Managing Antithrombotic Treatment in Intracerebral Hemorrhage and Ischemic Stroke Due to Cerebral Amyloid Angiopathy With Supporting Pathology.

Cerebral amyloid angiopathy (CAA) is a small vessel disease marked by β-amyloid deposition within cortical and leptomeningeal vessels. It is associated with a range of hemorrhagic manifestations, including lobar intracerebral hemorrhage (ICH), cortical superficial siderosis, and convexity subarachnoid hemorrhage (cSAH). In addition to hemorrhagic complications, cerebral ischemic events may occur due to impaired vascular reactivity and reduced cerebral perfusion. The coexistence of ischemic and hemorrhagic strokes in patients with biopsy-confirmed CAA poses significant diagnostic and therapeuti

Evaluation of Disease Severity Using CSF Biomarkers in Patients With Probable Cerebral Amyloid Angiopathy.

BACKGROUND AND OBJECTIVES: Cerebral amyloid angiopathy (CAA) is currently diagnosed using MRI-based Boston criteria. However, imaging markers represent downstream consequences of vascular injury and may not fully reflect disease severity. CSF biomarkers, particularly β-amyloid 1-42 (Aβ42) and β-amyloid 1-40 (Aβ40), may provide a more direct measure of vascular amyloid burden. We investigated the association between CSF biomarkers and hemorrhagic and nonhemorrhagic MRI markers of disease severity in probable CAA. METHODS: We conducted a retrospective multicenter cohort study including consecut

Pediatric Familial Cerebral Cavernous Malformation Associated With a Novel KRIT1 Initiation-Region Frameshift Variant.

BACKGROUND: Familial cerebral cavernous malformation (CCM) is an autosomal dominant vascular disorder with age-dependent penetrance and marked intrafamilial variability. KRIT1 loss-of-function variants are the most common genetic cause, but pediatric genotype-phenotype correlations remain limited. METHODS: Clinical, radiological, histopathological, and molecular findings of a pediatric family with suspected familial CCM were reviewed. Brain and spinal MRI, calvarial histopathology, whole-exome sequencing, segregation analysis, and targeted literature review were performed. RESULTS: The index

Method comparison and agreement of a fluorescence immunoassay-based HbA1c test with a routine HPLC method.

Alfimeprase, a fibrolase derivative with thrombolytic activity produced by recombinant DNA technology, was discovered by Amgen and was in development with Nuvelo for the treatment of stroke and catheter occlusion. However, development has been discontinued. Fibrolase is a zinc-containing metalloendopeptidase that was first isolated from the venom of the Southern copperhead snake, Agkistrodon contortrix contortrix. Alfimeprase directly degrades fibrin to break down clots. Alfimeprase is infused directly into the thrombus (side-hole catheter pushed through the entire clot) via multiple manual pu

Rollout of a care bundle for intracerebral haemorrhage across the North of England: a qualitative study.

BACKGROUND: A care bundle was developed and implemented at a hospital in North West England to improve the management of intracerebral haemorrhage (ICH) and was associated with a 44% reduction in 30-day case fatality. It was scaled out to the other hyperacute stroke units (HASUs) in the region. Findings showed the bundle could be successfully implemented into new settings, but challenges impeded its implementation. The findings were used to develop an implementation strategy to support roll-out outside the region. The ABC-ICH care bundle was then scaled up as a quality improvement initiative a

A machine learning approach to automate microinfarct and microhemorrhage screening in hematoxylin and eosin-stained human brain tissues.

Microinfarcts and microhemorrhages are characteristic lesions of cerebrovascular disease. Although multiple studies have been published, there is no one universal standard criteria for the neuropathological assessment of cerebrovascular disease. In this study, we propose a novel application of machine learning in the automated screening of microinfarcts and microhemorrhages. Utilizing whole slide images (WSIs) from postmortem human brain samples, we adapted a patch-based pipeline with convolutional neural networks. Our cohort consisted of 22 cases from the University of California Davis Alzhei

Cerebral Microbleeds and Their Association With Inflammation and Blood-Brain Barrier Leakage in Small Vessel Disease.

BACKGROUND: How cerebral microbleeds (CMBs) are formed, and how they cause tissue damage is not fully understood, but it has been suggested they are associated with inflammation, and they could also be related to increased blood-brain barrier (BBB) leakage. We investigated the relationship of CMBs with inflammation and BBB leakage in cerebral small vessel disease, and in particular, whether these 2 processes were increased in the vicinity of CMBs. METHODS: In 54 patients with sporadic cerebral small vessel disease presenting with lacunar stroke, we simultaneously assessed microglial activatio

Interleukin-1 Receptor-Associated Kinase-3 Aggravates Neuroinflammatory Injury After Intracerebral Hemorrhage via Activation NF-κB/IL-17A Pathway in Mice.

