Key neuroanatomical structures for post-stroke cognitive impairment.

The neuroanatomical substrate of vascular cognitive impairment (VCI) has traditionally included the subcortex of the brain, especially sub-frontal white matter circuits, strategic areas of single infarction that may mediate cognitive impairment such as the dominant thalamus or angular gyrus, and the left hemisphere, and bilateral brain infarcts or volume-driven cortical-subcortical infarctions reaching a critical threshold of tissue loss or injury. We provide an update on the neuroanatomical substrates of VCI and emphasize the following structures or areas: (1) new concepts in relation to hipp

Comparison of 7.0-T T₂*-magnetic resonance imaging of cerebral bleeds in post-mortem brain sections of Alzheimer patients with their neuropathological correlates.

BACKGROUND: In view of the increasing recognition of cerebral microbleeds (MCBs) with MRI, there is a need to validate their detection in post-mortem brains in patients with cerebrovascular diseases and dementia. MATERIALS AND METHODS: Out of 20 post-mortem brains of patients with Alzheimer dementia and with different cerebrovascular lesions, 45 large sections of the cerebral hemispheres, brainstem and cerebellum were submitted to a 7.0-T T₂*-weighted MRI, and afterwards compared to the histological detection of haematomas, MCBs and mini-bleeds (MNBs). RESULTS: The sensitivity, specificity,

Transpontine connection fibers between corticospinal tracts in hemiparetic patients with intracerebral hemorrhage.

OBJECTIVES: A connection of fibers between corticospinal tracts (CSTs) at the pons, originating from the CST of the affected hemisphere, has been observed in hemiparetic patients with stroke. The authors investigated the incidence and the clinical significance of transpontine connection of fibers (TCFs) in hemiparetic patients with intracerebral hemorrhage (ICH), using diffusion tensor tractography (DTT). SUBJECTS AND METHODS: Forty-two patients with ICH with weakness of the affected extremities at the time of DTI scanning and 41 age-matched control subjects were recruited. TCFs were classifi

[Reduplication of an event after right caudate nucleus haemorrhage].

BACKGROUND: Environmental reduplication which is characterized by reduplication of places has been reported in right hemispheric lesions, particularly but not only in the right frontal region. However, spatial delirium may follow right sub-cortical lesions. METHODS: We describe a 53 years-old man who had a reduplicative paramnesia for event alone after an intracerebral haematoma of the right caudate nucleus. RESULTS: MRI Scan showed also an extension of the right caudate nucleus haemorrhage into the ventricular system. Regional cerebral blood flow studied with 99Tcm-HMPAO showed a decrease o

A female case of West syndrome with remission of spasms following multiple cerebral hemorrhages.

Multiregional wide-distribution hemorrhages of the left hemisphere occurred at 1 month of age in a girl with congenital factor V deficiency. At the age of 4 months, symmetrical spasms appeared in clusters and electroencephalography showed diffuse background attenuation in the left side and hypsarrhythmia only in the right. Brain CT scan showed that the left hemisphere including Rolandic area was completely infarcted. She was diagnosed with West syndrome and spasms were not controlled by anti-epileptic drugs. Following multiple intracerebral and subarachnoid hemorrhaging involving the right hem

Clinical application of diffusion tensor tractography for elucidation of the causes of motor weakness in patients with traumatic brain injury.

Diffusion tensor tractography (DTT) is useful for elucidating the status of the corticospinal tract (CST). The purpose of this study was to investigate the usefulness of DTT for determining the causes of motor weakness in patients with traumatic brain injury (TBI). Five patients with TBI were recruited for this study. DTT was performed using 1.5-T with a Synergy-L Sensitivity Encoding (SENSE) head coil. DTT was obtained with termination criteria of FA < 0.2 and an angle change > 45 degrees . On the DTT of patient 1, who had diffuse axonal injury, the focal lesion was detected in the left pons,

Cortical reorganization demonstrated by diffusion tensor tractography analyzed using functional MRI activation.

The diffusion tensor tractography (DTT) allows the corticospinal tract(CST) to be visualized at the subcortical level and functional MRI (fMRI) is capable of precisely identifying activation sites at the cortex. Therefore, it seems that combined DTT/fMRI would allow more accurate evaluation of the state of the CST. We have attempted to demonstrate cortical reorganization in a patient with cortical hemorrhage using DTT analyzed by fMRI activations. Six normal subjects and a 12-year-old female patient with a hemorrhage in the left fronto-parietal cortex were recruited. fMRI was performed at 1.5-

Brain white matter hyperintensities are associated with carotid intraplaque hemorrhage.

PURPOSE: To retrospectively assess the relationship between carotid intraplaque hemorrhage (IPH), which indicates plaque instability, and brain white matter hyperintense lesions (WMHLs) by using a within-patient design. MATERIALS AND METHODS: All patients gave written informed consent for the initial magnetic resonance (MR) studies, and the institutional review board and local research ethics committee waived initial informed consent for the pooled analysis. A total of 190 patients with symptomatic carotid artery disease underwent fluid-attenuated inversion-recovery imaging of the brain and f

[Pure agraphia with topographical disorientation caused by right hemisphere lesion].

