The authors report a review of 290 patients admitted for the treatment of subarachnoid hemorrhage. Twenty-seven (9.3%) patients developed the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). The diagnosis was established by means of daily laboratory investigations (serum electrolytes and osmolality; urine sodium and osmolality; and fluid balance). The patients were divided into two groups (severe and mild SIADH) on the basis of clinical symptoms and signs and laboratory findings. High values of urine osmolality and sodium concentration in patients with low values of serum o

Computed tomographic scans in 81 consecutive patients wih subarachnoid hemorrhage due to ruptured aneurysm were analyzed for patterns of hemorrhage and lucency and correlated with the development of spasm and hydrocephalus. The circulation time was measured angiographically in representative cases of spasm. Hemorrhage corresponded in a general way to the fossa of aneurysm origin and, when there was parenchymal or ventricular hemorrhage, in more specific ways to anterior communicating, middle cerebral, and posterior inferior cerebellar artery aneurysms. Basal ganglionic hemorrhages due to aneur

The patient was a 48-year-old housewife, who had a sudden onset of severe headache followed by loss of consciousness for a few hours on the day of admission. Initially she showed slight restlessness due to headache, neck stiffness and subhyaloid hemorrhage. Four-vessel study revealed a basilar aneurysm on right retrograde brachial angiography and anterior communicating aneurysm on left carotid angiography. Two weeks after the onset, when she had no neurological deficit except for intermittent appearance of disorientation, both aneurysms were successfully clipped through right pterional approac

This is a report on 32 patients with subarachnoid haemorrhage caused by rupture of an intracranial arterial aneurysm in the 0-19 age group. Sixteen of the aneurysms were situated in the bifurcation of the internal carotid artery, 8 in the anterior communicating artery, 4 in the posterior communicating artery, two in PICA, one in the middle cerebral artery, and one in the pericallosal artery. Direct intracranial clipping of the neck of the aneurysm was carried out in 26 cases, proximal clipping in three, wrapping in two, and common carotid ligation in one case. There was one death from surgery.

The authors report a patient aged 58 years with a history of two episodes of subarachnoid haemorrhage and with arteriographic finding of four intracranial aneurysms: two on the internal carotid artery, one on the trifurcation of the middle cerebral artery, and one on the anterior communicating artery. Despite rather poor general and local condition (III clinical group) during the operation all four aneurysms were closed with clips. After a stormy postoperative course the condition of he patient improved and 6 months later he was feeling well, had no paresis, and only some memory disturbances r

A study of 788 cases of spontaneous subarachnoidal hemorrhages, which were observed in some neurological clinics during the past 5 years, permitted an analysis of the statistical data concerning the distribution and outcomes of this form of strokes depending upon the age of the patient and etiology. Among the observed population, individuals of a young and middle age prevailed (78% of the patients were younger than 60 years). The role of separate etiological factors is different in different age groups in individuals under 40, subarachnoidal hemorrhages are mainly produced by a rupture of cere

A 64-year-old woman had the syndrome of inappropriate ADH secretion (SIADH) after surgical repair of a ruptured aneurysm of the anterior communicating artery. SIADH began while the intracranial pressure was elevated, and resolved when the pressure returned to normal. Whether this syndrome was caused by direct hypothalamic lesions, local pressure on the hypothalamus secondary to enlargement of the third ventricle, or some other mechanism cannot be ascertained. The occurrence of intracranial pressure changes with SIADH is described.

A rare case was reported in which a brain tumor was associated with a cerebral aneurysm. A 59-year-old woman was admitted with right spastic hemiparesis and motor aphasia. She was in somnolence and complained of headache. She had two attacks of loss of consciousness about 12 years previously. Since then she had right spastic hemiparesis and motor aphasia as sequelae. Preoperative CT scan and carotid angiograms showed a malignant brain tumor in the left temporal region and an anterior communicating artery aneurysm. Operation was performed in one stage. At first the neck of the aneurysm was clip

