Isolated spinal aneurysms with spontaneous regression.

PURPOSE: To share our clinical experience with conservative management of isolated spinal arterial aneurysms (ISAs) and to identify clinical scenarios where conservative management may be appropriate, in the context of a literature review. METHODS: We performed a retrospective review of spinal angiograms from two German neuroradiology centers and conducted a systematic literature review of reported ISA cases. We analyzed demographics, clinical presentation, imaging findings, treatments, and outcomes. RESULTS: We identified seven patients (mean age 48; 4 women) with nine ISAs, eight of which

Cerebral vasospasm following clipping of unruptured middle cerebral artery aneurysms: a case report and literature review.

We report a 64-year-old woman who developed symptomatic vasospasm on postoperative day 7 after clipping of an unruptured right middle cerebral artery (MCA) aneurysm. Imaging revealed right MCA vasospasm, which resolved with oral antiplatelets and intravenous vasodilators. She was discharged without neurological deficits. Although a few reports describe vasospasm after clipping unruptured aneurysms, this is the first to detail the clinical course, radiological findings, and treatment response. Potential risk factors include female sex, age 40-60, smoking, trigeminal neuralgia, and complex aneur

Differential Effects of Murine Stroke Models on Dopaminergic Neurons, Glial Responses, and Neurobehavioral Outcomes.

Stroke is a leading cause of disability worldwide, often resulting in persistent motor, cognitive, and emotional impairments. While the hippocampus and amygdala play critical roles in post-stroke behavioral changes, specific neuronal alterations and prolonged glial responses within these regions across different stroke types remain unclear. This study investigates the behavioral, neuronal, and glial effects of subarachnoid hemorrhage (SAH), transient middle cerebral artery occlusion (tMCAO), and photothrombotic stimulation (PTS) in mice. SAH and tMCAO models exhibited significant motor deficit

Salvage coiling after the inadvertent proximal migration of a neuroform atlas stent into middle cerebral artery aneurysms: A report of two cases.

INTRODUCTION AND IMPORTANCE: During endovascular treatment (EVT) of middle cerebral artery (MCA) aneurysms using a Neuroform Atlas stent (NAS), inadvertent proximal migration of the NAS into the aneurysm may occur. CASE PRESENTATIONS: Patient 1 was a 49-year-old female with an unruptured MCA bifurcation aneurysm. During deployment of a 4 × 24-mm NAS from the upper trunk to the main trunk of the MCA, the proximal NAS inadvertently migrated into the aneurysm. Afterward, salvage coiling was continued. The aneurysm was coiled satisfactorily. After EVT, the patient recovered well. Patient 2 was a

Successful treatment of a recurrent dissecting aneurysm with flow diverter placement following occipital artery-posterior inferior cerebellar artery bypass.

Branch involvement can serve as a predicting factor of incomplete occlusion after flow diverter (FD) treatment, including pipeline embolization device. This study reports a case of subarachnoid hemorrhage owing to bilateral vertebral artery dissection with parent artery occlusion to the left side and stent-assisted coiling to the right side. However, a marked enlargement of the dissected lesion was observed at the right dissecting aneurysm at 6 months. Considering that the posterior inferior cerebellar artery (PICA) originating from the aneurysm could prevent aneurysm occlusion after the FD, w

Has a fast treatment transition from surgical to endovascular operations improved the survival of aneurysmal subarachnoid hemorrhage?

BACKGROUND: Several studies have attributed decreasing case fatality rates (CFRs) of aneurysmal subarachnoid hemorrhage (aSAH) to the gradually increasing use of endovascular treatment without considering improvements in other outcome-affecting factors. To assess the independent effect of a treatment modality on CFRs, we investigated CFR changes in a high-volume center rapidly transitioning from surgical to endovascular operations as the first-line treatment for all aSAH patients except those with middle cerebral artery (MCA) aneurysms. METHODS: We identified all surgically/endovascularly tre

Decompressive craniectomy in aneurysmal subarachnoid hemorrhage: can favorable outcome be achieved?

BACKGROUND: Decompressive craniectomy (DC) is a last-tier treatment for managing refractory intracranial hypertension in patients with aneurysmal subarachnoid hemorrhage (aSAH), though concerns persist about whether it primarily prolongs survival in a state of severe disability. This study investigated patient characteristics, surgical indications, complications, and outcomes following DC in aSAH. METHODS: In this Swedish, retrospective multi-center study, 123 aSAH patients treated with DC between 2008-2022 were included. Data collection included demographic details, aSAH characteristics, inj

Invisible Until It Burst: Unexpected Subarachnoid Hemorrhage From a Rapid-Onset Infectious Aneurysm in a Patient With Endocarditis.

