Although the survival rates of preterm infants have markedly improved, their neurodevelopmental outcomes remain poor. Historically, brain injury in preterm infants has been recognized as intraventricular hemorrhage or white matter injury, often resulting in cerebral palsy. More recently, increasing attention has focused on the elevated risk of neurodevelopmental disorders among preterm infants across a wide range of gestational ages. Furthermore, preterm birth is also associated with an increased risk of psychiatric disorders in adulthood, including depression and schizophrenia. However, the u

CONTEXT: Intracerebral hemorrhage (ICH) accounts for half of stroke attributable deaths and two thirds of DALYs loss. Past well designed trials (2005 STICH, 2013 STICH 2, 2019 MISTIE 3, 2024 ENRICH, 2024 SWITCH) did not demonstrate benefits of surgery, aside from ENRICH which suggested improved functional outcome thanks to minimal invasive surgery for lobar ICH, but not for deep seated thalamic ICH. Since 2020, retrospective studies suggest great functional benefit of minimal invasive surgery for brainstem ICH.

BACKGROUND: Mesenchymal stem cell (MSC) transplantation has emerged as a promising therapeutic strategy for intracerebral hemorrhage (ICH); however, the molecular mechanisms underlying its restorative effects remain poorly understood. This study evaluates the therapeutic effects of MSC transplantation following experimental ICH using bulk RNA sequencing, offering a comprehensive transcriptomic perspective.
METHODS: Adult Sprague-Dawley (SD) rats were assigned to Sham, ICH, and ICH plus MSC (ICH + MSC) groups. ICH was induced by collagenase injection into the striatum adjacent to the internal

RATIONALE: Brainstem hemorrhage is a rare but devastating subtype of intracerebral hemorrhage that frequently presents with impaired consciousness, quadriplegia, and recurrent infections. Integrative personalized medicine care (IPMC), which combines essential conventional treatments with personalized traditional Korean medicine interventions, has been suggested to improve functional outcomes and support immune regulation. By closely monitoring a patient with massive brainstem hemorrhage treated with IPMC, we observed neurological improvement, effective control of recurrent infections and infla

Hypertrophic olivary degeneration (HOD) is a delayed trans-synaptic consequence of injury to the dentato-rubro-olivary pathway - the Guillain-Mollaret triangle (GMT) - and represents one of the few conditions in which the inferior olivary nucleus (ION), a pivotal node of the olivopontocerebellar system, paradoxically enlarges rather than atrophies following disconnection. Its peak magnetic resonance imaging (MRI) appearance closely mimics a primary brainstem neoplasm, and failure to recognize it has led to unnecessary biopsy in documented cases. We report a case of a 79-year-old man with uncon

The blood-brain barrier (BBB), a highly specialized neurovascular structure indispensable for preserving cerebral homeostasis, exhibits significant impairment across diverse neurological pathologies; however, its therapeutic targeting persists as a formidable challenge due to the inherent complexity of its multicellular architecture and dynamic regulatory networks. Although the Wnt/β-catenin signalling pathway orchestrates the development and maintenance of the BBB, the clinical translation of Wnt-based interventions remains elusive. We fabricated functionalized extracellular vesicles derived

Hemorrhagic transformation (HT) and disruption of the blood-brain barrier (BBB) injury are critical pathological features of ischemic stroke, profoundly influencing the natural course of the disease and treatment outcome. This study aims to elucidate how bone marrow stromal cell-derived exosome (BMSC-exo) influence BBB injury and HT and the related molecular mechanisms, focusing on the core function of exosome miR-21a-5p. The HT model induced by middle cerebral artery occlusion (MCAO-HT) was treated with tail vein injection of BMSCs-exo, and HT, BBB injury and brain damage were assessed by mNS

The brain-derived neurotrophic factor (BDNF) plays a crucial role in neuroprotection, and we have previously demonstrated BDNF-mediated neuroprotective effects in mesenchymal stromal cells (MSCs). The present study aimed to investigate whether BDNF-overexpressing MSCs enhance the therapeutic efficacy of naïve MSCs in a preclinical model of severe neonatal intraventricular hemorrhage (IVH). We exposed primary rat neuronal cells to 40 U of thrombin overnight in vitro. Subsequently, the neuronal cells were co-cultured with either naïve MSCs or BDNF-overexpressing MSCs (1×105 cells in 1 mL media)

INTRODUCTION: Hemiplegic migraine (HM) is a rare subtype of migraine with aura, characterized by transient, unilateral motor weakness. Due to its rarity, treatment strategies are lacking. Monoclonal antibodies targeting the calcitonin gene-related peptide (CGRP) pathway have emerged as a potential therapeutic option.
CASE REPORT: A 64-year-old man was admitted to an Austrian stroke unit with acute aphasia, right-sided hemiplegia, and reduced consciousness. MRI showed no ischemia or hemorrhage. Cerebrospinal fluid analysis revealed pleocytosis (146 cells/μL) without signs of infection. By Day