A 69-year-old, right-handed, Japanese male patient presented with pure agraphia with topographical disorientation after hemorrhage in the right parietal lobe. Upon developing cerebral hemorrhage, he was referred to our hospital for close examination of agraphia. There was no paresis or clumsiness in his extremities. His speech was fluent and well-articulated. Neither aphasia nor reading impairments was found, although there was a clear writing impairment with effort and hesitation. His writing of both kanji and kana letters contained additional, absent or deformed strokes or parts. No hemianop

Cortico-subcortical distribution of microbleeds is different between hypertension and cerebral amyloid angiopathy.

BACKGROUND: The cerebral distribution of microbleeds in cerebral amyloid angiopathy (CAA) is quite different from that in hypertension, i.e., microbleeds in central gray matters are frequently found in patients with advanced hypertension (aHT), but not in patients with CAA. Distributions within the cortico-subcortical (CSC) area have not been compared between the diseases, and remain poorly understood. OBJECTIVES: We hypothesized that distributions of microbleeds differ in aHT and CAA even in the CSC area. METHODS: Out of a consecutive series of patients with intracerebral hemorrhage who und

[Surgical treatment of cortical and subcortical cavernomas. General principles and personal series of 20 cases treated between 2000 and 2006].

The surgical treatment of cortical or subcortical hemispheric cavernomas is founded on a series of questions: Is the cavernoma located in an eloquent or non-eloquent area? Is the cavernoma tangent to the cortex and visible immediately after the dura opening or deep seated in the hemisphere? Does the cavernoma lie in the depth of a sulcus and identifiable on the pretrans-sulcal approach MRI? Will perilesional tissue have to be removed to cure the epilepsy? What is the appropriate technology for each particular case: preoperative functional MRI, angiography, preoperative stereotactic guidance, p

48 year old male with sudden onset of right sided weakness.

This 48 year-old male presented with sudden right sided weakness and hypertension, and on CT was found to have a deep left hemispheric intracerebral hematoma. There were accompanying bilateral hypointensities in the occipital lobes, unchanged 1 day later, when expansion of the hematoma and uncal herniation resulted in death. The past medical history included liver transplantation followed by long term cyclosporin, features of thrombotic thrombocytopenic purpura (TTP) attributed to cyclosporin, type 1 diabetes mellitus and acute renal failure attributed to diabetic nephropathy. There was a 2-mo

Pathophysiological changes of the gastrointestinal tract in ischemic stroke.

OBJECTIVE: Dysphagia is common after stroke and represents a marker of poor prognosis. After ischemic stroke, dysphagia represents only one part of the clinical spectrum of changes in the gastrointestinal (GI) tract and includes GI hemorrhage, delayed GI emptying, and colorectal dysfunction. State-of-the-art imaging techniques have started to revolutionize to study the cortical and brainstem control of these GI symptoms. It has become increasingly obvious that GI alterations after stroke are complex and its recovery following stroke is even more so. METHODS: In this review, an electronic data

Chronic superior sagittal sinus thrombosis with phlebosclerotic changes of the subarachnoid and intracerebral veins.

An autopsy case of chronic thrombotic occlusion of the superior sagittal sinus (SSS) with an unusually protracted clinical course is reported. The patient was an 84-year-old man without any predisposing conditions for thrombosis. The clinical features were atypical and were characterized by recurrent cerebral subcortical hemorrhages. The autopsy revealed a large, organized thrombus in the SSS and extensive hemorrhagic infarction of the bilateral parasagittal region of the cerebral hemispheres. The hemorrhagic infarction was comprised of the aggregation of numerous minute foci of coagulation ne

Serial changes in the regional cerebral blood flow of patients with hypertensive intracerebral hemorrhage -- long-term follow-up SPECT study.

We analyzed serial changes in the regional cerebral blood flow (rCBF) of 13 patients with intracerebral hemorrhage by single photon emission computed tomography (SPECT) during the acute- to chronic stage (2 hr to 55 weeks). The (99m)Tc-ethyl cysteinate dimmer ((99m)Tc-ECD) was used as the nuclear mediator. The SPECT timing within 48 hours after the onset was considered to be acute stage, from 48 hours to 4 weeks to be subacute stage, and after 4 weeks to be chronic stage. The region of interest was each hemisphere in the whole brain without ventricles at the thalamic level. For semi-quantitati

Successful resection of a left insular cavernous angioma using neuronavigation and intraoperative language mapping.

Despite recent literature advocating the surgical removal of symptomatic Cavernous Angiomas (CA), even in critical brain areas, very few observations of insular CA surgery have been described, particularly in the left hemisphere. We report the case of a successful resection of a CA located in the dominant insula, using both neuronavigation and intra-operative functional mapping. This 33-year-old right-handed man harbored a left insular CA, revealed by generalized seizures following a bleed confirmed on MRI. The preoperative examination was normal. A stereotactic-guided surgery was performed un

Subcortical white matter metabolic changes remote from focal hemorrhagic lesions suggest diffuse injury after human traumatic brain injury.

OBJECTIVE: We used positron emission tomographic studies to prospectively examine the relationship between glucose and oxidative metabolism in the subcortical white matter (WM) acutely after traumatic brain injury (TBI). The objective was to determine the nature, extent, and degree of metabolic abnormalities in subcortical brain regions remote from hemorrhagic lesions. METHODS: Sixteen normal volunteers and 10 TBI patients (Glasgow Coma Scale score, 4-10; age, 17-64 yr; 6 with focal and 4 with diffuse injury) were studied. Each subject underwent dynamic positron emission tomographic studies u
