Mesenchymal stem cell transplantation improves motor recovery accompanied by region-specific transcriptomic responses following intracerebral hemorrhage in rats.

BACKGROUND: Mesenchymal stem cell (MSC) transplantation has emerged as a promising therapeutic strategy for intracerebral hemorrhage (ICH); however, the molecular mechanisms underlying its restorative effects remain poorly understood. This study evaluates the therapeutic effects of MSC transplantation following experimental ICH using bulk RNA sequencing, offering a comprehensive transcriptomic perspective. METHODS: Adult Sprague-Dawley (SD) rats were assigned to Sham, ICH, and ICH plus MSC (ICH + MSC) groups. ICH was induced by collagenase injection into the striatum adjacent to the internal

Pathological insights into cerebral amyloid angiopathy underlying intracerebral haemorrhage: population-based autopsy study.

Lobar intracerebral haemorrhage (ICH) is associated with cerebral amyloid angiopathy (CAA) pathology. Uncertainty remains about the mechanisms leading from CAA to ICH. We investigated the distribution and characteristics of CAA, and its clinical and neuropathological associations. Participants underwent research autopsy in the Lothian IntraCerebral Haemorrhage, Pathology, Imaging and Neurological Outcome (LINCHPIN) study. Neuropathologists rated tissue for CAA using standardised consensus criteria, as well as non-amyloid small vessel disease, Thal phase, and Braak stage. We compared the presen

Occipital Pial AVM Rupture in a Young Adult: Dual Intranidal Aneurysms, Solitary Parasagittal SSS Drainage, and Hematoma-Corridor Microsurgical Cure.

Background and Clinical Significance: Focal hemorrhagic severity associated with posterior convexity pial brain arteriovenous malformation (AVM) cases can be exacerbated by hemodynamic stress focusing on focal areas of architectural weakness and by superficial venous outflow being restricted by non-redundant superficial venous drainage. This clinical case report exemplifies how bedside neurologic localization and angioarchitectural characteristics can inform the selection of microsurgical approaches for the treatment of ruptured AVMs that are directed at reducing hemorrhage recurrence risk thr

Post-stroke rehabilitative mechanisms in individualized fatigue level-controlled treadmill training in rats.

Clinically, individualized training improves post-stroke motor function rehabilitation efficiency. However, the mechanisms underlying how individualized training facilitates recovery remain relatively unclear. Here, we explored the cortical and corticomuscular rehabilitative effects of post-stroke motor function recovery during individualized training using a rat model of intracerebral hemorrhage. Forced training or individualized fatigue-controlled training was provided from days 2 to 14 post-stroke. The fatigue-controlled training group exhibited superior motor function recovery and less cen

Contrasting patterns of leptomeningeal and parenchymal gadolinium extravasation in cerebral amyloid angiopathy: An MRI-based evaluation.

BACKGROUND: Previous studies suggest that blood-brain barrier (BBB) disruption may play a role in the pathophysiology of vessel rupture in cerebral amyloid angiopathy (CAA). Here, in a cross-sectional cohort study, we apply contrast-enhanced 3 Tesla magnetic resonance imaging (MRI) to test the hypothesis that the BBB is damaged in patients with CAA and to determine whether BBB leakage is associated with hemorrhagic brain injury in CAA. METHODS: Parenchymal BBB leakage rate (Ki) was assessed in the cortex and white matter with dynamic contrast-enhanced (DCE)-MRI and quantified with pharmacokin

Andexanet alfa Reduces Hematoma Expansion Following Controlled Cortical Impact in Mice Pretreated with Rivaroxaban.

OBJECTIVE: With the introduction of andexanet alfa, a specific antidote is now available to address life-threatening bleeding associated with factor Xa inhibitors. In this study, we explore its use in an experimental model of traumatic brain injury (TBI), mimicking a closed head trauma under rivaroxaban-induced anticoagulation. METHODS: Male C57BL6 mice were fed with rivaroxaban (10 mg/kg body weight). Subsequently, TBI was induced by controlled cortical impact (CCI) and andexanet alfa or placebo were administered as intravenous bolus injections. Edema and hemorrhage volume was quantified by

Dysfunction of inhibitory-excitatory cortical circuits induces density-dependent facilitation of motor-evoked potentials after subarachnoid hemorrhage.

