Endovascular treatment in patients with cervical or intracranial isolated internal carotid artery occlusion: A systematic review and meta-analysis.

The optimal treatment strategy for isolated internal carotid artery occlusion (IICAO) presenting as acute ischemic stroke (AIS) remains uncertain because these patients were largely excluded from pivotal thrombectomy trials. We compared endovascular treatment (EVT) with best medical treatment (BMT) for IICAO, assessing functional independence, mortality, and safety, and explored outcomes by occlusion site (cervical vs intracranial). Following PRISMA guidelines (PROSPERO CRD420251004624), PubMed, Embase, and Cochrane Library were searched through September 2025. Eligible studies enrolled adults

Retinal Microvascular Abnormalities, White Matter Injury, and Cognitive Impairment in Carotid Stenosis or Occlusion.

BACKGROUND: Severe carotid artery stenosis or occlusion (CASO) is associated with white matter injury and cognitive impairment, but markers of accompanying microvascular dysfunction remain incompletely characterized. We investigated the relationships among retinal microvascular abnormalities on optical coherence tomography angiography, white matter hyperintensities (WMH), and cognitive impairment in severe unilateral CASO. METHODS: In this multicenter cross-sectional study, 860 participants were included: 566 patients with severe unilateral CASO without acute cerebral infarction and 294 contr

Single-stage hybrid management of severe carotid stenosis and unruptured intracranial aneurysms using direct transcarotid access: Illustrative case.

BACKGROUND: The coexistence of symptomatic extracranial internal carotid artery stenosis (ICAS) and unruptured intracranial aneurysms (UIAs) is an uncommon but clinically challenging condition for which there is no standardized treatment strategy. The optimal treatment sequence remains controversial because carotid revascularization may theoretically alter intracranial hemodynamics, whereas endovascular treatment through a critically stenotic carotid artery presents significant technical challenges. Hybrid approaches may provide an alternative for carefully selected patients. CASE DESCRIPTION

[Invasive interventions for asymptomatic stenotic internal carotid artery lesions in paradigm of modern optimal drug therapy].

Internal carotid artery ostium is the most common localization of stenotic lesions of extracranial arteries. Stroke in carotid basin usually follows acute occlusion of internal carotid artery as a result of atherosclerotic plaque rupture and vascular thrombosis, ineffective anticoagulation in patients with cardiac arrhythmias. The main purpose of invasive interventions for internal carotid artery stenosis is prevention of postoperative stroke. Currently, two surgical interventions are common: carotid endarterectomy (CEA) and "protected" internal carotid artery stenting (CAS). According to reco

Early change and new onset of vascular pathology in cervical artery dissection: implications for surveillance.

INTRODUCTION: Antithrombotic therapy is central to stroke prevention after spontaneous cervical artery dissection (sCAD), but its efficacy may depend on the underlying vascular morphology. We examined the temporal course of vascular changes after sCAD. PATIENTS AND METHODS: Consecutive patients with first-time sCAD underwent prospective serial duplex sonography for up to 10 years. In this retrospective cohort analysis, pathology change was defined as recanalisation of an occluded artery or progression from stenosis to occlusion. New pathology resulting from dissection recurrence was considere

Abemaciclib and lenalidomide presenting with central retinal artery occlusion in a patient with breast cancer and multiple myeloma: a possible treatment-related association.

Central retinal artery occlusion (CRAO) is a vision-threatening vascular emergency that often causes permanent monocular visual loss. We report a woman in her late seventies with metastatic hormone receptor-positive, HER2-negative breast cancer and multiple myeloma who developed sudden, painless right monocular vision loss while receiving long-term abemaciclib and lenalidomide. Funduscopic examination showed diffuse retinal whitening, attenuated retinal arterioles and a cherry-red spot, confirming CRAO. She presented approximately 20 hours after symptom onset, outside the window for hyperacute

External carotid artery occlusion after carotid endarterectomy: Risk factors and exoscope use.

OBJECTIVE: Although carotid endarterectomy (CEA) is the standard treatment for carotid stenosis, the incidence, predictors, and clinical significance of external carotid artery (ECA) occlusion after CEA remain poorly characterized. We assessed these factors and whether exoscope introduction influenced the ECA occlusion rate. METHODS: We retrospectively analyzed 75 consecutive CEA cases with a patent preoperative ECA performed between January 2020 and December 2025 (mean age 73.4 years; 60 male). Preoperative ECA diameter was measured on MRA. Intraoperative indocyanine green (ICG) angiography

Takayasu Arteritis Presenting With Upper Extremity Ischemia Treated by Carotid-Humeral Bypass: A Case-Based Review.

