Subarachnoid hemorrhage (SAH) is a life-threatening cerebrovascular disorder characterized by high mortality and unfavorable long-term outcomes. During the COVID-19 pandemic, SARS-CoV-2 infection has been associated with systemic inflammation and coagulopathy, which may result in cerebrovascular complications such as intracranial hemorrhage (ICH). However, the characteristics and outcomes of COVID-19-associated SAH remain unclear, and favorable outcomes are rarely reported. We encountered two male patients with COVID-19-associated SAH, one with a non-aneurysmal SAH and another with a ruptured

This case study aims to describe the utility of RAPID for ANGIO software (RAPID ANGIO), a flat-panel detector CT-based perfusion and CT angiography-like imaging tool, in the supplementary diagnosis of symptomatic cerebral vasospasm (CVS) following subarachnoid hemorrhage (SAH). Three SAH patients were suspected of having symptomatic CVS, manifested by sudden disturbances of consciousness or focal neurological deficits during the periprocedural period of endovascular treatment (EVT) for ruptured intracranial aneurysms. RAPID ANGIO was used to evaluate vessel narrowing, infarct core, and hypoper

BACKGROUND: Thalamoperforating artery (TPA) pseudoaneurysms are relatively uncommon and often pose significant challenges in detection through vascular examination. Furthermore, their management, particularly via endovascular therapy, is complex and demanding. This article presents a case study of a patient diagnosed with an acute TPA pseudoaneurysm, highlighting its rarity and the intricacies involved in its treatment.
CASE PRESENTATION: A 49-year-old male was admitted to our hospital presentingwith headache and vomiting, with a medical history notable for hypertension and previous cardiac s

Patients with World Federation of Neurological Surgeons (WFNS) grade V aneurysmal subarachnoid hemorrhage (SAH) have poor outcomes, and no effective pharmacological treatment has been established. This study aimed to investigate whether any of the drugs currently used in Japan, where nimodipine is not approved, are effective for WFNS grade V SAH patients. From a database of SAH patients prospectively enrolled at nine hospitals in Japan between 2013 and 2024, 297 patients (mean age, 67.3 years) who underwent microsurgical or endovascular treatment for ruptured aneurysms and had WFNS grade V SAH

Introduction Autosomal dominant polycystic kidney disease (ADPKD) is associated with an increased prevalence of intracranial aneurysms (IAs), but the clinical factors influencing IA development remain unclear. This study aimed to identify the clinical factors associated with the presence or absence of IAs in patients with ADPKD. Methods We conducted a retrospective cohort study of 138 Japanese patients with ADPKD who underwent neuroradiological imaging between January 2004 and December 2020. Patients were categorized into IA (n=30) and non-IA (n=108) groups. Clinical characteristics, includi

OBJECTIVE: Whether radiological cerebral vasospasm (CVS) should be proactively treated following subarachnoid hemorrhage to reduce the development of delayed cerebral ischemia (DCI) and brain infarction remains unclear. In this bicentric retrospective study, we sought to determine if a first-line intravenous therapy (IVT) to early detect and treat CVS could be noninferior on neurological outcomes to a first-line endovascular therapy (ET) requiring intra-arterial procedures for confirmed CVS as the reference approach.
METHODS: Two cohorts of unselected patients exposed to the risk of CVS were

Dural arteriovenous fistula (dAVF) is thought to arise from triggers like venous sinus thrombosis or trauma, though many aspects remain unclear. We report the case of a 48-year-old woman who developed multiple asymptomatic dAVF-like findings two weeks after coil embolization for subarachnoid hemorrhage (SAH). Initial angiography showed no dAVFs. Postoperative angiography showed cavernous sinus and middle meningeal artery dAVF-like findings without cortical venous reflux. Conservative management was chosen, and follow-up angiography at three months showed spontaneous resolution. The incidental

BACKGROUND AND OBJECTIVES: Central retinal artery occlusion (CRAO) results in severe visual impairment. Intravenous thrombolysis (IVT) is a potential treatment for CRAO, but ambiguous data regarding efficacy and the possibility that the presence of cerebral infarctions may increase the risk of symptomatic intracranial hemorrhage (sICH) raise concerns. The aim of the current study was to evaluate the safety of IVT in patients with CRAO.
METHODS: This multi-center, international registry study included 66 patients with CRAO that presented within 4.5 h from symptom onset. Patients that received

Despite intracranial aneurysms smaller than 5 mm are generally considered to have a low risk of rupture, management of those located at the anterior communicating artery (ACoA) remains a significant clinical challenge. This study examined the association between ACoA location and rupture tendency in aneurysms smaller than 5 mm. We retrospectively reviewed patients diagnosed with intracranial aneurysms at our institution from January 2015 to September 2022. After excluding unruptured aneurysms, re-ruptures at previous surgical sites, and aneurysms with fusiform, dissecting, or blood blister mor

