Arterial complications in sickle cell disease (SCD), including stenoses and occlusions, are critical contributors to stroke. Townes SCD mice exhibit neurocognitive deficits and micro-vasculopathy, however stenoses and occlusions that could be causal to ischemic strokes have not yet been confirmed, which has led to challenges whether murine pathology reflects human pathology for strokes due to SCD. In our longitudinal study using label-free magnetic resonance angiography (MRA) to image carotid arteries in Townes SCD mice as they aged from 1 to 7 months, we identified multiple stenoses and occlu

BACKGROUND AND OBJECTIVES: Management of carotid lesions during endovascular therapy (EVT) in patients with acute ischemic stroke (AIS) due to tandem lesions (TL) remains controversial, and long-term prognosis data are scarce. We evaluated 90-day and 1-year follow-up outcomes of emergent carotid artery stenting (eCAS) vs non-eCAS in these patients.
METHODS: Data from a prospective mandatory population-based registry of Code Stroke in Catalonia (Spain) between 2017 and 2023 were analyzed. Patients with large vessel occlusion and ipsilateral ≥50% atheromatous cervical internal carotid artery (I

PURPOSE: To clarify the relationship between deep medullary veins (DMV) and risk of post-discharge stroke in chronic internal carotid artery occlusion (CICAO) patients.
METHODS: Hospitalized patients were divided into CICAO patients and control group without cerebral artery occlusion. SWI assessed DMV prominence in both cerebral hemispheres on a 0-3 scale. If the DMV scores of both hemispheres were consistent, DWV was not prominent (DMV-). Otherwise, DWV was prominent (DMV+) in the higher score hemisphere. In DMV + patients with CICAO, if the DMV score in the ipsilateral hemisphere of CICAO w

BACKGROUND: Hemodynamic failure in patients with steno-occlusive arterial disease is a major risk factor for stroke. Previous studies have identified impaired autonomic function in patients with carotid artery stenosis. Our study explores autonomic dysfunction and altered cerebrovascular hemodynamics in patients with stenosis and suspected hemodynamic failure.
METHODS: To assess autonomic nervous system dysfunction, patients underwent heart rate variability (HRV) testing, an active stand test, and the Valsalva maneuver with simultaneous monitoring of heart rate, blood pressure, and cardiac ou

OBJECTIVE: Mechanical thrombectomy is constantly advancing in terms of devices and techniques to enhance outcomes, and it is now commonly performed worldwide. Delayed stenosis is a potential complication that can occur following the procedure. However, its pathophysiology, risk factors, and time of onset remain unclear. We report 2 cases of delayed severe stenosis after mechanical thrombectomy.
CASE PRESENTATION: Case 1: A 70-year-old man was referred to our hospital with a right middle cerebral artery occlusion. We performed mechanical thrombectomy promptly. Complete recanalization was achie

Bypass surgery for common carotid artery occlusion presents significant challenges. In cases of internal carotid artery occlusion, superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass is the standard approach, as the STA blood flow remains intact. However, in common carotid artery occlusion, the STA blood flow is generally insufficient for a donor artery, necessitating alternative and more complex surgical strategies. This paper discusses the surgical approach for common carotid artery occlusion, focusing on donor artery selection and the relevant surgical anatomy.

PURPOSE: Mechanical thrombectomy (MT) is standard care for acute large vessel occlusion (LVO), but it fails in 10-20% of cases, often due to underlying intracranial artery stenosis (ICAS). In such cases, rescue stenting (RS), with or without angioplasty, may improve recanalization, but its clinical benefit remains debated. The purpose of this study was to define predictors of clinical outcome in this patient population.
METHODS: We conducted a retrospective multicenter study including 115 patients with ICAS-related occlusion of the middle cerebral artery (MCA) treated with MT and RS across 27

BACKGROUND: Beta-thalassemia intermedia (BTI) is a chronic hematologic disorder that may predispose to systemic complications, including rare cerebrovascular events. Moyamoya angiopathy (MMA) is a progressive steno-occlusive cerebrovascular disease, often presenting with recurrent strokes and transient ischemic attacks (TIAs). The association between BTI and MMA remains exceedingly rare.
CASE PRESENTATION: We report a 25-year-old woman with BTI who developed recurrent ischemic strokes and TIAs. Imaging studies revealed bilateral chronic infarcts and stenosis of distal internal carotid and mid

