A new lifeline for little brains: safe and effective intrathecal nicardipine use for refractory vasospasm in paediatric aneurysmal subarachnoid haemorrhage.

We present a case of aneurysmal subarachnoid haemorrhage in a preadolescent teenage patient from a right posterior inferior cerebellar artery origin aneurysm rupture. During coiling they had a re-rupture event with cerebral anoxia which necessitated decompressive craniectomy on bleed day three due to medically refractory intracranial hypertension. The patient then developed severe bilateral V4 and basilar vasospasm. In addition to blood pressure control and fluids, 4mg of intrathecal nicardipine was administered every eight hours into the ventriculostomy for 5 days. Vasospasm was markedly impr

Perimesencephalic non-aneurysmal subarachnoid hemorrhage: is there a need for repeat digital subtraction angiography?

PURPOSE: To assess the diagnostic yield and clinical relevance of repeated digital subtraction angiography (DSA) in patients with spontaneous perimesencephalic non-aneurysmal subarachnoid hemorrhage (PMSAH) and to evaluate DSA-related complication rates. METHODS: Retrospective analysis of 82 patients with PMSAH confirmed by non-contrast CT between March 2002 and February 2025. All patients underwent initial computed tomography angiography (CTA) and first DSA on average within 24 h. If no bleeding source was identified, a second DSA was performed after 10-14 days. Clinical data, radiological f

Traumatic vertebrobasilar pseudoaneurysms: diagnostic pitfalls on CT angiography with forensic implications - two case reports.

Traumatic pseudoaneurysms of the vertebrobasilar artery are rare and may occur without skull fractures. Standard CTA may miss or misinterpret these lesions due to technical limitations and low clinical suspicion, leading to significant diagnostic and medicolegal errors; We present two cases of fatal isolated traumatic subarachnoid hemorrhage (iTSAH), where optimized thin-slice CTA, identified pseudoaneurysms of the PICA and VA (V4 segment), confirmed by histopathology.

Perimesencephalic Hemorrhagic-Pontine Infarction Syndrome: Miler Fisher's Hint to Subarachnoid Paramedian Artery Rupture-A Case Report.

We report a 73-year-old man transferred for evaluation of suspected aneurysmal subarachnoid hemorrhage after his first-ever thunderclap headache episode. It was noted on the second day of the disease left hemiparesis. On MRI, the hemorrhage was of the perimesencephalic type and was associated with an early right paramedian pontine infarction. Angiography did not reveal an aneurismal source for the bleeding, arterial dissection, nor vertebrobasilar vasospasm. Synchronic pontine infarction with perimesencephalic hemorrhage is an unusual syndrome ascribed to the rupture of a perforator superficia

Are microsurgical techniques still indicated for posterior circulation aneurysms? A 10-year longitudinal analysis of clinical and radiological outcomes in the post-endovascular era.

OBJECTIVE: High rates of retreatment remain a concern after endovascular therapy (EVT) for posterior circulation aneurysms. The authors aimed to analyze the indications and outcomes of microsurgery for posterior circulation aneurysms in the endovascular era. METHODS: A single institutional prospectively maintained database was queried from 2015 to 2025. RESULTS: Sixty-seven aneurysms were treated with microsurgery. Females comprised 55.2% (n = 37) of the cohort, and the median age was 55.4 years. Aneurysms were located along the posterior inferior cerebellar artery (PICA) in 23 patients, dis

Simultaneous subarachnoid hemorrhage and cerebral infarction due to aneurysm rupture and occlusion of the parent terminal artery: A cause of angiogram-negative non-perimesencephalic subarachnoid hemorrhage.

Spontaneous subarachnoid hemorrhage (SAH) is most often caused by rupture of an intracranial aneurysm. However, in approximately 15-20% of cases, no clear source of bleeding can be identified. While angiogram-negative perimesencephalic SAH is characterized by localized blood clot distribution and is generally associated with favorable outcome, angiogram-negative non-perimesencephalic SAH presents a more diffuse blood pattern resembling aneurysmal hemorrhage and is linked to worse outcome, and its causes and etiology remain unclear. We hypothesize that rupture of small terminal artery aneurysms

Neuroprotective effects of hesperidin on cerebral vasospasm after subarachnoid hemorrhage in rats: an experimental study.

BACKGROUND: Subarachnoid hemorrhage (SAH) is a severe neurological emergency with high morbidity and mortality, primarily due to vasospasm and delayed ischemia. Hesperidin (HSP), a natural flavonoid, possesses strong antioxidant and vasoprotective properties. This experimental animal study examined HSP's neuroprotective role in oxidative stress and vascular remodeling after SAH, focusing on the extracellular regulated kinase 5-Kruppel-like factor 2-endothelial nitric oxide synthase (Erk5-KLF2-eNOS) pathway. METHODS: The study was conducted from 2021 to 2022. Forty female Wistar albino rats we

Confocal Raman microspectroscopy imaging reveals structural protein reorganization in human intracranial aneurysm tissue.

Intracranial aneurysms are pathological dilatations of cerebral arteries that may rupture and cause subarachnoid hemorrhage. Extracellular matrix remodeling, particularly of Collagen and Elastin, is central to aneurysm progression, but detailed molecular insights remain limited. In this proof-of-concept study, we applied confocal Raman microspectroscopy to matched aneurysmal and healthy basilar artery tissue from the same patient, complemented by an additional control from a second patient. Raman imaging was performed on cryo-sectioned samples using a 532 nm excitation laser. Data were process

Dissecting and partially thrombosed basilar artery aneurysm simultaneously treated with 2 different types of endovascular stents.

