OBJECTIVE: Subclavian artery (SA) stenoses are typically asymptomatic because of well-developed collateral circulation. In patients who have undergone coronary artery bypass grafting (CABG) using the internal thoracic artery (ITA), SA stenosis can induce coronary-subclavian steal syndrome and cause myocardial ischemia. Therefore, immediate interventions are required for such a dangerous condition. Furthermore, SA stenosis involving the origin of the vertebral artery (VA) requires simultaneous protection of the origin of the VA due to the risk of acute occlusion during the balloon angioplasty o

BACKGROUND: Giant cell arteritis (GCA) of the vertebral artery (VA) is a rare but serious cause of ischemic stroke, however, the long-term clinical and sonographic course of GCA patients with VA involvement (VA+) is poorly understood.
METHODS: All patients with suspected GCA who were consecutively referred to our ultrasound (US) lab over a 12-year-period were analyzed. US examination (GCA-specific) of the cranial and cervical arteries was performed. Patients with a positive US diagnosis of GCA were identified, and further analysis was restricted to VA+ patients. Follow-up data were extracted

INTRODUCTION: Dynamic vertebral artery insufficiency is a rare vascular phenomenon characterized by insufficient flow of the posterior cerebral circulation induced by dynamic motion of the head. Prior case reports have been limited to vertebral artery occlusion by cervical osteophytes or other structural impingements.
CASE REPORT: In this case report, we discuss the unusual case of a 61-year-old female with a history of vertigo, diplopia, nystagmus, and left subclavian stenosis admitted to the hospital for syncope elicited by a left head turn and left shoulder raise. Transcranial Doppler (TCD

BACKGROUND: Spinal cord infarction, though rare, represents a severe complication of neurointerventional procedures involving the posterior vertebrobasilar circulation.
METHODS: We report the case of a 49-year-old male with acute basilar artery occlusion treated with staged endovascular therapy. Initial thrombectomy and balloon angioplasty successfully restored basilar artery flow. However, drug-coated balloon angioplasty for residual basilar artery stenosis on the second procedure triggered cervical spinal cord infarction. Based on this case, we conducted a systematic literature review to id

A 46-year-old lady, with insulin-dependent type 2 diabetes, ischaemic heart disease, hypertension, high cholesterol, and being a current smoker, presented with acute left-sided weakness involving her upper and lower limbs. She was admitted with a clinical diagnosis of right lacunar ischemic stroke. Initial CT brain revealed an old right basal ganglia infarct, and CT carotid angiography revealed irregularities in the right middle cerebral artery with no occlusion, subtle mural thickening, and periarterial changes surrounding the origin of the great vessels arising from the aortic arch, features

RATIONALE: Vertebral artery stump syndrome (VASS) is an uncommon etiology of posterior circulation ischemic stroke. Previous studies have proposed several possible pathogenic mechanisms. However, the precise pathogenesis and initiating factors remain unclear. We present a case of VASS occurring following carotid artery stenting, in which thrombus formation was identified in the vertebral and basilar arteries using black-blood magnetic resonance imaging (MRI). Based on these findings, we propose a plausible pathogenesis and potential triggering factors.
PATIENT CONCERNS: We report a case of po

BACKGROUND: Patients suffering from Takayasu arteritis (TA) with stenosis or occlusion of supra-aortic trunks carry a high risk of cerebral vascular ischemic events (CVIEs). We explored the relationship between cerebral blood perfusion and CVIEs in TA. METHODS: Patients with involvement of supra-aortic trunks undergoing computed tomographic perfusion (CTP) were enrolled. The primary endpoint was CVIEs. The secondary endpoint was a poor outcome (modified Rankin scale, mRS > 3). The relationship between CTP parameters, ischemic deficit (time to maximum, Tmax >6 s) and ischemic core (cerebral blo

The treatment for vertebral artery stump syndrome (VASS) remains controversial. Here, we report a case of VASS in which cervical vertebral artery (VA) occlusion was performed. A 35-year-old man was admitted for left VA cerebellar infarction caused by left cervical VA dissection with severe stenosis on angiography, and was administered aspirin (100 mg/day). One month after discharge, the patient experienced recurrent stroke with cerebellar infarction. Digital subtraction angiography revealed that the origin of the left VA was occluded and that the VA received antegrade and retrograde flow via t

