Pure acute subdural hematomas (ASDHs) due to ruptured aneurysms without subarachnoid or intracerebral hemorrhage are rare. We report the case of a 26-year-old female who presented with a pure ASDH caused by a ruptured distal anterior cerebral artery (ACA). The patient complained of sudden headache and vomiting and was transferred to our hospital. On the ambulance journey to the hospital, her consciousness level decreased suddenly just after experiencing additional pain in the head. At admission, the consciousness level was 4 points on the Glasgow coma scale with bilateral pupil dilatation. Com

Aneurysmal subarachnoid hemorrhage(SAH) causes brain injury and systemic complications, including cardiopulmonary dysfunction, which mutually affect each other. Post-SAH brain injury includes early brain injury(EBI) and delayed cerebral ischemia(DCI). EBI is a non-iatrogenic pathology occurring within 72 h of clinical SAH, primarily induced by increased intracranial pressure, subsequent transient global cerebral ischemia, and extravasated blood components. DCI typically develops between days 4 and 14 after clinical SAH because of erythrolysis(free hemoglobin) and EBI-mediated reactions. EBI an

Lactococcus garvieae is a known fish pathogen associated with numerous aquacultural outbreaks. In humans, L. garvieae primarily causes infective endocarditis, but infections involving other organs have also been reported. We report the first case of ruptured infectious intracranial aneurysm associated with L. garvieae bacteraemia without concomitant infective endocarditis. The diagnosis of a left distal posterior cerebral artery mycotic aneurysm was based on a computed tomography angiogram, catheter angiogram and histopathological examination of the resected aneurysm. Here, we review the liter

We describe a 31-year-old woman who presented with acute right-sided weakness and was found to have a subarachnoid hemorrhage in the left Sylvian fissure and an ipsilateral frontal intraparenchymal hemorrhage. CT angiography revealed an occlusion of the left middle cerebral artery's M1 segment and a saccular aneurysm at its bifurcation. A cerebral angiogram confirmed these findings, and the patient subsequently underwent microsurgical aneurysm resection, which revealed a partially thrombosed pseudoaneurysm. Further stroke workup identified mitral valve vegetation, confirming the diagnosis of i

A non-aneurysmal subarachnoid hemorrhage (SAH) due to an aplastic or twig-like middle cerebral artery (Ap/T-MCA) is extremely rare. We describe a case of a non-aneurysmal SAH due to an Ap/T-MCA. A 38-year-old female patient experienced a sudden severe headache and was admitted to our hospital. A head CT scan revealed an SAH. Cerebral angiography revealed a non-aneurysmal subarachnoid hemorrhage due to an Ap/T-MCA. The patient was treated conservatively and had a good recovery. Because of the complexity of the vascular structure, we had difficulty in making a diagnosis of our case. Cone beam CT

Reversible cerebral vasoconstriction syndrome (RCVS) is a rare but significant cause of intracranial arteriopathy and stroke in young adults. The syndrome encompasses a spectrum of disorders radiologically characterized by reversible narrowing and dilation of intracranial arteries, often triggered by vasoactive drugs or the postpartum period. The hallmark clinical feature of RCVS is thunderclap headache with or without other neurological signs. Though endothelial dysfunction and sympathetic hyperactivation are hypothesized to be key mechanisms, the exact pathogenesis of RCVS is still unclear.

BACKGROUND: Subarachnoid hemorrhage is a severe neurological condition that requires prompt and appropriate treatment to prevent complications. Aneurysms are the most common cause of spontaneous subarachnoid hemorrhage. Conversely, basilar artery perforator aneurysms (BAPAs) are a rare etiology. There is no consensus on the optimal management of ruptured BAPAs in the acute setting.
CASE SUMMARY: We present a case series of 3 patients with ruptured BAPAs who were treated at our institution. Two patients had a modified Fisher grade of I, and one had a grade of IV on initial presentation. The an

Tentorial dural arteriovenous fistulas (DAVFs) are rare but highly dangerous vascular anomalies, constituting a small percentage of all intracranial DAVFs. Despite their infrequency, these lesions display aggressive characteristics, frequently leading to hemorrhage or neurological deficits due to their retrograde drainage into leptomeningeal veins, thus classifying them as Borden type III lesions. This case presents a middle-aged man who suffered cerebellar and subarachnoid hemorrhages resulting from a medial tentorial DAVF. Initial imaging revealed a high-flow vascular lesion, which was subse

