Continuous Epidural Infusion of Hydroxyethyl Starch for Spontaneous Intracranial Hypotension: A Retrospective Case Series.

BACKGROUND Spontaneous intracranial hypotension is a challenging clinical entity, conventionally managed through epidural blood patching. In a subset of refractory patients who have exhausted all standard-of-care therapies, suboptimal response or recurrence are major clinical concerns and can lead to prolonged symptoms, repeated interventions, and a heavier healthcare burden. Therefore, we describe an innovative intervention-continuous epidural infusion of hydroxyethyl starch-for the treatment of spontaneous intracranial hypotension, which is intended to provide sustained modulation of epidura

Risk prediction of traumatic and non-traumatic subdural hematoma after ischemic stroke: a nationwide competing-risk model with external validation.

Subdural hematoma (SDH) represents a growing hemorrhagic concern among ischemic stroke survivors receiving long-term antithrombotic therapy. Although aging, multimorbidity, and antithrombotic exposure are all contributors to SDH, there is limited evidence on subtype-specific risk factors in stroke populations, and existing studies have often overlook competing mortality. Practical tools to guide individualized antithrombotic stewardship remain absent from contemporary guidelines. A nationwide retrospective cohort study was conducted using Korean National Health Insurance Service data from 2010

A Case of Convexity Meningioma Presenting With Acute Subdural Hematoma: A Case Report With a Pooled Analysis of 63 Reported Cases.

Meningioma-associated acute subdural hematoma (ASDH) is an exceedingly rare clinical entity, and its hemorrhagic mechanisms remain poorly understood. A 65-year-old man presented with progressive headache without a history of trauma. Neurological examination revealed no focal deficits. Computed tomography demonstrated a left convexity acute subdural hematoma, while magnetic resonance imaging revealed a small adjacent extra-axial lesion with mild contrast enhancement. Digital subtraction angiography showed only minimal tumor staining and no evidence of vascular malformation. The patient underwen

Middle Meningeal Artery Embolization for Chronic Subdural Hematoma in Cancer Patients: A Propensity-Matched Cohort Study of 90-Day Outcomes.

BACKGROUND: Cancer patients with chronic subdural hematoma (cSDH) face higher recurrence and surgical risk. Middle meningeal artery embolization (MMAE) has emerged as a minimally invasive treatment for cSDH, but evidence is limited as cancer patients are mostly excluded from trials. METHODS: Using the TriNetX US Collaborative Network, we identified adults with nontraumatic cSDH and a neoplasm diagnosed within one year before the index event. We performed four 1:1 propensity-score-matched comparisons: MMAE versus medical management and surgery plus MMAE versus surgery alone (both within cancer

Non-traumatic bilateral subdural hematoma in a 1-year-old child with optic glioma: a rare case report and literature review.

INTRODUCTION AND IMPORTANCE: Subdural hematoma (SDH) is an uncommon but potentially life-threatening condition in pediatric patients, most frequently associated with trauma or coagulation disorders. Non-traumatic bilateral SDHs are particularly rare, especially in children undergoing treatment for central nervous system tumors. Early recognition is critical, as delayed diagnosis may result in significant morbidity and mortality. CASE PRESENTATION: We report the case of a 1-year-3-month-old male with a known optic glioma receiving chemotherapy, who presented with progressive head circumference

Recurrent nontraumatic acute subdural hematoma secondary to ruptured accessory meningeal artery aneurysm: A case report and literature review.

Spontaneous acute subdural hematoma (aSDH) is an uncommon neurosurgical emergency, and aneurysms of the accessory meningeal artery (AMA) are an exceptionally rare cause. We report a case of recurrent spontaneous aSDH attributed to rupture of an AMA aneurysm, which was identified as the most likely source of both the initial and recurrent hemorrhages, successfully treated with endovascular embolization. A 36-year-old hypertensive man presented with progressive headache and drowsiness without preceding trauma. Computed tomography (CT) demonstrated a left convexity acute-on-chronic subdural hemat

Breaking age barriers: spontaneous epidural haematoma in a child with sickle cell disease.

A middle childhood boy with homozygous sickle cell disease (SCD) on hydroxyurea, previously healthy, presented with acute bilateral limb pain progressing rapidly to altered sensorium and signs of raised intracranial pressure. Cranial CT revealed bilateral epidural haematomas with a large left fronto-parietal epidural haematoma (EDH) and midline shift. He underwent urgent craniotomy, mechanical ventilation, blood transfusion and phenobarbitone coma for autonomic instability. He recovered with initial right hemiparesis that improved to independent ambulation without limitation, with no recurrenc

Glucagon-Like Peptide-1 Receptor Agonists for Patients with Conservatively Managed Nontraumatic Subdural Hematoma.

BACKGROUND AND OBJECTIVES: Medical management options for nontraumatic subdural hematoma (SDH) remain limited, with only atorvastatin demonstrating benefit in a single phase 2 randomized clinical trial and mixed evidence for dexamethasone. Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) may benefit SDH outcomes. The objective of this study was to evaluate the association between GLP-1 RA use and rescue surgery rates and mortality in patients with medically managed nontraumatic SDH. METHODS: We conducted a retrospective cohort study using the TriNetX database from 2016 to 2025, including

Aneurysms of the meningeal arteries: systematic review and illustrative case series.

