Comatose state maintained during eight years following a vascular ponto-mesencephalic lesion.

A 24-year-old man survived for 8 years after a vascular lesion of the pons and midbrain. During these years a subresponsive comatose state with neurological signs extending from the oculomotor nuclei to the trigeminal and facial nuclei levels was present. A decorticated type of rigidity, with bilateral paralysis of the limbs accompanied by pyramidal signs, was also present. The EEG during the first months showed slow activity which afterwards gradually changed towards fast and alpha activities, maintained in the course of the years. The pathological diagnosis was a fibrous endarteritis, mainly

Problems of the pathogenesis, clinics, and therapy of panarteritis of the aorta and its branches.

138 patients with panarteritis of the aorta and its branches were examined, and the clinical and morphological findings were compared. The disease is more widespread than has been assumed so far, and is more frequent in young women. Localization of the process in the abdominal aorta with involvement (stenosis) of renal arteries causes renovascular hypertension, which often has a malignant course, especially with an ambilateral affection of renal arteries. In the treatment, repetitive therapeutic courses with corticosteroids and heparin are of fundamental importance. With an isolated affection,

[Pathogenesis of Wallenberg's syndrome].

In 1895 Wallenberg described a syndrome characterized by neurological symptoms produced by unilateral softening of the dorsolateral medulla oblongata. In the majority of cases the softening was caused by an arteriosclerotic-thrombotic occlusion of the homolateral intracranial vertebral artery or posterior inferior cerebellar artery, respectively. Occasionally an occlusion of the basilar artery or of the distal extracranial vertebral artery was responsible. In contradiction to the original thesis of Wallenberg, an embolic arterial occlusion could rarely be diagnosed. In particular cases the syn

Predicting the outcome of stroke: acute stage after cerebral infarction.

On admission to hospital during the acute phase of a stroke presumed due to ischaemic infarction in one cerebral hemisphere 93 patients were examined to determine the factors associated with a poor prognosis for immediate survival. The patients particularly at risk were those who were overtly unconscious and those with any combination of impaired consciousness, dense hemiplegia, and failure of conjugate ocular gaze towards the side of the limb weakness. Necropsy evidence suggested that these signs usually indicate infarction of the whole of one middle cerebral artery territory which is often s

Cerebral damage in neonates resulting from arteriovenous malformation of the vein of Galen.

Seven neonates with arteriovenous malformation of the vein of Galen are described. Six presented with cyanotic heart failure. Two cases had antenatal cerebral infarction, five periventricular leukomalacia, and three recent haemorrhagic infarction. Cerebral damage due to the malformation suggests that surgical correction of the arteriovenous malformation offers little to neonates, though after the neonatal period individuals can be cured by operation.
