Subclavian-to-internal carotid artery bypass with carotid endarterectomy for total occlusion of the common carotid artery under awake neurologic monitoring: a case series.

BACKGROUND: Total occlusion of the common carotid artery (CCA) from its origin is rare, occurring in less than 1% of brachiocephalic stenoses. Surgical revascularization is technically demanding, especially with tandem severe ipsilateral internal carotid artery (ICA) stenosis. Procedures under regional anesthesia with continuous neurologic monitoring are rarely described. OBJECTIVE: To evaluate outcomes of subclavian-to-internal carotid artery bypass with carotid endarterectomy (CEA) in symptomatic patients with total CCA occlusion and severe ipsilateral ICA stenosis under regional anesthesia

Hybrid surgical thromboembolectomy and crossover endovascular revascularization for acute lower-limb ischemia in a patient with active ovarian cancer receiving a PARP inhibitor (fuzuloparib): a case report and literature review.

Acute limb ischemia (ALI) is both limb- and life-threatening, and represents a vascular emergency. Malignancy produces a systemic hypercoagulable state that predisposes to arterial as well as venous thrombosis, and poly(ADP-ribose) polymerase (PARP) inhibitors-now central to maintenance therapy in ovarian cancer-have, in aggregate randomized data, been associated with only a borderline venous signal and no clearly demonstrated independent arterial signal. We report a 60-year-old woman with active high-grade serous ovarian carcinoma maintained on oral fuzuloparib who presented with Rutherford c

Recanalization After Acute Cervical Artery Dissection: A Multicenter International Study.

BACKGROUND AND OBJECTIVES: The rates and factors associated with complete recanalization after cervical artery dissection (CeAD) have not been comprehensively quantified in large cohorts. We sought to determine the 6-month rate of complete recanalization in CeAD, clinical and imaging factors associated with recanalization, and risk of recurrent ischemic stroke based on recanalization status. METHODS: This was a planned subanalysis of the Antithrombotic Treatment for Stroke Prevention in Cervical Artery Dissection study, a multicenter retrospective cohort including patients with imaging-confir

Temporal Trends in the Distribution of Extracranial Carotid and Intracranial Artery Disease Among South Korean Stroke Patients.

BACKGROUND AND PURPOSE: To examine 16-year temporal trends in the distribution of symptomatic extracranial carotid artery disease (ECAD) versus intracranial artery disease (IAD) among South Korean patients with anterior-circulation steno-occlusive ischemic stroke, and to explore how evolving vascular risk profiles and their management may have influenced these patterns. METHODS: Using the CRCS-K-NIH (Clinical Research Collaboration for Stroke in Korea-National Institute of Health) registry, we retrospectively included first-ever ischemic stroke patients admitted to 9 South Korean hospitals fr

Animal models for investigating post-stroke cognitive impairment: A neurovascular trafficking perspective.

Post-stroke cognitive impairment (PSCI) is a major cause of long-term disability after ischemic stroke, yet its mechanisms remain incompletely explained by infarct volume, lesion location, or conventional vascular risk factors alone. Animal models are indispensable for clarifying how ischemic injury evolves into delayed cognitive dysfunction and for determining which mechanisms are experimentally tractable, reproducible, and translationally relevant. This review examines PSCI from a neurovascular trafficking perspective, focusing on how laboratory animal models can be used to investigate secre

Magnetic resonance imaging vs computed tomography for baseline imaging evaluation in acute basilar artery occlusion.

BACKGROUND: The optimal imaging modality, magnetic resonance imaging (MRI) or computed tomography (CT), for identifying good treatment candidates among patients with acute basilar-artery occlusion (BAO) remains unclear. METHODS: This was a post hoc analysis of the ATTENTION trial. Patients were categorized into MRI or CT groups according to the baseline imaging modality. The primary outcome was mRS 0-3 at 90 days. The secondary outcomes included mRS score shift on an ordinal scale, early neurological deterioration, time from stroke onset to arterial puncture, and time from onset to randomizat

Stent retriever angioplasty with the Tigertriever as a bailout strategy for intracranial atherosclerotic disease-related large-vessel occlusion: illustrative cases.

