Epidemiology and treatment strategies of intracranial aneurysms associated with Loeys-Dietz syndrome: A systematic review, meta-analysis, and a case presentation.

BACKGROUND: Recent studies have suggested that intracranial aneurysms (IAs) may be a common arterial manifestation of Loeys-Dietz syndrome (LDS). However, the prevalence, characteristics, and treatment strategies remain unclear because of limited and fragmented evidence. METHODS: We searched PubMed and Scopus through April 2026. Inclusion criteria were studies reporting epidemiological characteristics or surgical interventions of LDS-associated IAs. Studies with high risk of bias according to an adapted Newcastle-Ottawa Scale were excluded from the meta-analysis. Prevalence and risk factors o

Efficacy and safety of endovascular coiling compared to microsurgical clipping in aneurysmal subarachnoid hemorrhage: a systematic review and meta-analysis of randomized trials.

BACKGROUND: Aneurysmal subarachnoid hemorrhage (aSAH) carries high morbidity and mortality despite advances in aneurysm repair. Microsurgical clipping and endovascular coiling are established treatment modalities, yet their relative benefits regarding functional outcome and long-term aneurysm occlusion remain debated. This study evaluates treatment effects stratified by aneurysm location and clinical grade. METHODS: We identified RCTs comparing clipping and coiling for ruptured intracranial aneurysms. Primary outcomes were favorable functional outcome at 1-year [modified Rankin Scale (mRS), a

Tiny (≤3 mm) Unruptured Intracranial Aneurysms: A 13-Year Contemporary Insight on Treatment Safety From a Multicenter Registry.

BACKGROUND AND OBJECTIVES: Tiny or very small unruptured intracranial aneurysms (≤3 mm) carry a very low estimated rupture risk, yet many are electively treated. Although factors influencing procedural risks have the potential to guide treatment selection, they remain poorly characterized in this population. METHODS: A retrospective cohort study was conducted using prospectively collected data from the multicenter Stroke Thrombectomy and Aneurysm Registry, including patients treated for unruptured aneurysms between 2013 and 2025. Tiny aneurysms were defined as ≤3 mm. Primary outcomes were int

Epidemiology of Aneurysmal Subarachnoid Hemorrhage in Shiga Prefecture, Japan: A Population-Based Study from the Shiga Stroke and Heart Attack Registry.

INTRODUCTION: Population-based data describing aneurysm location and treatment patterns in aneurysmal subarachnoid hemorrhage (SAH) remain limited in Japan. We aimed to describe the epidemiological characteristics of aneurysmal SAH using a large population-based stroke registry. METHODS: We conducted a descriptive epidemiological study using the Shiga Stroke and Heart Attack Registry, an ongoing population-based registry covering approximately 1.4 million residents of Shiga Prefecture, Japan. Among 1,056 patients with first-ever SAH identified between 2011 and 2015, 819 patients with aneurysm

Branch-Critical Clipping of a Ruptured Carotid-Posterior Communicating Aneurysm with Fetal PCA Configuration.

Background/Objectives: Aneurysmal subarachnoid hemorrhage (aSAH) involves a sudden onset of a perfusion-pressure injury from the initial insult combined with a secondary injury phase produced by delayed cerebral ischemia, cerebrospinal fluid circulation disturbances, and generalized instability of the patient's physiological state. The situation may be further complicated when there has been rupture of the aneurysm at the site of the carotid-posterior communicating (PCom) artery junction that occurs in conjunction with a fetal configuration of the posterior cerebral artery (fPCA), thereby maki

A Cautionary Tale of Balloon Assistance: Early Dissecting Aneurysm Formation after Posterior Communicating Artery Aneurysm Coiling.

OBJECTIVE: Balloon-assisted coiling of intracranial aneurysms is considered a safe and effective practice. Complications can rarely arise from balloon-assisted endovascular procedures, including thromboembolic events and dissection. CASE PRESENTATION: We present the case of a patient with a subarachnoid hemorrhage from a ruptured 3.5 × 2.5-mm right posterior communicating artery aneurysm. After embolization, the patient developed a new right carotid artery dorsal blood blister aneurysm at the site of prior embolization, which was treated with placement of a double layer of laser cut stents.

