[Hybrid treatment for tandem occlusion of the internal carotid and middle cerebral arteries in ischemic stroke].

The article presents a clinical case of hybrid treatment for tandem occlusion of the internal carotid artery (ICA) and the middle cerebral artery (MCA). The procedure was performed by a single surgical team in one operating room. First, an eversion carotid endarterectomy (ECEAE) for ICA was performed, followed by a common carotid artery (CCA) puncture and aspiration thrombectomy from MCA. The time from symptom onset to revascularization was 180 min. The outcome is favorable; the patient was discharged from the hospital with a neurological deficit of 2 points according to the National Institute

A Novel and Simple Score to Predict Embolic or Atherosclerotic Middle Cerebral Artery Occlusion Before Mechanical Thrombectomy: AHOC Score.

OBJECTIVE: The mechanical thrombectomy (MT) strategy obviously differs for acute middle cerebral artery occlusion (MCAO) stroke caused by embolism or atherosclerosis. Our study aimed to develop and validate a simple and universally applicable score for predicting etiology [embolism or intracranial arteriosclerosis (ICAS)] before MT in patients with acute MCAO stroke. METHODS: Between November 2019 and September 2022, we retrospectively enrolled eligible patients in our hospital as the training cohort. Additionally, consecutive patients between July 2023 and April 2024 were recruited as the va

Sequential bilateral common carotid artery occlusion in protein C deficiency and patent foramen ovale: illustrative case.

BACKGROUND: Common carotid artery occlusion (CCAO) is rare, with bilateral involvement being exceedingly uncommon. Its etiologies are heterogeneous, and sequential bilateral CCAO is exceptionally rare. Protein C deficiency is an inherited thrombophilia primarily associated with venous thromboembolism, while its role in arterial thrombosis remains unclear. In the presence of a patent foramen ovale (PFO), paradoxical embolism may contribute to arterial ischemic events. OBSERVATIONS: A 56-year-old man presented with sudden-onset motor aphasia and right hemiparesis. MR angiography demonstrated lo

Transient Ischemic Attack Progressing to Top-of-the-Basilar Syndrome: A Case Report.

BACKGROUND Top-of-the-basilar syndrome (TOBS) is a rare but clinically heterogeneous posterior circulation disorder characterized by acute ischemia affecting 2 or more territories supplied by the distal basilar artery, including the thalami, midbrain, occipital lobes, medial temporal lobes, and cerebellum. Intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) is an established treatment for eligible patients with TOBS, and reports of transient coma induced during rt-PA infusion remain exceedingly scarce. CASE REPORT A 77-year-old man with non-valvular atrial fibrillati

Convexity Subarachnoid Hemorrhage Due to Large Artery Atherosclerotic Stenosis or Occlusion: A Report of 3 Cases.

INTRODUCTION: Convexity subarachnoid hemorrhage (cSAH) is a rare subtype of nontraumatic subarachnoid hemorrhage. Large artery atherosclerotic stenosis or occlusion has emerged as a prevalent etiology in China. The diagnosis of cSAH is primarily based on imaging examinations. However, there are no established guidelines or consensus for its treatment. CASE REPORT: This report presented 3 cases of cSAH associated with atherosclerotic stenosis or occlusion. All 3 patients had significant large-artery stenosis or occlusion. In 2 cases (cases 1 and 3), computed tomography (CT) angiography reveale

Retrieval of an atherosclerotic plaque-vessel wall complex during mechanical thrombectomy: illustrative case.

BACKGROUND: Retrieval of an atherosclerotic plaque-vessel wall complex as a single specimen from the middle cerebral artery during mechanical thrombectomy (MT) is exceedingly rare. OBSERVATIONS: A 74-year-old man with atrial fibrillation underwent MT for right M1 occlusion. A red thrombus and a yellowish-white specimen were retrieved, and successful reperfusion was achieved. Histopathological examination of the retrieved specimen revealed an atherosclerotic plaque attached to the arterial wall, consistent with retrieval of the plaque-vessel wall complex as a single specimen. Postoperatively,

A coexisting pseudoaneurysm hidden behind a saccular distal middle cerebral artery aneurysm in long-standing postirradiation vasculopathy: illustrative case.

BACKGROUND: Delayed cerebrovascular complications after childhood cranial irradiation may include progressive vasculopathy and aneurysmal disease. However, attributing a ruptured aneurysm directly to prior irradiation can be difficult in patients with complex preexisting cerebrovascular pathology. The authors describe a diagnostic pitfall in a patient with long-standing postirradiation vasculopathy who presented with hemorrhagic distal middle cerebral artery (MCA) aneurysmal lesions. OBSERVATIONS: A 51-year-old man who had undergone whole-brain irradiation for pineal germinoma at 8 years of a

Slab maximum intensity projection images of rotational angiography are useful for evaluation of the perforating branch in a case of basilar artery incarceration secondary to clival fracture.

BACKGROUND: In the event of clivus exhibiting longitudinal fractures, the basilar artery (BA) may become entrapped at the fracture site, although this occurrence is infrequent. Occlusion of bilateral BA perforators, accompanied by entrapment of the BA, can result in brainstem ischemia, which often leads to unfavorable outcomes. We report here the usefulness of slab maximum intensity projection (MIP) images of rotational angiography in evaluating the BA perforators. CASE DESCRIPTION: A 73-year-old man fell from a two-meter stepladder and was transferred to the emergency department. His Glasgow

Inadvertent neuroform atlas stent deployment into a posterior cerebral artery circumflex branch with long-term patency during stent-assisted coiling of a basilar artery aneurysm.

