Four flow diverter stents for the treatment of a giant unruptured vertebrobasilar dissecting aneurysm: A case report.

The use of four consecutive flow diverter stents to treat a giant unruptured fusiform vertebrobasilar dissecting aneurysm (VBDA) is extremely rare, and their safety and efficacy have not been fully established. This case report describes the case of a middle-aged Chinese male patient who underwent endovascular treatment for a giant unruptured fusiform VBDA using four flow diverter stents deployed in a bridging sequence. Digital subtraction angiography revealed a fusiform aneurysm extending from the V4 segment of the left vertebral artery to the mid-basilar artery, measuring approximately 7.90

An Autopsy Case of CD4-Positive Lymphoproliferative Disorder at 38 Years Post-Transplantation Presenting With Cardiac Invasion and Cerebral Infarctions.

Lymphoma is a rare but life-threatening complication following solid organ transplantation. The vast majority of these lymphomas arise from B cells associated with Epstein-Barr virus (EBV), and a small number of cases have a T-cell origin. We here report a rare post-transplantation autopsy case of progressive T-cell lymphoma with cardiac invasion and extensive intravascular dissemination, which became evident at 38 years following kidney transplantation. The patient was a 72-year-old male who had received a kidney transplantation at 34 years of age. He had a 16-year history of recurrent cutane

Imaging the artery of Percheron: a pictorial review of associated pathology with important mimics of bithalamic abnormalities.

BACKGROUND AND PURPOSE: The Artery of Percheron (AoP) supplying bilateral paramedian thalami and rostral midbrain is a rare anatomical variant. In the event of occlusion of AoP, a characteristic pattern of ischaemia is seen, presenting as bithalamic signal abnormality on magnetic resonance imaging (MRI). However, this particular imaging finding has significant radiological and clinical overlap with other conditions, necessitating a comprehensive understanding of the imaging characteristics and potential differential diagnosis. The aim of this pictorial essay is to provide a visual documentatio

Neutrophil Extracellular Traps and Thrombolysis Resistance: New Insights for Targeting Therapies.

BACKGROUND: Thrombosis is linked to neutrophil release of neutrophil extracellular traps (NETs). NETs are proposed as a mechanism of resistance to thrombolysis. This study intends to analyze the composition of thrombi retrieved after mechanical thrombectomy, estimate the age and organization of thrombi, and evaluate associations with the use of thrombolysis, antiplatelets, and heparin. METHODS: This retrospective observational study involved 72 samples (44 from cerebral and 28 coronary arteries), which were stained with hematoxylin and eosin, anti-NE (neutrophil elastase) antibody, and anti-h

Pretreatment with a dual antiplatelet and anticoagulant (APAC) reduces ischemia-reperfusion injury in a mouse model of temporary middle cerebral artery occlusion-implications for neurovascular procedures.

BACKGROUND: Several neurovascular procedures require temporary occlusion of cerebral arteries, leading to ischemia of unpredictable length, occasionally causing brain infarction. Experimental models of cerebral ischemia-reperfusion injury have established that platelet adhesion and coagulation play detrimental roles in reperfusion injury following transient cerebral ischemia. Therefore, in a model of cerebral ischemia-reperfusion injury (IRI), we investigated the therapeutic potential of a dual antiplatelet and anticoagulant (APAC) heparin proteoglycan mimetic which is able to bind to vascular

Pulmonary edema in a young male with severe uncontrolled cardiovascular risk factors and pan-vascular atherosclerosis: a case report.

Cardiovascular risk factors are the cause of atherosclerotic disease, which can involve all the elastic and muscolo-elastic arteries. The etiopathogenesis of atherosclerosis is multifactorial since genetics, lifestyle, and comorbidities can be simultaneously involved. Clinical manifestations can be heterogeneous and include myocardial infarction, stroke, aortic aneurysms, renal artery stenosis, renal insufficiency, peripheral artery disease, etc. Currently, 70% of clinical events cannot be prevented with available drug therapy, statins included, and at least 10% of coronary events occur in app

Bilateral watershed infarcts due to hypoperfusion in the context of drug abuse: case report.

BACKGROUND: Watershed infarcts (WIs) are a distinct type of stroke with a varying clinical presentation that affects the border areas between the territories of two cerebral arteries and are typically associated with hemodynamic impairment and internal carotid artery stenosis. However, there is a paucity of data concerning its association with the history of recreational substance and drug abuse. METHODS/CASE REPORT: This case report presents a unique instance of bilateral internal watershed infarcts in a 23-year-old male with a history of polysubstance abuse, including methadone and cocaine.

