Fluctuating, but Not Forgotten: Recognizing Neuromuscular Junction Disorders.

The neuromuscular junction (NMJ), the connection between nerve and muscle, is an essential anatomic site that warrants careful consideration. Dysfunction at this junction often leads to skeletal muscle weakness with a multitude of clinical manifestations. Disorders of the NMJ primarily encompass myasthenia gravis, Lambert-Eaton syndrome, botulism, and organophosphate poisoning. This article will review diseases and disorders of the NMJ, their clinical presentations, and approach to management.

Laboratory Diagnostics of Botulism Cases in Livestock in Poland in 2022-2024.

Botulism is a neuroparalytic disease caused by exposure to botulinum neurotoxins produced by anaerobic spore-forming bacteria of the genus Clostridium. This disease occurs in both humans and wild and domestic animals, and is currently becoming an increasingly serious problem worldwide due to high animal mortality and economic losses. The clinical signs observed during the progression of botulism are nonspecific and difficult to unequivocally associate with this disease entity. The aim of this study is to present laboratory diagnostics of suspected botulism cases reported in Poland in 2022-2024

Genomic Characterization of Clostridium botulinum Isolates from Soil and Soybean Samples in High-Incidence Regions - Xinjiang, Inner Mongolia, and Qinghai PLADs, China, 2024.

WHAT IS ALREADY KNOWN ABOUT THIS TOPIC?: Foodborne botulism is prevalent in northwestern China, linked to traditional homemade foods. Recently, some cases have been linked to commercial vacuum-packaged ready-to-eat meat products. Soil is a potential contamination source, yet genomic information on environmental isolates from high-incidence regions remains scarce. WHAT IS ADDED BY THIS REPORT?: This study presents the first genomic characterization of 23 C. botulinum isolates obtained from soil and soybean samples in Northwest China. Four botulinum neurotoxin subtypes, A5(B3), B2, B3, and B4,

Infant Botulism and Honey Exposure: Global Epidemiology, Prevention Policies, and Communication Strategies.

AIM: This study summarised the global epidemiology, prevention policies, and public health communication strategies related to infant botulism and exposure to honey. METHODS: A narrative review of epidemiological data was carried out from 1976 to 2024, using surveillance reports, national health agency documents, and peer-reviewed literature. RESULTS: Cases were rare, but honey was the only consistently confirmed dietary source of infant botulism. The median age at onset was 3-4 months, and mortality was below 1% if the infants received intensive care support and specific therapy with human

Wound Botulism Complicating a Snake Bite Wound Following Traditional Application of Raw Goat Skin Successfully Treated with Botulinum Antitoxin: A Case Report.

Wound botulism is a rare, life-threatening neuroparalytic disease caused by neurotoxins produced by Clostridium botulinum in contaminated wounds. Traditional wound care practices, such as applying animal products, may increase the risk. We report the case of a 35-year-old woman from Afghanistan who presented with progressive descending neurological deficits, including bilateral ptosis, blurred vision, diplopia, nasal speech, dysphagia, and respiratory distress. Symptoms began nine months after a snake bite to the left lower leg that evolved into chronic non-healing calf ulcers. Traditional tre

Adverse neurological events following botulinum toxin type A: A case series of post-injection seizures and paralysis.

Within the field of rehabilitation and aesthetic medicine, botulinum neurotoxin (BoNT), particularly type A (BoNT-A), has gained significant popularity for its unique muscle-relaxing properties including muscular spasticity and reducing wrinkles. Nevertheless, its widespread use is accompanied by the inherent risk of poisoning. This report presents two distinct patterns of neurological adverse events observed across three patients involving BoNT-A. In the first pattern, Patients 1 and 2, who had cerebral infarction and cerebral hemorrhage, respectively, both developed seizures after BoNT-A inj

Phage display enabled the identification of single-domain nanobody antitoxins against botulinum neurotoxin serotype C.

Botulinum neurotoxin serotype C (BoNT/C) exhibits therapeutic efficacy and duration of action comparable to those of BoNT/A. It has been demonstrated to effectively manage movement disorders such as dystonia and blepharospasm, making it a highly promising alternative to BoNT/A. BoNT/C is also one of the primary etiological botulinum isoforms causing animal botulism and may pose a zoonotic risk. So, it is of great need to develop antidote that can treat human iatrogenic, foodborne and animal botulism caused by BoNT/C. Currently, the approved botulism antidotes primarily are polyvalent horse ser

Infantile Botulism in the Absence of Dietary Exposure to Honey: A Case Likely Related to Environmental Spore Exposure.

Infantile botulism is a rare but potentially life-threatening neuroparalytic condition caused by botulinum toxin produced by Clostridium botulinum colonizing the infant's gastrointestinal tract. Early manifestations are often subtle and nonspecific, frequently leading to delayed or missed diagnosis. We report a case of a four-week-old infant who developed progressive hypotonia and respiratory compromise secondary to infantile botulism. The diagnosis was confirmed by detection of botulinum toxin in the stool, and the patient was treated promptly with botulism immune globulin. This case highligh

Epidemiology of Foodborne Botulism Outbreaks in Romania, 2007-2024.

