Oculomotor nerve palsy in patients with cerebral aneurysms.

Twenty-six patients with oculomotor nerve palsy due to cerebral aneurysms were examined. There were six males and twenty females with a mean age of 55 years. 25 of the 26 aneurysms were located at the junction of the internal carotid and the posterior communicating artery and one was at the junction of the basilar artery and the superior cerebellar artery. Twelve patients had associated subarachnoid hemorrhage (SAH); the other 14 did not. The initial symptoms in many patients were ptosis and double vision. Twenty-one of the patients had total oculomotor nerve palsy, one had a sparing of medial

Postoperative recovery of third nerve palsy due to posterior communicating aneurysms.

Postoperative recovery of third nerve palsy due to a posterior communicating artery aneurysm was correlated with pre- and intraoperative factors. The only significant factor determining recovery was the degree of preoperative deficit. The presence of subarachnoid haemorrhage, timing of surgery and aspiration of the aneurysmal sac did not influence recovery.

[Basilar bifurcation aneurysm associated with internal carotid artery occlusion. Report of two cases].

Two cases of subarachnoid hemorrhage caused by rupture of a basilar bifurcation aneurysm associated with occlusion of the internal carotid artery (ICA) at the neck are presented. Case 1, a 71-year-old female, was hospitalized in a coma. Angiography demonstrated occlusion of the bilateral ICA, collateral blood supply through the branches of the foramen rotundum or vidian artery from the maxillary arteries and right posterior communicating artery, and a saccular aneurysm at the basilar bifurcation. The patient died 1 month later following rerupture of the aneurysm. Case 2, a 64-year-old male, wa

Medullary venous malformation with an arterial component associated with a ruptured aneurysm--case report.

We report the rare of occurrence of a medullary venous malformation (MVM) with an arterial component associated with a saccular aneurysm on the opposite side. This 49-year-old male patient was admitted with headache and vomiting. He was diagnosed as having a subarachnoid hemorrhage on the basis of bloody cerebrospinal fluid. Angiography revealed a saccular aneurysm at the junction of the internal carotid and posterior communicating arteries on the left side. A MVM with an arterial component was also seen in the right basal frontal lobe. On the seventh hospital day, the aneurysm was clipped via

Cerebral vasospasm after subarachnoid haemorrhage investigated by means of transcranial Doppler ultrasound.

Measurements of flow velocity in defined segments of the basal cerebral arteries can be obtained through the intact adult skull using 2 MHz pulsed Doppler ultrasound. We compared flow velocity in these vessels with findings from 56 cerebral angiographies obtained in 51 patients at from day 1 to day 21 after subarachnoid haemorrhage (SAH). The diameter of the proximal segment of the middle cerebral, anterior cerebral, and posterior cerebral arteries (MCA, ACA, and PCA, respectively) were measured from anteroposterior films produced in one angiographic laboratory. In patients investigated on day

Acute subdural hematoma from ruptured posterior communicating artery aneurysm.

Acute spontaneous subdural hematoma is infrequent in association with rupture of intracranial saccular aneurysm. In the majority of cases, aneurysms of the middle cerebral artery along the convexity or of the anterior cerebral artery along the interhemispheric fissure are found to be the culprits. We present two recent cases of internal carotid-posterior communicating artery aneurysms causing acute subdural hematoma with little or no subarachnoid hemorrhage and discuss the possible mechanisms for this occurrence.

[Usefulness of CT angiography for demonstrating cerebral aneurysms].

We report the usefulness of computed cerebral angiotomography (CT angiography) for demonstrating cerebral aneurysm and the clinical significance of CT angiography for ruptured cerebral aneurysm. Our modified method of CT angiography was easy and less time-consuming. Fifteen seconds after starting a single bolus injection, 1 ml/kg/25 seconds via cubital vein, of contrast medium (60% urograffin), 5 serial 5 mm thick-CT slices were scanned in every 6.5 seconds including 2 seconds of interval, beginning from an axial level 20 mm above the orbitomeatal line and ending at a level 40 mm. Early symptoms of subarachnoid haemorrhage due to aneurysms of the posterior communicating artery.

Ninety patients were studied who had been admitted to Dijkzigt Hospital in Rotterdam between January 1980 and April 1987 with subarachnoid haemorrhage (SAH) due to an aneurysm at or near the junction of the internal carotid artery with the posterior communicating artery (IC-PC). An evaluation was made of the early neuro-opthalmological symptoms and the mortality in patients with paresis of the oculomotor nerve (N.III). The importance of early recognition of the condition is emphasized and a prospective investigation of the early symptoms of SAH in ophthalmological practices in the Netherlands

Detection of unruptured familial intracranial aneurysms by intravenous digital subtraction angiography. Screening of two affected families.

The authors discuss the detection of intracranial aneurysms (IA) by means of intravenous digital angiography (ivDSA) in (a)symptomatic first degree relatives of families in which two or more individuals have IA. ivDSA is an almost noninvasive and low-risk diagnostic procedure. Screening, by means of ivDSA, of two affected families is described. In family I which includes 7 members with proven IA, ivDSA has been carried out in 36 asymptomatic individuals: in one, a 6 X 15 mm aneurysm was found at the left posterior communicating artery (PCoA). In family II, including one member with a proven IA

Basilar bifurcation aneurysm associated with bilateral internal carotid occlusion.

A case of subarachnoid hemorrhage from ruptured aneurysm of the basilar bifurcation associated with occlusions of both internal carotid arteries at the neck is presented. Each internal carotid artery mainly received collateral flow at the cavernous portion from the internal maxillary artery through anastomotic artery of the foramen rotundum. Posterior circulation also supplied collateral blood flow via the right posterior communicating artery. This case report suggests that hemodynamic stress may be regarded as an important factor in the formation or development of cerebral aneurysm.

