The assessment of carotid atherosclerosis using a new multipurpose ultrasound probe.

BACKGROUND AND PURPOSE: Multipurpose ultrasound probes combined with ultra-mobile ultrasound instrumentation have the potential to increase the availability and use of ultrasound examinations in the assessment of atherosclerotic burden and cardiac disease. The aim of this study was to compare the agreement of a newly developed multipurpose probe to a standard linear carotid probe in detection of atherosclerosis of the precerebral arteries. METHODS: We examined 103 patients with a multipurpose probe (General Electric, G9L MPP-9 MHz) and a standard linear probe (General Electric, Vivid 7-M12L-1

Fusion surgery for recurrent cerebellar infarctions due to bilateral atlantoaxial rotational vertebral artery occlusion.

STUDY DESIGN: Case report and literature review. OBJECTIVE: To describe a unique case of recurrent cerebral embolisms with "bilateral" atlantoaxial rotational vertebral artery occlusion (RVAO), and review the literature on this rare entity. SUMMARY OF BACKGROUND DATA: The few reports of bilateral bow hunter's stroke (BHS) contain either RVAO at the subaxial level, and BHS associated with bilateral RVAO at the atlantoaxial level has never been reported. The treatment strategy for this entity is controversial. METHODS: An 18-year-old female experienced a cerebellar embolism in the left superi

Functional assessment of high-grade ICA stenosis with duplex ultrasound and transcranial Doppler.

BACKGROUND: Duplex ultrasound (DUS) has shown a >90% accuracy compared to angiography, concerning the degree of internal carotid artery (ICA) stenosis. However, uncertainty may occur in a severe stenosis, in which peak systolic velocity (PSV) may decrease owing to high flow resistance or high backward pressure. We investigated intracranial collateral flows using transcranial Doppler (TCD) to further evaluate the hemodynamic significance of high-grade ICA stenosis. METHODS: In this retrospective study, 320 consecutive symptomatic patients were examined. The degree of ICA stenosis and collatera

Asymptomatic high flow subclavian steal in a patient with hemodialysis access.

INTRODUCTION: Subclavian steal phenomenon due to proximal subclavian artery stenosis or occlusion is not un-common but often remains asymptomatic. CASE REPORT: We describe the case of a 66-year-old man with end-stage renal disease hemodialysed through a brachio-brachial loop graft of the left forearm. Echo-Doppler precerebral examination showed a high reversed flow of 570 ml/min in the ipsilateral vertebral artery. After successful endovascular recanalization of the subclavian artery, access blood flow increased and vertebral flow decreased to 30 ml/min. Complete neurological examination was

[Dynamics of the recovery of disturbed functions in patients with stroke after carotid endarterectomy].

The recovery of disturbed functions was followed up for 1 year after ischemic stroke in 85 patients with atherosclerotic stenosis of internal carotid arteries. Fifty patients underwent carotid endarterectomy and 35 patients refused surgery. Somatic signs, neurological status measured with the NIH-NINDS, functional state (Barthel index), quality of life and neuropsychological status were assessed. The Doppler ultrasound of precerebral arteries and neurovisualization were performed. The significant positive changes, including the reduction of focal neurological deficit, the improvement of cognit

Atrial fibrillation is associated with increased risk of perioperative stroke and death from carotid endarterectomy.

BACKGROUND: Carotid endarterectomy is performed in high volume in the United States. Identifying patients with a higher risk of stroke and death after carotid endarterectomy can lead to modifications in care that would significantly reduce the occurrence of these events. This study evaluates whether atrial fibrillation is significantly associated with an increased risk of death or stroke for patients undergoing carotid endarterectomy. METHODS: This retrospective cohort study uses multivariable logistic regression analysis to assess the relationship between atrial fibrillation and death and/or

[Carotid stenosis--assessment and treatment].

BACKGROUND: About 12,000 people suffer an ischemic stroke in Norway every year; 20 % of them may be caused by emboli from precerebral arteries. We discuss the epidemiology of carotid artery stenoses and assessment upon suspicion, and provide an overview of medical treatment and indications for surgery. MATERIAL AND METHODS: The article is based on literature identified through non-systematic searches in PubMed, core medical journals and textbooks and the authors' experience from a vascular surgical department and stroke unit. RESULTS: Stroke symptoms or recurrent transient ischemic attacks (

[The final state of cerebral haemodynamics in elderly and senile age patients which died from various causes].

