A case of unilateral opercular syndrome associated with a subcortical lesion.

A patient who developed a unilateral opercular syndrome following a cerebrovascular accident is described. Computed tomography showed that the lesion did not affect the opercular cortex, but involved deep white matter and the head of the caudate nucleus of the left hemisphere. Persistent hypophonia and transient aphasia were associated. Comparison with previous cases is discussed.

[Neurological diagnosis and therapeutic measures in cerebral embolism].

UNLABELLED: Stroke is caused by intracerebral or subarachnoid hemorrhage in about 15% of clinical presentations and the remaining 85% result from ischemia. About 15% of ischemic strokes are caused by emboli arising from the heart. In younger patients (18 to 50 years) with ischemic strokes or transient ischemic attacks (TIA), the incidence of cardiac embolism is increased to 23 to 36%. DIAGNOSIS: a) SYMPTOMS: Individual neurologic symptoms of stroke do not provide sensitive or specific indications of the underlying mechanism. In 25 to 82% of patients with possible embolic stroke, there is an

Putaminal hemorrhage: clinical-computed tomographic correlations.

Ninety-two percent of 100 patients with putaminal hemorrhage were hypertensive. Of the eight normotensive patients, seven were substance abusers or had bilateral putaminal hemorrhages. The one other normotensive patient was less than 40 years old. The 100 hemorrhages had the following locations: 1. medial putaminal (17 cases; six were normotensive and less than 40 years old and five were substance abusers); 2. lateral putaminal extending through the external capsule (eleven cases); 3. putaminal-capsular and subcortical white matter (32 cases); 4. putaminal cerebral hemispheric (19 cases); 5. p

Central motor conduction in ischaemic and hemorrhagic cerebral lesions.

Electrical Transcranial Cortical Stimulation was performed in 20 healthy volunteers and 14 patients having supra- or infratentorial vascular lesions. Central Conduction Time (CCT) in the Pyramidal Tract (PT) was calculated by subtraction of the Peripheral Conduction Time and the spinal synaptic delay to the Motor Evoked Potential (MEP) latency. Twelve out of fourteen patients showed prolonged MEP latencies or CCTs in the affected corporal segments. Only in 3 cases, 2 with subcortical hematoma and other with bilateral ischaemic lesions, the PT was inexcitable. Two patients, one with a discrete

Persistent hypoglossal artery in combination with multifocal arteriovenous malformations of the brain: case report.

The persistence of embryonic cerebral vessels in the adult is not a frequent occurrence, neither is the presence of multifocal arteriovenous malformations (AVMs) of the brain. The most commonly reported type of persistent carotid-basilar anastomosis is the primitive trigeminal artery, followed by the primitive hypoglossal artery (PHA). In this report, a 30-year-old, right-handed woman hospitalized because of subarachnoid hemorrhage and harboring an intracerebral-intraventricular hematoma resulting from the rupture of one of two independent AVMs of the left cerebral hemisphere, was found also t

[Multiple hemorrhagic cerebral amyloid angiopathy associated with leukoencephalopathy].

The authors present a case of cerebral amyloid angiopathy (CAA) which caused multiple recurrent subcortical hemorrhages and leukoencephalopathy similar to Binswanger's disease. CT scan revealed bilateral hypodensity of the hemispheric white matter in addition to multiple subcortical hemorrhages. The difference between white and gray matter densities was 14 approximately 15 HU. T2-weighted MRI showed the hemispheric white matter as high intensity. These findings on CT and MRI are compatible with those observed in leukoencephalopathy like Binswanger's disease. A biopsy of the cortex was performe

[Somatosensory evoked potentials over the unaffected hemisphere in patients with unilateral intracerebral hematoma].

Somatosensory evoked potentials (SEPs) recorded from the unaffected hemispheres were studied in patients of less than one month after the onset of unilateral intracerebral hematoma. We examined 66 SEPs obtained from 49 patients with putaminal hemorrhage (midline shift 0-15 mm), 38 SEPs from 25 patients with thalamic hemorrhage (midline shift 0-8 mm), and 14 SEPs from 10 patients with subcortical hemorrhage (midline shift 0-12 mm). These examinations were made after electrical stimulation of the median nerve at the wrist by using square waves of 0.2 msec in duration delivered at a rate of 3 Hz.