BACKGROUND: Intracranial atherosclerotic disease-related large-vessel occlusion (ICAD-LVO) is associated with residual stenosis and early reocclusion following mechanical thrombectomy (MT), particularly in Asian populations. Conventional stent retrievers (SRs) are optimized for embolus removal and may not provide sufficient mechanical dilation. The Tigertriever is a radially adjustable SR with operator-controlled expansion and manual constriction, enabling stentoplasty and safe retrieval. OBSERVATIONS: This study retrospectively analyzed the data of 3 consecutive patients with ICAD-LVO who we

Modeling Moyamoya disease using patient-derived induced pluripotent stem cells and organoids.

The underlying mechanisms of Moyamoya disease (MMD), a rare, progressive cerebrovascular disorder marked by distal internal carotid artery stenosis or occlusion and the formation of fragile collaterals, remain poorly understood, in part because conventional models fail to capture the genetic, vascular, and neurovascular complexity of human MMD. Driven by recent advances in disease modeling, MMD research has moved beyond early endothelial progenitor cell studies toward patient-derived induced pluripotent stem cell (iPSC)-based platforms. Here, we summarize the generation and validation of MMD-s

Risk of early stroke recurrence and procedural stroke or death in carotid near occlusion patients scheduled for carotid endarterectomy.

BACKGROUND: Carotid near occlusion (NO) refers to distal internal carotid artery (ICA) narrowing caused by proximal carotid stenosis (CS). Stroke risk and optimal management of NO remain controversial. Our aim was to determine the prevalence of NO severities, rates of preoperative ischemic cerebrovascular events (iCVEs), 30-day procedural stroke and death, and hyperperfusion syndrome in patients with NO in a prospective CS cohort undergoing carotid endarterectomy (CEA). METHODS: We reanalyzed posthoc CTAs from 443 symptomatic and asymptomatic CS patients. Subtle NO (s-NO) was defined as a 1.0

Case report: Incidental finding of a duplicated right vertebral artery in a 17-year-old female with suspected systemic lupus erythematosus.

We present the case of a 17-year-old female with suspected systemic lupus erythematosus (SLE), receiving prednisolone and hydroxychloroquine, who was evaluated for a difference in blood pressure between the upper extremities and an elevated erythrocyte sedimentation rate (ESR). CT angiography (CTA) of the cervical vessels, performed because of clinical suspicion of vasculitis, incidentally demonstrated duplication of the right vertebral artery (VA), with both limbs originating from the right subclavian artery. The medial limb coursed posterior to the common carotid artery and anterolateral to

David G. Sherman Lecture: The Eternal Quest for Optimal Stroke Prevention.

Despite decades of clinical trial activity in stroke prevention, critical gaps remain in the evidence base guiding management of symptomatic carotid and intracranial stenosis and the quality of anticoagulation care delivered to patients with atrial fibrillation (AF). This article synthesizes recent evidence and ongoing research efforts addressing 2 enduring challenges: demonstrating the value of intensive medical therapy relative to interventional approaches and improving the quality of stroke prevention care in AF. Previous trials focused on intracranial stenosis established that aggressive,

Endovascular treatment in patients with cervical or intracranial isolated internal carotid artery occlusion: A systematic review and meta-analysis.

The optimal treatment strategy for isolated internal carotid artery occlusion (IICAO) presenting as acute ischemic stroke (AIS) remains uncertain because these patients were largely excluded from pivotal thrombectomy trials. We compared endovascular treatment (EVT) with best medical treatment (BMT) for IICAO, assessing functional independence, mortality, and safety, and explored outcomes by occlusion site (cervical vs intracranial). Following PRISMA guidelines (PROSPERO CRD420251004624), PubMed, Embase, and Cochrane Library were searched through September 2025. Eligible studies enrolled adults

Retinal Microvascular Abnormalities, White Matter Injury, and Cognitive Impairment in Carotid Stenosis or Occlusion.

