Endoscopic Evacuation of Intracerebral Hematoma Using a Modified 2.5-mL Syringe-Based Transparent Working Sheath.

BACKGROUND: Self-made syringe sheaths with larger outer diameters, such as 5-mL and 10-mL syringes, may theoretically increase the risk of white-matter disruption. Therefore, we describe a transparent working sheath based on a 2.5-mL syringe for neuroendoscopic evacuation of intracerebral hematoma. METHODS: This retrospective, single-centre technical note included 49 non-consecutive patients who underwent neuroendoscopic hematoma evacuation assisted by the modified sheath between June 2024 and December 2025. 3D Slicer and neuronavigation were used for localization. The coordinated two-surgeon

Selective Chemogenetic Blockade of Cerebello-Rubral Pathway by Double-Virus Infection Method Using Lentivirus and Adeno-Associated Virus in Rats.

Double-virus infection method of lentivirus and adeno-associated virus was used to reveal the contribution of cerebellum in rehabilitative training after intracerebral hemorrhage (ICH) in rats. Chemogenetic blockade of cerebello-rubral pathway was achieved by the injection of a lentiviral vector pseudotyped with fusion glycoprotein type E and carrying Cre recombinase into the red nucleus and an adeno-associated viral vector encoding hM4D(Gi)-mCherry in the double-floxed, inverted orientation into the cerebellar lateral nucleus. This silencing system induced by clozapine-N-oxide significantly r

Location-Specific Analysis of Hematoma Expansion in Intracerebral Hemorrhage.

BACKGROUND: Hematoma expansion (HE) is a potential modifier of intracerebral hemorrhage (ICH) outcome, yet reducing HE does not necessarily improve prognosis. This may reflect the complex relationship between location, baseline volume, HE, and outcomes, where different locations have varying hematoma growth and differing impacts on prognosis. Prior HE studies have oversimplified location categorization into lobar and deep. Therefore, we aim to investigate how HE varies across more specific location categorization and examine their interactions. METHODS: We retrospectively analyzed 691 nonanti

Fatal hemangioblastoma: a case report and literature review.

BACKGROUND: Hemangioblastoma (HB) is a benign tumor of the central nervous system (CNS), typically associated with a favorable prognosis following aggressive surgical resection. Spontaneous rupture and bleeding of these tumors are exceptionally rare, with only a small number of fatal cases reported. The rarity of such cases has impeded reliable epidemiological studies, underscoring the need to investigate the risk factors associated with HB-related mortality. CASE REPORT: A patient presented with a 12-hour history of headache and rapidly became comatose approximately 30 minutes after admissio

Dentate nucleus dilated perivascular spaces on magnetic resonance imaging in hypertensive arteriopathy.

BACKGROUND: Small vessel disease (SVD)-related MRI abnormalities include cerebral microbleeds (CMB), intracerebral hemorrhage (ICH) and supratentorial dilated perivascular spaces (DPVS). In the cerebellum, deep CMB/ICH involving dentate nucleus (DN) is associated with hypertensive arteriopathy (HA), whereas superficial cerebellar CMB/ICH is associated with cerebral amyloid angiopathy (CAA). We studied DPVS inside DN (DN-DPVS) in patients with HA or CAA. METHODS: We retrospectively analyzed DN-DPVS presence and number on T2-weighted 3T MRI (performed on follow-up in the vast majority of cases)

Ruptured intrameatal AICA aneurysm associated with a cerebellar pial arteriovenous shunt: a case report.

Intrameatal anterior inferior cerebellar artery (AICA) aneurysms associated with distal arteriovenous shunts are extremely rare. We report a ruptured intrameatal AICA aneurysm associated with a cerebellar pial arteriovenous shunt, most consistent with a pial arteriovenous fistula. A 68-year-old woman presented with posterior fossa subarachnoid hemorrhage. Digital subtraction angiography demonstrated a small intrameatal AICA aneurysm and a distal cerebellar arteriovenous shunt supplied by the AICA, brainstem perforators, and superior cerebellar artery. No definite nidus was identified, and cont

The relation between cerebral arteriovenous malformations and heart diseases in the pediatric population: a systematic review and meta-analysis.

