Cerebrovascular stenosis related to tyrosine kinase inhibitor for chronic myeloid leukemia: two illustrative cases.

BACKGROUND: Tyrosine kinase inhibitors (TKIs) improve prognosis in chronic myeloid leukemia (CML). Nilotinib and ponatinib, second- and third-generation TKIs, respectively, have been reported to cause adverse vascular occlusive events such as myocardial infarction and peripheral arterial disease. However, little is known about the risk of cerebral infarction associated with severe cerebrovascular stenosis, which is a late complication of TKIs. Herein, we report two cases of cerebrovascular stenosis associated with TKIs for CML. CASE PRESENTATION: A 53-year-old man with CML experienced transie

Carotid Endarterectomy Ameliorates Cognitive Impairment in Clinical and Experimental Unilateral Carotid Artery Stenosis.

BACKGROUND: Carotid endarterectomy (CEA) is widely used to treat carotid artery stenosis (CAS). However, the effects of CEA on unilateral CAS-induced cognitive impairment and the underlying mechanism remain poorly understood. METHODS AND RESULTS: Thirteen patients diagnosed with unilateral severe CAS underwent pre- and post-CEA assessments, including 18fluoro-2-deoxy-d-glucose positron emission tomography/magnetic resonance imaging, cognitive assessments, and routine blood tests before and after CEA. Unilateral carotid common artery occlusion and ligation release (reperfusion) surgeries were

Molecular Mediators of Neutrophil Primary Granule Release Following Acute Ischemic Stroke and their Associated Epigenetic Modulation by HDAC2.

High concentrations of neutrophil degranulation products in the plasma and thrombi are poor prognostic indicators in patients with acute ischemic stroke (AIS). This study aimed to identify candidate effectors capable of mediating neutrophil degranulation post-AIS, and to reveal their underlying epigenetic mechanisms. Microarrays and ChIP-seq were applied to analyze the neutrophils of patients with AIS. Cerebral ischemia was induced in C57/BL6 mice by middle cerebral artery occlusion (MCAO). Lipopolysaccharide was used to induce inflammation in HL-60 Cells. Protein and mRNA levels were assessed

Outcome of acute ischemic stroke with absent opacification of the cervical internal carotid artery at CT-angiography after endovascular treatment.

Purpose: Occlusion of the distal internal carotid artery can simulate a proximal occlusion of its cervical tract on CT angiography in patients with acute ischemic stroke, that is, pseudo-occlusion. As true and false carotid occlusions can present similarly on non-invasive imaging in patients undergoing endovascular treatment for stroke, our study aimed to evaluate clinical and technical differences of these conditions and the possible consequences of a misdiagnosis. Methods: We retrospectively reviewed consecutive patients who underwent mechanical thrombectomy for acute ischemic stroke at a si

Pipeline embolization device in treating middle cerebral artery aneurysms: a single-center experience in 69 consecutive patients.

PURPOSE: The pipeline embolization device (PED) has been established as an effective treatment option for aneurysms within its designated indications. Recently, its off-label applications, including treating middle cerebral artery (MCA) aneurysms, have increased. This study augmented the current literature by examining the outcomes of PED utilization for MCA aneurysms. METHODS: We collected data from consecutive patients with MCA aneurysms treated with PED between June 2016 and December 2021. Particularly, we collected data on patient demographics, aneurysm characteristics, procedural details

Superficial temporal artery-middle cerebral artery bypass for progressive cerebral infarction in invasive aspergillosis-induced total occlusion of internal carotid artery: A rare case and literature review.

Central nervous system (CNS) aspergillosis is a life-threatening infection primarily affecting immunocompromised patients and may lead to severe cerebral infarction through vascular invasion. However, there is limited data on the treatment options for aspergillosis-induced cerebral infarction especially surgical treatments such as superficial temporal artery (STA)-middle cerebral artery (MCA) bypass surgery. Herein, we present a case of cerebral infarction in a 59-year-old male with progressive right eye ptosis. Specifically, he had ipsilateral MCA stenosis originating from paranasal sinusitis

Dynamic PET imaging in patients with unilateral carotid occlusion shows lateralized cerebral hypoperfusion, but no amyloid binding.