BACKGROUND: Severe carotid artery stenosis or occlusion (CASO) is associated with white matter injury and cognitive impairment, but markers of accompanying microvascular dysfunction remain incompletely characterized. We investigated the relationships among retinal microvascular abnormalities on optical coherence tomography angiography, white matter hyperintensities (WMH), and cognitive impairment in severe unilateral CASO. METHODS: In this multicenter cross-sectional study, 860 participants were included: 566 patients with severe unilateral CASO without acute cerebral infarction and 294 contr

Endovascular treatment for basilar artery occlusion: A Bayesian network meta-analysis comparing time-to-treatment windows and medical management.

Background and objectivesBasilar artery occlusion accounts for a small fraction of ischemic strokes but carries a disproportionally poor natural history, with high rates of death and disability under best medical therapy (BMT). Endovascular treatment is used to treat these patients, though evidence of its safety and efficacy in different time windows compared to BMT remains conflicted. To address this, this Bayesian meta-analysis evaluates the outcomes of endovascular treatment for basilar artery occlusion performed in early (0-6 h) and late (>6 h) windows versus BMT.Materials and methodsFollo

Case report and literature review of stent detachment after vertebral artery stenting.

This study presents a case of severe stenosis at the origin of the left vertebral artery, which was complicated by stent detachment after stent implantation. We carried out a retrospective analysis of the patient's clinical data and undertook a review of relevant literature. Our goals were to investigate potential risk factors for stent detachment, assess how well conservative management works in such scenarios, and offer evidence to guide individualized treatment for this type of complication. The patient was a 70-year-old male who received a self-expanding Enterprise 2 stent at the origin of

Single-stage hybrid management of severe carotid stenosis and unruptured intracranial aneurysms using direct transcarotid access: Illustrative case.

BACKGROUND: The coexistence of symptomatic extracranial internal carotid artery stenosis (ICAS) and unruptured intracranial aneurysms (UIAs) is an uncommon but clinically challenging condition for which there is no standardized treatment strategy. The optimal treatment sequence remains controversial because carotid revascularization may theoretically alter intracranial hemodynamics, whereas endovascular treatment through a critically stenotic carotid artery presents significant technical challenges. Hybrid approaches may provide an alternative for carefully selected patients. CASE DESCRIPTION

[Invasive interventions for asymptomatic stenotic internal carotid artery lesions in paradigm of modern optimal drug therapy].

Internal carotid artery ostium is the most common localization of stenotic lesions of extracranial arteries. Stroke in carotid basin usually follows acute occlusion of internal carotid artery as a result of atherosclerotic plaque rupture and vascular thrombosis, ineffective anticoagulation in patients with cardiac arrhythmias. The main purpose of invasive interventions for internal carotid artery stenosis is prevention of postoperative stroke. Currently, two surgical interventions are common: carotid endarterectomy (CEA) and "protected" internal carotid artery stenting (CAS). According to reco

Endovascular Revascularization for Chronic Vertebral Artery Occlusion with Poor Collateral Circulation and a Large Perfusion Abnormality Identified by Automated Perfusion Imaging: A Case Report.

OBJECTIVE: Endovascular revascularization is increasingly used to treat symptomatic acute intracranial vertebral artery (ICVA) occlusion, with reported clinical benefit. In contrast, chronic total occlusion (CTO) is managed primarily with conservative therapy, and the indications for and efficacy of endovascular revascularization remain uncertain. CASE PRESENTATION: A 74-year-old man with right vertebral artery (VA) occlusion and left VA stenosis was followed at our outpatient clinic. He presented with a 40-day history of dizziness and gait disturbance. MRI showed no new acute infarction, whe

Early change and new onset of vascular pathology in cervical artery dissection: implications for surveillance.

INTRODUCTION: Antithrombotic therapy is central to stroke prevention after spontaneous cervical artery dissection (sCAD), but its efficacy may depend on the underlying vascular morphology. We examined the temporal course of vascular changes after sCAD. PATIENTS AND METHODS: Consecutive patients with first-time sCAD underwent prospective serial duplex sonography for up to 10 years. In this retrospective cohort analysis, pathology change was defined as recanalisation of an occluded artery or progression from stenosis to occlusion. New pathology resulting from dissection recurrence was considere
