Considering the current state of surgery for intracranial aneurysm, a philosophy for treating intracranial aneurysms associated with other entities is presented. The premise is based on a comprehensive review of the literature relative to the risk factors applied to intracranial aneurysms and personal experience in dealing with the combination of problems of aneurysms diagnosed in association with other intracranial aneurysms, vascular malformations, neoplasms, trauma, and inflammatory diseases. A patient's problem must be considered individually and comprehensively, using all known prognostic

The microsurgical correlates the pterional approach to the distal basilar circulation were evaluated in 20 cases of posterior circulation aneurysms, 50 human cadaver dissections, and a variety of other intracranial surgical lesions. The pterional approach permitted successful clipping of the aneurysm in 13 of 15 basilar bifurcation aneurysms, 1 of 2 basilar-posterior cerebral aneurysms, and 2 of 3 basilar-superior cerebellar aneurysms. In each of the failed attempts via the pterional approach, the subtemporal route ultimately resulted in proper clip application. The interposition of the poster

The syndrome of inappropriate antidiuretic hormone secretion (SIADH) developed approximately 7 days after a spontaneous subarachnoid hemorrhage in a 63-year-old woman with an anterior cerebral artery aneurysm. The hyponatremia associated with this syndrome resulted in a deterioration of the patient's clinical condition and focal neurological signs, which simulated the clinical deterioration after spontaneous subarachnoid hemorrhage that is often caused by other intracranial pathological conditions. The focal neurological signs in particular are likely to be interpreted as indicating one of the

A non operated aneurysm of the anterior communicating artery enlarged considerably over a seven years period; it finally thrombosed and behaved like a suprasellar tumor. The clinical course, the angiographic evolution and the anatomical examination allowed, in this case, to account for the increase in volume by progressive stretching of the wall, and not by incorporation of a false aneurysmal sack resulting from an encapsulated haematoma. Histologically, the wall of this giant aneurysm showed various changes : on the inner aspect, progressive thickening occurs, due to fibrous organization of m

Three hundred sixty-four patients who suffered a subarachnoid hemorrhage from an aneurysm at either the anterior (ACA) or posterior (PCA) communicating artery location and who were not surgically treated have been followed for up to 21 years in order to study the late morbidity and mortality. For patients surviving six months: (1) little improvement in morbidity was noted during the many years of subsequent follow-up; (2) in general, ACA patients fared better than PCA patients; (3) the neurological state on original admission was strongly correlated with the degree of morbidity; (4) other fact

The usefulness of hydrocortisone for the treatment of cerebral vasospasm was investigated using dogs by measuring changes in cerebral arterial diameter in angiograms as well as in other parameters such as blood pressure, intracranial pressure and CBF. The intrathecal (50-300 mg) or intravenous (100 mg/kg) injection of hydrocortisone resulted in a marked dilatation of cerebral arteries both in control animals and vasospastic animals after subarachnoid hemorrhage. The intrathecal injection caused a temporary rise in blood pressure immediately following the injection. Cerebrospinal fluid pressure

The pulsative inflow of blood to the brain causes fluctuations in the cerebral blood volume, and this is considered to be the main cause of the pulsations of the intracranial pressure (ICP). This statement allows discrimination between two different factors that influence the pulse amplitude of the ICP: 1. The pulsatile pattern of the cerebral arterial blood flow. 2. The slope of the intracranial pressure-volume curve. In order to clarify the relative contributions of the two factors we have developed a simulation model of the intracranial pressure-volume curve and the cerebral blood flow. Mea

A sixty-eight year old female patient died from a subarachnoid hemorrhage three days after experiencing sudden hearing loss in her right ear. Autopsy showed that the cause of death was a large aneurism of the basilar artery. Temporal bone sections uncovered a massive infiltrate of erythrocytes about the nerves in the internal auditory canal. The blood cells had extended along the nerve bundles to enter Rosenthal's canal and the other fluid channels of the modiolus, and could be found within all perilymphatic spaces. The presence of hemosiderin within the inner ear correlates clinically with th

Two patients with mycotic intracranial aneurysms were successfully treated with only antibiotic therapy. One patient, who had subacute bacterial endocarditis, rheumatic valvular disease, and an abscessed tooth, sustained a subarachnoid hemorrhage from a ruptured right middle cerebral artery trifurcation aneurysm. The other patient, who had Turner's syndrome and probable congenital aortic stenosis, developed multiple neurological findings during an ipisode of acute bacterial endocarditis precipitated by an infected ingrown toenail; a false aneurysm of the distal left middle cerebral artery and

The neurological manifestations of achondroplasia include megalencephaly, hydrocephalus, and progressive myelopathy or radiculopathy secondary to spinal stenosis. Subarachnoid and intracerebral hemorrhage have not been previously reported in association with achondroplastic dwarfism. This report describes two patients with achondroplasia who have died secondary to intracranial hemorrhage. At autopsy, the source of hemorrhage was found to be an arteriovenous malformation of the choroid plexus in one case and a ruptured aneurysm of the left middle cerebral artery in the other.