Takayasu's arteritis is a chronic large-vessel vasculitis that can lead to progressive arterial stenosis and occlusion, resulting in significant ischemic complications. We report the case of a 41-year-old woman with hypertension and a history of Takayasu's arteritis diagnosed at 12 years of age who presented with worsening left upper extremity claudication. Computed tomography angiography demonstrated complete occlusion of the left subclavian and axillary arteries. Given the severity of symptoms and extent of arterial disease, a carotid-to-mid humeral bypass using a polytetrafluoroethylene gra

Data augmentation method for computer-aided diagnosis using specular reflection.

UNLABELLED: Drug-resistant Plasmodium falciparum has been on the rise, mainly due to point mutations in the pfDHFR or dihydrofolate reductase gene. This has heightened urgency to discover and develop new antimalarial drugs. This research utilized Pharmacophore Modeling, Virtual Screening, Molecular Docking and Molecular Dynamics Simulation techniques to identify potential pfDHFR inhibitors from virtual databases. Ligand-based pharmacophore hypothesis (H1) (r = 0.949, r2 = 0.883) was formulated using a training set of 28 compounds that demonstrated pfDHFR inhibitory activity across four orders

MR-VWI concentric ring sign: a potential imaging feature of internal carotid artery pseudo occlusion and predictive value for successful recanalization.

BACKGROUND: Predicting the success of endovascular recanalization in non-acute internal carotid artery occlusion (NICAO) remains a challenge. OBJECTIVE: To examine the hypothesis that the concentric ring sign observed on high-resolution magnetic resonance vessel wall imaging (MR-VWI) could serve as a potential imaging biomarker to improve the accuracy of predicting recanalization success and guide treatment decisions. METHODS: A retrospective analysis was conducted on patients with NICAO who underwent endovascular treatment at our institution between January 2020 and December 2023. Baseline

Relevance of cervical internal carotid artery patency after thrombectomy in tandem occlusion. Are we missing an opportunity to revascularize?

BACKGROUND: Treatment options for cervical internal carotid artery (c-ICA) occlusion in tandem occlusions (TOs) include emergent carotid artery stenting (eCAS) and angioplasty. We attempted to determine the impact of c-ICA reocclusion on the risk of recurrent ischemic stroke (IS) and stroke-related death, as well as functional independence. METHODS: Patients with TOs undergoing endovascular thrombectomy (EVT) from April 2016 to October 2024 were included. The primary outcome was the 90-day composite of recurrent IS and stroke-related death. Secondary outcomes included the rate of 90-day funct

Endovascular treatment for Moyamoya-associated intracranial aneurysms: a systematic review and meta-analysis.

BACKGROUND: Moyamoya disease (MMD) is a rare cerebrovascular disorder characterized by progressive stenosis of the distal internal carotid arteries, leading to increased collateral flow and the development of an abnormal vascular network, which imposes shear stress on small vessels and increases the risk of aneurysm formation. Endovascular treatment (EVT) has emerged as a potential approach for managing MMD-associated intracranial aneurysms (MMD-IA), but its safety and efficacy remain unclear. OBJECTIVE: This systematic review and meta-analysis aimed to evaluate the safety, efficacy, and long

Biphasic Progression of Radiation-Induced Carotid Stenosis: The Predictive Role of Low-Density Plaque.

BACKGROUND AND PURPOSE: Radiation therapy (RT), a primary treatment for head and neck cancer (HNC), increases the risk of radiation-induced carotid stenosis (RICS). This study examines the progression of carotid artery total occlusion (CATO) in patients with HNC after RT, focusing on low-density plaque (LDP) as a predictor, an aspect underexplored in prior research. MATERIALS AND METHODS: A retrospective cohort study assessed 44 patients with HNC who underwent RT, using 2021 follow-up data. Carotid stenosis progression was quantified via CT scans before and after LDP detection. Patients with

The "Stentriever Sandwich" Technique: Management of Stent Retriever Entrapment after Carotid Stenting in Tandem Stroke.

Tandem large-vessel occlusion refers to simultaneous occlusion of a major intracranial and extracranial artery. Endovascular treatment of tandem lesions presents substantial technical and strategic challenges. We present a case treated with mechanical thrombectomy followed by acute extracranial carotid artery stent placement. During treatment of a re-occluded intracranial vessel, the stent retriever became entrapped within the struts of the newly placed carotid stent, resulting in rapid thrombus formation. We describe our approach to managing this complication when efforts to disentangle the d

Ultrasonographic evaluation of vascular remodeling and patency after drug-coated balloon treatment for arteriovenous fistula stenosis.

BACKGROUND: Drug-coated balloons (DCBs) have shown promise in prolonging arteriovenous fistula (AVF) patency by inhibiting neointimal hyperplasia. However, few studies have longitudinally evaluated vascular remodeling after DCB treatment using ultrasound. This study aimed to evaluate vascular remodeling after DCB treatment using high-frequency ultrasound. METHODS: In this retrospective single-center study, 50 patients with single AVF stenosis were treated with DCB and followed by ultrasound at four time points: pre-treatment, immediately post-treatment, 3 months, and 6 months. Patients were c