Endoscopy's ability to provide close observation, deep magnification, and multi-angle views has proven to be an effective tool for minimally invasive craniotomy in neurosurgery. However, no large case series have been published on the use of fully endoscopic-assisted minimally invasive keyhole craniotomy for clipping intracranial aneurysms (IAs). To evaluate the value of fully endoscopic-assisted minimally invasive keyhole craniotomy in the treatment of anterior circulation aneurysms. A retrospective analysis was conducted on 20 patients who underwent fully endoscopic-assisted minimally keyhol

Terson syndrome (TS) is an intraocular hemorrhage associated with subarachnoid hemorrhage (SAH), often linked to specific intracranial aneurysm locations. This review and meta-analysis aim to explore the relationship between aneurysm location and the risk of developing TS. A systematic search was conducted using PubMed and Scopus with the terms "Terson's Syndrome" OR "Terson Syndrome." Studies involving adult patients in English were included. Data on aneurysm location, TS incidence, and clinical outcomes were extracted. The risk of bias was assessed using the Risk Of Bias In Non-randomized St

We report a rare case of a 60-year-old woman who initially presented with subarachnoid hemorrhage and was later diagnosed with a pituitary abscess 11 months after coil embolization. Poor oral hygiene and cultures positive for Escherichia coli (E. coli) from both the throat and pituitary abscess pus suggested that the infection had a dental origin. Because of the rarity of aneurysms with atypical morphology and location, the previously treated multiple intracranial aneurysms-including an anterior communicating artery aneurysm, a true posterior communicating artery aneurysm, and an anterior wall

Cerebral aneurysms are localized dilations occurring at weakened areas within the brain's arterial circulation. They often occur at the branching points of smaller vessels and are usually saccular in shape, but they can also have fusiform or blister-type shapes. Internal carotid artery (ICA) aneurysms are believed to represent 30 to 50% of all intracranial aneurysms. Most cerebral aneurysms are asymptomatic and are often discovered incidentally during neuroimaging or autopsy. When rupture occurs, it often leads to subarachnoid hemorrhage, which is associated with high morbidity and mortality.

Subarachnoid purulence is generally formed secondary to the rupture of brain abscesses. Early diagnosis of subarachnoid purulence unrelated to brain abscesses is challenging because of the lack of specific neurological or radiological findings. In this article, we report a case of localized subarachnoid purulence mimicking aneurysmal subarachnoid hemorrhage. A 74-year-old woman with diabetes and a history of rhinosinusitis presented with a sudden deterioration of the level of consciousness, tonic posture, and fever. A head computed tomography (CT) scan revealed a high-density area in the left

Background and Clinical Significance: Although rupture of aneurysms at the internal carotid-posterior communicating artery (ICA-PCom) junction accounts for a small percentage of all ruptured intracranial aneurysms, they are clinically relevant due to their proximity to perforator-rich cisterns, the optic-carotid-oculomotor pathways and flow-diverting zones, as well as their high likelihood for causing early neurological instability. Additionally, ruptured ICA-PCom aneurysms that have multiple lobulations are associated with increased variability in wall shear stress, local inflammatory remodel

Brain ultrasonography has emerged as a valuable diagnostic and monitoring tool in intensive care, complementing other point-of-care techniques. Transcranial colour-coded duplex (TCCD) offers a rapid, non-invasive, repeatable, and cost-effective bedside me-thod that has gained significant interest among intensivists. TCCD enables visualisation of the circle of Willis and adjacent cerebral arteries, supporting its use in various critical scenarios, including vasospasm following subarachnoid haemorrhage, elevated intracranial pressure, hydrocephalus, acute ischaemic stroke, cerebral circulatory a

Background and objectives Several Asian populations abroad are reported to have a higher prevalence of spontaneous intracranial hemorrhages (sICH) and a greater proportion of all stroke types attributed to ICH compared to non-Asians. However, the causes are unknown, and few studies have examined the issue among Asian Americans. This report aims to highlight some less common, but not rare, clinical features that could bear on several pathophysiological factors, by presenting a selected case series of 13 Asian American patients admitted to a Boston-based healthcare system and hospital. Methods T

BACKGROUND: Infective endocarditis (IE) is a rare, but potentially fatal, infectious disease. One of the common complications of IE is the embolization of endocardial vegetation with subsequent intracerebral artery obstruction that causes acute ischemic stroke. Herein, we present a case report of a patient presenting with a neurological manifestation that turned out to be a complication of IE.
CASE ILLUSTRATION: We present the case of a patient with a chief complaint of left-sided hemiplegia. Blood test results revealed signs of infection. Computed tomography (CT) of the head revealed extensi

BACKGROUND: Cerebral damage after aneurysmal subarachnoid hemorrhage (SAH) results from various, sometimes unrelated causes. After the initial hemorrhage trauma with an increase in intracranial pressure, induced vasoconstriction, but also microcirculatory disturbances, inflammation and pathological electrophysiological processes (cortical spreading depolarization) can occur in the course of the disease, resulting in delayed cerebral ischemia (DCI). In the neuroradiological context, cerebral vasospasm (CVS) remains the focus of diagnostic imaging and endovascular therapy as a frequent component
