Subarachnoid haemorrhage during premenopausal dienogest therapy in a patient with multiple risk factors.

We present the third case of subarachnoid haemorrhage (SAH) during dienogest (DNG) therapy documented in Japan. A woman in her 40s with a history of smoking and a family history of SAH developed a sudden severe headache and was diagnosed with SAH. Angiography revealed a ruptured anterior communicating artery aneurysm (9.4 mm), which was successfully treated with a Woven EndoBridge. DNG induces a hypooestrogenic state, with serum oestradiol levels dropping to 30 pg/mL after 1 year, potentially increasing the risk of aneurysm rupture. We revised our protocol accordingly to screen for SAH risk fa

Coarctation of the aorta and subarachnoid hemorrhage from an anterior communicating artery aneurysm in an 11-year-old: illustrative case.

BACKGROUND: The authors report the case of an 11-year-old boy who presented with sudden-onset severe headache owing to a subarachnoid hemorrhage (SAH) due to rupture of an anterior communicating artery aneurysm. OBSERVATIONS: Clinical examination revealed a discrepancy in blood pressure between the upper and lower extremities, prompting further cardiovascular evaluation. Imaging identified a previously undetected coarctation of the distal aortic arch (CoA). The patient had been considered healthy and asymptomatic prior to presentation. LESSONS: The coexistence of CoA and an intracranial aneu

BACKGROUND: Aneurysmal subarachnoid haemorrhage (aSAH) is associated with high morbidity and mortality and is often complicated by rebleeding, hydrocephalus, infection and secondary brain injury. CASE PRESENTATION: We present a case of a previously healthy man in his thirties who suffered a ruptured anterior communicating artery aneurysm with secondary complications. Brain imaging revealed severe injury to the corpus callosum and cingulate gyrus. He developed akinetic mutism, characterised by severely reduced initiative and an inability to communicate his needs, resulting in complete dependen

Microsurgical Clip Reconstruction of Multiple Contiguous Aneurysms of the Anterior Communicating Artery Complex: Subfrontal Exposure and Endoscopic Verification-A Case Illustration.

BACKGROUND AND IMPORTANCE: Multiple contiguous aneurysms of the anterior communicating artery (ACoA) complex present significant technical challenges due to their irregular geometry and the density of critical hypothalamic perforators. Standard microscopic visualization is often limited by optical obstruction from clip blades or line-of-sight constraints during complex reconstruction. CLINICAL PRESENTATION: A 62-year-old woman presented with aneurysmal subarachnoid hemorrhage. Preoperative digital subtraction angiography and volumetric segmentation identified 4 distinct aneurysmal origins dis

Case Report: Hemorrhagic-Thrombotic Escalation After Intraprocedural Rupture During Stent-Assisted Coiling: A Case-Based Narrative Review and Staged Communication Model.

Intraprocedural rupture (IPR) during stent-assisted coiling (SAC) after stent deployment can create a narrow and rapidly changing management problem: hemorrhage control, anticoagulation reversal, acute thrombotic occlusion, and postprocedural cerebrospinal fluid diversion may all become urgent within the same clinical sequence. We report a fatal IPR during SAC of an unruptured anterior communicating artery (AComA) aneurysm and use the case as an anchor for a targeted case-based narrative review. A 71-year-old woman underwent SAC for a 5.1-mm posteriorly directed AComA aneurysm with a bleb afte

When ST Elevation Is Not STEMI: Autonomic-Mediated Repolarization Abnormalities After Subarachnoid Hemorrhage.

BACKGROUND Subarachnoid hemorrhage (SAH) is a neurological emergency accounting for 5% of all strokes, with mortality exceeding 50% in patients over 80 years of age. Aneurysmal SAH is particularly lethal in the elderly due to atypical presentations, including ECG abnormalities mimicking acute coronary syndromes, leading to delayed diagnosis and high complication rates. This case report highlights the diagnostic pitfalls of SAH-induced STEMI mimicry. CASE REPORT A in their 90s White woman with a remote history of diabetes presented to the Emergency Department (ED) with sudden-onset occipital he

[Reflections on the management of a case of neurogenic stress cardiomyopathy complicated by acute submassive pulmonary embolism].