Flow diverters (FDs) have been employed in the treatment of unruptured intracranial vertebral artery dissecting aneurysms (IVADAs), yielding seemingly favorable outcomes. Despite FD treatment, aneurysm persistence (incomplete occlusion post-initial treatment) can occur in some patients, potentially leading to recurrent symptoms or complications. This study aims to explore the risk factors associated with the persistence of unruptured IVADAs that have undergone treatment with FDs. The study encompassed 77 unruptured IVADAs from 75 consecutive patients who received treatment with FDs. At a media

Vertebral artery occlusion is a condition that can reduce blood flow to the vertebrobasilar system, which may cause various neurological symptoms. We report the case of a 34-year-old female patient who had a cesarean delivery under spinal anesthesia 19 days before presentation. She had no identifiable risk factors. The patient presented to the Family Health Center with complaints of dizziness. She exhibited numbness on the right side of the face, right arm, and right side of the tongue, as well as weakness in her right arm and leg, and was subsequently referred to the emergency department via

BACKGROUND: An occipital artery-vertebral artery (OA-VA) anastomosis is rarely observed on cerebral angiography. The authors present a case of posterior circulation infarction during carotid artery stenting (CAS) due to an OA-VA anastomosis.
OBSERVATIONS: A 73-year-old male presented with mild right internal carotid artery (ICA) stenosis, which progressed to near occlusion over 18 months. Cerebral angiography revealed anterograde flow from the external carotid artery (ECA) to the vertebrobasilar system via the OA-VA anastomosis, which was not recognized by the surgeons before treatment. Background: Vertebrobasilar artery occlusion (VBAO) is a life-threatening condition with often nonspecific symptoms, making early diagnosis challenging. Timely intervention is crucial, especially in cases involving distal vertebral artery stenosis. Case Report: A 65-year-old male presented with acute vertigo, dizziness, and visual disturbances, along with ipsilateral sixth cranial nerve palsy. His medical history included a treated abdominal aortic aneurysm, hypercholesterolemia, and hypertension. CT angiography (CTA) revealed an occlusion in the V4 segment of the right vertebral artery. Fibromuscular dysplasia (FMD) is characterized by the proliferation of connective tissue and smooth muscle fibers within the arterial wall, without inflammatory or atherosclerotic components, leading to stenosis, occlusion, or aneurysm formation and consequent impairment of perfusion in the affected organ. The renal, internal carotid, and vertebral arteries are most frequently involved. Secondary arterial hypertension and stroke are common clinical manifestations of FMD, as illustrated in the present case. UNLABELLED: Clevidipine (Cleviprex), a late-generation dihydropyridine calcium channel antagonist available as a lipid emulsion for intravenous infusion, is approved in the US for the reduction of blood pressure (BP) when oral therapy is not feasible or desirable. Intravenous clevidipine is effective in the treatment of both acute preoperative and postoperative hypertension in adult cardiac surgery patients, and with a rapid onset and short duration of action the drug can be easily titrated for predictable BP control. Moreover, in terms of controlling acutely elevated BP in this patient popula

BACKGROUND: Vertebral-basilar artery dissecting aneurysms (VADAs) are an uncommon phenomenon in all fields of cerebrovascular disease. The flow diverter (FD) can be used as an endoluminal reconstruction device that promotes neointima formation at the aneurysmal neck and preserves the parent artery. To date, imaging examinations such as CT angiography, MR angiography, and DSA are the main methods used to evaluate the vasculature of patients. However, none of these imaging methods can reveal the situation of neointima formation, which is of great importance in evaluating occlusion of VADAs, espe

INTRODUCTION: Radioulnar synostosis is an uncommon complication of forearm fractures and presents with varying degrees of restricted forearm movement. The diaphysial distal third synostosis is less common and excision of the synostosis is fraught with risk of re-ossification. Use of inert or biological interposing material has thus been accompanied with the synostosis excision and various methods have been described. There is still no consensus on the ideal treatment method.
CASE REPORT: We, hereby, report a case of a long-standing radioulnar synostosis with rotational restriction of movement

Vertebrobasilar insufficiency (VBI) is a significant medical condition that results from a lack of adequate blood flow to the posterior circulation of the brain. The first-line treatment involves the use of antiplatelet therapy, but in cases where patients are not responsive to drug therapy, surgical management is the next viable option. In the past, open endarterectomy was the preferred surgical approach for treating critical VBI patients. However, due to its high mortality rates and severe peri-procedural complications, its usage has decreased. Instead, the endovascular approach has emerged