Intraoperative monitoring of motor-evoked potentials (MEPs) is important for preventing serious ischemic complications during aneurysm surgery following subarachnoid hemorrhage (SAH). Reduced intraoperative MEP amplitude indicates corticospinal tract neuronal damage, which should be avoided during surgical maneuvers. We aimed to investigate whether the MEP amplitude immediately before surgery can serve as both a control value for intraoperative MEP and an indicator of the pathophysiology of cortical excitability and inhibitory-facilitatory circuits immediately after SAH and neurological functi

Diagnostic performance of prehospital large-vessel occlusion detection scales in patients with suspected acute stroke: a retrospective validation study. The Akershus Study of Ischemic Stroke and Thrombolysis.

INTRODUCTION: Prehospital stroke scales are widely used to identify patients with suspected stroke. The scales have shown potential to identify suspected large-vessel occlusion (LVO) for direct-to-thrombectomy triage, yet it remains unclear which scales perform best, and whether performance varies by patient characteristics. PATIENTS AND METHODS: In this retrospective diagnostic accuracy study, we analysed 1150 consecutive adults admitted under the stroke code protocol at a primary stroke centre (January 2015-December 2017). The cohort included ischaemic stroke, transient ischaemic attack, in

mPFC hemorrhage induces hippocampal CA1 dendritic spine plasticity and attentional deficits in mice.

BACKGROUND AND PURPOSE: Damage to a specific brain region, in addition to its local effects, can influence brain function by inducing neuroplasticity in remote areas. This study hypothesized that hemorrhage of the medial prefrontal cortex (mPFC) is associated with deficits in shifting attention through structural alterations in the hippocampal CA1 region, an area with known functional connections to the mPFC. EXPERIMENTAL APPROACH: C57BL/6 mice were subjected to intracerebral hemorrhage (ICH) or sham surgery. Neurological function was assessed using the neurological deficit score (NDS) on dif

Intraoperative neurophysiological monitoring for motor function preservation during AVMs resection: Indication or redundancy? Beyond the doctrine of "all-or-nothing".

The resection of cerebral arteriovenous malformations (AVMs) involving eloquent motor regions remains one of the most challenging tasks in neurosurgery due to the risk of irreversible motor deficits. Historically, such lesions have often been managed conservatively or with radiosurgery, particularly when located near primary motor cortex or corticospinal tracts. However, the advent of intraoperative neurophysiological monitoring (IONM) allows for real-time feedback on motor pathway function, potentially enabling safer resections in areas previously considered inoperable. This study investigate

Intracerebral Hemorrhage in a Middle-Aged Male With Classical Ehlers-Danlos Syndrome in the Absence of Macrovascular Lesions: A Case Report.

Ehlers-Danlos syndrome (EDS) is a heterogeneous connective tissue disorder characterized by tissue fragility, joint hypermobility, and variable vascular involvement. Although intracerebral hemorrhage (ICH) is classically associated with vascular EDS, emerging evidence suggests that cerebrovascular complications may occur across multiple EDS subtypes, even in the absence of overt vascular malformations. We report a 39-year-old Saudi male with genetically confirmed classical Ehlers-Danlos syndrome (cEDS) associated with COL5A1 variants who presented with acute decreased level of consciousness, e

Open Versus Laparoscopic Radical Hysterectomy Using Myoma Screw for Early-Stage Cervical Cancer- A Retrospective Analysis from Pre-LACC Era.

BACKGROUND: Diphallus, also known as penile duplication, is a rare malformation, seen once in every 5 to 6 million births. Newborns showing this condition present higher mortality rates due to malformations and infections. The underlying etiology of this malformation is uncertain, but it is thought to be associated with trauma, drug use, or infections that may affect fetal the mesoderm between the 23rd and 25th day of pregnancy. Our objective is to describe this rare malformation - diphallus - through magnetic resonance imaging, as well as additional findings. CASE: A Three-month-old male pat

Cerebral Amyloid Angiopathy With Progressive Cortical Superficial Siderosis and Convexity Subarachnoid Hemorrhage in an Apolipoprotein E (APOE) ε2/ε2 Homozygous Patient: A Case Report and Literature Review.

