Severe Recurrent Hemorrhage of an Oncogenic Vertebrobasilar Junction Aneurysm Following Stent-Assisted Coil Embolization in Metastatic Testicular Choriocarcinoma.

Oncogenic cerebral aneurysms (OCAs) are rare, catastrophic lesions typically arising from distal middle cerebral artery (MCA) branches. We present a case involving an atypical OCA at the vertebrobasilar (VB) junction secondary to metastatic mixed testicular cancer predominantly comprising choriocarcinoma, which resulted in fatal rebleeding despite initial stent-assisted coil embolization. A male in his 30s with metastatic choriocarcinoma and leptomeningeal carcinomatosis presented with ambiguous subarachnoid hemorrhage (SAH) on initial MRI. Subsequent CT angiography (CTA) identified an outpouc

Active Cerebrospinal Fluid Exchange Versus External Ventricular Drainage in IVH and SAH: A Meta-Analysis.

BACKGROUND: Intraventricular hemorrhage and subarachnoid hemorrhage are associated with neuroinflammation and increased morbidity and mortality. Rapid clearance of blood from the cerebrospinal fluid space may mitigate neuroinflammatory cascades and improve clinical outcomes. This systematic review and meta-analysis aimed to compare the clinical effectiveness of active cerebrospinal fluid exchange, including neuroendoscopic lavage and IRRAflow systems, versus external ventricular drainage in patients with intraventricular hemorrhage or subarachnoid hemorrhage. METHODS: The study followed Prefe

'Carotid spells' - transient focal neurological episodes associated with convexity subarachnoid haemorrhage in severe ipsilateral extracranial internal carotid artery stenosis: a case report and concise literature review.

BACKGROUND: Convexity subarachnoid haemorrhage (cSAH) is most frequently caused by cerebral amyloid angiopathy (CAA), often presenting with transient focal neurological episodes (TFNEs, a.k.a. amyloid spells). In younger patients, posterior reversible encephalopathy, reversible cerebral vasoconstriction syndrome, and sinus thrombosis are the most frequent aetiologies. Reports on extracranial internal carotid artery (ICA) stenosis-associated cSAHs are exceptional. CASE PRESENTATION: A 46-year-old female with a history of heavy smoking presented with repetitive stereotypical right-sided sensory

Roles of Sirtuin-1 in the Prevention of Intracranial Aneurysm Rupture in Mice.

BACKGROUND: SIRT1 (sirtuin-1) regulates various cellular and metabolic processes in the vasculature. SIRT1 is an NAD+-dependent deacetylase that deacetylates transcription factors that control vascular inflammation and remodeling. Given that pathways regulated by SIRT1 are implicated in the pathophysiology of intracranial aneurysms, we hypothesized that activation and overexpression of SIRT1 prevent aneurysm rupture by suppressing vascular inflammation. METHODS: Intracranial aneurysms were induced in mice by systemic hypertension and a single intracisternal injection of elastase. We analyzed

Late-Onset Contrast-Induced Encephalopathy After Stent-Assisted Coiling: A Case Report and Review of the Literature.

INTRODUCTION: Contrast-induced encephalopathy (CIE) is a rare condition with a broad spectrum of neurological symptoms. With the increasing use of endovascular techniques, the disease is becoming ever more important. To date, recent studies have primarily described patients with early-onset CIE, in which symptoms appear shortly after the intervention. This case report represents one of the few reported cases of late-onset CIE. CASE DESCRIPTION: A 53-year-old woman with a re-recurrent anterior communicating artery aneurysm with coil impaction and a history of two aneurysmal subarachnoid hemorr

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

PMMNet: A Dual Branch Fusion Network of Point Cloud and Multi-View for Intracranial Aneurysm Classification and Segmentation.

Intracranial aneurysm (IA) is a vascular disease of the brain arteries caused by pathological vascular dilation, which can result in subarachnoid hemorrhage if ruptured. Automatically classification and segmentation of intracranial aneurysms are essential for their diagnosis and treatment. However, the majority of current research is focused on two-dimensional images, ignoring the 3D spatial information that is also critical. In this work, we propose a novel dual-branch fusion network called the Point Cloud and Multi-View Medical Neural Network (PMMNet) for IA classification and segmentation.

Association between cerebral oxygenation and usual parameters of cerebral perfusion in critically ill patients with acute brain injury.

