Flow-Augmentation Bypass Surgery: Indications and Decision-Making.

Flow-augmentation bypass aims to enhance blood flow to hypoperfused brain regions in patients with cerebrovascular steno-occlusive diseases. This surgical procedure is indicated for ischemic and hemorrhagic moyamoya vasculopathy and for selected patients with chronic steno-occlusive disease and acute ischemic stroke. Flow-augmentation bypass has been rigorously evaluated in randomized clinical trials, including the EC-IC Bypass Trial, the Carotid Occlusion Surgery Study (COSS), the Japanese Adult Moyamoya (JAM) trial, and the recent Carotid and Middle Cerebral Artery Occlusion Surgery Study (C

Neurological outcomes after carotid endarterectomy in patients with contralateral carotid occlusion: One-year follow-up.

INTRODUCTION: Neurological complications occur after CEA, independent of CCO. These neurological complications appear to occur more frequently in CEA patients with CCO. In our study, we aimed to share the perioperative and postoperative 1-year neurological results of CEA patients. METHODS: Patients who underwent CEA between January 2017 and January 2023 were retrospectively examined. Patients who underwent CEA with total occlusion of the contralateral internal carotid artery (ICA) were included in the study (42 patients). The patients were routinely checked on the 15th day, 6th month and 12th

High Middle Cerebral Artery Wall Shear Stress in Branch Atheromatous Disease: A Computational Fluid Dynamics Analysis.

AIM: Branch atheromatous disease (BAD), characterized by the occlusion of perforating branches near the orifice of a parent artery, often develops early neurological deterioration because the mechanisms underlying BAD remain unclear. Abnormal wall shear stress (WSS) is strongly associated with endothelial dysfunction and plaque growth or rupture. Therefore, we hypothesized that computational fluid dynamics (CFD) modeling could detect differences in WSS between BAD and small-vessel occlusion (SVO), both of which result from perforating artery occlusion/stenosis. METHODS: This cross-sectional o

The effect of first pass effect in mechanical thrombectomy for acute terminal internal carotid artery occlusion.

INTRODUCTION: This study aimed to elucidate the impact of the first-pass effect (PFE) on patients with acute terminal internal carotid artery occlusion. METHODS: We conducted a retrospective analysis of patients with acute terminal internal carotid artery occlusion who underwent endovascular treatment. PFE was defined as achieving complete revascularization with a single use of the thrombectomy device, without the requirement for salvage therapy during the procedure. The primary functional outcome was measured using the 90-day modified Rankin Scale (mRS). mRS score of 0-3 was defined as a fav

Lactobacillus acidophilus promotes cognitive function recovery via regulating microglial peroxisomal function in cerebral ischemia.

Cerebral ischemia causes significant mortality and is accompanied by inflammatory reactions that exacerbate damage. While the gut microbiota is linked to clinical outcomes, specific treatments and mechanisms remain unclear. Our metagenomics linked low Lactobacillus abundance to cognitive decline in patients with cerebral ischemia. L. acidophilus supplementation alleviated post-ischemic deficits in murine middle cerebral artery occlusion and bilateral carotid artery stenosis models. The neuroprotective effects of L. acidophilus were driven by enhanced gut absorption of linoleic acid (LinA). L.

ABGCEPD for acute symptomatic isolated cervical internal carotid artery occlusion to improve reperfusion and reduce distal embolization.

The optimal endovascular strategy for acute symptomatic isolated cervical internal carotid artery occlusion (sICAO) management remains unclear because of the anatomical complexity and high risk of distal embolization. This study looks into the safety, effectiveness, and outcomes of a novel Aspiration-Based Dual-Protection Strategy Under Proximal Balloon Occlusion and Distal Embolic Protection Device (ABGCEPD) for sICAO treatment. This retrospective study involved 65 patients with acute moderate-to-severe stroke who underwent endovascular treatment using ABGCEPD at three hospitals in China betw

Carotid artery calcification as a predictor of systemic vascular events.

