Transcranial Doppler ultrasound in the evaluation of collateral blood flow in patients with internal carotid artery occlusion: correlation with cerebral angiography.

PURPOSE: To determine the accuracy of transcranial Doppler (TCD) ultrasound for evaluation of collateral supply through the circle of Willis in patients with internal carotid artery (ICA) occlusion. METHODS: The evaluation of the collateral pathways through the circle of Willis with TCD ultrasound and with cerebral angiography was compared in 40 patients (30 men, 10 women; mean age, 55 +/- 9 years) in a total of 44 ICA occlusions of which 43 had a suitable ipsilateral temporal bone window for TCD examination. RESULTS: By TCD, a patent anterior communicating artery is indicated by a reversal

Transcranial Doppler sonography and magnetic resonance angiography in the assessment of collateral hemispheric flow in patients with carotid artery disease.

BACKGROUND AND PURPOSE: The aim of this investigation was to compare the respective efficacy of transcranial Doppler sonography (TCD) and magnetic resonance angiography (MRA) for the assessment of intracranial hemodynamics in patients with extracranial occlusion or severe stenosis of the internal carotid artery (ICA). METHODS: Twenty-five patients with unilateral ICA occlusion (n = 20) or tight stenosis (n = 5) demonstrated by duplex scanning or angiography were studied with both TCD and MRA. Three-dimensional time-of-flight MRA was used for the evaluation of extracranial-intracranial ICAs. C

Massive infarcts involving the territory of the anterior choroidal artery and cardioembolism.

BACKGROUND AND PURPOSE: At neuropathological examination, the territory of the anterior choroidal artery is frequently found to be involved in massive infarcts of the internal carotid artery territory. The aim of our study was to analyze the clinical spectrum, the course, and the mechanism of these massive infarcts compared with the rare infarcts involving only the anterior choroidal artery territory. METHODS: Retrospective clinical examination and pathological study were performed in 35 patients with cerebral infarcts affecting at least the territory of the anterior choroidal artery. RESULT

Moyamoya disease: diagnostic accuracy of MRI.

Our purpose was to evaluate the diagnostic accuracy of MRI in moyamoya disease. We studied 30 patients with this disease, comparing MRI and angiographic findings. The diagnostic value of MRI was evaluated for occlusive lesions, collateral vessels, and parenchymal lesions. In all patients bilateral occlusion or stenosis of the supraclinoid internal carotid artery and proximal anterior and middle cerebral arteries was clearly shown by MRI, and staging of the extent of occlusion agreed with angiographic staging in 44 (73%) of 60 arteries. MRI, particularly coronal images, clearly showed basal cer

High resolution, magnetization transfer saturation, variable flip angle, time-of-flight MRA in the detection of intracranial vascular stenoses.

OBJECTIVE: Factors that restrict 3D TOF MRA are limited resolution, saturation of flow, and degree of background suppression. We evaluated MRA for intracranial stenoses by using a 3D TOF technique that minimizes these factors. MATERIALS AND METHODS: Twenty-nine patients underwent MRA and intraarterial digital subtraction angiography (DSA). The MRA studies were performed on a 1.5 T Siemens SP 4000 system. Integrated techniques applied to the conventional 3D TOF acquisition included the following: (a) 256 x 256 matrix with a 140 mm FOV and 0.9 mm slice thickness, yielding a 0.54 x 0.54 x 0.9 mm

The role of the circle of Willis in carotid occlusion: assessment with phase contrast MR angiography and transcranial duplex.

PURPOSE: To study the collateral pathways recruited after occlusion of the internal carotid artery (ICA), and to evaluate its influence on the impairment of hemispheric blood flow supply and development of low flow infarcts. METHODS: 38 patients with ICA occlusion (18 asymptomatic; five transient ischaemic attacks; and 15 strokes) were included. Infarcts on cerebral MR scanning were categorised in order to differentiate patients with territorial infarcts or no lesion (group I; n = 22) from those with brain damage due to low flow (group II; n = 16). Patency and direction of flow in the communi

Anastomosis of occipital artery to posterior cerebral artery with interposition of superficial temporal artery using occipital interhemispheric transtentorial approach: case report.

BACKGROUND: Superficial temporal artery (STA)-superior cerebellar artery (SCA) anastomosis, and STA-posterior cerebral artery (PCA) anastomosis are considered suitable as surgical procedures for the treatment of patients with significant stenosis or occlusion in the rostral portion of the basilar artery and patients with significant stenosis or occlusion of the posterior cerebral artery, respectively. However, several authors have reported frequent and serious complications of these surgical procedures, including temporal lobe retraction edema and hematoma. In this study, we introduce a new su

Cortical blood flow and cognition after extracranial-intracranial bypass in a patient with severe carotid occlusive lesions. A three-year follow-up study.

The long-term effect of extra-intracranial arterial bypass on cerebral circulation was examined. Cortical blood flow and cognitive ability were evaluated pre- and up to 3 years post-bypass in a 58-years-old man with severe carotid occlusive lesions, who presented with 3 transient cerebral ischaemic attacks which resulted in mental deterioration over 3 years. Regional cerebral blood flow (rCBF) was evaluated pre- and up to 33 months post-bypass by 123Iodine N-isopropyl-p-iodoamphetamine (IMP) single-photon emission CT (SPECT). Mental abilities were evaluated before and up to 33 months after sur

Regressing intracranial carotid occlusions in childhood.

