From 1973 to 1980, 177 cases with ruptured aneurysms of the anterior communicating artery (Aco) have been operated on using various microsurgical procedures. Since 1974, 136 cases were directly operated on using the microsurgical anterior interhemispheric (AIH) approach: a modification of Lougheed's approach. The overall mortality was 5%. The early operations, within 1 week of onset of subarachnoid haemorrhage had a 3% mortality in grade 1 and 2 patients; a 16% mortality in grade 3 and 4 and a 25% mortality in grade 5 patients. The rate of cases in which the patient was independent following s

In patients with symptomatic aneurysms of the posterior communicating artery, the prognosis of oculomotor palsy mainly depends on the interval between the onset of palsy and the time of operation, and furthermore on the degree of preoperative deficit and the development of the cranial nerve lesion. The incidence of ultimately complete or incomplete palsy is the same in cases with subarachnoid haemorrhage and without rupture ("warning symptom"). In many cases, an initially incomplete paresis develops to a complete ocular palsy within eight days. Ptosis is generally the first symptom, and it fre

Two cases of pulseless disease associated with intracranial aneurysms, one located at the tip of the basilar artery and the other at the left internal carotid-posterior communicating artery junction which developed with episodes of subarachnoid hemorrhage due to their rupture, are presented. Whether the aneurysms and pulseless disease occurred independently or whether the aneurysms developed secondarily from the altered hemodynamic flow produced by pulseless disease is unknown; however, it is certain that abnormal intracranial hemodynamic forces play a role in the development of aneurysms.

Computed tomographic scans in 81 consecutive patients wih subarachnoid hemorrhage due to ruptured aneurysm were analyzed for patterns of hemorrhage and lucency and correlated with the development of spasm and hydrocephalus. The circulation time was measured angiographically in representative cases of spasm. Hemorrhage corresponded in a general way to the fossa of aneurysm origin and, when there was parenchymal or ventricular hemorrhage, in more specific ways to anterior communicating, middle cerebral, and posterior inferior cerebellar artery aneurysms. Basal ganglionic hemorrhages due to aneur

The patient was a 48-year-old housewife, who had a sudden onset of severe headache followed by loss of consciousness for a few hours on the day of admission. Initially she showed slight restlessness due to headache, neck stiffness and subhyaloid hemorrhage. Four-vessel study revealed a basilar aneurysm on right retrograde brachial angiography and anterior communicating aneurysm on left carotid angiography. Two weeks after the onset, when she had no neurological deficit except for intermittent appearance of disorientation, both aneurysms were successfully clipped through right pterional approac

This is a report on 32 patients with subarachnoid haemorrhage caused by rupture of an intracranial arterial aneurysm in the 0-19 age group. Sixteen of the aneurysms were situated in the bifurcation of the internal carotid artery, 8 in the anterior communicating artery, 4 in the posterior communicating artery, two in PICA, one in the middle cerebral artery, and one in the pericallosal artery. Direct intracranial clipping of the neck of the aneurysm was carried out in 26 cases, proximal clipping in three, wrapping in two, and common carotid ligation in one case. There was one death from surgery.

A case of the persistent left primitive hypoglossal artery associated with an intracranial aneurysm was reported. The thirty-year-old man was admitted to hospital in the diagnosis of subarachnoid hemorrhage at seven days after the onset. Bilateral carotid angiograms revealed a saccular aneurysm at the right internal carotid-posterior communicating artery junction and a persistent left hypoglossal artery arising from the left internal carotid artery at the level of the C-1 vertebra. Retrograde vertebral angiograms revealed right hypoplastic vertebral artery (2mm in diameter) and markedly hypopl

The angiographically documented spontaneous thrombosis of an internal carotid-posterior communicating aneurysm in a 63-year-old woman is reported. Serial angiography performed 10, 17, and 29 days after the initial attack of subarachnoid hemorrhage demonstrated progressive thrombosis of the aneurysm. Clinical analysis of this case and a review of the literature suggest that vasospasm and the relative narrowness of the aneurysmal neck played crucial roles in the spontaneous thrombosis of the aneurysm, although neither is definitely responsible.

A 57-year-old woman complained of severe headache and vomiting of a week's duration. Spinal tap showed bloody CSF and cerebral angiography revealed the "true" posterior communicating artery aneurysm. Left frontotemporal osteoplastic craniotomy and trapping of the aneurysm were performed on August 28, 1979. After operation right hemiplegia, left oculomotor palsy and the disturbance of consciousness developed. Postoperative CT scan showed a hemorrhagic infarction at the left basal ganglia. Since then she has been getting better and was discharged, able to walk, from our hospital 6 months after o

Angiographically equivocal aneurysmal dilatation at the junction of the internal carotid and the posterior communicating arteries was noted in a patient with documented subarachnoid hemorrhage. This was initially thought to be infundibular dilatation. A craniotomy revealed an actual aneurysm, which was obliterated successfully. Whether infundibular dilatation is a preaneurysmal state or a benign dilatation is controversial. The management of angiographically noted infundibular dilatation in cases of "essential" subarachnoid hemorrhage is discussed.