Incidental Identification of an Intracranial Vertebral Artery Aneurysm Related to Unilateral Vertebral Artery Occlusion in a Patient With a Ruptured Internal Carotid-Posterior Communicating Artery Aneurysm: A Case Report.

An intracranial aneurysm located on the vertebral artery (VA) following unilateral VA occlusion is rare. Little has been described about this entity. An 85-year-old drowsy woman was transported to our hospital. A screening CT scan showed subarachnoid hemorrhage and secondary acute hydrocephalus. CT angiography revealed a right internal carotid-posterior communicating (IC-Pcom) artery aneurysm, an intracranial VA aneurysm, and left VA occlusion. The intracranial VA aneurysm was located on the lateral side of the left VA, near the origin of the anterior spinal artery. Following external ventricu

Sequential rupture of multiple intracranial aneurysms with rapid de novo bleb formation: A case report.

BACKGROUND: Managing multiple intracranial aneurysms (MIAs) in subarachnoid hemorrhages (SAHs) can be challenging, especially when rapid morphological changes such as de novo bleb formation can increase the risk of sequential rupture. In addition, identifying the rupture source is often difficult in the case of symmetrical hematoma distribution. CASE DESCRIPTION: This study investigated a 66-year-old woman with an SAH and three aneurysms: Right and left internal carotid-posterior communicating (IC-PC) aneurysms and an anterior communicating artery (Acom) aneurysm. Magnetic resonance imaging w

Bridging Technique with a Stent Retriever for Iatrogenic Internal Carotid Artery Occlusion during Coil Embolization of a Ruptured Aneurysm.

OBJECTIVE: Iatrogenic arterial dissection is a rare complication in neuroendovascular procedures, with an incidence of 0.15%-0.4% in the cervical carotid artery. In ruptured aneurysm cases, preoperative antiplatelet therapy is usually not administered, and immediate carotid artery stenting (CAS) in the event of occlusive dissection may lead to thrombosis. Even when antiplatelet drugs are administered urgently, their delayed onset may lead to in-stent thrombosis, resulting in cerebral ischemia. We report a case of intraoperative iatrogenic internal carotid artery (ICA) occlusion during coil emb

From Benign Symptoms to Subarachnoid Hemorrhage: A Pediatric Case.

Pediatric intracranial aneurysms are rare and represent an uncommon but life-threatening cause of subarachnoid hemorrhage in children. Their diagnosis is usually delayed because early symptoms are nonspecific and often mimic benign pediatric conditions. We report the case of a 13-year-old boy who presented with a one-week history of progressive headache and vomiting and was repeatedly evaluated at multiple healthcare facilities, where he was treated for suspected gastroenteritis and sinusitis. He clinically declined on the day of presentation with sudden worsening of headache, photophobia, and

Posterior circulation vascular variants and intracranial aneurysm formation: An imaging-based study with implications for risk stratification.

BACKGROUND: Subarachnoid hemorrhage (SAH) from Intracranial Aneurysms (IA) represents a leading cause of death and morbidity from neurological deficits. Emerging research has identified that variations in vascular anatomy may contribute to endothelial dysfunction, hemodynamic stress, and pharmacological responsiveness in cerebrovascular tissues. OBJECTIVES: This study aimed to determine whether variations in the posterior circulation vascular system are associated with the development of aneurysms. Additionally, it was explored whether these variations could provide additional risk assessment

Management of Aneurysmal Subarachnoid Hemorrhage During Pregnancy with a Devastating Clinical Course: A Case Report.

Background: Aneurysmal subarachnoid hemorrhage (SAH) during pregnancy is rare, occurring in approximately 0.01-0.05% of pregnancies, most commonly in the third trimester. Its management is particularly challenging, requiring careful consideration of both maternal and fetal outcomes. Methods: We report the case of a 32-year-old woman at 31 weeks of gestation who presented with severe headache and left third cranial nerve palsy. Imaging revealed diffuse SAH with significant obstructive hydrocephalus and a 5 mm left posterior communicating artery aneurysm. Following multidisciplinary discussion,

Intraoperative Heparin Resistance and Internal Carotid Artery Occlusion After Andexanet Alfa Reversal During Endovascular Treatment of a Ruptured Cerebral Aneurysm: A Case Report.