Due to their unique pathophysiological states, neurocritical care patients are at a significantly increased risk of acute pulmonary embolism. Typical symptoms are frequently masked by neurological deficits, resulting in delayed diagnosis. Furthermore, clinical management is highly challenging owing to the concurrent high risk of intracranial hemorrhage. On February 26, 2025, Linyi People's Hospital admitted a 67-year-old male patient who developed neurogenic stress cardiomyopathy (NSC) and acute submassive pulmonary embolism following surgery for a ruptured aneurysm. The patient presented with

Stent-assisted coiling with LVIS Evo and HydroCoil embolic system: 1-year results of the SEALANT study.

BACKGROUND: More high-quality evidence for stent-assisted coiling (SAC) of cerebral aneurysms is needed. Randomized controlled trials have shown lower recurrence with the HydroCoil Embolic System (HES) compared with bare platinum coils, and self-adjudicated LVIS Evo SAC studies have demonstrated favorable safety and technical success. The objective of the SEALANT study is to investigate the safety and effectiveness of this device combination. METHODS: SEALANT is an open-label, prospective, single-arm, multicenter study including patients with aneurysms up to a maximum size of 12 mm. The prima

Early rupture of an anterior communicating artery pseudoaneurysm after transsphenoidal surgery: A case report and clinical implications.

BACKGROUND: Intracranial pseudoaneurysm after transsphenoidal surgery (TSS) is rare, particularly in the anterior communicating artery (AcomA). CASE DESCRIPTION: A 47-year-old female underwent extended TSS for a giant pituitary adenoma with gross-total resection. She developed a subarachnoid hemorrhage on the night of surgery. Angiography revealed a ruptured AcomA pseudoaneurysm, which was successfully treated with coil embolization. The patient achieved good recovery (modified Rankin Scale 1). CONCLUSION: Early postoperative hemorrhage after TSS should prompt early vascular imaging, as time

Delayed Remote Intracerebral Hemorrhage following Coil Embolization Using a Simple Technique for Ruptured Anterior Communicating Artery Aneurysm: A Case Report of Subarachnoid Hemorrhage.

INTRODUCTION: Delayed remote intracerebral hemorrhage (rICH) is a well-established complication following flow diverter placement for intracranial aneurysms, with a reported incidence of 5.7%-8.5%. Although rare, similar events have also been reported after coil embolization, predominantly in cases in which adjunctive techniques are used. We report a case of delayed rICH after coil embolization performed with a simple technique in a patient at high risk of thromboembolism. CASE PRESENTATION: A 62-year-old female presented with subarachnoid hemorrhage due to a ruptured anterior communicating a

Association of Diagonal Earlobe Crease (Frank's Sign) with Intracranial Aneurysm Rupture at Small Sizes.

BACKGROUND: The diagonal earlobe crease (DELC), or Frank's sign, has been associated with systemic vascular degeneration. Whether this cutaneous marker correlates with intracranial aneurysm (IA) instability remains unclear. METHODS: We conducted a hospital-based observational registry study including 186 consecutive patients with IA. DELC was dichotomized into absent/low-grade versus high-grade creases. Associations between DELC and aneurysm rupture, rupture at small sizes (<7 mm), and clinical outcome were analyzed using multivariate logistic regression models adjusted for demographic and va

Isolated intraventricular hemorrhage from an anterior communicating artery aneurysm via the lamina terminalis: illustrative case.

BACKGROUND: Rupture of an anterior communicating artery (ACoA) aneurysm causing isolated intraventricular hemorrhage (IVH) without accompanying subarachnoid hemorrhage (SAH) is exceptionally rare. Although an anatomical mechanism has been proposed, direct supporting evidence remains scarce. OBSERVATIONS: A 78-year-old male presented with isolated IVH initially attributed to hypertensive hemorrhage. Retrospective review of MR angiograms, prompted by a persistent hyperdense focus in the third ventricle on follow-up CT, revealed a small aneurysm at the right A1-A2 junction. Three-dimensional CT

Case Fatality of Subarachnoid Hemorrhage by Aneurysm Location: A Population-Based Study From Finland and New Zealand.

