Woven EndoBridge (WEB) device is an intrasaccular flow disruption device designed for the treatment of wide-necked aneurysms.1, 2 Once deployed into the aneurysm it initiates intra-aneurysmal stasis, thrombosis and occlusion, and flow diversion at the interface of the neck of the aneurysm with the parent vessel.3 The device offers the advantage of reduced operating time and reduced radiation exposure to the patient and the operator. We present a case of a patient in their 70s with a subarachnoid haemorrhage secondary to a ruptured aneurysm at the bifurcation of the right middle cerebral ar

BACKGROUND: Patients with aneurysmal subarachnoid hemorrhage (aSAH) are often hospitalized for 21 days after aneurysm rupture due to the risk of complications. However, some never experience complications and are unlikely to benefit from a prolonged hospitalization. The aim of this study is to derive a risk assessment model (RAM) using data from the first 14 days of hospitalization to identify low-risk patients for early discharge, at day 15 or after.
METHODS: Patients ≥ 18 years old with an acute aSAH at a Comprehensive Stroke Center from 2017 to 2024 were included. Baseline demographics, aS

Pericallosal artery aneurysms (PCAA) are relatively rare intracranial aneurysms that present unique challenges in diagnosis and management. This study provides a comprehensive review of the literature to assess demographic patterns, risk factors, treatment approaches, and complications associated with PCAA. Data from 23 studies were analyzed using Python with libraries such as Pandas and Matplotlib. Descriptive statistics and crosstabulations explored the relationships between treatment modalities (microsurgical, endovascular, combined) and complications, including hydrocephalus, vasospasms, a

The authors describe the case of multiple rare distal anterior cerebral artery (DACA) aneurysms related to a middle cerebral artery (MCA) aneurysm, an azygos, and an undescribed anterior cerebral artery (ACA) anatomical variation. A 61-year-old woman in a severe clinical state was diagnosed with subarachnoid hemorrhage (SAH) secondary to a ruptured anterior A3 DACA aneurysm. The patient also had unruptured kissing superior A3 DACA aneurysms, an atherosclerotic DACA aneurysm between the anterior and superior DACA aneurysms, and an unruptured MCA left aneurysm. Two pericallosal anatomical variat

BACKGROUND: The Tubridge flow diverter (TFD) has become a widely used device for reconstructing parent vessels and sealing complicated aneurysms in China. However, its application to managing distal small to medium-sized dissecting aneurysms has not been extensively investigated. This study aimed to evaluate the safety and effectiveness of the TFD and the factors affecting healing in patients with dissecting aneurysms in the middle cerebral artery (MCA).
METHODS: Patients with dissecting aneurysms in the MCA who were treated with the TFD from 2019 to 2023 were included. According to the O'Kel

BACKGROUND: A giant intracranial aneurysm (GIA) measures ≥ 25 mm and is a rare, challenging condition to treat with a poor prognosis. Selecting the appropriate treatment approach, whether surgical, endovascular, or a combination of both, is crucial for achieving favorable outcomes. However, the unique characteristics of each GIA imply that no single treatment option can be considered optimal universally. This study aimed to evaluate the treatment strategies and clinical outcomes at our center.
METHODS: This retrospective, single-center study conducted between 2012 and 2018, included 20 patien

Surgery-related cerebral infarction (SRCI) is an important complication after aneurysmal subarachnoid hemorrhage (aSAH), and detailed assessment of the characteristics and risk of SRCI is insufficient. We aimed to analyze the risk factors for SRCI in different treatment modalities after aSAH. A total of 472 patients were included in this retrospective study, including 169 in clip and 303 in endovascular. Regarding radiologically observable SRCI as the outcome, we used univariate and multivariate statistical analyses of risk factors and summarized the characteristics of SRCI. The incidence of S

A 51-year-old woman presented with acute onset of a severe headache, and was found to have diffuse subarachnoid hemorrhage with prominent cisternal and left cortical convexity blood on head computed tomography. The first 2 conventional angiograms were negative for aneurysm, but a third angiogram revealed a mycotic aneurysm of a distal left middle cerebral artery branch. Brain biopsy, associated with clipping of the aneurysm, demonstrated pathology consistent with vasculitis. Over the course of a month, she developed diffuse, serpiginous dolichoectasia of the cerebral arteries. Further investig

Partially thrombosed and dissecting giant peripheral aneurysms present significant challenges for treatment with both direct surgical and endovascular approaches. We report a case of partially thrombosed, dissecting giant aneurysm in the peripheral segment of the middle cerebral artery treated with straightforward endovascular parent artery occlusion without saccular packing. A 30-year-old male presented with sudden loss of consciousness and subarachnoid hemorrhage and was transferred to our hospital. On admission, his level of consciousness was assessed as Japan Coma Scale 100 and Glasgow Com

PURPOSE: Vasospasm is a potentially devastating condition that can develop in pediatric patients secondary to traumatic, inflammatory, infectious, and neoplastic etiologies and carries a high risk of morbidity and mortality (Isola et al. 2022, Krishna et al. 2005). Management is often adapted from adult to pediatric care given its relative rarity in children. There is level 3 evidence for the use of intrathecal (IT) calcium channel blocker (nicardipine) administration to treat refractory vasospasm in the adult subarachnoid hemorrhage (SAH) population. Its use has yet to be investigated in chil
