Botulinum neurotoxins (BoNTs) are the bacterial proteins responsible for the flaccid paralysis and lethal effects of botulism. They act by inhibiting neurotransmitter release, primarily at peripheral cholinergic nerve terminals, and have traditionally been classified into seven serotypes, designated with letters A through G. Over a long history of investigation, scientists and physicians have learned to harness the selectivity and potency of these neurotoxins to advance therapeutic applications. These advances have come by leveraging the basic science understanding of how the BoNT's work. The

Botulinum neurotoxins (BoNTs) are zinc-dependent endopeptidases that cleave presynaptic SNARE proteins and arrest acetylcholine release, producing flaccid paralysis with protracted recovery. We report two cases of a mother and son who developed severe food-borne botulism after eating Fesikh (traditional salted fish) with rapidly progressive descending paralysis requiring ventilation, prominent dysautonomia (hypotension responsive to fluids, bradycardia, dry mucosae, constipation with abdominal distension, as well as urinary retention), and intact consciousness. Both received trivalent antitoxi

Clostridium botulinum serotype B (Group I and II) is a Gram-positive, anaerobic bacterium renowned for producing botulinum neurotoxin type B (BoNT/B), a potent zinc-dependent endopeptidase. This mini-review explores the microbiological and genetic foundations of BoNT/B-producing strains, emphasizing their taxonomic classification, genomic plasticity, and toxin gene cluster organization (e.g., bont/B, ntnh, ha). The review highlights the bacterium’s environmental adaptability, driven by horizontal gene transfer and plasmid- or phage-encoded toxin genes, which influence strain diversity and toxi

Weakness is a challenging and broad chief complaint in emergency medicine, necessitating rapid and accurate differentiation among numerous etiologies. Effective diagnosis involves recognizing symptoms of central and peripheral neurologic disorders, neuromuscular junction diseases, nutritional deficiencies, and toxin exposures. Prompt identification and intervention in conditions such as Guillain-Barré syndrome, transverse myelitis, botulism, and nutritional deficiencies are crucial. Despite vaccines reducing cases of polio, tetanus, and diphtheria, vigilance remains essential due to their pote

BACKGROUND: Botulinum toxin type A is widely used to block acetylcholine release in the treatment of chronic sialorrhea, muscle spasticity, and dystonia. We aim to develop a user-friendly method for detecting cases of medical botulinum toxin poisoning.
METHODS: The mice poisoning model was established by injecting or gavage with Botulax®, and the poisoning dose and symptoms were observed. The residual levels of toxin in poisoned mice were detected by high-resolution mass spectrometry and sandwich ELISA, respectively.
RESULTS: Two hours after poisoning, no residual botulinum toxin was found b

BackgroundBotulinum toxin is widely used for the treatment of spasticity in pediatric patients with cerebral palsy. Although regarded as safe, rare but severe systemic complications, such as iatrogenic botulism, may arise.Case PresentationWe present the case of an 8-year-old boy with cerebral palsy who experienced global muscle weakness, bilateral ptosis, dysphagia, and respiratory distress after botulinum toxin A (Dysport). The clinical presentation was consistent with iatrogenic botulism. The patient was administered botulinum antitoxin, resulting in a gradual enhancement of the condition. S

BACKGROUND: Infant botulism accounts for many new botulism cases each year. Geographic case clustering has been previously described in California. This study aimed to describe whether case clustering was present in Victoria, Australia.
METHODS: We conducted a retrospective case series of infant botulism presenting to Victorian hospitals between 1978 and 2024. Moran's I test for spatial autocorrelation was used to determine if spatiotemporal clustering was present.
RESULTS: Twelve cases of infant botulism presented to Victorian hospitals in the study period. Using Moran's I statistic in anal

OBJECTIVE: Based on the US Food and Drug Administration Adverse Event Reporting System (FAERS), signal mining of adverse drug events (AEs) caused by Botulinum Toxin Type A (BoNTA) was performed to explore its safety implications for the treatment of cerebral palsy (CP).
METHODS: The OpenVigil 2.1 platform was used to extract AE reports on BoNTA from the FAERS database, covering the period from the fourth quarter of 2003 to the second quarter of 2024. Safety data were analyzed using the Reporting Odds Ratio (ROR) and Proportional Reporting Ratio (PRR), with BoNTA designated as the primary susp