Posterior inferior cerebellar artery (PICA) dissecting aneurysms are rare and typically present with subarachnoid hemorrhage (SAH) or ischemic symptoms, with a high risk of rebleeding in the acute phase. This case presents an atypical ruptured PICA aneurysm with a hematoma confined to the craniocervical junction and cervical cord, leading to a delayed diagnosis - a 41-year-old male with an atypical presentation of headache and neck pain without neurological deficits. Initial magnetic resonance imaging (MRI) revealed a hematoma extending from the craniocervical junction to the cervical spinal c

BACKGROUND: Endovascular transmural targeting of cervical and cranial perivascular structures is a novel approach for minimally invasive delivery of therapeutics. Components of the autonomic nervous system are in close anatomic proximity to major cervical vasculature and, therefore, represent potential targets for intervention. The superior cervical ganglion (SCG) is a discrete structure of interest for this approach, as sympathetic blockade may have therapeutic effects for various conditions. Variability of SCG location and its relationship to large cervical vessels, and the feasibility of en

Intracranial aneurysms, characterized by abnormal dilations of cerebral arteries, pose significant health risks due to their potential to rupture, leading to subarachnoid hemorrhage with high mortality and morbidity rates. This paper aim is to explore the innovative application of nanoparticles in treating intracranial aneurysms, offering a promising avenue for enhancing current therapeutic strategies. We took into consideration the pathophysiology of cerebral aneurysms, focusing on the role of hemodynamic stress, endothelial dysfunction, and inflammation in their development and progression.

BACKGROUND: Local angiotensin activity is thought to play a critical role in arterial wall homeostasis and remodeling, and impaired arterial wall integrity contributes to the pathogenesis of subarachnoid hemorrhage (SAH). Here we aimed to assess the association between pharmacologic inhibition of angiotensin-converting enzyme and subsequent nontraumatic SAH.
METHODS: In a retrospective cohort study based on Optum's Clinformatics Datamart deidentified database records (2000-2021), we included all subjects with a history of hypertension. We collected medication history, and the exposure of inte

The clinical manifestation of intracranial mycotic aneurysms (ICMAs), which are rare but carry high risks of rupture and early mortality, remains poorly understood. We present a case of an ICMA that rapidly developed and ruptured after the diagnosis of meningitis caused by Streptococcus salivarius (S. salivarius), which rarely causes bacterial meningitis. A 54-year-old man presented with a headache that worsened on coughing, without altered consciousness or vomiting. He was diagnosed with pneumocephalus due to a meningoencephalocele, which was detected on contrast-enhanced magnetic resonance i

Spontaneous convexity subarachnoid hemorrhage (cSAH) is a vascular disease different from aneurysmal SAH in neuroimaging pattern, causes, and prognosis. Several causes might be considered in individual patients, with a limited value of the patient's age for discriminating among these causes. Cerebral amyloid angiopathy (CAA) is the most prevalent cause in people > 60 years, but reversible cerebral vasoconstriction syndrome (RCVS) has to be considered in young people. CAA gained attention in the last years, but the most known manifestation of cSAH in this context is constituted by transient foc

This comprehensive study analyzes 346 surgically treated intracranial aneurysms, emphasizing the importance of understanding risk factors and prevalent characteristics in patients. Intracranial aneurysms, primarily of the saccular or berry type, significantly contribute to nontraumatic subarachnoid hemorrhages and demonstrate a rising incidence due to advances in imaging techniques. The study highlights a gender discrepancy in aneurysm occurrence and a higher prevalence in individuals over 30 years old. The research delves into various aspects, including aneurysm localization, diameter, neck d

Anterior communicating artery (ACom) aneurysm, one of the most frequent types of intracranial aneurysm rupture, usually results in a subarachnoid hemorrhage (SAH) with intraventricular hemorrhage. We describe a case of an ACom aneurysm rupture with subarachnoid, intraparenchymal, and subdural hemorrhages (SDH) with midline shift in a 55-year-old woman. Decompressive craniectomy was performed to evacuate the SDH with subsequent microsurgical clipping of the aneurysm. Postoperative angiogram showed occlusion of the ACom aneurysm without residual flow. Mechanisms to explain multimodal bleeding wi