[A case of venous dural sinus thrombosis presenting dementia syndrome. An autopsy case].

We report clinical and pathological features of a case of dementia syndrome due to dural sinus thrombosis. In three years before admission, the patient, a 64-year old man, had had four convulsive attacks; scattered calcification shadows were disclosed by plain CT. Ten months before the admission, he complained of progressive memory disturbance and dyscalculia. General physical examination showed no remarkable abnormalities except for bruit at the left mastoid process. Although his time orientation was poor, he was otherwise fully awake. Neurologically, memory disturbance, finger agnosia, and d

[Clinical and etiological study of crossed cerebellar diaschisis. Report of two cases].

With the great progress in the imaging technique of cerebro-cerebellar perfusion and metabolism, it has been revealed that supratentorial cerebrovascular disorders often cause some reduction of contralateral cerebellar blood flow and metabolism. The phenomenon, termed crossed cerebellar diaschisis (CCD), is interpreted to be brought about by transneuronal deactivation of cerebellum via the corticopontocerebellar pathway and usually not to accompanied by limb-incoordination. We have experienced 2 cases presenting ataxia clinically and CCD in positron emission tomography (PET) which are thought

Three variant forms of subcortical aphasia in Chinese stroke patients.

Five right-handed patients with subcortical aphasia that involved the left hemisphere subcortical lesion sites were subjected to CT scans. Given their etiology, two cases were infarctions and the other three were hemorrhages. Two of the patients presented an involvement of the anterior limb of the internal capsule and of the basal ganglia and an anterior superior white-matter lesion extension. In both cases slow scanty dysarthric speech was noted; one had markedly impaired auditory comprehension, and the others were only partially impaired. The third patient presented an involvement of the pos

Traumatic basal ganglia hemorrhage: clinicopathologic features and outcome.

Traumatic basal ganglia hemorrhage (TBGH) is probably secondary to rupture of lenticulostriate or anterior choroidal arteries. We evaluated 6 consecutive cases of this entity to define its clinical and pathologic dimensions. Relative frequency of TBGH was 3% (3 left, 3 right) in this acute rehabilitation population. Lesion size and associated pathology varied. Contralateral hemiparesis, present in all, recovered to varying extents, apparently related to lesion location (posterior limb, internal capsule, or midperiventricular white matter), not size. Prolonged muteness occurred in 4 of 6; these

[A case of multiple cerebral hemorrhage related to cerebral amyloid angiopathy].

A case of multiple cerebral hemorrhage in the bilateral parietal lobes related to cerebral amyloid angiopathy (CAA) is reported. A 66-year-old man was admitted to our hospital because of headache and vomiting on Feb. 21, 1983. He had no history of hypertension, diabetes mellitus, recent head injury, or dementia. Four days prior to admission, he had behaved as if he were blind, but, had denied any blindness. On examination, he was confused and disoriented. Blood pressure was 130/80 mmHg. Abnormal findings on neurological examination included memory disturbance, apraxia for dressing, right-left

[Electrographic reflection of vegetative protective and compensatory reactions in craniocerebral trauma].

The authors substantiate the assumption that the presence of high-amplitude superslow potential fluctuations in registration from the skin surface of the head is determined by destabilization of subcortical intercentral relationships which comprises inhibition of stem and activation of posterohypothalamic structures in craniocerebral trauma. The constant potential reacts only to the sharpest period of a severe craniocerebral trauma and reflects acute inflammatory and vascular phenomena on the cortical level in the first 24 hours; maintenance or appearance of increased positive values of the co

[MR of ischemic brain diseases. A comparison with CT, PET (18-fluorodeoxyglucose) and angiographic results].

