Dry and dusty outback: infant botulism.

Infant botulism is a rare but life-threatening neuroparalytic syndrome caused by intestinal colonisation of Clostridium botulinum resulting in a symmetrical descending flaccid paralysis. This gram-positive, rod-shaped anaerobic bacillus is universally found in soil with previous links to ingestion of honey and dust contaminated with C. botulinum spores. We present a case of a baby boy with this rare condition eventually linked to dust on a rural outback property. This young patient was ventilated for 90 days. Initial serology for botulism toxin was negative, although C. botulinum spores were e

Botulinum Neurotoxins as Two-Faced Janus Proteins.

Botulinum neurotoxins are synthetized by anaerobic, spore-forming bacteria that inhibit acetylcholine release at the level of the neuromuscular and autonomic cholinergic junctions, thus inducing a series of symptoms, the most relevant of which is flaccid paralysis. At least seven serotypes and over 40 subtypes are known, and they are among the most poisonous natural substances. There are different forms of botulism according to the route of contamination, but the clinical manifestation of descending symmetric flaccid paralysis is consistent, regardless of the route of contamination. It is very

Recombinant expression of receptor binding domains of all eight subtypes of botulinum neurotoxin type A for generation of antitoxins with broad reactivity.

BACKGROUND: Botulinum neurotoxin type A (BoNT/A) represents a major threat to global public health because of its most potent toxicity with the longest persistence. Several camelid single-domain antibodies (or VHHs) have been reported to exhibit high neutralizing activity against the receptor binding domain (H C) of the BoNT/A subtype used to generate them. However, it remains unclear if these VHHs can neutralize effectively H C of other BoNT/A subtypes. This study aimed to generate H C domains of all eight BoNT/A subtypes and to screen for VHHs with broad reactivity against these domains. ME

Global Occurrence of Infant Botulism: 2007-2021.

We sought to summarize the worldwide occurrence and epidemiology of infant botulism (IB) from 2007 to 2021. Data were collected through active and passive surveillance, via hospital inquiries to the Infant Botulism Treatment and Prevention Program regarding Human Botulism Immune Globulin Intravenous for patients with suspected IB, by contact with epidemiologists in countries with historically high case counts and by searching the medical literature. A case was defined as an infant with laboratory-confirmed botulism that was not due to ingestion of food containing botulinum toxin. Eighteen coun

An Effective Prophylactic and Therapeutic Protection Against Botulinum Type A Intoxication in Mice and Rabbits Using a Humanized Monoclonal Antibody.

Botulinum neurotoxins (BoNTs) are the most potent toxins on Earth and are classified as Category A biological agents. BoNTs lead to paralysis in humans and cause botulism. Antibody therapeutics can effectively treat toxin-mediated infectious diseases. In this study, we generated a pharmaceutical humanized monoclonal antibody (HZ45 mAb) to prevent or treat botulism. HZ45 binds to the heavy chain receptor (HCR) domain of the toxin, preventing the toxin from entering the cell. The mAb was produced using hybridoma technology and phage display. We evaluated HZ45 mAb for the neutralization of BoNT s

A systematic review of the avian antibody (IgY) therapeutic effects on human bacterial infections over the decade.

The overuse of antibiotics worldwide, especially during the Coronavirus pandemic, has raised concerns about the rise of antibiotic resistance and its side effects. Immunoglobulin Y, a natural protein that specifically targets foreign antigens, holds promise as a potential therapeutic option, particularly for individuals with sensitive immune systems. Despite numerous studies on IgY, the optimal administration method, effective dose, target antigen, and potential side effects of this antibody remain areas of active research and challenge. This review selected and evaluated articles published in

Botulism in the Highlands: Understanding the High-Altitude Effect.

Botulism is a foodborne, life-threatening neuroparalytic disease caused by Clostridium botulinum, an anaerobic, gram-positive, spore-forming bacteria. Its incidence is unknown and underestimated due to missed diagnoses, especially in remote areas. It is divided into four main clinical types: Infant, adult-onset foodborne, iatrogenic, and wound botulism. The overall goal of the article is to focus on the geographic differences in incidence, including its propensity to high-altitude areas, and shed light on the theories behind this propensity.

Exploring parents' knowledge, attitudes and practices on honey and botulism in the West Bank, Palestine: a cross-sectional study.

BACKGROUND: Although nutritionally beneficial, honey may harbor Clostridium botulinum spores, posing risks to infant botulism (IB). Infants under one year are vulnerable due to immature gut flora, with IB potentially causing severe symptoms, such as respiratory failure. Despite global awareness, cultural beliefs influenced caregivers' practices in Palestine. This study evaluated Palestinian parents' knowledge, beliefs, and feeding practices regarding honey to guide targeted interventions. METHODS: This cross-sectional study  (August-September 2024) surveyed 469 Palestinian parents (aged ≥18 y

Tracing and characterization of foodborne botulism caused by the new MLST type Clostridium botulinum A2 in Hebei province, China.

Foodborne botulism caused by botulinum neurotoxin (BoNT) remains an important form of botulism worldwide, with a high mortality rate and prolonged hospitalization time. Clostridium botulinum (C. botulinum) is the main microorganism responsible for producing BoNTs. This study reports a case of foodborne botulism caused by a C. botulinum subtype A2 strain from pickled eggs. We detected the BoNT gene using real-time PCR and the BoNT through the mouse bioassay (MBA) from both the patient's feces and the pickled eggs and isolated C. botulinum A strains. The genetic SNP and phylogenetic tree analysi

Detoxification techniques for bacterial toxins: A pathway to effective toxoid vaccines.

Bacterial toxins play a critical role in the virulence of many pathogens, leading to serious diseases such as tetanus, diphtheria, botulism, and entrotoxemia. As key virulence factors, these toxins cause significant tissue damage and disease manifestations in infected hosts. Vaccination against these toxins through toxoid vaccines, composed of inactivated forms of the toxins, represents a vital strategy for preventing toxin-mediated diseases. However, creating effective toxoid vaccines necessitates meticulous detoxification processes that ensure the loss of toxicity while retaining the immunog

The Scientific Benefits of a Statewide, Standardized, Coastal Wetland Monitoring Program in Hawa'i.

In this viewpoint, we provide a scientific justification for a statewide, standardized, coastal wetland monitoring program for Hawai'i, USA. Hawaiian coastal wetlands provide important habitat for endangered waterbirds, invertebrates, plants, and the Hawaiian hoary bat ('ōpe'ape'a; Lasiurus semotus) as well as support Indigenous food systems. Currently, numerous agencies and groups in Hawai'i collect data on coastal wetlands, but information is not typically shared and methods are not standardized. A statewide, standardized, coastal wetland monitoring program with a centralized database would