To prospectively evaluate the diagnostic efficacy of combining head and neck computed tomography angiography (CTA) with the ABCD2 score (Age, Blood pressure, Clinical features, Duration, Diabetes score) for detecting carotid artery stenosis in patients with transient ischemic attacks (TIAs) over an extended follow-up period. In this single-center prospective cohort study, 416 TIA patients (January 2020-January 2025) were stratified into high-risk (ABCD2 ≥4, n = 200, 48.08%) and low-risk (ABCD2 <4, n = 216, 51.92%) groups. High-risk patients underwent head-neck CTA within 2 hours of symptom ons

INTRODUCTION: Intracranial atherosclerotic disease (ICAD), prevalent in over 65% of Asian populations, is a major cause of global cerebrovascular morbidity and mortality through cerebral hypoperfusion. Despite guideline-directed medical therapy, recurrent ischaemic events remain common due to inadequate collateral circulation. Previous randomised controlled trials (ie, International Extracranial-Intracranial Bypass Study, carotid occlusion surgery study, carotid and middle cerebral artery occlusion surgery study) demonstrated high perioperative risks of extracranial-to-intracranial bypass surg

Unilateral asymptomatic middle cerebral artery stenosis or occlusion (MCAs/o) is an ideal human model for investigating the neural consequences of chronic cerebral hypoperfusion. Using a discovery-validation approach, this study aimed to characterize functional abnormalities in hypoperfused brain regions of unilateral MCAs/o and assess their neurobehavioral implications. In a discovery cohort comprising 41 patients with unilateral MCAs/o and 30 matched controls, patients exhibited significantly impaired performance on bilateral grooved pegboard tests (GPT, P < 0.05). BACKGROUND: Common carotid artery (CCA) occlusion frequently results in compromised hemodynamics of the ipsilateral hemisphere with risk of infarction but also a lack of blood flow in the ipsilateral superficial temporal artery (STA), requiring a more complex revascularization strategy than a standard extracranial-intracranial (EC-IC) bypass when indicated.
METHOD: We describe the performance of a "Bonnet bypass" using a radial artery interposition graft (RAIG) from the contralateral STA to the ipsilateral middle cerebral artery (MCA). A groove is drilled in the skullcap to position the RAIG

OBJECTIVES: Indirect revascularization after bypass surgery for intracranial atherosclerotic steno-occlusive disease (ICAD) remains controversial. We aimed to evaluate indirect revascularization after direct bypass surgery in patients with ICAD.
METHODS: Among 245 patients with ICAD who underwent direct bypass surgery between 2003 and 2022, 191 patients were ultimately enrolled in this study. Treatment outcomes were compared between combined bypass (n = 162 [84.8 %]) and direct bypass alone (n = 29) groups. Multivariable analyses were performed to identify significant predictors of indirect r

The combined technique (simultaneous use of a stent retriever and contact aspiration) is widely used for mechanical thrombectomy to treat acute large-vessel occlusions, but its clinical benefits remain unclear. We compared the efficacy and safety of different vessel-recanalization strategies on clinical outcomes across age groups. We analyzed 301 consecutive patients with internal carotid or middle cerebral artery occlusions. Between January 2017 and March 2021, 145 patients underwent single-device mechanical thrombectomy (stent retriever or contact aspiration) as the first-line strategy. Betw

INTRODUCTION: Carotid atherosclerotic disease (CAD) is a major cause of stroke, often requiring a combination of medical and surgical interventions. Current guidelines have established well the role of interventions such as carotid endarterectomy and carotid artery stenting (CAS) for unilateral carotid disease. However, there is still a paucity of evidence on the timing, procedural order, and complication rate of these procedures when there is bilateral carotid involvement. Hyperperfusion syndrome (HPS), with or without associated intracerebral hemorrhage, although rare, is a major source of m

BACKGROUND: Extracranial-intracranial (EC-IC) bypass surgery is performed to reduce the risk of ipsilateral cerebral infarction in selected patients with chronic intracranial atherosclerotic steno-occlusive disease (ICAD) with reduced cerebral blood flow (CBF). However, postoperative watershed shift induced ischemic stroke (WSIS) may occasionally occur despite maintained bypass patency and improved CBF. We report the incidence and characteristic features of WSIS after superficial temporal artery-middle cerebral artery (STA-MCA) bypass for chronic WSIS.
METHODS: We retrospectively analyzed 158
