The Unusual Suspect: Artery of Percheron Occlusion in a Young Healthy Patient.

The artery of Percheron (AOP) is a rare anatomical variant. It is a single arterial trunk that supplies the bilateral thalami and midbrain. It is the result of an anatomical variant of the diencephalic irrigation in which the thalamic paramedian arteries arise with a common trunk from the posterior cerebral artery. The AOP is associated with a clinical syndrome characterized by bilateral vertical gaze palsy, memory impairment, and hypersomnia. Occlusion of the AOP can result in bilateral thalamic infarction with highly variable presentations that often mimic conditions such as encephalitis or

Acute Ischemic Stroke in Non-Arteritic Anterior Ischemic Optic Neuropathy.

Background: Non-arteritic anterior ischemic optic neuropathy (NAION) is a neuroophthalmological disorder characterized by impaired blood flow to the optic nerve head. There is uncertainty about whether, in some cases, NAION may be caused by proximal embolism of the posterior ciliary arteries. Diffusion-weighted magnetic resonance imaging (DWI-MRI) can provide evidence of concurrent cerebral infarction that may indicate a common embolic etiology. Methods: Adults with ophthalmological diagnosis of NAION who underwent cerebral DWI-MRI within 14 days from onset of visual impairment were included i

Subarachnoid Hemorrhage From Ruptured Pseudoaneurysm Secondary to Dissection of the Anterior Cerebral Artery: A Case Report.

Arterial dissections occur when blood enters and separates the layers of the arterial wall. They are typically classified as either subintimal dissections, which are associated with ischemia or infarction, or subadventitial dissections, which can lead to subarachnoid hemorrhage (SAH). Dissections most commonly occur extradurally, particularly in the extracranial segments of the carotid and vertebral arteries. When intradural, they tend to involve the distal vertebral arteries and the supraclinoid internal carotid artery (ICA). Less commonly, dissections affect the anterior cerebral artery (ACA

Transient Cortical Blindness: A Rare Sequelae Following Endovascular Embolization of a Basilar Tip Aneurysm.

A 60-year-old woman with hypertension and a family history of aneurysms was found to have an incidental basilar tip aneurysm following a fall at Alicante airport. She had no history of polycystic kidney disease and was a non-smoker. The aneurysm measuring 1.2 cm was discovered following imaging after the head injury. The patient was counselled about treatment options, including endovascular embolization, which she opted for after understanding the cumulative risk of rupture. The procedure was performed via right common femoral artery (CFA) and left superficial femoral (SF) access under ultraso

How Topographic Diffusion-Weighted Imaging Patterns can Predict the Potential Embolic Source.

PURPOSE: To develop an imaging prediction model for patients with embolic stroke of undetermined source (ESUS), we investigated the association of topographic diffusion-weighted imaging (DWI) patterns with potential embolic sources (PES) identified by transesophageal echocardiography. METHODS: From a total of 992 consecutive patients with embolic stroke, 366 patients with the ESUS group were selected. ESUS was defined as no atrial fibrillation (Af) within 24h from admission and no PES after general examination. Clinical variables include age (> 80years, 70-80 years), sex, vascular risk factor

Posttraumatic Basal Ganglia Infarction by Lenticulostriate Artery Injury in Adult Patients: A Review.

Post-traumatic striatocapsular infarction (SCI) due to lenticulostriate artery (LSA) damage is rare. Most cases reported are in children. We discuss the pathogenesis and differential diagnosis of this kind of SCI after trauma in adult patients. The most common etiology of non-traumatic SCI are an embolism from the proximal artery, cardiogenic embolism, and atherosclerotic plaque in the proximal middle cerebral artery (MCA). However, injury of the LSA after trauma may lead to hemorrhagic infarction in the basal ganglia (BG). Post-traumatic SCI due to LSA damage might be associated with hemorrha

Tumor Embolization via the Meningohypophyseal and Inferolateral Trunk in Patients with Skull Base Tumors Using the Distal Balloon Protection Technique.

BACKGROUND AND PURPOSE: Tumor embolization through the meningohypophyseal trunk and inferolateral trunk is known to be effective in skull base tumors; however, microcatheter cannulation into these arteries is difficult, and the number of cases that can be safely embolized is limited. In this study, we present a novel embolization procedure for the meningohypophyseal trunk and inferolateral trunk using the distal balloon protection technique and detail its clinical efficacy and complication risks. We developed this procedure to allow safe embolization in patients who cannot be adequately cannul

Rat Model of Middle Cerebral Artery Occlusion.

Stroke is the third-leading cause of death and the leading cause of acquired adult disability worldwide. Several ischemic stroke models are currently available. However, mimicking focal cerebral ischemia (FCI) is the most common. The formation of an embolic or thrombotic occlusion at or near the middle cerebral artery causes most events in FCI. The current protocol closely mimics the etiology of human stroke and ensures that the results obtained are highly relevant. The method described in this protocol yields reproducible results. The success of this model in ischemic research can be examined

A novel NIR-II albumin-escaping probe for cerebral arteries and perfusion imaging in stroke mice model.