Foodborne botulism, caused by ingestion of pre-formed botulinum neurotoxin, is the most common form of botulism. While large outbreaks linked to commercial foods are rare, smaller outbreaks associated with home-processed products are more frequent, reflecting local dietary habits and traditional preservation practices. The aim of this paper is to provide a public health overview of reported foodborne botulism outbreaks in Romania over an 18-year period to raise awareness among clinicians and public health officials. Between 2007 and 2024, a total of 337 foodborne botulism cases were reported i

Non-Wound Tetanus Mimicking Botulism in a Low-Resource Setting: Early Recognition and Successful Intensive Care Management.

INTRODUCTION: Tetanus is a vaccine-preventable, toxin-mediated neurologic emergency caused by Clostridium tetani. Although classically associated with contaminated wounds, the portal of entry may be trivial or unrecognized, resulting in so-called non-wound presentations that can delay diagnosis. Early tetanus may also mimic other toxin-mediated syndromes, including botulism, particularly when cranial or bulbar symptoms predominate. CASE PRESENTATION: A 41-year-old man in Somalia presented with rapidly progressive trismus, neck stiffness, dysarthria, perioral paresthesia, and marked masseter r

Critical role of empiric antitoxin therapy in suspected Clostridium baratii infection: a case report.

BACKGROUND: Clostridium baratii infections, though rare, can result in life-threatening botulism due to the production of botulinum neurotoxin. The rapid administration of antitoxin is the cornerstone of treatment to mitigate neurological damage and prevent mortality. Given the difficulty in obtaining rapid diagnostic confirmation, empiric administration of antitoxin is often justified. CASE SUMMARY: This paper describes a case of C. baratii infection with complete recovery after empiric treatment with antitoxin prior to confirmatory testing returning. Additionally, this manuscript explores t

The perfect storm in a coastal pond: duckweed, botulism, bird mortalities, and recurrent blooms of Euglena sanguinea.

In summer 2024, a freshwater coastal pond in northwest Spain (A Congorza, Cangas) experienced its first massive mallard (Anas platyrhynchos) mortalities, repeated in 2025. The pond is covered by Lemna minor and develops a red scum during summer. Molecular, pigment, and morphological analyses allowed identification of Euglena sanguinea as the responsible species for the latter. Water, tissues, and maggots from dead specimens were analyzed to assess physico-chemical characteristics, plankton composition, clinical signs, and toxins in order to explain these mortalities. Poor water quality paramet

[Botulism in Japan].

Botulism is a paralytic disease caused by the neurotoxin mainly produced by Clostridium botulinum. Botulism is classified as infant, foodborne, wound, adult intestinal toxemia, iatrogenic, and inhalation botulism. In Japan, type E botulism associated with the consumption of "izushi," a traditional fermented fish product, was prevalent before 2000. Since the 2000s, type A botulism has become more common. In some cases, foodborne botulism was suspected, but no incriminating food or sporadic cases were identified. Therefore, foodborne botulism should be considered even in the absence of a clear h

Development of novel 2-[(8-hydroxyquinolin-7-yl)(phenyl)methylamino]benzoic acid derivatives for inhibiting the catalytic activity of botulinum neurotoxins type B and F: in silico, in vitro & in vivo evaluation.

Botulinum neurotoxins (BoNTs) are the most pernicious toxin and category 'A' bioterrorism agent, responsible for 'botulism', a rare but fatal disease. Toxins are divided into seven toxinotypes from A-G, where A, B, E, and F are accountable for human botulism. The available treatment strategies are inadequate and ineffective for post-neuronal intoxication. We report novel 2-[(8-hydroxyquinolin-7-yl)(phenyl)methylamino]benzoic acid derivatives targetingthe catalytic site of VAMP-acting serotypes (BoNT/B and F). Inhibitory potential of the synthesized compounds was studied using a framework of in

Foodborne Botulism in an Immunocompetent Adult Following the Consumption of a Prepackaged Soup.

Botulism is a potentially fatal neuroparalytic disease that has historically been associated with outbreaks from improperly stored foods. Today, foodborne botulism in adults is rare. Because of its rarity, early symptoms may go unrecognized, leading to rapid disease progression. Prompt recognition and early administration of antitoxin are critical, as there is currently no agent that reverses the neurological damage caused by the toxin. In this case, botulism was associated with the consumption of a commercially prepackaged, non-canned soup, rather than the classic association with improperly

Botulism Type F, a Rare Type of Botulism that Can Masquerade as a Posterior Fossa Stroke.

BACKGROUND: Botulism is a rare, life-threatening condition caused by potent neurotoxins that inhibit the release of acetylcholine into the neuromuscular junction, resulting in cranial nerve palsies, flaccid paralysis, and respiratory compromise. Most cases are caused by type A and B toxins produced by Clostridium botulinum. CASE REPORT: Here, we describe a 41-year-old woman who presented with acute onset binocular diplopia, nausea/vomiting, slurred speech, and unsteady gait, symptoms that resemble a posterior fossa stroke. The patient was initially admitted to the stroke service; however, her