Partially thrombosed basilar artery aneurysms are large or giant lesions with a poor prognosis, posing significant treatment challenges. Symptomatic cases may result in brainstem compression, increased stroke risk, or rupture leading to subarachnoid hemorrhage. Endovascular therapy is the preferred treatment over surgical clipping due to lower morbidity and mortality rates. However, outcomes depend on aneurysm location, morphology, and device selection. Stent-assisted coiling is widely used, with the Accero and Acclino stents offering distinct advantages. The Accero stent, a self-expanding bra

Management of Intracranial Perforator Aneurysms.

Intracranial perforator aneurysms are rare but potentially life-threatening vascular lesions that can lead to subarachnoid hemorrhage. Initial imaging is frequently negative, so a high level of suspicion is important for efficient detection of perforator aneurysms. The management of these lesions is challenging due to their location and delicate surrounding structures. Conservative management, endovascular treatment, and microsurgical clipping have been used to manage perforator aneurysms successfully. Because perforator aneurysms remain an underappreciated source of subarachnoid hemorrhage ev

Effects of colchicine on SAH-induced vascular damage in an experimental subarachnoid hemorrhage model.

BACKGROUND/AIM: Subarachnoid hemorrhage (SAH) is a severe cerebrovascular condition associated with high morbidity and mortality. Inflammation and oxidative stress play critical roles in its pathophysiology, especially in the development of secondary vascular damage. This study aimed to evaluate the protective effects of colchicine on vascular integrity and biochemical markers following SAH in an experimental rat model. MATERIALS AND METHODS: Eighteen female Sprague Dawley rats were randomly divided into 3 groups: Control (no SAH), SAH (induced without treatment), and SAH + colchicine (receiv

Intravenous thrombolysis as an adjunct to endovascular thrombectomy in acute basilar artery occlusion: a GRADE-assessed systematic review and meta-analysis.

Acute basilar artery occlusion (BAO) is a rare but devastating type of ischemic stroke that significantly impacts outcomes. This systematic review and meta-analysis aim to evaluate the efficacy and safety of combining IVT with EVT (EVT + IVT) versus EVT alone in treating acute BAO, focusing on functional independence, mortality, reperfusion success, and hemorrhagic and procedural complications. We conducted a comprehensive search up to May 2025. Effect sizes were expressed as odds ratios (ORs) with 95% confidence intervals (CIs), and statistical significance was determined using p-values. Successful conservative management for ruptured basilar artery perforator aneurysm associated with early hematoma expansion in right temporal lobe without aneurysm rerupture: illustrative case.

BACKGROUND: Early intracerebral hematoma (ICH) expansion without aneurysm rerupture is extremely rare in patients with a ruptured basilar artery perforator aneurysm (BAPA), and the treatment strategy has not been established. OBSERVATIONS: A 47-year-old male was rushed to the authors' hospital due to loss of consciousness. CT imaging showed diffuse subarachnoid hemorrhage (SAH) and ventriculomegaly, and 3D CT angiography (CTA) showed a BAPA. Ventricular drainage was performed, and conservative treatment for the BAPA was started. However, CT imaging on SAH day 1 showed an enlarged ICH in the r

Microsurgical clipping of ruptured wide-neck Aneurysms: A comparative analysis with Woven EndoBridge data.

BACKGROUND: Due to their complex configuration, wide-neck aneurysms (WNAs) present significant challenges. The intrasaccular Woven EndoBridge (WEB) device was introduced in 2010. However, complete occlusion rates with WEB remain relatively low (40-60 %). The aim of this study was therefore to evaluate our institutional experience with microsurgical clipping of ruptured WNAs and to compare these results with a cohort of WNAs treated with the WEB device. METHODS: A retrospective study of consecutive adult patients with ruptured WNAs who underwent microsurgical clipping at our institution betwee

Microsurgical Clipping of Blood Blister-like Anterior Communicating Artery Aneurysm in a Patient with Total Occlusion of Left Internal Carotid Artery Aneurysm.

Blister-like aneurysms represent a rare subtype characterized by a wide neck and dissecting appearance, predisposing them to perioperative rebleeding due to fragile walls. These aneurysms predominantly occur at non-branching sites of the internal carotid artery (ICA). Still, they may also manifest at atypical locations, including the anterior communicating artery (AComA), anterior cerebral artery, middle cerebral artery, posterior cerebral artery, and basilar artery.1 Treatment of blister-like aneurysms typically involves a combination of microsurgery and endovascular procedures, with no unive

Double origin of the superior cerebellar artery from the Basilar artery and the P1 segment of the posterior cerebral artery.

A 50-year-old patient admitted for a subarachnoid hemorrhage underwent a successful surgical clipping of two aneurysms involving the anterior communicating artery and the supraclinoïd and ophthalmic segments of the right internal carotid artery. The analysis of the vertebrobasilar system through the brain computed tomography angiography and 3D modeling revealed a double origin of the right superior cerebellar artery from the basilar artery and the posterior cerebral artery. Two channels arose separately from the basilar artery and the P1 segment of the right posterior cerebral artery then merg