In patients with acute brain injury (ABI), optimizing cerebral perfusion parameters relies on multimodal monitoring. This include data from systemic monitoring-mean arterial pressure (MAP), arterial carbon dioxide tension (PaCO2), arterial oxygen saturation (SaO2), hemoglobin levels (Hb), and temperature-as well as neurological monitoring-intracranial pressure (ICP), cerebral perfusion pressure (CPP), and transcranial Doppler (TCD) velocities. We hypothesized that these parameters alone were not sufficient to assess the risk of cerebral ischemia. We conducted a retrospective, single-center stu

Has a fast treatment transition from surgical to endovascular operations improved the survival of aneurysmal subarachnoid hemorrhage?

BACKGROUND: Several studies have attributed decreasing case fatality rates (CFRs) of aneurysmal subarachnoid hemorrhage (aSAH) to the gradually increasing use of endovascular treatment without considering improvements in other outcome-affecting factors. To assess the independent effect of a treatment modality on CFRs, we investigated CFR changes in a high-volume center rapidly transitioning from surgical to endovascular operations as the first-line treatment for all aSAH patients except those with middle cerebral artery (MCA) aneurysms. METHODS: We identified all surgically/endovascularly tre

Evaluating the Balloon-Assisted Rapid Intermittent Sequential Coiling Technique for the Treatment of Wide-Neck Cerebral Aneurysms.

BACKGROUND: Balloon-assisted coiling (BAC) is acknowledged as an auxiliary method for the endovascular treatment of difficult wide-necked cerebral aneurysms (WNCAs). An intracranial stent may be necessary as a supportive scaffold when the anatomical conditions are unfavorable, as BAC alone may be inadequate to avoid coil protrusion into the parent artery. We aimed to evaluate the safety of the balloon-assisted rapid intermittent sequential coiling technique (BARISCT) and the effectiveness of BARISCT in reducing the risk of coil prolapse in the primary artery during the coiling of WNCA. METHOD

Invasive aspergillosis leading to fatal cerebral hemorrhage: a case report and comprehensive literature review.

Aspergillus fumigatus significantly threatens immunocompromised individuals by causing potentially fatal diseases, primarily invasive aspergillosis (IA). IA, mostly affecting the lungs, can disseminate to other organs, including the central nervous system (CNS), leading to high mortality rates. We present the case of an 84-year-old immunocompetent male with a history of significant comorbidities, who developed a lethal subarachnoid and tetraventricular hemorrhage. Initial symptoms included paroxysmal neuralgic pain and ophthalmic manifestations. Despite diagnostic efforts, the infection was co

Decompressive craniectomy in aneurysmal subarachnoid hemorrhage: can favorable outcome be achieved?

BACKGROUND: Decompressive craniectomy (DC) is a last-tier treatment for managing refractory intracranial hypertension in patients with aneurysmal subarachnoid hemorrhage (aSAH), though concerns persist about whether it primarily prolongs survival in a state of severe disability. This study investigated patient characteristics, surgical indications, complications, and outcomes following DC in aSAH. METHODS: In this Swedish, retrospective multi-center study, 123 aSAH patients treated with DC between 2008-2022 were included. Data collection included demographic details, aSAH characteristics, inj

Embolization of a spontaneous middle meningeal arteriovenous fistula that caused intracranial hemorrhage due to aneurysm rupture.

A middle meningeal arteriovenous fistula (MMAVF) is a direct arteriovenous shunt between the middle meningeal artery (MMA) and adjacent venous structures. Most MMAVFs result from MMA tears due to trauma or iatrogenic injury. Spontaneous MMAVF as a result of true MMA aneurysm rupture is rare. Here, we report such a rare case. A 60-year-old male presented with subarachnoid, ventricular and epidural hemorrhages. He was previously healthy and denied a recent history of head injury or surgery. Upon examination, he was alert and could answer questions correctly. Computed tomography (CT) did not reve

Characteristics of Cerebral Infarction Due to Anterior Cerebral Artery Dissection: A Review of 16 Cases.

OBJECTIVE: Intracranial artery dissection is a recognized cause of cerebral infarction and subarachnoid hemorrhage. In Japan, anterior cerebral artery dissection (ACD) represents one of the causes leading to ischemia. This study compares our clinical experience with existing literature to characterize the distinctive features of ischemia-onset anterior cerebral artery dissection (infarction-anterior cerebral artery dissection (iACD)). MATERIALS AND METHODS: We conducted a retrospective analysis of 16 iACD cases identified among 2,776 cerebral infarction patients hospitalized between 2010 and
