Cortical auditory disorder caused by bilateral strategic cerebral bleedings. Analysis of two cases.

The authors present the anatomical and clinical features of cortical auditory dysfunction in two patients, in whom hypertensive bleedings destroyed the cortical auditory centres in both hemispheres. The second haemorrhage occurred four years after the first bleeding in both cases. The bleedings isolated the cortical hearing centres due to the destruction of the right and left temporal white matter. During the clinical course the symptomatology has changed in both patients: pure word deafness has transformed to cortical deafness and generalised auditory agnosia. Normal pattern of brain stem aud

Prolonged unilateral vasodilatation and brain edema in fulminant hepatic failure, associated with symptomatic seizure.

We report prolonged unilateral vasodilatation and hemispheric brain edema in a 49-year-old man with fulminant hepatic failure (FHF). The patient presented with a tonic-clonic seizure caused by a hypertensive subcortical hemorrhage in the left parietal lobe. Serial computed tomography (CT) scans showed progressive darkening of the ipsilateral hemisphere, suggesting hemispheric cerebral infarction, but the patient did not show clinical signs of deterioration. Brain magnetic resonance angiography showed dilation of the large arteries of the left hemisphere. Evaluation of cerebral blood flow 7 day

Tourettism and dystonia after subcortical stroke.

The term "tourettism" has been used to describe Tourette syndrome (TS)-like symptoms secondary to some specific cause. Tics associated with attention deficit hyperactivity disorder (ADHD), obsessive-compulsive disorder (OCD), or both, are commonly present in TS, but this constellation of symptoms has been rarely attributed to stroke. We describe two boys who suffered a subcortical stroke and subsequently developed hemidystonia, tics, and behavioral comorbidities. Both had right hemispheric stroke involving the basal ganglia at 8 years of age, and in both the latency from the stroke to the onse

Hyperperfusion encephalopathies: hypertensive encephalopathy and related conditions.

BACKGROUND: Hypertensive encephalopathy (HTE) is a syndrome typified by headache, seizures, and neurologic signs associated with increased systemic blood pressures; edema in the subcortical white matter is seen on imaging studies and is usually reversible, although infarction or hemorrhage may supervene. Based on previous work, we theorize that HTE is associated with increased perfusion to the brain. Syndromes related to HTE may also be encountered in clinical situations in which perfusion to the brain is acutely increased without systemic hypertension (i.e., after treatment of high-grade caro

Two types of auditory neglect.

Auditory neglect, defined as inattention to stimuli within the left hemispace, is mostly reported in association with left ear extinction in dichotic listening. However, it remains disputed as to how far dichotic extinction reflects a primary attentional deficit and is thus appropriate for the diagnosis of auditory neglect. We report here on four patients who presented left ear extinction in dichotic listening following right unilateral hemispheric lesions. Auditory spatial attention was assessed with two additional tasks: (i) diotic test by means of interaural time differences (ITDs), simulat

Chinese writing function in patients with left versus right putaminal hemorrhage.

This study was designed to explore the Chinese writing function of patients with subcortical stroke. Two cohorts of patient subjects with either the left or the right subcortical strokes, mainly putaminal hemorrhagic strokes, and one group of normal controls participated in the study. All participants received a writing test battery including the three aspects of writing function, that is, Spontaneous Writing, Writing to Dictation, and Writing from Copy, as well as a battery of non-writing linguistic tests. Comparing with normal controls revealed that writing function change occurred in both p

Xenon-induced flow activation in patients with cerebral insult who undergo xenon-enhanced CT blood flow studies.

BACKGROUND AND PURPOSE: Stable xenon-enhanced CT ((s)Xe/CT) has gained wide acceptance in the assessment of regional cerebral blood flow (rCBF) in patients with intracranial abnormalities. The aim of this study was to test whether the contrast medium (ie, (s)Xe) itself directly induces relevant changes in rCBF, thereby distorting any valid determination of cerebral perfusion by using (s)Xe/CT. METHODS: To characterize the degree and temporal dynamics of (s)Xe-induced flow activation, a thermal diffusion (TD)-based microprobe was placed subcortically into the frontal lobe on either hemisphere

Asymmetries of visual attention after circumscribed subcortical vascular lesions.

OBJECTIVE: To investigate the role of the basal ganglia and the thalamus for basic processes of visuospatial attention METHODS: Fifteen patients with acute circumscribed vascular lesions (10 with haemorrhage and five with infarction) were included in the study. The lesions were confined exclusively to subcortical structures, such as the basal ganglia, internal capsule, and thalamus, which was confirmed by initial CT on the day of referral and MRI taken 14-28 days after clinical onset. These patients were examined with two computerised attentional tasks (one detection and one search task) meas

[Primary intracerebral hemorrhage. Retrospective study of 72 operated cases].