BACKGROUND: Neuroinflammatory reactions are crucial factors in secondary brain damage following intracerebral hemorrhage (ICH). Although previous studies have shown that IRAK3 is involved in immune responses, the potential effects of IRAK3 on ICH remain unclear. METHODS: Collagenase IV-induced ICH mouse model. Western blotting was used to determine the expression of IRAK3 at different time points following ICH. Immunofluorescence was used to investigate the cellular localization of IRAK3. The ICH model was treated with recombinant human IRAK3 (rh-IRAK3) or IRAK3 siRNA via an intracerebroventr

Predictors and Implications of Myocardial Injury in Intracerebral Hemorrhage.

PURPOSE: Myocardial injury, indicated by an elevation of high-sensitive cardiac Troponin (hs-cTnT), is a frequent stroke-related complication. Most studies investigated patients with ischemic stroke, but only little is known about its occurrence in patients with intracerebral hemorrhage (ICH). This study aimed to assess the frequency, predictors, and implications of myocardial injury in ICH patients. METHODS: Our retrospective analysis included 322 ICH patients. We defined myocardial injury as an elevation of hs-cTnT above the 99th percentile (i.e. 14 ng/L). Acute myocardial injury was define

When Seizures Lead to the Incidental Finding of Silicone Oil Migration.

Silicone oil, commonly used as a temporary intraocular tamponade in retinal detachment repair, may have retrobulbar migration as a possible complication of the procedure to the cerebral ventricles. This report presents the case of a 73-year-old male patient with multiple cardiovascular risk factors and diffuse atherosclerotic disease who underwent vitrectomy of the left eye several years earlier due to retinopathy and was diagnosed through retrospective analysis of all cerebral imaging studies, with the migration of silicone oil particles, from the vitreous humor of the eye to the intraventric

Remission of alcohol use disorder following traumatic brain injury with focal orbitofrontal cortex hemorrhage: case report and network mapping.

BACKGROUND: The orbitofrontal cortex (OFC) and its role in the regulation of urges/compulsion has been identified as a critical component of circuit-based addiction models. Building on such models, it was recently shown that brain lesions disrupting addictive behavior can be mapped to a common brain circuit. METHODS: We present a case of a 42-year-old woman with chronic treatment-refractory alcohol use disorder who experienced early remission following a traumatic brain injury (TBI) with focal left OFC intracerebral hemorrhage. Using a network mapping approach (normative connectome, n = 1000)

Cerebral Amyloid Angiopathy: Clinical Presentation, Sequelae and Neuroimaging Features-An Update.

The prevalence of cerebral amyloid angiopathy (CAA) has been shown to increase with age, with rates reported to be around 50-60% in individuals over 80 years old who have cognitive impairment. The disease often presents as spontaneous lobar intracerebral hemorrhage (ICH), which carries a high risk of recurrence, along with transient focal neurologic episodes (TFNE) and progressive cognitive decline, potentially leading to Alzheimer's disease (AD). In addition to ICH, neuroradiologic findings of CAA include cortical and subcortical microbleeds (MB), cortical subarachnoid hemorrhage (cSAH) and c

Aging, mitochondrial dysfunction, and cerebral microhemorrhages: a preclinical evaluation of SS-31 (elamipretide) and development of a high-throughput machine learning-driven imaging pipeline for cerebromicrovascular protection therapeutic screening.

Cerebral microhemorrhages (CMHs, also known as cerebral microbleeds) contribute to vascular cognitive impairment and dementia (VCID), with aging and hypertension being key risk factors. Mitochondrial oxidative stress is a hallmark of cerebrovascular aging, leading to endothelial dysfunction. This study tests the hypothesis that increased mitochondrial oxidative stress contributes to age-related CMH susceptibility and evaluates the mitochondrial-targeted antioxidative peptide SS-31 (elamipretide) as a potential protective agent in an aged, hypertensive mouse model. Concurrently, we developed a

Reversible Cerebral Vasoconstriction Syndrome Secondary to Paroxetine and Opioid Use in a Middle-Aged Woman: A Case Report.

Reversible cerebral vasoconstriction syndrome (RCVS) is a condition that usually presents with recurrent thunderclap headaches, focal neurological deficits, and seizures, secondary to transient cerebral vascular spasms. Although the mechanism is not well understood, medications that modulate serotonergic activity, such as selective serotonin reuptake inhibitors (SSRIs), may induce RCVS. In this case, we present a 39-year-old female patient with thunderclap headaches and a multitude of neurological deficits, including vision loss and musculoskeletal weakness. Her medical history is significant

Pediatric cerebral arteriovenous malformation diagnosed 11 years after neonatal thalamic hemorrhage: illustrative case.

BACKGROUND: Arteriovenous malformations (AVMs) are known as congenital vascular disorders. However, ruptured AVMs in neonates are extremely rare. A hypothesis suggests that micro-AVM or de novo AVMs enlarge during childhood, eventually acquiring the pathological features of adult AVMs. The authors describe a case of a pediatric AVM diagnosed 11 years after neonatal intracerebral hemorrhage. OBSERVATIONS: A 16-day-old neonate presented with sudden vomiting and convulsions. CT revealed a left thalamic hemorrhage extending into the left lateral ventricle. MRI showed no bleeding sources, includin