Meningeal artery aneurysms are a rare but clinically significant pathology, distinct from cerebral aneurysms and classified as traumatic (trAA) or non-traumatic (non-trAA). Their natural history, optimal diagnosis, and management remain poorly defined. A PRISMA-guided systematic review was conducted (PubMed/MEDLINE, Scopus, Web of Science) up to July 2025. We analyzed all reported cases of meningeal artery aneurysms to develop a novel pathophysiology-based classification and evaluate treatment outcomes. A supplementary institutional series of nine combat-related trAA cases is presented. Among

Population-based estimates suggest middle meningeal artery embolization for subdural hematomas could significantly expand the scope of neurovascular therapies.

BACKGROUND: This study quantifies the impact of middle meningeal artery embolization (MMAE) for subdural hematomas (SDHs) by estimating a target population. METHODS: A population-based study at a tertiary hospital, the main SDH facility for a four-county population, used primary ICD-10 codes over 3 years to collate SDH hospitalizations. Clinical and imaging data confirmed traumatic versus non-traumatic and acute versus non-acute (mixed or chronic) SDH. The MMAE-eligible population included patients with non-traumatic, non-acute SDH aged ≥18 years plus patients with 'traumatic' but non-acute S

Left atrial appendage closure for patients with atrial fibrillation at high intracranial haemorrhagic risk.

BACKGROUND AND OBJECTIVES: Although left atrial appendage closure (LAAC) is performed in patients with non-valvular atrial fibrillation (NVAF) at increased risk of intracranial haemorrhage (ICH), outcome data are scarce. We assessed the detailed neurological indications for LAAC and outcomes after LAAC in high ICH risk patients. METHODS: Study population included consecutive patients with NVAF who underwent LAAC in a single hospital network between January 2015 and October 2021 because of prior ICH or the presence of high ICH risk imaging markers on brain MRI (cerebral microbleeds (CMBs)). Spontaneous haemorrhage in subdural hygroma in an Alzheimer's patient on lecanemab.

We report a female patient in her 70s with mild cognitive impairment due to Alzheimer's disease (AD) who developed a left temporal lobar haemorrhage after the third dose of lecanemab, accompanied by non-traumatic haemorrhage in a pre-existing subdural hygroma, and was diagnosed with amyloid-related imaging abnormalities with haemorrhage. The patient exhibited mild verbal paraphasia and word recall difficulty but had no other major neurological deficits. There was no additional intracranial abnormality during the course of hospitalisation. This case highlights the importance of considering subd

Improved healthcare utilization and economic outcomes of chronic subdural hematoma treatment with middle meningeal artery embolization compared to conventional surgical drainage.

BACKGROUND: Middle meningeal artery embolization (MMAE) emerges as an alternative to conventional surgical drainage (CSD) for chronic subdural hematomas (cSDH). Several studies have suggested that MMAE improves the cost efficacy of cSDH treatment. However, further comprehensive analyses of the outcomes and healthcare costs of MMAE are necessary. METHODS: Merative MarketScan Research Database from 2017-2022 was used to compare demographics, reoperation rates, complications, healthcare utilization, and payments of patients being treated with CSD, CSD and adjunctive MMAE, or standalone MMAE for

Association between interhospital transfer status and outcomes following nontraumatic subdural hemorrhage.

BACKGROUND: Patients with nontraumatic subdural hemorrhage often require immediate surgical intervention that may not be available at community hospitals and are therefore transferred to tertiary care centers. This study aims to evaluate the effects of interhospital transfer (IHT) on postoperative complications and outcomes following ntSDH. METHODS: The National Surgical Quality Improvement Program (NSQIP) database was queried for patients treated for ntSDH from January 2014 to January 2020. Baseline demographics, clinical characteristics, postoperative complications, and postoperative outcom

Spinal cord injury in abusive and accidental head injury in children, a neuropathological investigation.

The diagnosis of abusive head trauma (AbHT) in children is a challenging one that needs to be differentiated from natural disease and accidental head injury (AcHT). There is increasing evidence from the Neuroradiology field showing spinal cord injury in children subject to AbHT, which has, so far, been poorly investigated pathologically. In this study we retrospectively reviewed the forensic records of 110 paediatric head injury cases over an eight-year-period. The records included detailed circumstances of death and clinical history alongside neuropathology, ophthalmic pathology and osteo-art

Missed diagnosis; non-traumatic retroclival haematoma in adults, brief case report with review and evaluation of similar cases in the literature.

Retroclival subdural haematomas (RSH) are a rare occurrence, accounting for 0.3% of acute extra-axial haematomas. Although typically associated with trauma, non-traumatic causes include coagulopathy, pituitary apoplexy and vascular anomalies. The presence of cases due to non-traumatic causes can present significant diagnostic challenges. This article aims to shed light on the intricacies of non-traumatic RSH by conducting a review of the literature and presenting a case study of a 74-year-old woman on anticoagulants. The primary objective is to enhance our understanding of the diagnosis and tre

Standalone Middle Meningeal Artery Embolization Versus Conservative Management for Nontraumatic Subdural Hematoma.

BACKGROUND: Recent randomized control trials have demonstrated the efficacy and safety of middle meningeal artery embolization (MMAE) as an adjunct to conventional management for patients with nonacute subdural hematoma (SDH); however, a large majority of trial participants received surgical evacuation as part of the standard of care. Thus, the efficacy and safety of standalone MMAE compared with conservative management (CM) for patients with nonsurgical SDH are unclear. METHODS: This was a retrospective cohort study of the 2019 to 2021 Nationwide Readmissions Database in the United States. P