Enlargement and rupture of distal basilar artery aneurysm after iatrogenic carotid occlusion.

A case of angiographic enlargement and fatal rupture of a previously asymptomatic distal basilar aneurysm in a 12-year-old girl is reported. She had been treated by carotid sacrifice for a giant intracavernous carotid aneurysm. After superficial temporal-middle cerebral artery bypass, this patient underwent a trapping procedure and decompression of her symptomatic giant aneurysm. Despite postoperative patency of her bypass graft, the involved middle cerebral circulation was irrigated substantially by retrograde flow through her posterior communicating artery. An incidental distal basilar aneur

Persistent hypoglossal artery associated with basilar artery aneurysm.

A saccular aneurysm at the tip of the basilar artery in a patient with a persistent hypoglossal artery is reported. The persistent hypoglossal artery was arising from the left internal carotid artery. Both vertebral and posterior communicating arteries were absent bilaterally. The combination of a persistent hypoglossal artery and a basilar bifurcation aneurysm is a very rare occurrence. The anatomy of the cerebral circulation in this unique case is important for surgical planning.

Unusual intercarotid anastomosis associated with anterior communicating artery aneurysm. Case report.

The case is presented of a 34-year-old man with subarachnoid hemorrhage from rupture of an anterior communicating artery aneurysm. The magnetic resonance imaging and angiographic findings are reported. Angiography showed the aneurysm plus an unusual anastomosis between the intracavernous portions of both internal carotid arteries. The anastomosis crossed posterior to the base of the dorsum sellae and was associated with absence of the right internal carotid artery and the A1 segment of the right anterior cerebral artery. This is the 11th case of such an unusual intercarotid anastomosis reporte

[Extracranial internal carotid occlusion and coexisting ipsilateral intracranial internal carotid aneurysm].

A rare case of extracranial internal carotid occlusion with a coexisting ipsilateral internal carotid aneurysm is reported. A 50-year-old male had a sudden onset of severe headache, vomiting and right motor weakness on May 14, 1984. Two days later the patient was transferred to our hospital. On admission he was alert but presented with nuchal rigidity and right moderate hemiparesis. He had an episode of a blunt head injury 12 years previously, but no history of hypertension, diabetes mellitus or cerebral stroke. A computed tomography revealed mild subarachnoid hemorrhage and mild ventricular d

Anatomical patterns of hypoplastic posterior communicating arteries and their implications for cerebrovascular diseases.

A total of 114 undersized posterior communicating arteries (PCoA-s, external diameter less than 1 mm) and 126 larger ones were investigated histologically. Microscopic examination of hypoplastic PCoA-s revealed five different structural patterns. Two of them showed microscopic appearances characteristic of intracranial arteries seen in adults or newborns without vascular disease. Two other types resembled the histologic picture of Moya Moya disease. The last type was thin-walled and dilated. All PCoA-s larger than 1 mm displayed the adult pattern. The PCoA-s with the Moya Moya-like structure a

Sylvian fissure arteriovenous malformations.

We have operated on 16 cases of arteriovenous malformation (AVM) in and around the sylvian fissure. We call these lesions "sylvian fissure AVMs" and classify them into four subdivisions, namely, pure, lateral, medial, and deep AVMs. By others, they have been variously called AVMs of the basal ganglia, insula, anterior choroidal artery, frontal lobe, or temporal lobe. These sylvian fissure AVMs showed similar angiographic findings: the feeders in all cases were branches of the middle cerebral artery; in some cases, additional feeders from the anterior and posterior choroidal and posterior commu

A non-invasive Doppler ultrasound method for the evaluation of patients with subarachnoid hemorrhage.

Measurements of flow velocity in the basal cerebral arteries can be obtained non-invasively through the intact skull using 2 MHz pulsed Doppler ultrasound. We investigated 47 patients with subarachnoid hemorrhage and compared flow velocity with angiography. Flow velocity in the middle and posterior cerebral arteries (MCA and PCA) was inversely related to the lumen diameter. Further clues to diagnose MCA spasm were obtained from the ratio of MCA flow velocity divided by the velocity in the distal extracranial internal carotid artery. In the individual patient, the two anterior cerebral arteries

[Stenosis of the posterior cerebral artery (P1) discovered at the time of a meningeal hemorrhage and simulating an arterial aneurysm].

A case is reported, in which a patient 54 years old, was admitted after a SAH. The CT Scan (D1) demonstrated blood in the peri-peduncular cisterns. A first angiogram through femoral route was performed at D1. There was no vascular malformation on both carotid territories. An arterial ectasia was demonstrated on the basilar artery, arising at the origin of the left superior cerebellar artery. The left posterior cerebral artery was supplied only by the left carotid artery through a dilated posterior communicating artery. A second left vertebral angiogram was performed at D8, to make sure whether

Surgical treatment of incidental intracranial aneurysms.

It is clear that more incidental aneurysms will be encountered in the future. Approximately 5% or more of the population harbors these lesions, and advancing technology can be expected to demonstrate them with increasing regularity. Multiple aneurysms will also be found in at least 18% of patients with subarachnoid hemorrhage due to aneurysms. The best estimates suggest a rate of hemorrhage approximating 1%/year for incidental aneurysms and a 0.4 to 0.65% annual mortality rate for these lesions. It has also been shown that even small aneurysms may enlarge and bleed unpredictably with the passa
