Experimental subarachnoid hemorrhage in the cynomolgus monkey: evaluation of treatment with hypertension, volume expansion, and ventilation.

We evaluated the treatment regime of dopamine-induced hypertension in association with volume expansion and ventilatory support in an experimental subarachnoid hemorrhage (SAH) model using the cynomolgus monkey. Regional cerebral blood flow, vessel caliber, intracranial pressure, and other pertinent physiological parameters were monitored throughout each study. We report the results for nine animals receiving treatment after an induced SAH and compare them with results obtained in a group of five animals not treated after SAH. Improvements in cerebral blood flow, vessel caliber, and morbidity

A pathological study of intracranial aneurysms particularly of aneurysms other than saccular ones.

The author has investigated grossly and microscopically 90 intracranial aneurysms among 63 individuals, autopsied at the Tokyo-to Medical Examiner Office and partly at the 2nd Department of Pathology, Showa University School of Medicine. These aneurysms consisted of 74 of the saccular type, 5 of the arteriosclerotic type, 6 of the spindle-shaped aneurysm of the vertebrals and 5 of the dolichoectatic type, respectively. In this paper, the author has discussed the differences of pathogenesis of the above mentioned four types of aneurysms. The saccular aneurysm is thought to occur from turbulence

[Ruptured ICPC junction aneurysm revealing angiographically only small bulging (author's transl)].

Two cases revealed only small bulging at ICPC junction on carotid angiogram after subarachnoid hemorrhage were reported. One case was treated conservatively and other was operated. The diagnosis and operative indication of these cases were discussed. The authors think that all of these cases, whether these bulging are classified angiographically as aneurysm or junctional dilatation, should be operated positively when CT scan shows high density area due to subarachnoid hemorrhage around them and there are no other findings inducing subarachnoid hemorrhage on angiograms.

Surgical treatment of asymptomatic and incidental intracranial aneurysms.

A consecutive series of 78 patients underwent surgery for 106 aneurysms between 1972 and 1978; this group included 14 patients with subarachnoid hemorrhage who had multiple, unruptured, incidental aneurysms (20 additional aneurysms). These aneurysms were operated on with no mortality. In 15 other patients, who underwent angiography for various nonhemorrhagic disorders, a total of 18 asymptomatic aneurysms were found. These were also operated on with no mortality. Three of the 29 patients had postoperative hemiplegia, which persists in only one. The author recommends that all aneurysms should b

SIADH after subarachnoid hemorrhage and craniotomy.

A 64-year-old woman had the syndrome of inappropriate ADH secretion (SIADH) after surgical repair of a ruptured aneurysm of the anterior communicating artery. SIADH began while the intracranial pressure was elevated, and resolved when the pressure returned to normal. Whether this syndrome was caused by direct hypothalamic lesions, local pressure on the hypothalamus secondary to enlargement of the third ventricle, or some other mechanism cannot be ascertained. The occurrence of intracranial pressure changes with SIADH is described.

The clinical and pathological spectrum of brain-stem vascular malformations. Long-term course stimulates multiple sclerosis.

Three cases of brain-stem vascular malformation with progressive and fluctuating clinical courses of longer than one year were studied. One patient with a rare brain-stem arteriovenous malformation lived for 20 years with symptoms notably similar to those of multiple sclerosis (MS). One case each of cavernous hemangioma and capillary telangiectasis are also described. The clinical, roentgenographic, and pathologic features of these three cases plus 18 other cases of brain-stem vascular malformations from the literature, with progressive or intermittent clinical courses and with survival of lon

Angiography study of cerebral vasospasm following the rupture of intracranial aneurysms: Part II. Relation between the site of aneurysm and the occurrence of the vasospasm.

The relation between the occurrence of vasospasm and the site of ruptured aneurysm was investigated. Vasospasm tended to occur initially on arteries close to the aneurysm, later extending to other locations. Contralateral spasm was more frequent the closer the site of the aneurysm to the mid-sagittal line. The spasm observed angiographically varies in relation to the site of the aneurysm; the site of the hematoma in the subarachnoid space was supposed to be the cause of such variation.

[Posterior subtemporal transtentorial approach for a lower basilar trunk aneurysm (author's transl)].

A lower basilar trunk aneurysm is rare and it has been difficult to operate on this kind of aneurysm which is located in so-called "no man's land". We have recently operated on the aneurysm which was located between the vertebrobasilar junction and the origin of AICA. The aneurysm was approached posterior-subtemporal-transtentorially and wrapped with a muscle piece because of its broad neck. After the operation the patient developed amnestic aphasia which, however, disappeared 4 months postoperatively. The advantage of this approach is that it enables a better visualization of the lower basila

Recurrent subarachnoid hemorrhage associated with aminocaproic acid therapy and acute renal artery thrombosis. Case report.

Epsilon aminocaproic acid (EACA) has been used to prevent rebleeding in patients with subarachnoid hemorrhage (SAH). Although this agent does decrease the frequency of rebleeding, several reports have described thrombotic complications of EACA therapy. These complications have included clinical deterioration and intracranial vascular thrombosis in patients with SAH, arteriolar and capillary fibrin thrombi in patients with fibrinolytic syndromes treated with EACA, or other thromboembolic phenomena. Since intravascular fibrin thrombi are often observed in patients with fibrinolytic disorders, EA

Management of extracranial carotid stenosis and intracranial aneurysms.