Carotid-ophthalmic aneurysms have intimate relationships with the ipsilateral optic nerve and other neighboring structures. Direct operation for treatment of these aneurysms has been considered particularly hazardous and difficult. Therefore, some authors have recommended an indirect operation (carotid artery ligation in the neck) as the procedure of choice. A Consecutive series of eight patients with carotid-ophthalmic aneurysms in presented; a direct operation was performed in seven instances. There was no operative mortality and the morbidity was very low. The direct attack seems to be reli

It is accepted that cerebrovascular dilatation is a constant response during the advanced stage of intracranial hypertension. Severe intracranial hypertension ultimately associates with profound vasodilatation and reduces cerebral blood flow to zero. This irreversible state is called cerebral vasomotor paralysis by Langfitt et al (1965). The rich adrenergic nerve supply of the cerebral vessels suggests that pressor amines possibly affect the cerebral circulation and the cerebral vascular tone. This study is to investigate the reactivity of norepinephrine and phentolamine on intracranial pressu

Thirty-two cases of rupture of an aneurysm during angiography were reviewed to determine whether there is an immediate, acute phase of intracranial arterial spasm in human beings. Unfortunately, in the 11 instances of major extravasation into the adjacent subarachnoid spaces (the cases most likely to demonstrate arterial spasm), the arteries either were not filled or were obscured by the escaping contrast medium. In 13 patients who demonstrated arterial spasm, the vasospasm definitely (5), probably (6), or possibly (2) was related to a prior episode of bleeding. There was no evidence of arteri

Thirty-two cases of carotid-ophthalmic aneurysms are reviewed. As with intracranial aneurysms in other positions they present mainly with subarachnoid haemorrhage but, in spite of their close proximity to the optic nerve, visual involvement is infrequent. They are more common in women, more frequent on the left side, and more prone to multiplicity. In cases of multiple aneurysms a carotid-ophthalmic aneurysm is usually an incidental finding. Detailed angiographic studies employing various projections are required before treatment can be planned. Yet angiography does not always disclose some of

An operative case of 12-year-old boy with a saccular aneurysm at the anterior communicating artery was reported. He had episodes of occasional headache during one year before admission. He was attacked by a severe headache associated with nausea and vomiting, and was admitted to Ooita Pref. Hospital under the diagnosis of subarachnoid hemorrhage four days later. On admission physical examinations revealed almost normal findings except for moderate dehydration and a blood pressure of 130/70 mmHg. Routine examinations (blood, serum including total cholesterol, urine, ECG and plain chest X-film)

Clinical use of four-vessels angiography increased the frequency of detection of intracranial aneurysm in patients who had episode of subarachnoid hemorrhage. However, some cases of subarachnoid hemorrhage did not show intracranial and intraspinal source of bleeding angiographically. Bjökesten and Troupp pointed out that some cases who were negative in angiography may have a very small intracranial aneurysm. Hassler described the minute aneurysm sized less than 2 mm in diameter from finding autpsy of the cases of subarachnoid hemorrhage and he emphasized that source of subarachnoid hemorrhage

One case with Wallenberg's syndrome followed by the neck clipping of the posterior inferior cerebellar aneurysm was reported. The patient was 49 years old female with the subarachnoid hemorrhage, who had previously no history of the cardiovascular disease. The vertebral angiogram revealed a saccular aneurysm of the left vertebral artery at the origin of the posterior inferior cerebellar artery. The preoperative neurological examination were normal, except for the slight degree of the meningeal irritation. The surgical intervention was successfully performed on 39th day after the subarachnoid h

The author measured the level of creatine phosphokinase (CPK) in 35 cerebrospinal fluid (CSF) specimens from 30 patients with acute subarachnoid hemorrhage, and correlations were sought with 17 other clinical and laboratory parameters. Elevations of CSF CPK have no diagnostic specificity. However, they do dhow a statistically significant correlation with the existence of any destructive process in cerebral tissue (hydrocephalus, infarction, intraparenchymal hemorrhage, or intraventricular clot). Yet arterial spasm without infarction does not raise the CSF CPK level. During the preoperative man

Thirteen cases of vertebral aneurysm at the origin of PICA (VA-PICA aneurysm) were operated on at the Department of Neurological Surgery of Kitano Hospital from March, 1970 through July, 1975. Those included 9 cases of saccular aneurysm and 5 cases of fusiform aneurysm (Table 1). The incidence of VA-PICA aneurysms among our whole series of intracranial aneurysms was 4.2%. Patients with subarachnoid hemorrhage were subjected to our routine 4 vessel angiography. For those with suspected vertebral aneurysm vertebral angiography was performed in a transoral projection. In this method, when the ang

Cerebral infarction and petechial hemorrhage are seen in autopsied brains with subarachnoid hemorrhage as most important findings. However, the pathogenesis for the development of these lesions are still obscure. We have been doing clinico-pathological study on the cerebral petechial hemorrhages following subarachnoid hemorrhage. In this report, the findings of these hemorrhagic lesions were presented in detail by means of Perls-Stieda's iron staining. The results are as follows. Cerebral petechial hemorrhages were seen in 3 out of 13 autopsied cases with subarachnoid hemorrhage and almost all