Sporadic cerebral amyloid angiopathy (CAA) is characterized by progressive amyloid β-protein (Aβ) deposition in small cortical and leptomeningeal vessels. It is associated with intracerebral hemorrhage (ICH), cognitive decline, and gait disturbance. Among its hemorrhagic manifestations, cortical superficial siderosis (cSS) and convexity subarachnoid hemorrhage (cSAH) are gaining increasing attention, particularly in individuals carrying the apolipoprotein E (APOE) ε2 allele. A cSAH typically represents an acute-phase lesion caused by blood leakage into the subarachnoid space, often presenting

Noninvasive mapping of visuospatial attention by navigated repetitive transcranial magnetic stimulation in patients with parietal lobe tumors.

OBJECTIVE: Visuospatial neglect corresponds to a burdening cognitive deficit with reduced space attention and disturbed stimuli detection of the contralateral side. Unilateral strokes, tumor lesions, or intracerebral hemorrhage may cause it. Identifying specific areas responsible for the onset of visuospatial neglect has proven difficult. The authors hereby aimed to detect neglect-positive areas in patients with parietal gliomas undergoing tumor resection through navigated repetitive transcranial magnetic stimulation (nrTMS). METHODS: The authors performed a monocentric prospective study that

The impact of different types of brain injuries on cerebral perfusion in preterm infants: an arterial spin labeling : MRI study.

Cerebral blood flow (CBF) is crucial for supplying the brain with the oxygen and nutrients necessary for its proper development and metabolism. The poor autoregulation of CBF is believed to play a role in the development of brain injury in preterm infants. This study aimed to investigate the characteristics of CBF in perinatal brain injury. In this retrospective study, 108 healthy preterm infants (HP), 26 infants with intracranial hemorrhage (IVH), and 16 infants with periventricular leukomalacia (PVL) were included. CBF was assessed using arterial spin labeling imaging in the frontal, tempora

A Comparison of Three Perfusion Algorithms in Patients at Risk of Delayed Cerebral Ischemia After Subarachnoid Hemorrhage.

Background/Objectives: Delayed cerebral ischemia (DCI) after an aneurysmal subarachnoid hemorrhage (aSAH) often presents with bilateral vasospasm and cortical spreading depolarizations. Computer tomography perfusion (CTP) is the prevailing screening method for detecting early changes in the cerebral blood flow. Commonly used CTP thresholds include an rCBF < 30% for the core volume and a Tmax > 6 s for hypoperfused tissue detection in acute ischemic stroke. These stroke algorithm computing thresholds compared to the contralateral hemisphere may or may not apply to detect tissue at risk of DCI.

Early Motor Cortex Connectivity and Neuronal Reactivity in Intracerebral Hemorrhage: A Continuous-Wave Functional Near-Infrared Spectroscopy Study.

Insights into motor cortex remodeling may enable the development of more effective rehabilitation strategies during the acute phase. We aim to assess the affected and unaffected motor/premotor/somatosensory cortex resting state functional connectivity (RSFC) and reactivity with continuous wave functional near-infrared spectroscopy (cw-fNIRS) in patients with ICH compared to age, sex, and comorbidity-matched subjects. We enrolled patients with acute-subacute hemispheric ICH (n = 37; two were excluded due to artifacts) and grouped them according to the side (right and left) of the stroke. Matche

Microstructural brain changes and their impact on hemorrhagic stroke recovery: a neurite orientation dispersion and density imaging-based study.

BACKGROUND: Intracerebral hemorrhage (ICH) induces profound microstructural damage, particularly to axonal integrity and connectivity. This disruption severely compromises the efficiency and precision of neural signal transmission, serving as a key determinant of post-ICH functional outcome. Precise evaluation of these changes is therefore essential for prognostication and guiding targeted rehabilitation strategies. This study aimed to investigate the correlation between the patterns of microstructural changes in brain tissue following ICH and the patients' functional outcome and cognitive sta

Perforator-Sparing Microsurgical Clipping of Tandem Dominant-Hemisphere Middle Cerebral Artery Aneurysms: Geometry-Guided Reconstruction of a Wide-Neck Bifurcation and Dorsal M1 Fusiform Lesion.

Background and Clinical Significance: Tandem pathology at the dominant-hemisphere middle cerebral artery (MCA)-combining a wide-neck bifurcation aneurysm that shares the neck with both M2 origins and a short dorsal M1 fusiform dilation embedded in the lenticulostriate belt-compresses the therapeutic margin and complicates device-first pathways. We aimed to describe an anatomy-led, microscope-only sequence designed to secure an immediate branch-definitive result at the fork and to remodel dorsal M1 without perforator compromise, and to place these decisions within a pragmatic perioperative fram