BackgroundCarotid occlusive disease is a risk factor for cognitive decline. A possible underlying etiology is that hemodynamic impairment results in decreased cerebral perfusion, exacerbated amyloid-β accumulation (Aβ) and poorer cognitive performance.ObjectiveWe aimed to determine whether patients with unilateral internal carotid artery (ICA) occlusion have less cerebral perfusion and more Aβ in the ipsilateral than in the contralateral hemisphere, and whether perfusion and Aβ are associated with cognitive functioning.MethodsWe included 20 patients (age 67.2 ± 7.0 years, 8 females, MMSE 29 [2

Effect of intravenous alteplase before endovascular therapy for atherothrombotic stroke-related large vessel occlusion: subanalysis of the RESCUE AT-LVO registry.

BACKGROUND: Whether intravenous thrombolysis (IVT) should be administered prior to endovascular therapy (EVT) in patients with atherothrombotic stroke-related large vessel occlusion (AT-LVO) remains unclear. This study aimed to assess the efficacy and safety of IVT administered before EVT in this patient population. METHODS: We analysed the data from a multicentre registry of patients who underwent EVT for AT-LVO. Patients were categorised based on presumed mechanism of occlusion: in situ occlusion (intracranial group) or embolism from cervical artery occlusion/stenosis (tandem group). We com

Host-Microbial Cometabolite Ursodeoxycholic Acid Protects Against Poststroke Cognitive Impairment.

BACKGROUND: Poststroke cognitive impairment (PSCI) is a common residual disability after stroke, often underestimated and underdiagnosed. We previously found that ursodeoxycholic acid (UDCA), a host-microbiota cometabolite, ameliorates brain damage in stroke mice. Based on these findings, we aimed to evaluate the predictive value of UDCA for PSCI risk in a prospective cohort study. METHODS AND RESULTS: We recruited 202 patients with mild acute ischemic stroke and 63 patients with symptomatic large-artery atherosclerotic stenosis as the modeling and external validation cohorts, respectively. Multiple synchronous intracranial dural arteriovenous fistulas.

BACKGROUND: Intracranial multiple dural arteriovenous fistulas (DAVFs) are rare, with most reports limited to small case series and basic descriptive analyses. To better understand this condition-often linked to lower cure rates and higher progression risk-we conducted a comparative analysis of their angiographic characteristics, clinical presentations, and outcomes. METHODS: Data were retrospectively collected from the Dural Arteriovenous Fistula Research and Management in China (DREAM-INI) database. Lesions were classified as multifocal or diffuse based on angiographic features, and as prog

Impacts of obstructive sleep apnea on collateral circulation in stroke patients with proximal middle cerebral artery stenosis or occlusion.

BACKGROUND: Obstructive sleep apnoea (OSA) is a common sleep-related breathing disorder that can negatively affect the prognosis of cardiovascular events. However, OSA may facilitate the development of collateral circulation via hypoxic preconditioning. This study aimed to investigate the relationship between OSA and intracranial collateral circulation among stroke patients with proximal middle cerebral artery (MCA) stenosis or occlusion. METHODS: Patients with symptomatic unilateral proximal MCA stenosis or occlusion were enrolled. All patients underwent polygraph and computed tomographic an

Successful Mechanical Thrombectomy and Balloon Angioplasty in Two Children With Arterial Ischemic Stroke Caused by Focal Cerebral Arteriopathy.

BACKGROUND: Mechanical thrombectomy (MT), although well established in adults, is gaining acceptance as a viable treatment in pediatric patients, especially with cardioembolic etiology. However, there is an ongoing debate about the use of MT in children with arterial ischemic stroke due to suspected focal cerebral arteriopathy (FCA). Here, we present two cases of pediatric arterial ischemic stroke with high suspicion of FCA successfully treated with MT, highlighting its potential utility in this population. In addition, both cases were treated with balloon angioplasty successfully; thus this r