We analysed 72 cases of primary intracranial hemorrhage surgically treated from 1970 to 1999. The hemorrhages were diagnosed by computerized axial tomography in 52 patients. Most hematomas were situated in the cerebral hemispheres (30 percent in thalamus-basal ganglia region and 50 percent in the subcortical matter). There were 10 patients with cerebellar hemorrhage. Hypertension (based in blood pressure recordings in the hospital and history) was found in 24 patients (33 percent). The most frequent findings were coma, intracranial hypertension and hemimotor deficit. The death rate registered

[Immunohistochemical methods in the differential diagnosis of primary traumatic and subsequent secondary cerebral changes].

In a 22-year-old man, driver of a personal motor vehicle, who died within 39 hours after a traffic injury, the authors made histological and immunohistochemical examinations of the brain focused on differentiation of primary traumatic and subsequent secondary changes. In haematomas the authors revealed the presence bi- and trivalent iron by Turnbull's and Perl's reaction as well as glycophorin by immunohistochemical reactions. White matter lesions were evaluated histologically by staining according to Palmgren and immunohistochemically by detection of neuron-specific enolase, beta-amyloid prot

Right-sided neglect following a left subcortical lesion.

Unilateral spatial neglect is, in humans, typically consequent to lesions of the right hemisphere and pertains to the left hemispace. Although neglect is maximally frequent after right cortical lesions, it may also ensue from lesions confined to right subcortical structures. By contrast, hemispatial neglect consequent to left hemispheric lesions occurs less frequently. Reports of neglect following lesions to left subcortical structures are even more rare and largely anecdotal. Here we report on a right-handed man who had two successive left-sided brain lesions at an interval of 10 years from o

A 35-year-old man with cerebral hemorrhage and pheochromocytoma: the second brain-dead organ donor in Japan.

A 35-year-old man was brought into the emergency room of Keio University Hospital by ambulance because of a sudden onset of coma. His Glasgow Coma Scale was 3 and his blood pressure 150/100 mmHg. CT scanning revealed a subcortical hemorrhage 8 cm in diameter. His respiration deteriorated rapidly, and an emergency craniotomy was performed for hematoma removal and cerebral decompression. Postoperatively the patient remained in a deep coma (GCS = 3) requiring respiratory support. The family presented an organ donor card previously signed by the patient, and brain death was confirmed in accordance

Experimental investigation of cerebral contusion: histopathological and immunohistochemical evaluation of dynamic cortical deformation.

We used a new approach, termed dynamic cortical deformation (DCD), to study the neuronal, vascular, and glial responses that occur in focal cerebral contusions. DCD produces experimental contusion by rapidly deforming the cerebral cortex with a transient, nonablative vacuum pulse of short duration (25 milliseconds) to mimic the circumstances of traumatic injury. A neuropathological evaluation was performed on brain tissue from adult rats sacrificed 3 days following induction of either moderate (4 psi, n = 6) or high (8 psi, n = 6) severity DCD. In all animals, DCD produced focal hemorrhagic le

A case of amnestic syndrome caused by a subcortical haematoma in the right occipital lobe.

A case of an amnestic syndrome caused by a subcortical haematoma in the right occipital lobe is reported. A 62-year-old right-handed man presented with a sudden onset of headache to the hospital. On admission, he had a left homonymous hemianopsia, disorientation and recent memory disturbance, but had normal remote memory and digit span. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a subcortical haematoma in the right occipital lobe. These findings suggest that the patient's amnesia was caused by a lesion of the retrosplenial region in the non-dominant hemisphere.

Language related brain potentials in patients with cortical and subcortical left hemisphere lesions.

The role of the basal ganglia in language processing is currently a matter of discussion. Therefore, patients with left frontal cortical and subcortical lesions involving the basal ganglia as well as normal controls were tested in a language comprehension paradigm. Semantically incorrect, syntactically incorrect and correct sentences were presented auditorily. Subjects were required to listen to the sentences and to judge whether the sentence heard was correct or not. Event-related potentials and reaction times were recorded while subjects heard the sentences. Three different components correl

[Diagnosis and treatment of intracerebral hemorrhage].

INTRODUCTION: Intracerebral hemorrhage (ICH) is the main hypertensive mechanism occurring as the consequence of structural changes in the small perforating vessels of the cerebral hemispheres and brain stem. DEVELOPMENT: These vascular lesions cause deeply situated hemorrhages in the cerebral hemispheres (basal grey nuclei and thalamus) and brain stem (pons); less common sites are in the subcortical white matter and cerebellum. There are many non-hypertensive causes of ICH including: amyloid cerebral angiopathy, vascular malformations, intracranial tumours, the use of anticoagulant and fibrin

Imaging findings in diffuse axonal injury after closed head trauma.

Even in patients with closed head trauma, brain parenchyma can be severely injured due to disruption of axonal fibers by shearing forces during acceleration, deceleration, and rotation of the head. In this article we review the spectrum of imaging findings in patients with diffuse axonal injuries (DAI) after closed head trauma. Knowledge of the location and imaging characteristics of DAI is important to radiologists for detection and diagnosis. Common locations of DAI include: cerebral hemispheric gray-white matter interface and subcortical white matter, body and splenium of corpus callosum, b