Oxidative stress (OS) is a hallmark of secondary brain damage after intracerebral hemorrhage (ICH), contributing to the progression of neurological damage and poor clinical outcomes. While mesenchymal stem cell-derived exosomes (MSC-Exo) demonstrate antioxidative potential, the specific mechanisms underlying their protective effects, particularly concerning mitochondrial dynamics, remain unclear. This study identifies OPA1-mediated mitochondrial fusion as a novel mechanism through which MSC-Exo alleviates oxidative stress and brain injury after ICH. In vivo fluorescence imaging and immunofluor

Clearance of aberrant cerebral amyloid-β (Aβ) deposits represents a promising therapeutic strategy for Alzheimer's disease (AD), yet current anti-Aβ immunotherapy raises safety concerns due to frequent adverse effects. Extracellular targeted protein degradation (eTPD) offers an approach for safe and efficient clearance of disease-causing proteins. Here, we develop a next-generation eTPD platform, synthetic peptide-programmed lysosome-targeting chimeras (SPYTACs), using entirely synthesized bispecific peptides. Leveraging low-density lipoprotein receptor-related protein 1 (LRP1), SPYTACs effect

BACKGROUND: Intracerebral hemorrhage (ICH) represents one of the most severe forms of cerebrovascular injury, characterized by high mortality and lasting neurological dysfunction. Neural stem cells (NSCs), which are indispensable for neurogenesis, are mainly distributed in the hippocampus, a brain region essential for learning and memory. However, alterations in hippocampal NSCs following ICH and the mechanisms that mediate these changes remain poorly defined. The present study investigates the effects of ICH on hippocampal NSCs, focusing on the regulatory role of MTHFD2 in maintaining mitocho

Intracerebral hemorrhage (ICH) is a devastating neurological condition with high mortality and morbidity. Despite advancements in medical and surgical treatments, the long-term prognosis of ICH remains poor. Stem cell therapy emerges as a promising therapeutic strategy, offering potential benefits in neuroprotection and neuroregeneration. Preclinical studies have demonstrated the efficacy of stem cell therapy in reducing brain injury and improving neurological function after ICH. In particular, the application and mechanisms of engineered stem cells in neural repair following ICH warrant furth

This study aimed to investigate the effects and potential mechanisms of bone marrow mesenchymal stem cell-derived exosomes (BMSC-Exos) on secondary brain injury (SBI) after intracerebral hemorrhage (ICH) in rats. BMSC-Exos were isolated and characterized by transmission electron microscopy and Western blot analysis of exosomal markers (CD9, CD63, and TSG101). A rat model of ICH was established by intracerebral injection of Type IV collagenase, and a PC12 cell model of ICH was induced by hemin exposure. Neurological deficits was evaluated using the modified neurological severity score (mNSS). H

Intracerebral hemorrhage (ICH) remains a devastating condition with no available therapies that can effectively mitigate secondary injury and promote neurological repair. This research presents a novel combinatorial regenerative strategy, concurrently delivering adipose-derived stromal vascular fraction (SVF) within an adhesive self-assembling peptide (HGF-RADA16-IKVAV) nanohydrogel (HGF). In a clinically relevant rat model of ICH with hematoma evacuation, the combined therapy of HGF and SVF demonstrated synergistic and enhanced efficacy. In the short term, the combined therapy demonstrated he

Traumatic brainstem injuries represent some of the most severe and life-threatening forms of head trauma, demanding a thorough understanding of their profound clinical implications. These injuries assume critical significance given the brainstem's role in controlling vital autonomic functions. Brainstem damage may be observed in many cases of head injury, occurring as primary injury or as hemorrhagic and ischemic complications from brain herniation and intracranial hypertension. This pattern is commonly seen as a secondary event in cases such as extra-axial or subdural hemorrhage, or intracere

BACKGROUND AND PURPOSE: Intracerebral hemorrhage (ICH) is a devastating acute neurological condition with high mortality and disability. Induced pluripotent stem cell-derived neural progenitor cells (iPSCNPCs) have been shown to promote behavioral recovery by enhancing neural connectivity and providing trophic support. As the adenosine monophosphate-activated protein kinase (AMPK)/ mammalian target of rapamycin (mTOR) signaling pathway is a key regulator of autophagy in stroke, we investigated its role in the context of iPSCNPC transplantation for ICH.

BACKGROUND: Cerebral venous malformations (CVMs) generally remain silent and asymptomatic in the vast majority of cases. CVM-associated lethal intracerebral hemorrhage is extremely rare.
METHODS: Here we presented a case with chest tightness, shortness of breath, fever and convulsive seizures. Chest and brain computed tomography, and laboratory testing were performed.
RESULTS: The patient suffered a sudden cardiac arrest and expired despite aggressive resuscitation efforts. Autopsy analysis revealed a venous vascular malformation located in the pons and medulla oblongata, and brainstem hemor

BACKGROUND: Neuroinflammation following intracerebral hemorrhage (ICH) is a critical contributor to secondary brain injury. Lactylation modification, a novel epigenetic regulatory mechanism, may participate in pathological processes by modulating inflammatory gene expression. This study investigates whether exosomes derived from human umbilical cord mesenchymal stem cells (hUCMSC-exos) alleviate ICH-induced neuroinflammation by regulating lactylation modification via the PI3K/AKT/PKM2/H3K18la axis.
METHODS: In vivo, an ICH model was established by stereotactic injection of collagenase IV into