BACKGROUND: Stent-assisted coiling (SAC) is an established option for wide-neck intracranial aneurysms. However, ischemic complications such as in-stent thrombosis or stenosis remain, particularly when stents are deployed in small-caliber vessels. The Neuroform Atlas is generally recommended for parent vessels ≥2 mm, and data on its use in small-caliber arteries arising from the posterior cerebral artery (PCA), including the circumflex branch, are limited. CASE DESCRIPTION: A 51-year-old woman with autosomal dominant polycystic kidney disease and prior renal transplantation presented with sud

Stent-assisted coil embolization of a fenestration-related aneurysm of an infraoptic anterior cerebral artery: A case report.

BACKGROUND: An infraoptic course of the anterior cerebral artery (ACA) is a rare congenital vascular anomaly that may be associated with intracranial aneurysms and other segmental anomalies. The endovascular experience in this setting remains limited. CASE DESCRIPTION: A 66-year-old man was incidentally found to have an unruptured 8.0-mm aneurysm arising at the proximal apex of a fenestrated left infraoptic ACA. The precommunicating segment of the left infraoptic ACA formed a fenestration whose thick left and thin right limbs each gave rise to an A2 segment before rejoining into a common trun

Tandem Lesion Strokes: To Stent or Not to Stent, Insights From a Decade of Experience at a Comprehensive Stroke Center in Argentina.

BACKGROUND: Tandem lesion strokes, defined by the coexistence of an intracranial large- or medium-vessel occlusion and a cervical internal carotid artery stenosis or occlusion, represent a challenging subtype of acute ischemic stroke. Optimal endovascular management remains controversial, particularly regarding the role of emergent carotid artery stenting (eCAS) during mechanical thrombectomy (MT). We aim to compare the safety and efficacy of eCAS versus MT alone in patients with anterior circulation tandem lesion strokes treated at a comprehensive stroke center. METHODS: We conducted a retro

Same lesion, different outcomes: hemodynamic correlates of ischemic stroke in unilateral high-grade internal carotid artery stenosis or occlusion - a retrospective transcranial Doppler study.

BACKGROUND: High-grade stenosis or occlusion of the internal carotid artery (ICA) is a leading cause of ischemic stroke (IS). However, patients with severe ICA lesions often experience varying outcomes. This study investigated the relationship between cerebral hemodynamics, as detected by transcranial Doppler ultrasound (TCD), and the occurrence of IS in patients with high-grade stenosis or occlusion of unilateral ICA. METHODS: A total of 272 patients diagnosed with high-grade stenosis or occlusion of unilateral ICA were retrospectively enrolled in the study. The baseline data, laboratory exa

Recurrent Transient Paresthesias After Revascularization for Moyamoya Disease: Hemodynamic Diagnostic Challenges in a Patient With Post-radioiodine Hypothyroidism.

Moyamoya disease (MMD) is a chronic progressive cerebrovascular arteriopathy characterized by stenosis or occlusion of the terminal internal carotid arteries (ICAs) with compensatory collateral vessel formation. Although surgical revascularization reduces ischemic risk, recurrent neurologic symptoms may persist and can create a diagnostic challenge when structural imaging remains stable. We report the case of a 31-year-old Black woman with a prolonged course of MMD complicated by prior hemorrhagic and ischemic cerebrovascular events who continued to experience recurrent transient unilateral pa

Hybrid revascularization for acute common carotid artery occlusion after transcarotid transcatheter aortic valve implantation: illustrative case.

BACKGROUND: Transcarotid transcatheter aortic valve implantation (TC-TAVI) is an alternative access route for patients in whom transfemoral TAVI is not suitable. Although carotid complications are uncommon, they may occur because of direct carotid manipulation. We report a rare case of acute common carotid artery occlusion after TC-TAVI treated with hybrid revascularization. OBSERVATIONS: A 94-year-old woman underwent TC-TAVI through the left common carotid artery because severe iliofemoral calcification precluded transfemoral access. After the procedure, delayed emergence from general anesth

Assessment of cerebrovascular reactivity in middle cerebral artery stenosis using non-hypercapnic resting-state fMRI: a potential biomarker for hemodynamic alterations.

BACKGROUND: Cerebrovascular reactivity(CVR), a key indicator of cerebrovascular reserve, is crucial for evaluating cerebrovascular pathophysiology. This study employed resting-state MRI (rs-MRI) to assess CVR alteration in patients with unilateral middle cerebral artery stenosis or occlusion (MCA-S) under non-hypercapnic conditions, comparing them with healthy controls. METHODS: A total of 41 patients with unilateral MCA-S and 50 age-, sex-, and education-matched normal controls (NC). All underwent rs-MRI and neuropsychological assessments. CVR was derived from rs-fMRI frequency band signals,

Staged Carotid Endarterectomy Following Percutaneous Transluminal Angioplasty for Hemodynamically Compromised Internal Carotid Artery Stenosis at High Risk of Hyperperfusion: A Case Report.

Cerebral hyperperfusion syndrome (CHS) is a rare but potentially devastating complication after carotid revascularization in patients with severe carotid stenosis and impaired cerebral hemodynamics. We report a case of staged carotid endarterectomy (CEA) performed to reduce the risk of postoperative CHS. A 78-year-old woman underwent ophthalmologic evaluation for decreased vision in the left eye and was referred to our department because ocular ischemia related to ipsilateral carotid artery disease was suspected. Carotid ultrasonography demonstrated severe stenosis of the left internal carotid