Infective endocarditis (IE) can lead to serious neurological complications, including septic embolism and infectious intracranial aneurysms (IIAs). Although IIAs are rare, their rupture often results in catastrophic outcomes. Predicting their formation, especially within a short period, remains a clinical challenge. We present the case of a man in his 70s who was newly diagnosed with colon cancer. During preoperative evaluation, transthoracic echocardiography revealed vegetations on the aortic and mitral valves, leading to a diagnosis of IE caused by Streptococcus sanguinis. On the third day o

Continuous Intra-arterial Infusion of Verapamil for Severe Vasospasm Treatment After Subarachnoid Hemorrhage: A Case Report.

Delayed cerebral ischemia can occur after subarachnoid hemorrhage (SAH) and is associated with the development of symptomatic vasospasm. Despite medical treatments for vasospasm, it can be refractory. Endovascular approaches, including bolus injection or continuous infusion of verapamil via in situ placement of a microcatheter, can serve as rescue therapy. A 39-year-old male patient with a history of HIV-1 infection was brought to the Emergency Department (ED) after being found unconscious at home. He presented with a Glasgow Coma Scale score of 9 (E2V1M6), left hemiparesis, and isocoric, phot

Association between M1 tortuosity and intracranial hemorrhage after mechanical thrombectomy using a stent retriever for M2 occlusions.

BackgroundMechanical thrombectomy (MT) is an important treatment for middle cerebral artery (MCA) M2 occlusions. However, it carries a risk of intracranial hemorrhage (ICH), especially with a stent retriever. We investigated the correlation between MCA M1 tortuosity and ICH after MT using a stent retriever for M2 occlusions, along with other clinical outcomes.MethodsThis single-center, retrospective study included patients with isolated MCA M2 occlusions treated using a stent retriever. M1 tortuosity was assessed using pre-MT imaging, with patients classified into tortuous (≥ 2 angulations) an

A Simplified Operation for the Endovascular Perforation Murine Model of Subarachnoid Hemorrhage.

The endovascular perforation model is commonly utilized as a method to simulate subarachnoid hemorrhage (SAH) in experimental studies. Comprehensive and reliable as it is, it requires complicated techniques. This protocol simplifies the model in some detail, combined with neurological scoring. We use a nylon suture to loop and pull the common carotid artery (CCA) to block blood flow temporarily other than traditional ligation, which offers a clear operative field. The utilization of the electrocautery pen to fuse the blood vessels reduces the risk of bleeding. Moreover, we use a filament with

Advancements in neuromonitoring for predicting cerebral vasospasm in aneurysmal subarachnoid hemorrhage using near-infrared spectroscopy: an observational study and review of the literature.

INTRODUCTION: Aneurysmal subarachnoid hemorrhage (aSAH), a severe form of hemorrhagic stroke, poses significant diagnostic and management challenges, particularly in predicting and managing cerebral vasospasm and delayed cerebral ischemia. This study explores the predictive value of near-infrared spectroscopy (NIRS) in high-grade aSAH patients. MATERIAL AND METHODS: This observational study included 16 high-grade aSAH patients treated at a single institution from June 2020 to November 2023. Neuromonitoring incorporated daily transcranial Doppler and continuous NIRS, alongside routine computed

Dual Woven EndoBridge deployment and neuroendoscopic hematoma evacuation for a ruptured multilobulated middle cerebral artery aneurysm: A case report.

BACKGROUND: Ruptured wide-necked or multi-lobulated intracranial aneurysms pose a challenge in the acute phase of subarachnoid hemorrhage (SAH), in which antiplatelet therapy is generally contraindicated. The Woven EndoBridge (WEB) device enables intrasaccular occlusion without requiring antiplatelet agents. However, the treatment of multilobulated aneurysms in this setting remains technically demanding. CASE DESCRIPTION: An octogenarian patient presented with a poor-grade SAH due to a ruptured multilobulated aneurysm of the right middle cerebral artery (MCA) accompanied by intracerebral and

Infective Endocarditis-Associated Cerebral Embolism With Successful Early Thrombectomy Followed by Subarachnoid Hemorrhage Six Hours Post-treatment: A Case Report.

Infective endocarditis (IE)-associated cerebral embolism can lead to severe complications, including subarachnoid hemorrhage (SAH). Inflammation spreading from the embolus to the arterial wall may cause vascular fragility, increasing the risk of hemorrhage even after successful recanalization. Furthermore, mechanical stress during thrombectomy may intensify this fragility, potentially precipitating hemorrhage. However, the timing of the onset of these hemorrhagic complications remains unclear. We report a 71-year-old man with IE who presented with sudden consciousness disturbance three months