BACKGROUND AND OBJECTIVES: According to region-specific hospital-based studies, the location of ruptured intracranial aneurysm (RIA) influences the case fatality rate (CFR) of aneurysmal subarachnoid hemorrhage (SAH). However, little is known about whether CFRs vary by RIA location in population-based studies that include prehospital SAH deaths. We assessed whether CFRs and CFR-trends differ by RIA location using whole population data from Finland and New Zealand. METHODS: We used externally validated administrative databases to identify all nonhospitalized and hospitalized SAH cases in Finla

Incidental Identification of an Intracranial Vertebral Artery Aneurysm Related to Unilateral Vertebral Artery Occlusion in a Patient With a Ruptured Internal Carotid-Posterior Communicating Artery Aneurysm: A Case Report.

An intracranial aneurysm located on the vertebral artery (VA) following unilateral VA occlusion is rare. Little has been described about this entity. An 85-year-old drowsy woman was transported to our hospital. A screening CT scan showed subarachnoid hemorrhage and secondary acute hydrocephalus. CT angiography revealed a right internal carotid-posterior communicating (IC-Pcom) artery aneurysm, an intracranial VA aneurysm, and left VA occlusion. The intracranial VA aneurysm was located on the lateral side of the left VA, near the origin of the anterior spinal artery. Following external ventricu

Sequential rupture of multiple intracranial aneurysms with rapid de novo bleb formation: A case report.

BACKGROUND: Managing multiple intracranial aneurysms (MIAs) in subarachnoid hemorrhages (SAHs) can be challenging, especially when rapid morphological changes such as de novo bleb formation can increase the risk of sequential rupture. In addition, identifying the rupture source is often difficult in the case of symmetrical hematoma distribution. CASE DESCRIPTION: This study investigated a 66-year-old woman with an SAH and three aneurysms: Right and left internal carotid-posterior communicating (IC-PC) aneurysms and an anterior communicating artery (Acom) aneurysm. Magnetic resonance imaging w

Microsurgical clipping of paraclinoid aneurysms: a 12-year single-centre experience with surgical nuances and outcomes.

Paraclinoid aneurysms, arising from the internal carotid artery between the proximal dural ring and the posterior communicating artery, pose significant microsurgical challenges due to their proximity to critical neurovascular structures. Despite advances in endovascular techniques, surgical clipping offers definitive exclusion with lower recurrence. This study evaluates surgical outcomes and operative nuances in clipping paraclinoid aneurysms over a 12-year period. A retrospective review was conducted of 116 patients with paraclinoid aneurysms who underwent surgical clipping at a tertiary cen

Subtonsillar Approach to a Polylobulated Posterior Inferior Cerebellar Artery Aneurysm: A Case Illustration.

BACKGROUND AND IMPORTANCE: Dysplastic and polylobulated aneurysms of the posterior inferior cerebellar artery (PICA) present significant technical challenges because of their irregular geometry and proximity to the lower cranial nerves and brainstem perforators. When endovascular options carry elevated risks, microsurgical clip reconstruction remains a definitive treatment to restore the vascular architecture. CLINICAL PRESENTATION: A 46-year-old woman presented for the elective surgical management of an unruptured, polylobulated left PICA aneurysm. She had a history of subarachnoid hemorrhag

Endovascular Management of Aneurysmal Subarachnoid Hemorrhage in People Living With HIV: Comparable Outcomes in a High-Prevalence Setting.

BACKGROUND AND OBJECTIVES: The relationship between aneurysmal subarachnoid hemorrhage (aSAH) and HIV is poorly understood, particularly regarding endovascular outcomes in high-prevalence regions, where current guidelines favor endovascular management for ruptured saccular aneurysms amenable to both coiling and clipping, although some suggest surgical clipping may be preferable for people living with HIV (PLWH). This research investigates and contrasts the clinical features, laboratory results, imaging characteristics, therapeutic approaches, associated complications, as well as short-term and