Though detected with increasing frequency, intracranial carotid artery dissection remains less common in infancy. We report on 3 otherwise healthy children aged 8, 12 and 15 years who presented with focal headache and stroke secondary to intracranial carotid occlusive disease consistent with arterial dissection. In 2 cases this was precipitated by strenuous physical exertion. The protean angiographic configuration included long tapered narrowing with focal stenosis, beaded narrowing with Moya Moya vascular network and 'string sign'; occlusion of the anterior cerebral artery was always present.

[Traumatic middle cerebral artery occlusion].

A 16-year-old boy developed motor aphasia and right hemiplegia after head trauma caused during Rugby football club activity. About 2 hours after trauma, these neurological deficits improved dramatically. CT scan on admission revealed no abnormal finding. But, on the next day, CT scan revealed an irregular low density area at the left basal ganglia. Cerebral angiography showed a slight narrowing at the left carotid siphon, severe stenosis at M1 portion of the left middle cerebral artery with vasospasm of its distal branches and occlusion of the left anterior temporal artery. The extracranial po

Anterior choroidal artery territory infarcts. Study of presumed mechanisms.

BACKGROUND AND PURPOSE: A previous study suggested that occlusive diseases of small penetrating arteries account for most anterior choroidal artery (AChA) territory infarcts, but half of the patients did not have an echocardiogram. Cases of AChA territory infarcts associated with internal carotid artery stenosis or atrial fibrillation suggest that this hypothesis may be wrong. The aim of this study was to determine the mechanism of 16 nonselected consecutive AChA territory infarcts. METHODS: The study population consisted of 8 men and 8 women aged 17 to 89 years. They underwent a computed tom

Encephaloduroarteriosynangiosis (EDAS) for the treatment of childhood moyamoya disease.

Moyamoya disease is defined by the angiographic demonstration of stenosis or occlusion of the vessels of the anterior circulation at the base of the brain and the concomitant development of collateral blood supply. Untreated, the disease is often clinically progressive, resulting in significant neurologic sequelae. Encephaloduroarteriosynangiosis (EDAS), which involves the transposition of a segment of a scalp artery onto the surface of the brain, is a surgical treatment aimed at improving collateral blood flow. Six children underwent 8 EDAS procedures and were followed from 6 months to 9 year

[Unilateral moyamoya disease associated with acrofacial vitiligo in a 13-year-old patient--case report].

Cerebrovascular Moyamoya disease (MMD) is a clinical entity characterized by stenotic or occlusive lesions around the terminal portions of the internal carotid arteries (ICA) and the formation of abnormal vascular networks at the base or convexity. This condition is sporadic, and there are no pathological explanations for this cause. A case with unilateral juvenile MMD is reported. A 13-year-old girl with sudden episode of left limbs weakness, accompanied by transient dysphasia and seizures attack, was admitted to our hospital for further investigation. Computed tomography scanning showed an a

Multiple cerebral aneurysms associated with aortitis syndrome--case report.

A 48-year-old female presented with four aneurysms in the anterior half of the circle of Willis associated with aortitis syndrome. All the aneurysms were successfully clipped. In general, intracranial hemodynamic change, due to stenosis or occlusion of carotid arteries, is considered to initiate aneurysm growth in the vertebrobasilar system in this syndrome, but renal hypertension was probably involved in our patient. Careful screening for multiple aneurysms, even in the anterior circulation, should be undertaken in patients with aortitis syndrome who present with an aneurysm.

The anatomy of the posterior communicating artery as a risk factor for ischemic cerebral infarction.

BACKGROUND: After the occlusion of an internal carotid artery the principal source of collateral flow is through the arteries of the circle of Willis, but the size and patency of these arteries are quite variable. Study of the anatomy of the collateral pathways in patients with internal-carotid-artery occlusion with or without infarction in the watershed area of the deep white matter may identify patterns that afford protection from ischemic infarction. METHODS: Using conventional magnetic resonance imaging and three-dimensional phase-contrast magnetic resonance angiography, we evaluated 29 c

Evaluation of cerebral vasoreactivity by three-dimensional time-of-flight magnetic resonance angiography.

BACKGROUND AND PURPOSE: Cerebral vasoreactivity is an important indicator of the reserve capacity of the cerebral circulation. To make a quantitative analysis of cerebral vasoreactivity in individual major arterial territories, we evaluated the response to acetazolamide using three-dimensional time-of-flight magnetic resonance angiography. METHODS: We examined 10 healthy volunteers and 6 patients with unilateral stenosis of the middle cerebral artery by a 1.5-T superconducting magnetic resonance imaging system. After a baseline vascular image was obtained, each subject received 17 mg/kg IV of

[A case of emergency carotid endarterectomy to cope with vertebrobasilar insufficiency].

This is a report of an emergency carotid endarterectomy preformed on a 59 year-old male. He had been suffering from TIA of vertebrobasilar insufficiency since 1 day prior to admission. Angiography revealed severe stenosis and ulceration of the left cervical carotid bifurcation and occlusion of the bilateral vertebral arteries. Six hours after angiography, he showed hemiparesis, decerebrated posture and consciousness disturbance. These neurological signs were considered to be derived from occlusion of the left carotid bifurcation and disappearance of the collateral circulation via the left post