Cerebral arteriovenous malformations (AVMs) and congenital heart diseases (CHDs) are rare, complex conditions that often present diagnostic and therapeutic challenges-especially when they coexist in pediatric patients. Though individually uncommon, their association raises important questions about shared developmental origins, systemic implications, and long-term outcomes. Understanding this relationship is crucial for improving early recognition, tailoring treatment strategies, and reducing morbidity and mortality in affected children. This systematic review and meta-analysis explored the re

Hypertrophic Olivary Degeneration Following Brainstem Hemorrhage in a Patient with Tremor: A Case Report with Serial MRI Follow-Up.

Background: Hypertrophic olivary degeneration (HOD) is a rare neurological condition resulting from trans-synaptic degeneration of the inferior olivary nucleus (ION) following disruption of the dentato-rubro-olivary pathway, also known as the Guillain-Mollaret triangle (GMT). Although the clinical and radiologic features of HOD have been previously described, the precise temporal correlation between clinical symptom onset and manifestations on magnetic resonance imaging (MRI) remains difficult to establish, and the factors contributing to accelerated disease progression are poorly understood.

Incidence, clinical characteristics and outcome of spontaneous cerebellar intracerebral haemorrhage in a Norwegian population.

INTRODUCTION: Cerebellar ICH accounts for 5%-10% of spontaneous ICH, yet cerebellar-specific incidence data are sparse and the unselected clinical spectrum and outcomes remain poorly characterised. PATIENTS AND METHODS: We retrospectively identified all adults (≥18 years) in the catchment area of a Norwegian stroke centre with spontaneous cerebellar ICH between 2009 and 2024, using discharge codes and radiological verification. We calculated crude and age-standardised incidence rates with 95% CIs, and assessed clinical characteristics, functional outcome (mRS) and mortality to 1 year. RESULT

Cerebellar hyperperfusion following endovascular obliteration of dural arteriovenous fistula: illustrative case.

BACKGROUND: Cerebral hyperperfusion after reconstructive surgery can develop intracerebral hemorrhage. However, there is no previous report of cerebellar hyperperfusion after surgical treatment for dural arteriovenous fistula (DAVF). In this report, the authors describe a case of cerebellar hyperperfusion following endovascular obliteration of DAVF. OBSERVATIONS: A man in his 70s had a progressing cognitive disturbance. Preoperative angiography showed a high-grade DAVF at the right sigmoid sinus with occlusion of the ipsilateral internal jugular vein. Since the superior sagittal sinus (SSS) w

Transient Ischemic Attack Progressing to Top-of-the-Basilar Syndrome: A Case Report.

BACKGROUND Top-of-the-basilar syndrome (TOBS) is a rare but clinically heterogeneous posterior circulation disorder characterized by acute ischemia affecting 2 or more territories supplied by the distal basilar artery, including the thalami, midbrain, occipital lobes, medial temporal lobes, and cerebellum. Intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) is an established treatment for eligible patients with TOBS, and reports of transient coma induced during rt-PA infusion remain exceedingly scarce. Endoscopic Hematoma Evacuation for Spontaneous Intracerebral Hemorrhage: A Single-Center Retrospective Analysis of 136 Consecutive Cases.

BACKGROUND: Endoscopic hematoma evacuation is an established minimally invasive treatment for spontaneous intracerebral hemorrhage (ICH), yet real-world data encompassing diverse hemorrhage locations, elderly patients, and antithrombotic use remain limited. METHODS: We retrospectively reviewed 136 consecutive patients who underwent endoscopic hematoma evacuation for spontaneous ICH at a single stroke center in Japan (April 2018-February 2026) using a dilated burr-hole approach with the Neuroport transparent sheath. Surgical indications followed the Japanese Stroke Society guidelines. The prim

Neuroprotective effects of myricetin against rotenone-ınduced parkinsonian neurotoxicity: a multidimensional analysis in rats.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by dopaminergic neuronal loss, oxidative stress (OS), and α-synuclein pathology. As OS is a major contributor to neuronal injury in PD, therapeutic approaches targeting redox homeostasis have attracted increasing interest. Rotenone (RTN)-induced Parkinsonian-like features closely resemble key aspects of human PD. Myricetin (MYR), a natural flavonol with antioxidant and anti-apoptotic properties, has emerged as a potential neuroprotective agent. This study aimed to evaluate the neuroprotective effects of MYR agai

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Gamma knife radiosurgery for cavernous malformations: a comprehensive study on symptom relief, hemorrhage rates, and histopathological changes.

