A case is presented of a 64-year old woman with pulseless disease associated with multiple intracranial aneurysms which were located in the internal carotid circulation at the left internal carotid-ophthalmic artery and the left internal carotid-posterior communicating junction, and in the basilar-vertebral circulation, at the left basilo-anterior inferior cerebellar artery junction and at the top of the basilar artery. Autopsy verified that her death was due to rupture of the aneurysm at the top of the basilar artery. From the cerebral angiographic and autopsy findings, it can be presumed tha

A case of agenesis of the left internal carotid artery, common carotid artery and main trunk of external carotid artery with multiple cerebral aneurysms is presented. No similar case has been reported before. A 70-year-old man who had severe headache was admitted to the neurological department of Matsuyama Shimin Hospital. On admission he demonstrated nuchal rigidity and motor aphasia. CT scan revealed subarachnoid hemorrhage with intracerebral hematoma in the left temporal area. Right common carotid angiography, bilateral retrograde brachial angiography and aortography demonstrated an agenesi

One case of chronic subdural hematoma associated with an unruptured cerebral aneurysm detected by cerebral computed angiotomography is reported. A 44-year-old female slipped and hit her head without loss of consciousness, one month ago. Recently she complained of headaches and visited the department of Neurosurgery, Washokai Sadamoto Hospital on May 21, 1985. There were no physical and neurological signs on examination. Plain CT scans showed a crescent-shaped high density area in the left frontal region with a slight mass sign. She was diagnosed as having a possible chronic subdural hematoma a

The authors summarize the findings of previous studies relating to the natural history of aneurysms and arteriovenous malformations (AVM's). Ruptured aneurysms have their highest rate of rebleeding on Day 1, and at least 50% will rebleed during the 6 months after the first hemorrhage. Thereafter, the rate drops to at least 3% a year. This is the same rate as seen in anterior and posterior communicating artery aneurysms treated by anterior cerebral artery clipping and carotid ligation; these operations provide immediate protection but do not result in long-term diminution of the risk of rebleed

There were thirty-two autopsied cases of ruptured aneurysms at the junction of the internal carotid and posterior communicating arteries in the file of Montefiore Medical Center from 1948 to 1983 (Table 1). The age range of the patients was 11-83 years. Seven were men and twenty-five were women. Fifteen had previous surgery; either clipping of the neck of the aneurysm or ligation of the common carotid artery. Analysis of the hemorrhage associated with the ruptured aneurysms revealed two distinct patterns. One is subarachnoid hemorrhage alone (18 cases) and another is subarachnoid hemorrhage an

Most of "so-called" posterior communicating artery aneurysms previously reported, originated from the internal carotid-posterior communicating junction. Aneurysms arising from the posterior communicating artery itself are very rare. The abducens nerve palsy caused by cerebral aneurysm is also very rare. We are reporting a case with the saccular aneurysm arising directly from the distal half of the posterior communicating artery presenting the abducens nerve palsy. This 73-year-old woman who had no treatment with hypertension for several years was admitted for sudden onset of severe headache, v

A case of subarachnoid hemorrhage complicated by neurogenic pulmonary edema and neurogenic myocardial damage is reported. A 50-year-old woman was admitted following the sudden onset of headache and disturbance of consciousness due to a ruptured internal carotid posterior communicating artery aneurysm on the right side. She showed respiratory failure due to pulmonary edema, which subsequently improved with the mechanical ventilation. After that, she manifested chest distress and hypotensive episode then occurred. An ECG showed QS wave and ST elevation which suggested the presence of inferolater

The incidence of an unilateral oculomotor palsy due to aneurysms in the junction of the internal carotid-posterior communicating artery is about 30 to 40%. The prognosis of these palsies is dependent of several factors. These are: 1) Is the palsy complete or incomplete? 2) Could the aneurysms be surgically treated within 14 days after onset of palsy? 3) Was decompression of the oculomotor nerve achieved by surgery? Generally the levator palpebrae recovers the fastest and most frequently. The bulbus movements have the least tendency to recover. An incomplete palsy has an essentially better prog

A review of a series of 312 giant intracranial aneurysms treated at University Hospital in London, Ontario, showed that 93 of those aneurysms were located between the intracavernous portion and the bifurcation of the internal carotid artery. Sixty-five of those aneurysms were carotid ophthalmic, 12 were located in the internal carotid-posterior communicating-anterior choroidal artery regions and 16 involved the internal carotid artery bifurcation. For the majority, clinical presentation was related to the mass of the aneurysms and compression of surrounding structures such as visual pathways,

An 18-year-old male medical student presented with subarachnoid hemorrhage. Other than stiffness of the neck, the neurological examination yielded normal findings. Bloody cerebrospinal fluid was obtained by lumbar puncture. Bilateral carotid arteriography revealed a right internal carotid artery aneurysm at the level of the posterior communicating artery. The angiogram of the left carotid artery was entirely normal. A right trapping procedure followed. The patient was asymptomatic for 4 years, when a repeat subarachnoid hemorrhage occurred. A new (mirror) aneurysm on the opposite (left) intern