BACKGROUND: Coronary calcification is a well-known predictor of coronary events, yet the impact of carotid artery calcification on systemic vascular events (e.g., cerebral ischemic, coronary, and peripheral artery events) remains unclear. The aim of this study was to determine whether carotid calcification can be used to predict systemic vascular events. METHODS: This single-center, retrospective cohort study included 194 patients who had a history of vascular disease, including carotid stenosis or occlusion, coronary artery disease, valvular heart disease, ischemic stroke, or transient ische

Bilateral Internal Carotid Artery Spasms Triggered by Elongated Styloid Processes: A Case Report.

Elongated styloid processes that interact with the internal carotid arteries (ICAs) have been documented to lead to carotid artery stenosis, occlusion, or dissection, resulting in transient ischemic attack (TIA) or cerebral infarction. This study reports a unique case of bilateral ICA vasospasms triggered by elongated styloid processes and neck movements during physical activity, highlighting a potential vasospastic origin. A 52-year-old man experienced TIAs with visual disturbances while playing golf. Diagnostic imaging, including magnetic resonance angiography (MRA) and dynamic cerebral angi

The "One Coin Bypass" Technique as a Minimally Invasive Navigation-Guided Superficial Temporal Artery to Middle Cerebral Artery Bypass.

BACKGROUND: Minimally invasive cerebral revascularization techniques are increasingly sought to reduce surgical trauma while preserving efficacy. The "One Coin bypass" (OCB) is a navigation-guided superficial temporal artery to middle cerebral artery (STA-MCA) bypass performed through a coin-sized craniectomy and 3.0 cm skin incision. This study aimed to evaluate its clinical feasibility and surgical benefits. METHODS: We retrospectively analyzed 29 patients who underwent single-vessel STA-MCA bypass for symptomatic internal carotid or MCA stenosis or occlusion between 2017 and 2022. The OCB

Comparison of Exoscopic and Microscopic Superficial Temporal Artery to Middle Cerebral Artery Bypass.

PURPOSE: Exoscopes have been used as a reliable surgical tool in the neurosurgical field. They could improve surgical field visibility with 4K three-dimensional (4K-3D) monitors and provide alleviation for the physical strain by allowing surgeons to maintain a neutral posture. In this study, we report our initial experience with exoscopic superficial temporal artery (STA)-middle cerebral artery (MCA) bypass compared to the conventional microscope at our institution. METHODS: This study included 58 patients with ischemic status, including internal carotid artery (ICA) stenosis, ICA occlusion,

A case series of cerebrovascular abnormalities in Townes sickle cell mice visualized with magnetic resonance imaging and angiography.

Arterial complications in sickle cell disease (SCD), including stenoses and occlusions, are critical contributors to stroke. Townes SCD mice exhibit neurocognitive deficits and micro-vasculopathy, however stenoses and occlusions that could be causal to ischemic strokes have not yet been confirmed, which has led to challenges whether murine pathology reflects human pathology for strokes due to SCD. In our longitudinal study using label-free magnetic resonance angiography (MRA) to image carotid arteries in Townes SCD mice as they aged from 1 to 7 months, we identified multiple stenoses and occlu

Prominence of deep medullary veins associated with the risk of stroke in unilateral chronic internal carotid artery occlusion patients.

PURPOSE: To clarify the relationship between deep medullary veins (DMV) and risk of post-discharge stroke in chronic internal carotid artery occlusion (CICAO) patients. METHODS: Hospitalized patients were divided into CICAO patients and control group without cerebral artery occlusion. SWI assessed DMV prominence in both cerebral hemispheres on a 0-3 scale. If the DMV scores of both hemispheres were consistent, DWV was not prominent (DMV-). Otherwise, DWV was prominent (DMV+) in the higher score hemisphere. In DMV + patients with CICAO, if the DMV score in the ipsilateral hemisphere of CICAO w

[Bypass Surgery for Common Carotid Artery Occlusion].

Bypass surgery for common carotid artery occlusion presents significant challenges. In cases of internal carotid artery occlusion, superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass is the standard approach, as the STA blood flow remains intact. However, in common carotid artery occlusion, the STA blood flow is generally insufficient for a donor artery, necessitating alternative and more complex surgical strategies. This paper discusses the surgical approach for common carotid artery occlusion, focusing on donor artery selection and the relevant surgical anatomy.