Multimodal detection and analysis of microplastics in human thrombi from multiple anatomically distinct sites.

BACKGROUND: Microplastic (MP) pollution has emerged as a significant environmental concern worldwide. While extensive research has focused on their presence in marine organisms and ecosystems, their potential impact on human health, particularly on the circulatory system, remains understudied. This project aimed to identify and quantify the mass concentrations, polymer types, and physical properties of MPs in human thrombi surgically retrieved from both arterial and venous systems at three anatomically distinct sites, namely, cerebral arteries in the brain, coronary arteries in the heart, and

COVID-19 Infection Raising Suspicions of a Connection between Vascular Damage and Thrombus Formation.

Although endothelial damage has been hypothesized to be associated with coronavirus disease 2019 (COVID-19)-related cerebral infarction based on the specificity of the viral cellular invasion pathway, no case has been reported to date. We herein report a 51-year-old Japanese woman who presented with neck pain one week after COVID-19 infection. Computed tomography and magnetic resonance imaging revealed inflammation of the carotid and vertebral arteries. Ultrasonography revealed multiple flap-like structures that were assumed to be thrombi. Although the patient had no cerebral infarction, this

Constructing a Transient Ischemia Attack Model Utilizing Flexible Spatial Targeting Photothrombosis with Real-Time Blood Flow Imaging Feedback.

Transient ischemic attack (TIA) is an early warning sign of stroke and death, necessitating suitable animal models due to the associated clinical diagnostic challenges. In this study, we developed a TIA model using flexible spatially targeted photothrombosis combined with real-time blood flow imaging feedback. By modulating the excitation light using wavefront technology, we precisely created a square light spot (50 × 250 µm), targeted at the distal middle cerebral artery (dMCA). The use of laser speckle contrast imaging (LSCI) provided real-time feedback on the ischemia, while the excitation

Long-term outcomes of carotid endarterectomy with eversion technique in patients with an open circle of Willis: A multicenter registry study.

IntroductionCarotid endarterectomy (CEA) is a surgery aimed at removing atherosclerotic plaque from the carotid artery. There are classical and eversion CEA techniques. The eversion technique is the most popular because it does not require the use of implants. Eversion CEA is also associated with a lower risk of restenosis in the late postoperative period. During the surgery, the carotid artery is clamped and blood flow to the brain comes from the contralateral carotid artery and vertebral arteries. However, if the circle of Willis (COW) is not closed, compensatory mechanisms for protecting th

Bilateral thalamic infarction with posterior cerebral artery variant (the arcade artery): A case report.

RATIONALE: Bilateral thalamic infarction is a rare type of posterior circulation stroke, and it often presents with a reduced level of consciousness in the elderly. Arteriosclerosis is the primary etiology of bilateral thalamic infarction, including conditions such as native vessel stenosis or arterial-to-arterial embolism. Cardiogenic or paradoxical embolism can also lead to thrombosis of the perforator branches innervating the thalamus, and these emboli tend to disintegrate and lead to multiple lesions, even in elderly patients. Epidemiology, Pathophysiology, and Imaging of Atherosclerotic Intracranial Disease.

Intracranial atherosclerotic disease (ICAD) is one of the most common causes of stroke worldwide. Among people with stroke, those of East Asia descent and non-White populations in the United States have a higher burden of ICAD-related stroke compared with Whites of European descent. Disparities in the prevalence of asymptomatic ICAD are less marked than with symptomatic ICAD. In addition to stroke, ICAD increases the risk of dementia and cognitive decline, magnifying ICAD societal burden. The risk of stroke recurrence among patients with ICAD-related stroke is the highest among those with conf

BACKGROUND: Takayasu arteritis (TA), a rare systemic macrovasculitis in children, affects multiple arteries, causing necrosis, remodeling, stenosis, or dilatation. Due to nonspecific symptoms, early diagnosis is challenging, and it is often missed until organ dysfunction occurs. TA poses a significant health threat, with few reports on acute cerebral infarction in pediatric cases. CASE PRESENTATION: An 11-year-old girl presented with TA-induced thrombosis in the right middle cerebral artery's upper trunk. Despite prompt thrombolysis restoring blood flow, brain tissue necrosis and dysfunction