An 11-year-old girl with renal artery fibromuscular hyperplasia and a berry aneurysm of the anterior communicating artery with subarachnoid haemorrhage is reported. She presented with subarachnoid hemorrhage and was shown to have hypertension attributable to unilateral renal artery fibromuscular hyperplasia. Cerebral angiography showed an aneurysm of the anterior communicating artery. The aneurysm was not treated surgically, but control of the hypertension was obtained by renal revascularisation with an aortorenal graft of saphenous vein. The patient remained well subsequently, and follow-up a

A 52-year old man suffered from a subarachnoid hemorrhage, out of a ruptured aneurysm of the anterior communicating artery. Due to the subarachnoid hemorrhage a hydrocephalus developed presenting the symptoms of akinesia and mutism. Arterial spasms of the anterior cerebral arteries led to bilateral ischemic infarction allowed by a distal anterior syndrome and signs of general brain damage. The combination of these complications of the ruptured aneurysm caused a complex syndrome which in the beginning resembled an apallic syndrome.

Clinical analysis of postoperative complications was made in 1,000 cases of intracranial saccular aneurysm. Psychological symptoms, motor disturbances, and aphasia were observed in 107 cases (11.5%), 74 cases (7.1%) and 20 cases (2.1%), respectively, in 939 discharged cases. Water and electrolyte disturbances and gastro-intestinal bleeding were found in 60 cases (6.0%) and 19 cases (1.9%), respectively, in 1,000 surgically treated patients including those that died during hospitalization. Psychological symptoms were most apt to develop in cases of vertebrobasilar artery aneurysm, multiple aneu

This report describes the analysis of 216 CT pictures of 85 patients with ruptured aneurysms which consist of 29 anterior communicating, 27 internal carotid, 27 middle cerebral and 2 basilar arterial aneurysms, including 18 cases with multiple aneurysms. The intervals between CT scanning and the last subarachnoid hemorrhage were various from 6 hours to 4 months. The first CT scanning was made in 40 cases within 3 days, in 54 cases within 7 days and in 11 cases more than one month after the hemorrhage. CT findings showed no abnormality in 9 patients who were classified as Grade I (Hunt and Kosn

A patient had a subarachnoid hemorrhage in 1969. Four-vessel angiography demonstrated an infundibular widening of the posterior communicating artery alone. In 1978, nine years later, angiography demonstrated that the infundibular widening had enlarged and developed into an aneurysm, though unruptured. Another aneurysm of the anterior communicating artery was also found. Both were confirmed at operation and clipped. Review of the pertinent literature emphasizes that the infundibular widening may be preaneurysmal.

The author has operated on 40 patients with giant intracranial aneurysms, using various surgical approaches. Giant aneurysms predominated in females (3:1) and were most common in the age group 30 to 60 years. Patients presented with subarachnoid hemorrhage (17), visual disturbance (18), chronic headache (14), transient or progressive hemispheric deficit (6), seizure (2), dementia (2), and cerebrospinal fluid rhinorrhea (1). Giant aneurysms were located at the carotid artery (25), the basovertebral artery (8), the anterior communicating artery (5), and the middle cerebral artery (2). Eight of 4

A non operated aneurysm of the anterior communicating artery enlarged considerably over a seven years period; it finally thrombosed and behaved like a suprasellar tumor. The clinical course, the angiographic evolution and the anatomical examination allowed, in this case, to account for the increase in volume by progressive stretching of the wall, and not by incorporation of a false aneurysmal sack resulting from an encapsulated haematoma. Histologically, the wall of this giant aneurysm showed various changes : on the inner aspect, progressive thickening occurs, due to fibrous organization of m

Measurements of the regional cerebral blood flow (rCBF) were performed 44 times on patients with subarachnoid hemorrhage, using the 133-Xe clearance method with aid of the 8 channel detector unit. The patients consisted of 31 preoperative cases within 3 weeks of the last subarachnoid hemorrhage. The sites of the ruptured aneurysms included 11 anterior communicating arteries, 8 middle cerebral arteries, 7 internal carotid arteries and 3 anterior cerebral arteries. In 2 cases, the aneurysms were not identified with the cerebral angiograms. The purpose of this study was to investigate the effect