A 40-year-old man had a severe headache of sudden onset and progressive palsy of the third cranial nerve from a minor subarachnoid hemorrhage. Despite assessment by two primary care physicians the possibility of serious neurologic disease was not raised and the urgency of the situation not appreciated. Two weeks after the onset of the headache the patient was urgently admitted to hospital at the request of a neuro-ophthalmologist. Cerebral angiography revealed an aneurysm of the posterior communicating artery, which was surgically occluded with a clip. The patient had a satisfactory recovery,

A case is reported of malignant schwannomatosis (malignant transformation of von Recklinghausen's disease) with catecholamine production in a patient with multiple intracranial aneurysms. The patient had a history of episodic hypertension and elevated levels of catecholamines in the serum and 24-hour urinary excretion. Postmortem examination revealed diffuse central nervous system (CNS) dissemination of the tumor from the thoracolumbar spinal malignant schwannoma. A high concentration of catecholamines was demonstrated in the tumor tissue, and histochemical and electron microscopy studies sugg

This study of the posterior communicating artery (PCoA) in 126 cranial cavities of adult cadavers revealed some hitherto unreported anomalies. These included: the origin of one PCoA from the ophthalmic artery inside the optic canal; the presence of three PCoA's on one side; the occurrence of two posterior cerebral arteries of different origin on one side; and the existence of junctional dilatations at both ends of a PCoA. Junctional dilatation at the commencement of the artery was seen in 6.3% of cases. More common anomalies were a fetal type of posterior cerebral artery in 31.7% and macroaneu

Two patients with persistent trigeminal artery associated with a ruptured arterial intracranial aneurysm arising from the ipsilateral internal carotid artery at the level of the posterior communicating artery are presented. One patient underwent successful clipping of the aneurysm. The other patient died on the x-ray table shortly after admission during the performance of emergency carotid angiography. In each patient, the anomalous vessel was considered to be incidental.

Three patients with cerebral aneurysms newly growing and enlarging for 2 to 10 years are reported. Case 1, a 54-year-old woman, had subarachnoid hemorrhage due to rupture of an intracranial aneurysm, growing from a small residual aneurysmal neck on the left internal carotid artery 10 years after the repair of the aneurysm. Case 2, a 63-year-old man, had a junctional dilatation on the left internal carotid-posterior communicating artery, developing into ruptured aneurysm about 10 years after the first hemorrhage. Case 3, a 52-year-old man, had multiple aneurysms on the bilateral bifurcations of

The charts of 84 patients admitted to the Neurosurgery Service of the Washington University Medical Center between January, 1960, and July, 1981, with aneurysms at or near the junction of the internal carotid and posterior communicating arteries (ICA-PoCA) were reviewed. Special emphasis was placed on the pupillary size and reactivity of 51 patients with oculomotor nerve involvement. Seven initially had normal pupils, representing 8% of the total group and 14% of those presenting with oculomotor palsies. In 4 of these patients pupillary involvement developed within 5 days, and in 1 pupillary i

The patient, 68 year-old, male, was admitted to our clinic with episode of the second attack of subarachnoid hemorrhage in October, 1979. The cerebral angiogram demonstrated an aneurysm arising from the junction of posterior communicating artery and aberrant origin of the anterior choroidal artery. Additionally, bilateral posterior cerebral arteries were filled only through the anterior circulation via the posterior communicating artery. The patient was successfully operated with clipping of the aneurysm through the pterional approach. The aneurysm arising from the posterior communicating arte

A baboon model of subarachnoid hemorrhage (SAH) has been developed to study the changes in cerebral blood flow (CBF), intracranial pressure (ICP), and cerebral edema associated with the acute stage of SAH. In this model, hemorrhage was caused by avulsion of the posterior communicating artery via a periorbital approach, with the orbit sealed and ICP restored to normal before SAH was produced. Local CBF was measured in six sites in the two hemispheres, and ICP monitored by an implanted extradural transducer. Following sacrifice of the animal, the effect of the induced SAH on ICP, CBF, autoregula

In 25 per cent of patients with persistinc primitive trigeminal artery, and in 27 percent of patients with primitive hypoglossal artery, vascular malformations or a history of subarachnoid haemorrhage were found. There is a high incidence of aneurysms at the site of origin of the persisting embryonic vessels. No relation was found between the persisting embryonic vessels and the age, sex, and time of subarachnoid haemorrhage. Opacification of posterior cerebral arteris, vertebral arteries, and posterior communicating arteries was analysed, and variations of the Circulus arteriosus Willisi were

Three hundred and sixty-four patients who suffered a subarachnoid haemorrhage from an aneurysm at either the anterior (ACA) or posterior (PCA) communicating artery location and who were not surgically treated have been followed for up to 21 years in order to study the late morbidity and mortality. For the 213 patients surviving to six months or longer: (1) little improvement in morbidity was noted during the many years of subsequent follow-up; (2) in general, ACA patients fared better than PCA patients; (3) the neurological state on original admission was strongly correlated with the degree of