A 56-year-old woman receiving oral edoxaban (30 mg/day) was transferred to our hospital with impaired consciousness and headache. Head computed tomography revealed a subarachnoid hemorrhage and a 7-mm aneurysm located at the junction of the left internal carotid artery-posterior communicating artery. Andexanet alfa was administered 11.5 hours after the patient's last dose of edoxaban to reduce the potential rebleeding risk before aneurysm treatment. During coil embolization, acute occlusion of the left internal carotid artery was observed. Mechanical thrombectomy was subsequently performed, ac

Eye Discomfort and Blurred Vision Preceding Oculomotor Nerve Palsy in a Warning Headache:A Case Report.

Cerebral aneurysms can present with warning headaches before rupture or during minor leaks. Internal carotid artery-posterior communicating artery (IC-PC) aneurysms are among the few aneurysms for which the clinical observation of growth and associated symptoms is possible. Enlargement or morphological changes may worsen oculomotor nerve (ON) palsy, intensified headache, or orbital pain, indicating an impending rupture that requires urgent intervention. Notably, blurred vision with eye discomfort preceding ON palsy during impending rupture is rarely reported. Herein, we present a case highligh

Catastrophic Rebleeding during Woven EndoBridge Treatment of a Large Ruptured Internal Carotid Artery Aneurysm with a Fetal-Type Posterior Communicating Artery: A Case Report.

Intraprocedural rupture during Woven EndoBridge (WEB) treatment is most commonly reported in the early deployment phase, whereas rebleeding after substantial deployment is considered rare. We report a patient in their early 90s with poor-grade subarachnoid hemorrhage caused by a large ruptured internal carotid artery aneurysm with a fetal-type posterior communicating artery. After initial deployment, the bleb was already covered by the lateral surface of the device. During partial resheathing and repositioning to preserve the branch, the WEB rotated slightly, altering the configuration of the

"Double Trouble": Ultra-Early Formation and Rupture of a De Novo Terminal Basilar Artery Aneurysm After Aneurysmal Subarachnoid Hemorrhage.

INTRODUCTION: The formation of de novo intracranial aneurysms (DNIAs) often occurs months or years after the initial identification of an intracranial aneurysm. Consensus on surveillance after surgery remains elusive. CASE PRESENTATION: A 53-year-old woman presented with a recurrent subarachnoid hemorrhage (SAH) caused by a rapidly expanding de novo basilar tip aneurysm, which developed and ruptured within 6 days of a previous SAH due to the rupture of a posterior communicating artery aneurysm. Both SAH incidents were successfully treated with microsurgical clipping. A computed tomography ang

Ischemic Complications After Parent Artery Occlusion for Ruptured Posterior Cerebral Artery Aneurysm With a Fetal-Type Posterior Communicating Artery: A Case Report.

Posterior cerebral artery (PCA) aneurysms are uncommon, and many are considered to be dissecting aneurysms. In ruptured cases, parent artery occlusion (PAO) is often selected as a treatment strategy. However, the morphology of the posterior communicating artery (PCoA) may influence the risk of postoperative ischemic complications. We report the case of a 76-year-old man with a sudden-onset headache and impaired consciousness. He was diagnosed with subarachnoid hemorrhage (SAH) and Hunt and Kosnik grade V. Imaging revealed a ruptured aneurysm located in the distal P2 segment of the left PCA. Th

Microsurgical clipping of paraclinoid aneurysms: a 12-year single-centre experience with surgical nuances and outcomes.

Paraclinoid aneurysms, arising from the internal carotid artery between the proximal dural ring and the posterior communicating artery, pose significant microsurgical challenges due to their proximity to critical neurovascular structures. Despite advances in endovascular techniques, surgical clipping offers definitive exclusion with lower recurrence. This study evaluates surgical outcomes and operative nuances in clipping paraclinoid aneurysms over a 12-year period. A retrospective review was conducted of 116 patients with paraclinoid aneurysms who underwent surgical clipping at a tertiary cen
