Comparative analysis of blood flow velocity in middle cerebral artery stenosis by transcranial doppler ultrasonography and contrast-enhanced high-resolution magnetic resonance imaging.

BACKGROUND: This study aimed to investigate the optimal cutoff value of transcranial Doppler (TCD) ultrasonography for detecting middle cerebral artery (MCA) stenosis and differences in MCA blood flow velocities between plaques with different characteristics using contrast-enhanced high-resolution magnetic resonance imaging (CE-HR-MRI). METHODS: A total of 122 patients with MCA stenosis detected using TCD underwent CE-HR-MRI. Peak systolic velocity (PSV), mean flow velocity (MFV), and end-diastolic velocity (EDV) of the stenotic and distal segments were recorded. The stenotic/distal MFV ratio

Impaired cerebral autoregulation in patients with moyamoya disease and moyamoya syndrome: A prospective cross-sectional study.

Moyamoya disease and syndrome (MMDS) are rare cerebrovascular etiologies defined by stenosis of the distal intracranial internal carotid artery and proximal branches, with secondary development of small-vessel collaterals. Current techniques to characterize MMDS severity generally provide measurements in resting conditions (degree of stenosis, vessel collateralization, and regional oxygen delivery). In addition, some measurements of cerebral perfusion are performed with induced hypercapnia or acetazolamide to assess chemoreactivity. Despite the battery of exams, current techniques do not provi

Intracranial Dotter angioplasty using Tenzing (Tenzingplasty) for acute ischemic stroke with underlying intracranial atherosclerotic disease.

INTRODUCTION: Catheter-mediated (Dotter) angioplasty has been previously described for extracranial/peripheral arteries. Tenzing (Route 92 Medical, San Mateo, California, USA), a shelf-reducing delivery catheter, has an atraumatic tapered distal tip that progressively enlarges to maximal outer diameter of 1.2 mm (Tenzing 5), 1.6 mm (Tenzing 7), and 2.1 mm (Tenzing 8). OBJECTIVE: To report our initial experience treating acutely symptomatic intracranial atherosclerotic disease (ICAD) using Dotter angioplasty with Tenzing (Tenzingplasty). METHODS: After institutional review board approvals, we

Internal carotid arteries behind the hyoid bone as a clue for identifying the flip-flop phenomenon.

A 62-year-old woman presented with sudden left hemiparesis. Magnetic resonance imaging (MRI) revealed scattered acute infarctions in the right middle cerebral artery territory. MR angiography (MRA) revealed no significant stenosis in the major intracranial or extracranial arteries, but a filling defect was noted at the origin of the right internal carotid artery (ICA). On day 5, MRA demonstrated a change in the positional relationship between the ICA and the external carotid artery. Although carotid ultrasonography initially revealed the ICA was located behind the hyoid bone, making plaque ide

High-resolution MRI-based Radiomics and Machine Learning for Identification of Middle Cerebral Artery Vulnerable Plaques.

INTRODUCTION: To construct a vulnerable plaque model utilizing high-resolution magnetic resonance imaging (HR-MRI) based radiomics features and traditional features. MATERIALS AND METHODS: 110 patients with unilateral middle cerebral artery (MCA) stenosis and available HR-MRI data were enrolled. 50 plaques exhibiting acute stroke signals within the stenotic MCA region were categorized into the vulnerable plaque group, whereas the remaining 60 plaques were assigned to the stable plaque group. A traditional model was constructed by calculating the radiological plaque features. Radiomics feature

Hemodynamic impact of blood viscosity in intracranial atherosclerotic arteries with varying stenosis severity: A non-newtonian computational fluid dynamics patient specific study.

Intracranial atherosclerotic stenosis (ICAS) is a major cause of ischemic stroke, yet geometric stenosis alone may not fully reflect the functional hemodynamic burden of a lesion. This study used computational fluid dynamics (CFD) with a shear-thinning non-Newtonian Carreau viscosity model to quantify the combined effects of stenosis severity and blood viscosity on intracranial hemodynamics. A middle cerebral artery (MCA) stenosis model with an original ~70% narrowing was reconstructed from computed tomography angiography, and additional idealized stenosis variants (30%, 50%, and 90%) were gen

Optimal heart rate threshold for phase selection in beta-blocker-free coronary CT angiography using second-generation motion correction.

PURPOSE: Beta-blocker-free coronary CT angiography (CCTA) improves workflow, but is often limited by motion artifacts at higher heart rates (HRs). Advances such as faster gantry rotation and second-generation motion correction (SSF2) may overcome these limitations. We aimed to determine the optimal HR cutoff for phase selection using SSF2. METHODS: We retrospectively analyzed 368 patients who underwent CCTA without beta-blockers using a 256-row CT scanner (rotation time, 0.234 s). Systolic and diastolic phases were acquired over two consecutive heartbeats, with images reconstructed with and

Pheochromocytoma complicated with acute myocardial infarction and cerebral infarction: a case report.

BACKGROUND: Pheochromocytoma is a relatively rare neuroendocrine tumor originating from chromaffin cells of the adrenal medulla. Characterized by excessive catecholamine release, it typically presents with paroxysmal or sustained hypertension, palpitations, and other related symptoms. Catecholamine-mediated sustained hypertension can cause vascular endothelial injury, atherosclerotic plaque formation, and vasospasm, thereby increasing the risk of cardiovascular and cerebrovascular events. Cases of acute myocardial infarction and cerebral infarction at the same time are extremely rare. Owing to

Diagnosis of Bilateral Bow Hunter’s Syndrome in an Adolescent Using Dynamic Cervical Magnetic Resonance Angiography: A Case Report.