This study aims to demonstrate the effect of gamma knife radiosurgery (GKRS) on symptoms, hemorrhage rates, and histopathological changes in patients with cavernous malformations (CMs), regardless of whether the symptomatic lesions are hemorrhagic. This single-center retrospective study evaluated symptomatic patients with single CMs treated with GKRS between 2016 and 2023. The patients' demographic data, presenting symptoms, GKRS radiation dose, complications developed during follow-up (hemorrhage, radiotoxicity), the rate of symptom improvement, and histopathological changes of surgically rem

Clinical and radiological presentation of cavernomas according to the Zabramski classification.

OBJECTIVE: Cerebral cavernous malformations (CCMs) are a cluster of abnormal vessels of the brain. CCMs have a low risk for intracerebral hemorrhage and appear increasingly often as incidental findings on MRI. Zabramski classification has been used to describe the radiological features of CCMs. How the Zabramski classification associates with the clinical symptoms at presentation and clinical course of the disease is not well established. METHODS: The authors reviewed the medical records of conservatively managed and surgically treated CCM patients over a 20-year period treated in Kuopio and

Occipital cerebral microbleeds as a predictor of intracranial hemorrhage in posterior reversible encephalopathy syndrome.

BACKGROUND AND OBJECTIVES: Intracranial hemorrhage (ICH), which often occurs in cases of posterior reversible encephalopathy syndrome (PRES), is regarded as a significant predictor of poor disease outcomes. While ICH and cerebral microbleeds (CMBs) can occur due to uncontrolled hypertension, the relationship between these conditions remains unclear. METHODS: This retrospective case-control study categorized PRES patients according to the presence of ICH on the day of symptom onset. The presence of CMBs or intracranial bleeding was assessed using gradient echo (GRE), susceptibility-weighted im

Acute Obstructive Hydrocephalus Due to Posterior Reversible Encephalopathy Syndrome Successfully Treated With Continuous Ventricular Drainage: A Case Report.

Posterior reversible encephalopathy syndrome (PRES) is an acute-onset neurological disorder characterized by various symptoms, including headache, visual disturbances, and seizures. The causes of PRES include hypertension, eclampsia, sepsis, renal failure, autoimmune diseases, and the use of chemotherapeutic agents or immunosuppressants. Although the exact pathophysiology remains unclear, it is thought to involve vasogenic edema and endothelial dysfunction. PRES generally has a favorable prognosis, and with appropriate early intervention, neurological function typically recovers without residu

PINK1 activation by MTK458 ameliorates neurological impairments and pyroptosis after intracerebral hemorrhage in mice.

Intracerebral hemorrhage (ICH) is often linked to severe neurological impairments, including cognitive deficits and anxiety-like behaviors. This study aimed to evaluate the therapeutic potential of PTEN-induced kinase 1 (PINK1), which is activated during ICH, as a target for mitigating these effects. C57/BL6 wild-type mice underwent ICH induction through an intrastriatal injection of autologous blood. The PINK1 activator, MTK458, was administered daily doses of 10-50 mg/kg starting one week before ICH induction and continuing for three days post-surgery. The modified neurological severity scor

Yield of MRI in patients with spontaneous deep intracerebral hemorrhage.

PURPOSE: Hypertensive hemorrhage is the most common type of nontraumatic intracerebral hemorrhage (ICH), and it characteristically originates in deep structures, particularly the basal ganglia, internal capsules, thalami, brainstem, and cerebellum. While advanced imaging modalities like MRI can help uncover culprit lesions in cases of unexplained ICH, we hypothesized that the yield of brain MRI would be low in patients with spontaneous deep intracerebral hemorrhage. METHODS: With IRB approval, we retrospectively reviewed cases of deep ICH at a single tertiary care academic center over a 5-yea