The results of MRI and CT in 55 patients with brain infarcts were compared; in 26 of these cases an additional PET examination was obtained in order to study the regional glucose utilisation. MRI was superior to CT, demonstrating 11% more of the infarcts, particularly during the first 24 hours, in small lesions confined to the grey or subcortical white matter and in infratentorial ischemic lesion. On the other hand, only CT was able to show fresh hemorrhage, although MRI was the method of choice to demonstrate old blood collections. To characterise the follow up of an infarct, CT and MRI were

Temporal evolution of hypoxic-ischaemic brain lesions in asphyxiated full-term newborns as assessed by computerized tomography.

Hypoxic-ischaemic brain lesions may be detected as low density (LD) areas by means of computerized tomography (CT), but the clinical significance of such LD areas has been controversial. Since timing might be a critical factor, we studied the temporal evolution of LD areas in 9 asphyxiated term babies who had had two or more CT, and compared the changes to the neurodevelopmental outcome. Scans were classified according to the elapsed time after asphyxia as early (day 1-7, n = 6), intermediate (week 2-4, n = 7; week 4-7, n = 3) and late CT (3 months or more, n = 7). In early scans, no, or only

Leukoencephalopathy in diffuse hemorrhagic cerebral amyloid angiopathy.

We have studied 12 patients with diffuse hemorrhagic cerebral amyloid angiopathy clinically and at postmortem examination. The brains in 8 patients had diffuse bilateral loss of myelin in the hemispheric white matter sparing the U fibers, corpus callosum, and internal capsules. The periventricular areas were predominantly affected. Microscopic examination of the white matter showed an association with subacute or chronic edematous lesions: spongiosis, swollen oligodendroglia, widening of the perivascular spaces with edema fluid or siderophages, hyalinization of the blood vessel walls, incomple

[Classifications and therapeutic indications in spontaneous intracerebral hematomas in the absence of any vascular malformation. Preliminary study apropos of 65 cases].

CT Scan and I.C.P. monitoring help us in management of spontaneous intracerebral haemorrhage to decide if surgical evacuation is necessary or not. The various opinions expressed in literature seem to depend on the lack of correlations between clinical state, CT Scan and I.C.P. monitoring. Three groups of patients were studied: Group I: patients with ingravescent coma during the first three days. Their natural history is death. Group 2: patients with disturbances of consciousness which appear, persist or increase during the first week. Group 3: awake patients with spontaneous improvement. In gr

Real time ultrasound characteristics of the acute intracerebral hemorrhage as studied in the canine model.

Real time intraoperative sonography is a valuable tool for visualizing subcortical mass lesions. Although most solid lesions are hyperechogenic, little is known about the ultrasound characteristics of intracerebral hemorrhage and subsequent hematoma formation. We sought to determine the sonographic characteristics of an acute intracerebral hematoma, to study its evolution, and to explore factors responsible for its echogenicity. Acute intracerebral hematomas were created in adult mongrel dogs using heparinized or unheparinized autologous whole blood injected under sonographic visualization int

[Right hemiasomatognosia and sensation of amputation caused by left subcortical lesion. Role of callosal disconnection].

A 72-year-old right-handed hypertensive man presented with a right brachial monoplegia, and hypesthesia of the right half of the body to touch and pricking, sparing the face. A CT scan 2 weeks later showed a spontaneous hyperdense area corresponding to a left subcortical parietal hematoma. The patient used his spontaneous language to express body image disturbances: intense prolonged feeling of amputation related to the upper limb and foot on the right side, hemiasomatognosia without anosognosia, autotopagnosia. Also associated were a right-sided visual negligence and a more general inability

Subcortical aphasia: two clinical-CT scan correlation studies.

Two clinical-CT scan studies were undertaken in order to investigate the frequency and the characteristics of aphasia following strictly subcortical left hemisphere lesions. In Study 1, all patients whose CT scan, carried out within a given period, showed subcortical lesions in both hemispheres, were evaluated for presence and type of aphasia; conversely, in Study 2 the records of all patients referred to our Aphasia Unit for language evaluation within a given period, were examined for presence of subcortical lesions on the CT scan. In Study 1 aphasia was found in two out of six patients. In S