BACKGROUND: Bow hunter's syndrome (BHS), also known as rotational vertebral artery occlusion syndrome, is a hemodynamic disorder caused by mechanical compression of the vertebral artery during head rotation or hyperextension. This compression may lead to transient occlusion or significant stenosis, resulting in posterior circulation ischemia. BHS is relatively rare in clinical settings, and most reported cases occur in middle-aged or elderly patients. Its etiology is commonly associated with degenerative cervical conditions, such as osteophyte formation or disc herniation. In addition, the con

An Rnf213-deficient hypoperfusion model reveals Moyamoya-relevant neurovascular remodeling.

Moyamoya disease (MMD) is a chronic cerebrovascular disorder characterized by progressive stenosis of the terminal portions of the bilateral internal carotid arteries and their major branches, accompanied by the development of an abnormal vascular network at the base of the brain. Although RNF213 is the major susceptibility gene, its relatively low penetrance in MMD suggests that genetic predisposition requires synergistic interaction with environmental stressors to induce the characteristic pathological vascular remodeling. Studies have demonstrated that patients with MMD exhibit mild stenosi

Distinct Clinical Phenotypes in Moyamoya Disease: A Multicenter Comparison of Ischemic and Hemorrhagic Presentations.

BACKGROUND AND OBJECTIVES: Moyamoya disease (MMD) is a progressive occlusive arteriopathy marked by stenosis of the major cerebral arteries and the development of fragile basal collaterals. Although ischemic and hemorrhagic phenotypes of MMD are well described, comparative evidence evaluating differences in surgical safety and long-term treatment response between these subgroups remains limited. This study aimed to compare perioperative complications and long-term stroke risk after revascularization surgery in patients with ischemic-type vs hemorrhagic-type MMD. METHODS: We conducted a multic

Coronary intimal proliferation despite lipid reduction during the first year after heart transplantation.

Cardiac allograft vasculopathy (CAV) is a major cause of mortality after heart transplantation, characterized by intimal proliferation-induced coronary stenosis. The first year post-transplantation represents a critical period for CAV development, with most intimal proliferation occurring during this timeframe. This study aimed to investigate the temporal changes in lipid content and its relationship with intimal proliferation during the first year after heart transplantation. Serial three-vessel near-infrared spectroscopy intravascular ultrasound (NIRS-IVUS) and optical coherence tomography (

A Framework with Transformer-Based Model for Cerebrovascular Stenosis Detection in Magnetic Resonance Angiography.

Accurate identification of cerebrovascular stenosis is essential for early stroke prevention and effective clinical management. Magnetic resonance angiography provides non-invasive 3D visualization of cerebral vessels, but reliable automated stenosis detection remains challenging due to anatomical complexity and imaging variability. This study aims to develop an automated, robust, and clinically useful transformer-based deep learning framework for detecting stenosis in 3D brain MRA scans. We propose a framework designed for cerebrovascular stenosis detection. It first automatically localizes t

A Mechanism-Based Classification of Mechanical Compression of the Extracranial Carotid Arteries: A Narrative Review.

BACKGROUND: Mechanical external compression of the extracranial carotid arteries is a rare but potentially treatable cause of cerebral ischemia. Despite increasing reports of individual mechanisms, a unified and clinically applicable classification system is currently lacking. To propose a mechanism-based classification of mechanical carotid artery compression and to review the literature on diagnosis and management of this condition. METHODS: A narrative review of the literature was performed using PubMed/MEDLINE, Web of Science, and Google Scholar databases. Articles describing mechanical (

Long-segment cervicothoracic spinal cord injury occurring after cerebral angiography: a case report.

BACKGROUND: Spinal cord injury (CIS) is an exceedingly rare complication of cerebral angiography, with only nine cases since 1962. Pathogenesis remains poorly understood. CASE PRESENTATION: A 67-year-old diabetic woman presented with dizziness and normal neurological examination. During cerebral angiography, repeated injections of ioversol contrast medium (total 100 mL; approximately 20 min at a single site) were performed into the right subclavian artery for vertebral artery imaging due to vascular tortuosity. The angiography showed normal cerebrovascular morphology, without emboli, stenosis

Pre-existing intracranial arterial stenosis independently predicts delayed cerebral ischemia after aneurysmal subarachnoid hemorrhage.

PURPOSE: Delayed cerebral ischemia (DCI) remains a significant cause of morbidity and mortality following aneurysmal subarachnoid hemorrhage (aSAH). Despite identification of numerous predictors, risk remains variable even among patients with similar hemorrhage severity suggesting presence of unrecognized modifiers. One factor receiving little attention is intracranial arterial disease (ICAD): stenosis and calcification. We aim to examine their additive impacts to known modifiers. METHODS: We analyzed 526 patients with aSAH treated at a single, tertiary care center between 2015 and 2025. DCI

Effects of intraoperative inhalational versus intravenous anesthesia on perioperative neurological outcomes and long-term prognosis in patients with moyamoya disease undergoing revascularization surgery: a systematic review and meta-analysis.

BACKGROUND: Moyamoya disease (MMD) is a rare cerebrovascular disorder characterized by progressive stenosis of intracranial carotid arteries and abnormal collateral network formation. Surgical revascularization is the standard treatment for stroke prevention, but the optimal intraoperative anesthetic strategy to minimize perioperative neurological complications remains controversial. This meta-analysis compares inhalational versus intravenous anesthesia for perioperative neurological outcomes in MMD patients undergoing revascularization. METHODS: We systematically searched PubMed, Embase, Coc