A 29-year-old woman with a five-year history of systemic lupus erythematosus was seen after having had a grand mal convulsion, her first manifestation of a seizure disorder. A lumbar puncture revealed the presence of subarachnoid blood, and angiography demonstrated a fusiform aneurysm of the left posterior communicating artery. The patient's neurological status deteriorated despite the use of corticosteroids in high dosage, and she died three weeks after admission. Necropsy disclosed focal transmural angiitis at the site of the ruptured aneurysm. This report describes a radiographically and pa

213 cases of so-called posterior communicating artery aneurysm submitted to surgical operation without using microscope were reviewed from the technical point of view. Aneurysm project boradly to two directions, i.e. postero-inferior and lateral directions. We have treated these anuerysms with two very distinct operative approaches in order to reach the aneurysmal neck from the right angle. Frontal approach is used for the aneurysms projecting laterally. In contrast, the aneurysms projecting postero-inferior are reached laterally by mobilizing the tip of the temporal lobe. Of 213 patients, 15

A patient had a subarachnoid hemorrhage in 1969. Four-vessel angiography demonstrated an infundibular widening of the posterior communicating artery alone. In 1978, nine years later, angiography demonstrated that the infundibular widening had enlarged and developed into an aneurysm, though unruptured. Another aneurysm of the anterior communicating artery was also found. Both were confirmed at operation and clipped. Review of the pertinent literature emphasizes that the infundibular widening may be preaneurysmal.

The author reports on 41 aneurysms of the proximal internal carotid artery (PICA) demonstrated in 36 patients with subarachnoid hemorrhage. The patients included a striking preponderance of women, and there was a high incidence of multiple aneurysms. In cases with multiple aneurysms the PICA aneurysm was usually found incidentally, a more distal aneurysm on the internal carotid artery being the source of hemorrhage. An infundibulum at the origin of a posterior communicating artery was unusually common in these patients. The origin of the ophthalmic artery is proposed as the angiographic landma

The author has operated on 40 patients with giant intracranial aneurysms, using various surgical approaches. Giant aneurysms predominated in females (3:1) and were most common in the age group 30 to 60 years. Patients presented with subarachnoid hemorrhage (17), visual disturbance (18), chronic headache (14), transient or progressive hemispheric deficit (6), seizure (2), dementia (2), and cerebrospinal fluid rhinorrhea (1). Giant aneurysms were located at the carotid artery (25), the basovertebral artery (8), the anterior communicating artery (5), and the middle cerebral artery (2). Eight of 4

A man of 35 years, who had had three attacks of subarachnoid hemorrhage in the previous 3 years, was admitted to hospital with complaints of headache and priapism. There had been intermittent priapism with abnormal acceleration of sexual desire since the first attack, and erection of the penis had persisted with intolerable pain after the last attack of subarachnoid hemorrhage. A carotid angiogram revealed an aneurysm at the junction of the left internal carotid and posterior communicating arteries. Clipping of the aneurysmal neck was successfully performed. However, priapism continued for 22

In order to study the role of sympathetic nervous system and vasoactive amines in cerebral arterial spasm, the following experiments were performed in dogs. In the first experiment, two weeks after bilateral removal of the superior cervical ganglion subarachnoid hemorrhage was produced by the puncture of the posterior communicating artery in six dogs. Second, the arterial blood of non-reserpinized dog was injected into the cisterna magna of another six reserpinized dogs. Diameter changes of the cerebral basal arteries before and after the experimental subarachnoid hemorrhage were observed util

An infundibulum at the origin of the posterior communicating artery is observed to such an extent that it provokes little interest or comment. Although considered preaneurysmal by some, no serious consideration is given to its surgical removal. We present a patient with a subarachnoid hemorrhage, a middle cerebral artery aneurysm, and bilateral posterior communicating artery infundibula to draw attention to the importance of the infundibulum in certain circumstances.

Three hundred sixty-four patients who suffered a subarachnoid hemorrhage from an aneurysm at either the anterior (ACA) or posterior (PCA) communicating artery location and who were not surgically treated have been followed for up to 21 years in order to study the late morbidity and mortality. For patients surviving six months: (1) little improvement in morbidity was noted during the many years of subsequent follow-up; (2) in general, ACA patients fared better than PCA patients; (3) the neurological state on original admission was strongly correlated with the degree of morbidity; (4) other fact