Clostridia that produce neurotoxins are highly relevant organisms to public health. While cases of botulism [caused by C. botulinum and other organisms that produce botulinum neurotoxin (BoNT)] are rare, the severity of this disease necessitates robust epidemiologic surveillance to promptly identify and mitigate outbreaks. Next generation sequencing (NGS) can provide additional support to these investigations through single nucleotide polymorphism (SNP)-based analysis, phylogenetic reconstruction, toxin subtyping, and structural analysis. Until recently, testing for this disease was restricted

International borders pose health risks due to the intense movement of people and products, requiring surveillance and specific public health interventions. In Brazil, territorial diversity accentuates these challenges. The aim of the present study was to analyze the pattern of the mandatory immediate reporting of public health events in border regions of Brazil and more specifically in the state of Paraná from 2010 to 2019. Fourteen events from the National List of Compulsory Notification of Diseases that require immediate reporting to the three levels of government were analyzed using data f

UNLABELLED: Clostridium butyricum exhibits a dual role, acting not only as a probiotic but also as an opportunistic pathogen associated with neonatal necrotizing enterocolitis (NEC) and infant botulism. We aimed to establish high-resolution genotyping frameworks to improve molecular surveillance and outbreak investigations. We analyzed 297 C. butyricum genomes, including 200 isolates from preterm neonates across 13 French neonatal intensive care units over a 20-year period and 97 publicly available genomes. A core-genome multilocus sequence typing (cgMLST) scheme was developed using chewBBACA,

Botulinum neurotoxin serotype A (BoNT/A), a Class A bioterrorism agent, poses significant poisoning risks, with diagnosis challenges and no specific antidotes, making vaccination the most effective prevention. However, traditional needle-based vaccines are painful, thermally unstable, require trained professionals for administration, and carry infection risks, leading to low patient compliance. To overcome these limitations, we introduce a self-applicable, thermostable, heterogeneous microneedle (MN) patch for delivering a recombinant protein vaccine based on the C-terminal heavy chain of BoNT

This study presents novel clinical presentations of botulism, observed in both patients and goats from the same farm. Their symptoms appeared at the same time, suggesting a possible relation to animal visceral botulism. Interestingly, weight loss and weakness were the persistent signs, but not life-threatening as expected in botulism.

A young man presented with progressive cranial nerve dysfunction following participation in a communal meal. Initial investigations and imaging were inconclusive; Guillain-Barré syndrome and viral encephalitis were considered in the differential diagnosis. He was admitted to the intensive care unit (ICU) due to the potential need for respiratory support. The progression of neurological dysfunction, combined with relevant dietary exposure, raised clinical suspicion for botulism. The patient was treated with heptavalent botulinum antitoxin, and serum testing later confirmed the presence of botul

Foodborne botulism is a potentially fatal disease caused by Clostridium botulinum neurotoxins (BoNTs). These spore-forming bacteria are ubiquitous in the environment and can contaminate various food products, especially raw vegetables. During the preparation of home-made preserves, favorable conditions of anaerobiosis, temperature, salinity, and pH can lead to spore germination and toxin production. BoNTs can reach neuromuscular junctions where they block the release of acetylcholine. In this study, we present a case of foodborne botulism associated with the consumption of chili peppers preser

BACKGROUND: Botulism is a rare but potentially fatal illness caused by botulinum neurotoxin, with outbreaks reported globally in humans, animals, and the environment. In the absence of a global surveillance system, open-source intelligence (OSINT) offers real-time insight into botulism activity.
METHODS: Using the Artificial-Intelligence-driven OSINT early warning system EPIWATCH®, we extracted a dataset published between 31 December 2016 and 1 January 2025 and filtered by our search terms to identify botulism outbreaks. Data collected included country, event date, location, symptoms, outbrea

Botulinum toxin (BoNT) is extensively used across a wide range of therapeutic and cosmetic applications due to its efficacy in inhibiting neuromuscular transmission and its well-established safety profile. While adverse effects are typically localized and transient, systemic complications - particularly iatrogenic botulism - are rare but potentially life-threatening events. Iatrogenic botulism results from unintended systemic spread of the neurotoxin, which can occur even at doses commonly used for cosmetic purposes. We report a rare and severe case of iatrogenic botulism in a previously healt