Twenty patients with extracranial carotid stenosis and intracranial aneurysms are reviewed. Fifteen of these patients had transient ischemic attacks (TIA's) and incidental aneurysms. The other five presented with symptoms referrable to an aneurysm, and angiography revealed significant carotid stenosis. None of the patients who presented with TIA's and underwent endarterectomy suffered subarachnoid hemorrhage. However, those patients who presented with symptoms referrable to an aneurysm and underwent endarterectomy seem to be at greater risk for subarachnoid hemorrhage.

[Effectiveness of antifibrinolytic, edema-preventing and hypotensive agents in the prevention of recurrent bleeding from intracranial aneurysms].

On the basis of a material of 561 patients with ruptured intracranial aneurysms treated at the Department of Neurosurgery, Institute of Neurology, Medical Academy in Cracow in the years 1971 to 1977 the authors discuss the effectiveness of conservative treatment for prevention of recurrent bleeding from these aneurysms. The whole group of 561 patients were divied into two subgroups: 361 patients (group A) receiving standard antifibrilytic treatment, drugs preventing oedema formation and reducing the arterial blood pressure, and 254 patients (group B) who were not given antifibrinolytic agents

[Diagnosis and pathological analysis of ruptured cerebral aneurysm by CT (author's transl)].

This report describes the analysis of 216 CT pictures of 85 patients with ruptured aneurysms which consist of 29 anterior communicating, 27 internal carotid, 27 middle cerebral and 2 basilar arterial aneurysms, including 18 cases with multiple aneurysms. The intervals between CT scanning and the last subarachnoid hemorrhage were various from 6 hours to 4 months. The first CT scanning was made in 40 cases within 3 days, in 54 cases within 7 days and in 11 cases more than one month after the hemorrhage. CT findings showed no abnormality in 9 patients who were classified as Grade I (Hunt and Kosn

[Surgical treatment of the internal carotid-posterior communicating artery aneurysm--from the experience of 213 cases (author's transl)].

213 cases of so-called posterior communicating artery aneurysm submitted to surgical operation without using microscope were reviewed from the technical point of view. Aneurysm project boradly to two directions, i.e. postero-inferior and lateral directions. We have treated these anuerysms with two very distinct operative approaches in order to reach the aneurysmal neck from the right angle. Frontal approach is used for the aneurysms projecting laterally. In contrast, the aneurysms projecting postero-inferior are reached laterally by mobilizing the tip of the temporal lobe. Of 213 patients, 15

Direct surgical treatment of giant intracranial aneurysms.

The author has operated on 40 patients with giant intracranial aneurysms, using various surgical approaches. Giant aneurysms predominated in females (3:1) and were most common in the age group 30 to 60 years. Patients presented with subarachnoid hemorrhage (17), visual disturbance (18), chronic headache (14), transient or progressive hemispheric deficit (6), seizure (2), dementia (2), and cerebrospinal fluid rhinorrhea (1). Giant aneurysms were located at the carotid artery (25), the basovertebral artery (8), the anterior communicating artery (5), and the middle cerebral artery (2). Eight of 4

[Liliequist membranotomy for patients with ruptured intracranial aneurysms (author's transl)].

In order to reconstruct the blocked CSF pathway, we attempted to excise the Liliequist membrane in 22 patients with subarachnoid hemorrhage resulted from ruptured intracranial aneurysms. After clipping the aneurysmal neck, the Liliequist membrane was reached through the space between the optic nerve and the internal carotid artery within the same operative field. As a result, the incidence of postoperative ventricular dilatation was remarkably reduced in comparison with control cases without Liliequist membranotomy. The necessity for the shunt operation for post-SAH hydrocephalus was also redu

Detection of cryptic vascular malformations by computerized tomography. Report of two cases.

Two cases of cryptic vascular malformation that were not demonstrated by cerebral angiography were detected by computerized tomography. One of these patients had a cavernous angioma in the fourth ventricle with recurrent subarachnoid hemorrhages, and the other harbored a small arteriovenous malformation and intracerebral hematoma. The usefulness and limitations of computerized tomography in the identification of cryptic vascular malformations are discussed.

Giant aneurysms in CT and angiography.

Giant intracranial aneurysms are frequently observed as incidental findings on CT and angiograms obtained for purposes other than subarachnoid hemorrhage, such as slowly evolving nerve palsy or hemiparesis. Since giant aneurysms often thrombose, the CT scan may demonstrate a larger aneurysmal volume than the angiogram. This paper presents CT and angiogram findings in cerebral aneurysms with a diameter of 25 mm-60 mm. The discussion covers: sort of aneurysm (congenital, arteriosclerotic); type and degree of thrombosis; long-term results after spontaneous thrombosis; and CT examination after usi

Anterior inferior cerebellar artery aneurysms. Case report.

A 54-year-old man experienced a right occipital headache accompanied by a roaring sound, nausea, vomiting, right facial weakness, and stiff neck. Vertebral angiography revealed an aneurysm of the right anterior inferior cerebellar artery (AICA) at the internal acoustic meatus which was later excised with favorable results. The literature is reviewed; operations have been reported in eight other cases. Inconstant waxing and waning cerebellopontine angle symptoms and signs can be found when a history of subarachnoid hemorrhage is lacking.