The state of cerebral haemodynamics in 62 elderly patients was analysed 2-4 days before death because of various causes: cardiovascular, cerebrovascular, malignant tumours and others. Cerebral haemodynamics was investigated with the help of dopplerographyc of pre-cerebral and cerebral arteries. Moreover, the medical data of all died patients and their blood vessels were investigated with the help of pathomorphological and morphometrical methods. It was discovered that the final state of cerebral haemodynamics depends on not only cerebrovascular pathology, but on some others backgrounding and a

[Operative and endovascular treatment of carotis stenosis--when is it indicated?].

BACKGROUND: Stroke is the most common cause of disability in Norway. Most strokes are ischemic, and 25-30% are caused by emboli from atherosclerotic plaques in pre-cerebral arteries. The aim of this study was to review the literature on effectiveness of stroke prevention by surgical and endovascular treatment of carotid bifurcation stenoses. MATERIAL AND METHODS: Search of the PubMed and Cochrane Library. Relevant textbook chapters and personal experience have also supported the evaluation. RESULTS AND INTERPRETATION: Prevention of stroke by carotid endarterectomy is documented in several la

Non-malignant disease mortality in meat workers: a model for studying the role of zoonotic transmissible agents in non-malignant chronic diseases in humans.

BACKGROUND: Current research efforts have mainly concentrated on evaluating the role of substances present in animal food in the aetiology of chronic diseases in humans, with relatively little attention given to evaluating the role of transmissible agents that are also present. Meat workers are exposed to a variety of transmissible agents present in food animals and their products. This study investigates mortality from non-malignant diseases in workers with these exposures. METHODS: A cohort mortality study was conducted between 1949 and 1989, of 8520 meat workers in a union in Baltimore, Ma

A preliminary clinical scale to predict the risk of in-hospital death after carotid endarterectomy.

OBJECTIVE: Carotid endarterectomy (CEA) remains the gold standard for the treatment of carotid disease, with mortality rates generally at 0.4% to 1.7%. Controversy remains with regards to its role in the treatment of the high-risk surgical population. We developed a new clinical scale incorporating weighted risk factors into a single numerical score that correlates with the risk of in-hospital death after CEA. We propose that this tool may serve to prospectively identify the high-risk patient. METHODS: We performed a retrospective analysis of 10 years (1994 to 2003) of the Maryland hospital d

Extracranial carotid artery stenosis.

BACKGROUND: Carotid endarterectomy is the most common surgical procedure used to treat stenosis of the extracranial precerebral carotid artery. Data from several randomized controlled trials are available to help guide its use in specific patient subgroups. Carotid angioplasty with stenting is also being performed, and clinical trials comparing this procedure with carotid endarterectomy are in progress. SUMMARY OF REPORT: For patients with symptomatic high-grade (ie, 70% to 99%) stenosis, carotid endarterectomy is associated with an overall benefit (risk ratio estimate for the combined end po

Life expectancies among survivors of acute cerebrovascular disease.

BACKGROUND AND PURPOSE: Stroke survivors represent a large group of persons for whom age-differentiated life expectancy tables do not exist. Such tables are vital for many purposes. The aim of the present study was to estimate age- and sex-specific life expectancies among individuals who have survived the acute phase (1 month) of a cerebrovascular disease (CVD). METHODS: All patients who were registered with the Swedish National Hospital Discharge Registry with an admission for CVD (ICD codes 430 to 438) between January 1, 1989, and November 30, 1993, and were alive at the end of 1993 (N=103

Gender differences in the treatment of cerebrovascular disease.

OBJECTIVE: Previous studies have shown that women receive fewer invasive procedures for the treatment of coronary artery disease than men, but gender differences in cerebrovascular disease have not been well studied. Our objective was to explore differences in the treatment of stroke between men and women. DESIGN: Secondary database analyses. STUDY PARTICIPANTS: We examined the use of carotid endarterectomy in a nationally representative sample of Medicare enrollees aged 65 to 84, hospitalized for a principal diagnosis of stroke in 1992, the "all strokes group". We also studied a subgroup of