In order to guide the formulation of post-stroke treatment strategy in time, it is necessary to have real-time feedback on collateral circulation and revascularization. Currently used near-infrared II (NIR-II) probes have inherent binding with endogenous albumin, resulting in significant background signals and uncontrollable pharmacokinetics. Therefore, the albumin-escaping properties of the new probe, IR-808AC, was designed, which achieved timely excretion and low background signal, enabling the short-term repeatable injection for visualization of cerebral vessels and perfusion. We further ac

Overcoming challenges in the treatment of massive pulmonary embolism in the elderly: Role of extracorporeal membrane oxygenation.

Population aging represents a paramount medical and socio-demographic challenge globally. As living standards improve and medical technology advances, the elderly population experiences an increasing number of detected and treated pulmonary embolisms (PE). However, rescuing massive pulmonary embolism (MPE) in the elderly remains a difficult task. Conventional thrombolysis or surgical thrombolysis might be contraindicated, leading extracorporeal membrane oxygenation (ECMO) to emerge as a treatment modality for MPE in the elderly. Nevertheless, data are scarce regarding the use of ECMO as a stan

[A case of numb chin syndrome caused by postcentral gyrus infarction].

We report a case of numb chin syndrome caused by a small cortical infarction in the postcentral gyrus. A 67-year-old man suddenly developed numbness in his right lower lip and the chin. There were no apparent abnormal neurological symptoms other than numbness. MRI revealed a fresh small infarction in the left postcentral gyrus, which corresponds with the somatosensory area of the lower lip and the chin drawn by Penfield and Rasmussen. MRA showed no significant stenosis in the main trunk of the cerebral arteries. A soft plaque with irregular wall was detected in the left carotid bifurcation on

Embolization of Ruptured Aneurysms in the Intracranial Peripheral Arteries Using N-Butyl Cyanoacrylate Glue: A Case Series.

Ruptured aneurysms in peripheral arteries are rare, and an optimal treatment strategy has not yet been established. Herein, we evaluated the efficacy and safety of endovascular treatment of peripheral aneurysms located in small, tortuous vessels. In the present study, we report five consecutive cases of small, peripherally located ruptured aneurysms successfully embolized using N-butyl cyanoacrylate glue, with or without the adjunct use of coils. Five ruptured fusiform peripheral artery aneurysms in five consecutive patients were treated with parent artery occlusion using N-butyl cyanoacrylate

Performance Assessment of an Electrostatic Filter-Diverter Stent Cerebrovascular Protection Device: Evaluation of a Range of Potential Electrostatic Fields Focusing on Small Particles.

Silent Brain Infarction (SBI) is increasingly recognized in patients with cardiac conditions, particularly Atrial Fibrillation (AF) in elderly patients and those undergoing Transcatheter Aortic Valve Implantation (TAVI). While these infarcts often go unnoticed due to a lack of acute symptoms, they are associated with a threefold increase in stroke risk and are considered a precursor to ischemic stroke. Moreover, accumulating evidence suggests that SBI may contribute to the development of dementia, depression, and cognitive decline, particularly in the elderly population. The burden of SBI is s

Long-term outcomes of carotid endarterectomy with eversion technique in patients with an open circle of Willis: A multicenter registry study.

IntroductionCarotid endarterectomy (CEA) is a surgery aimed at removing atherosclerotic plaque from the carotid artery. There are classical and eversion CEA techniques. The eversion technique is the most popular because it does not require the use of implants. Eversion CEA is also associated with a lower risk of restenosis in the late postoperative period. During the surgery, the carotid artery is clamped and blood flow to the brain comes from the contralateral carotid artery and vertebral arteries. However, if the circle of Willis (COW) is not closed, compensatory mechanisms for protecting th

Bilateral thalamic infarction with posterior cerebral artery variant (the arcade artery): A case report.

RATIONALE: Bilateral thalamic infarction is a rare type of posterior circulation stroke, and it often presents with a reduced level of consciousness in the elderly. Arteriosclerosis is the primary etiology of bilateral thalamic infarction, including conditions such as native vessel stenosis or arterial-to-arterial embolism. Cardiogenic or paradoxical embolism can also lead to thrombosis of the perforator branches innervating the thalamus, and these emboli tend to disintegrate and lead to multiple lesions, even in elderly patients. PATIENTS CONCERNS: A 69-year-old man presented to our emergenc

Epidemiology, Pathophysiology, and Imaging of Atherosclerotic Intracranial Disease.

Intracranial atherosclerotic disease (ICAD) is one of the most common causes of stroke worldwide. Among people with stroke, those of East Asia descent and non-White populations in the United States have a higher burden of ICAD-related stroke compared with Whites of European descent. Disparities in the prevalence of asymptomatic ICAD are less marked than with symptomatic ICAD. In addition to stroke, ICAD increases the risk of dementia and cognitive decline, magnifying ICAD societal burden. The risk of stroke recurrence among patients with ICAD-related stroke is the highest among those with conf
